Fibpredictor: a computational method for rapid prediction of amyloid fibril structures.

Amyloid fibrils are important in diseases such as Alzheimer's disease and Parkinson's disease, and are also a common instability in peptide and protein drug products. Despite their importance, experimental structures of amyloid fibrils in atomistic detail are rare. To address this limitation, we have developed a novel, rapid computational method to predict amyloid fibril structures (Fibpredictor). The method combines β-sheet model building, β-sheet replication, and symmetry operations with side-chain prediction and statistical scoring functions. When applied to nine amyloid fibrils with experi

Low Wages as Occupational Health Hazards.

The history of occupational medicine has been characterized by ever-widening recognition of hazards, from fires in 1911 to asbestos in the 1960s, to job strain in the 1990s. In this essay, we argue for broadening the recognition further to include low wages. We first review possible mechanisms explaining the effects of wages on health or health behaviors. Mechanisms involve self-esteem, job satisfaction, deprivation, social rank, the "full" price of bad health, patience, and the ability to purchase health-producing goods and services. Second, we discuss empirical studies that rely on large, ty

Vaccines against leptospirosis.

Vaccines against leptospirosis followed within a year of the first isolation of Leptospira, with the first use of a killed whole cell bacterin vaccine in guinea pigs published in 1916. Since then, bacterin vaccines have been used in humans, cattle, swine, and dogs and remain the only vaccines licensed at the present time. The immunity elicited is restricted to serovars with related lipopolysaccharide (LPS) antigen. Likewise, vaccines based on LPS antigens have clearly demonstrated protection in animal models, which is also at best serogroup specific. The advent of leptospiral genome sequences

Prophylactic mastectomy.

With availability of genetic testing and development of statistical models for risk stratification, more women are being identified as having increased risk for breast cancer. A number of risk-reducing treatment options with varying efficacy exist for them, including frequent surveillance, chemoprevention, prophylactic salpingo-oophorectomy (PSO), and prophylactic mastectomy (PM). Those most likely to benefit from PM are BRCA gene carriers and those who have a strong family history of breast cancer. Prevetive PM remains controversial, however. There are no randomized controlled trials to subst

Strategies to identify genes for complex diseases.

Genes underlie numerous human diseases and traits. Although we have witnessed a great deal of success in identifying disease-susceptible genes, the task remains challenging for most of the complex diseases. This paper reviews evidence for the role of genetic factors in complex diseases including breast cancer, diabetes and multiple sclerosis. We then describe strategies that can potentially optimize our chance of success in identifying genes involved in complex diseases. Advances in molecular biology, particularly mapping of the human genome, statistical methods that provide more accurate mode

RF 46-790 versus paracetamol: effect on post-operative pain.

The analgetic efficacy of a new non-steroidal anti-inflammatory drug fluproquazone (RF 46-790, Sandoz A. G., Switzerland) was tested versus paracetamol in a double-blind parallel group study using pain after surgical removal of impacted third molars. The patients received 3 doses of 100 mg fluproquazone or 500 mg paracetamol. Statistical analysis was made for the first and last dose. No statistically significant difference was found between the two treatment groups with respect to the first and last dose taken. Only minor complaints of side effects were noted with no statistically significant

Development, assessment and validation of a novel prediction nomogram model for risk identification of tracheobronchial tuberculosis in patients with pulmonary tuberculosis.

OBJECTIVE: Tracheobronchial tuberculosis (TBTB), a specific subtype of pulmonary tuberculosis (PTB), can lead to bronchial stenosis or bronchial occlusion if not identified early. However, there is currently no available means for predicting the risk of associated TBTB in PTB patients. The objective of this study was to establish a risk prediction nomogram model for estimating the associated TBTB risk in every PTB patient. METHODS: A retrospective cohort study was conducted with 2153 PTB patients. Optimised characteristics were selected using least absolute shrinkage and selection operator re

Predicting major complications in patients undergoing laparoscopic and open hysterectomy for benign indications.

BACKGROUND: Hysterectomy, the most common gynecological operation, requires surgeons to counsel women about their operative risks. We aimed to develop and validate multivariable logistic regression models to predict major complications of laparoscopic or abdominal hysterectomy for benign conditions. METHODS: We obtained routinely collected health administrative data from the English National Health Service (NHS) from 2011 to 2018. We defined major complications based on core outcomes for postoperative complications including ureteric, gastrointestinal and vascular injury, and wound complicati

Moderate carpal tunnel syndrome: Factors associated with long-term surgery risk after corticosteroid injection.

Corticosteroid injection is commonly used for moderate carpal tunnel syndrome (CTS), but factors associated with subsequent surgery remain unclear. This study evaluated baseline clinical, electrophysiological, and ultrasonographic factors associated with long-term surgery risk. This retrospective cohort included 73 of 84 participants with electrodiagnostically moderate CTS from a randomized trial comparing ultrasound-guided and landmark-guided injections. All received a single corticosteroid injection at baseline. Carpal tunnel release was ascertained through structured telephone interviews, w

Explainable artificial intelligence (XAI) in schizophrenia: enhancing transparency in clinical decision-making.

Schizophrenia is a chronic and severe psychiatric disorder affecting nearly 1% of the global population, characterized by positive symptoms such as hallucinations and delusions, negative symptoms including avolition and affective flattening, and cognitive impairments that compromise social and occupational functioning. Despite advances in psychiatry, diagnosis remains largely symptom-based, relying on criteria from the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) and the International Classification of Diseases (ICD-11), highlighting the need for objective biomarkers and impro

Non-invasive periodontal screening using self-reported-oral-health (SROH) questionnaire and salivary biomarkers: development and validation of machine learning models.

