Technical feasibility of an angle-free ultrasound-assisted approach for challenging lumbar puncture in complex spinal anatomy: a two-patient case series.

BACKGROUND: Lumbar puncture is particularly challenging in patients with abnormal spinal anatomy, such as severe scoliosis or obesity. Conventional ultrasound‑assisted neuraxial techniques have two fundamental limitations: inaccurate projection of the probe's geometric center onto the skin when the probe is tilted and difficulty replicating the exact probe angle during needle insertion. These limitations often result in multiple needle passes, prolonged procedure time, and increased patient discomfort and complication risk. We developed an ultrasound‑assisted technique designed to standardize

[Myelopathies cervical arthroses. Apropos of 14 personal cases, 12 of which were operated on].

Reviewing fourteen cases, the authors show that cervical myelopathies present by spastic quadriplegia associated in half of the cases with sensory impairment and in a third of cases with radicular signs in the upper limbs. Tomography always shows posterior discal osteophytosis, sometimes gross, which reduces the lumen of the canal more particularly since it is associated (eleven times out of fourteen) with constitutional stenosis, of variable severity. Opaque or gaseous myelography makes it possible to exclude malformation or new formation and will demonstrate cord constriction within the cana

Hirayama disease: an uncommon cause of motor neuron disease.

We herein report the case of a 23-year-old man with a 3-year history of progressive right-hand weakness, leading to functional impairment. A neurological examination revealed signs of lower cervical motor neuron involvement, including the reverse split hand sign and the Wartenberg's sign. Electrophysiological studies showed reduced compound muscle action potentials (CMAPs) in the right ulnar and median nerves, with preserved sensory conduction and neurogenic changes in the myotomes from C7 to T1. A conventional cervical spine magnetic resonance imaging (MRI) scan was unremarkable, but a flexio

Insufficiency of trunk extension and impaired control of muscle force in Parkinson's disease with camptocormia.

OBJECTIVE: To examine the aetiology of parkinsonian camptocormia, a non-fixed pathological forward bending of the trunk, by measuring trunk muscle activation and force regulation in Parkinson patients with (PD + CC) and without (PD) camptocormia matched for disease severity, and in age- and sex-matched healthy controls (HC). METHODS: The isometric forces of trunk extension and flexion were measured in PD + CC, PD and HC. Neuromuscular efficiency (increase of extension force per increase of paravertebral muscle surface electromyography signal) and the ability to maintain a constant submaximal

Assessment of striatal & postural deformities in patients with Parkinson's disease.

BACKGROUND & OBJECTIVES: Though striatal and postural deformities are known to occur commonly in atypical Parkinsonism patients, these may also be seen in patients with Parkinson's disease (PD). These are frequently misdiagnosed as joint or orthopaedic pathology leading to unnecessary investigations. This study was conducted to observe the various striatal and postural deformities among patients with PD in India. METHODS: This study was conducted at a tertiary care teaching institute in north# India. Seventy consecutive patients with PD diagnosed as per the modified UK Brain Bank criteria wer

Sporadic lower motor neuron disease with a snake eyes appearance on the cervical anterior horns by MRI.

OBJECTIVE: Lower motor neuron disease (LMND) is the term generally used to describe diseases in which only lower motor neuron signs are detected. A snake eyes appearance on magnetic resonance imaging (MRI) is associated with a wide spectrum of neurological conditions including LMND. The author reports on three unique LMND patients with upper limb muscle weakness and atrophy who show a snake eyes appearance by MRI. PATIENTS AND METHODS: The patients were aged 18, 40 and 52 years, respectively, at the onset of the disease and had a longstanding clinical course (more than 10 years for two patien

Treatment of camptocormia with continuous subcutaneous infusions of apomorphine: 1-year prospective pilot study.

Camptocormia is defined as a marked dystonic flexion of the trunk in the sagittal plane. Camptocormia responds poorly to botulinum toxin injections, manipulation with dopaminergic treatment, and deep brain stimulation. We designed a prospective pilot study to assess the effect of apomorphine infusions on camptocormia. Five patients were enrolled. All five patients responded well to this treatment. The fact that camptocormia responds so well to apomorphine may be explained by the sustained stimulation of the ventrolateral striatal D1 receptors, alleviating this type of dystonia.

Progressive muscular atrophy variant of familial amyotrophic lateral sclerosis (PMA/ALS).

Twelve cases of adult-onset progressive muscular atrophy variant of amyotrophic lateral sclerosis (PMA/ALS) were studied in a small rural population of 1500 in the Republic of Belarus (former Soviet Union). The patients were members of three apparently related kindreds, each showing autosomal dominant pattern of disease inheritance. The average age at clinical onset ranged from 26 to 57 years (mean, 40 years). Each patient suffered from skeletal muscle weakness and wasting, starting in the limbs and spreading to the trunk and neck, with very limited bulbar and no upper motor neuron involvement

Severe motor tics causing cervical myelopathy in Tourette's syndrome.