BACKGROUND: Accurate and accessible screening tools for periodontitis are essential for early detection and disease prevention. This study evaluated a non-invasive diagnostic approach integrating sociodemographic data, self-reported oral health (SROH) questionnaires, and salivary biomarkers, using both conventional statistical and machine learning (ML) predictive models. METHODS: Seventy-seven adults completed a validated SROH questionnaire and provided saliva samples for quantification of six biomarkers: interleukins (IL-1β, IL-6), tumour necrosis factor (TNF-α), matrix metalloproteinases (M

On misconceptions about the Brier score in binary prediction models.

The Brier score is a widely used metric in epidemiological and clinical research for evaluating the accuracy of probabilistic predictions for binary outcomes, such as disease occurrence, treatment response, and screening performance. Despite its popularity, the Brier score is frequently misunderstood, leading to flawed interpretation of prediction models and potentially misguided public health and clinical decisions. This study aims to didactically clarify common misconceptions about realised Brier scores and to provide practical, statistically rigorous guidance for its correct interpretation

Emotional Intelligence as a Differentiated Personal Resource: Component-Level Associations with Burnout Among Higher-Education Teachers.

Drawing on the personal-resource logic of the Job Demands-Resources (JD-R) model, this study examines emotional intelligence (EI) not as a single global trait but as a differentiated set of self-regulatory resources associated with distinct burnout dimensions. Using cross-sectional, self-report data from higher-education instructors (N = 292; complete-case N = 161-185 for the multivariable models), EI was assessed with the Bar-On Emotional Quotient Inventory and burnout with the BO-SE Burnout Screening Scales (mental, emotional, physical, and social exhaustion, plus an aggregated index); hiera

Towards intelligent air quality forecasting using integrated machine learning framework with variational mode decomposition and catboost feature selection.

Predicting air pollution is crucial in improving air quality (AQ), which consequently provides benefits to the ecosystems and human health. AQ predictions often make use of Machine Learning (ML) approaches; nevertheless, these methods are not without their limitations. The main contribution of this research is to develop an efficient framework using machine learning (ML) for forecasting daily air quality metrics for Sulfur dioxide (SO2) and Nitrogen dioxide (NO2) in Changping, China. The suggested ML method is based on a set of local weights and a kernel extreme learning machine (LWKELM) model

Heart rate variability as a predictor of future depressive symptoms: A systematic review and meta-analysis.

Depression has limited early detection strategies. Heart rate variability (HRV), an indicator of autonomic regulation, has been linked to depression cross-sectionally, but its prospective value remains uncertain. We performed a meta-analysis and systematic review examining whether HRV is prospectively associated with later depressive symptom levels. PubMed, Embase, Cochrane, and PsycINFO (inception November 3, 2024) were searched for longitudinal studies of initially non-depressed baseline HRV and at least one-month follow-up assessing depressive or internalizing symptoms. Meta-analyses used m

Comparative Analysis of Deep Neural Networks for Automated Ulcerative Colitis Severity Assessment.

BACKGROUND: Ulcerative colitis (UC) is a chronic inflammatory bowel disease characterized by continuous inflammation of the colon and rectum. Accurate disease assessment is essential for effective treatment, with endoscopic evaluation, particularly the Mayo Endoscopic Score (MES), serving as a key diagnostic tool. However, MES measurement can be subjective and inconsistent, leading to variability in treatment decisions. Deep learning approaches have shown promise in providing more objective and standardized assessments of UC severity. METHODS: This study utilized publicly available endoscopic

Comparing personality dysfunction, maladaptive personality traits, and borderline personality disorder as models of emotion dysregulation in three adult samples.

The categorical borderline personality disorder (BPD) diagnosis identifies people who struggle to manage negative emotions. As the field transitions to dimensional personality disorder (PD) models, it is important to know whether alternative diagnostic constructs capture emotion regulation difficulties to the same degree. If not, it may make sense to modify the dimensional models or else retain the BPD syndrome to preserve its incremental utility for clinical description, treatment planning, and prognosis. In three adult samples (total N = 1,197), we modeled self-rated emotion dysregulation as

Income and Rurality Impact Overall Survival but not Cause-Specific Survival in Patients With Chondrosarcoma: A Population-Based Study From the SEER Database.

BACKGROUND AND OBJECTIVES: Previous studies evaluating socioeconomic status (SES) in bone malignancies such as chondrosarcoma used the Cox Proportional Hazards model, which might overestimate risk compared to cause-specific models like the Fine-Gray model. This study aims to evaluate the prognostic significance of income status in chondrosarcoma using both models. METHODS: We performed a retrospective cohort study using the Surveillance, Epidemiology, and End Results (SEER) database for patients diagnosed with chondrosarcoma. Patients were stratified by SES indicators, including income levels

Identification of structural fragments and field point-based design of novel p38α MAPK inhibitor: Integrating 2D and 3D-QSAR models with advanced in-silico techniques.

Dysregulation of p38α MAP kinase (MAPK14) increases the production of pro-inflammatory cytokines, causing pathogenesis of inflammatory, oncological, and neurodegenerative diseases. Till date, no orally effective p38α MAPK inhibitor exists in clinics, which may be due to poor selectivity and off-target effects. This emphasizes the urgent need for the design of potential p38α MAPK inhibitors. In this study, SMILES-based 2D-QSAR and field-point-based 3D QSAR models were developed to guide the design of novel p38α MAPK inhibitors. A dataset of 207 molecules was used to developed 2D-QSAR models via