We describe two patients with Tourette's syndrome in whom severe motor tics involving the neck were complicated by cervical myelopathy. The first patient, a 21-year-old man, had complex tics consisting of violent twisting and extending movements of the neck preceded by an irresistible urge to produce the abnormal postures. Two years after onset of these tics, paraesthesias, sensory deficits up to the level of C4, and a gait disturbance developed. Neuroimaging studies confirmed compressive myelopathy, and the symptoms gradually improved with botulinum toxin injections in the posterior cervical

Dissociated motor loss in the upper extremities. Clinical features and pathophysiology.

Dissociated motor loss occurring in the upper extremities with and without lower extremity myelopathy was evaluated in patients with cervical spondylosis. The presence of dissociated motor loss without attendant myelopathy was correlated with selected compression of the anterior nerve root in the lateral spinal canal, close to the intervertebral foramen. The clinical and radiologic feature differentiating these two dissociative syndromes were reviewed.

Late-onset progressive radiculomyelopathy in patients with cervical athetoid-dystonic cerebral palsy.

Three patients with severe athetoid-dystonic type of cerebral palsy involving the neck musculature developed in their fourth to fifth decade progressive cervical radiculomyelopathy associated with vertebral spondylarthrotic compressive lesions in addition to their long-standing neurological syndrome. It is likely that the late-onset myelopathy is linked to continuous torsion, compression and 'wear and tear' of the cervical spinal cord, induced by the involuntary movements. In 2 patients decompressive cervical spinal surgery was ineffective, suggesting irreversible damage.

Longitudinal spinal cord MRI in adult 5q-SMA: biomarker and pathophysiological insights.

BACKGROUND: Spinal muscular atrophy (SMA) is a progressive neuromuscular disease caused by biallelic SMN1 mutations and widespread lower motor neuron degeneration. Objective biomarkers to monitor progression and treatment response are critically needed. Spinal cord (SC) MRI, including diffusion tensor imaging (DTI), is a promising non-invasive approach, but data in adults remain scarce. METHODS: We conducted an exploratory longitudinal study in 26 adults with 5q-SMA (type 2: n = 4; type 3: n = 22; median age 29.5 years) and 27 controls undergoing 3 T cervical SC MRI. Gray matter (GM) and tota

Parkinson's disease-associated camptocormia: biopsy-confirmed focal myositis and treatment options.

BACKGROUND: The etiology of the disabling symptom camptocormia/dropped head in Parkinson's disease (PD) is still debated. Many PD patients develop involuntary flexion of the spine in an upright position that does not respond to dopaminergic treatment. OBJECTIVES: To study the histopathology of paraspinal muscles in PD-associated camptocormia with potential consequences for therapy. METHODS: We report a cohort of 66 patients with PD (mean age 68.4 ± 8.1 years) and associated camptocormia according to consensus criteria. Paravertebral muscle biopsy was performed in all patients with consecutiv

Deep brain stimulation for Parkinson's disease-related postural abnormalities: a systematic review and meta-analysis.

Deep brain stimulation (DBS) has become a well-established treatment modality for Parkinson's disease (PD), especially regarding motor fluctuations, dyskinesias, and tremor. Although postural abnormalities (i.e., Camptocormia [CC] and Pisa syndrome [Pisa]) are known to be a major symptom of PD as well, the influence of DBS on postural abnormalities is unclear. The objective of this study is to analyze the existing literature regarding DBS for PD-associated postural abnormalities in a systematic review and meta-analysis. In compliance with the Preferred Reporting Items for Systematic Reviews an

The effect of postural deformities on back function and pain in patients with Parkinson's disease.

BACKGROUND: Postural deformities, such as Pisa syndrome (PS), and camptocormia and antecollis (C&A) are common in patient with Parkinson's disease (PwPD). These deformities can lead to back disability and pain with different mechanisms, including abnormal loading or stress on soft tissues as muscles, lumbar discs and ligaments. OBJECTIVES: To evaluate the effect of different postural deformities including PS and C&A on back function and pain in PwPD. METHODS: The function, disability and pain were assessed by Oswestry disability index (ODI) and brief pain inventory (BPI). All participants co

Immediate effect of spinal magnetic stimulation on camptocormia in Parkinson's disease.

OBJECTIVES: Spinal cord stimulation is a potential therapeutic option for the treatment of Parkinson's disease (PD)-associated symptoms. Repetitive trans-spinal magnetic stimulation (rTSMS) is a non-invasive and safe alternative for stimulation of spinal pathways that has not been studied for therapeutic efficacy in PD. We assessed the benefits of rTSMS on camptocormia, an often treatment-resistant postural abnormality observed in PD patients. METHODS: We compared rTSMS to sham stimulation in PD patients with camptocormia in a single-centre, randomised, single-blind, crossover, placebo-contro
