Surgical anatomy of the posterior shoulder: effects of arm position and anterior-inferior capsular shift.

The purposes of this study were to evaluate anatomically various surgical intervals to the posterior shoulder and to determine the effects of varying arm positions and anterior-inferior capsular shift (AICS) on the relation of the posterior neurovascular structures to fixed bony landmarks. Fourteen cadaveric shoulders were dissected. The posterior surgical anatomy was defined, and the distances from fixed bony landmarks to neurovascular and musculotendinous structures were determined with digital calipers. Measurements were made with the arm in various positions and repeated after AICS. The mo

Modified superficial musculoaponeurotic system face-lift: a review of 327 consecutive procedures and a patient satisfaction assessment.

BACKGROUND: A conventional superficial musculoaponeurotic system (SMAS) face-lift is well established because it allows the skin envelope to be rotated in a bit more lateral direction than the cephalad redirection of the SMAS flap. This ensures an individualization of the treatment plan according to the needs of the patient and avoids a postoperative stretched look. However, this technique has some limitations with respect to its long-term effects on the sagging tissue, inadequate lifting of the malar fat pad, and flattening of the nasolabial fold. METHODS: The procedure described by the auth

Construction and validation of nomograms to predict central lymph node metastasis in clinical node-negative unilateral papillary thyroid carcinoma.

The scope of neck lymph node dissection remains controversial for unilateral papillary thyroid carcinoma (UPTC) patients with no clinical evidence of lymph node metastasis (cN0). This study aims to build and validate a model for predicting central lymph node metastasis (CLNM) in UPTC patients through preoperative basic information and intraoperative rapid frozen pathology results. Retrospective analysis covered 1928 patients with PTC from the Wuhan Union Hospital database (2010-2020), randomly split into training and validation sets in a 7:3 ratio. Univariate and multivariate logistic regressi

Impact of nodal status evaluation on therapeutic strategy for clinically unifocal T1b/small T2 node negative papillary thyroid carcinoma.

PURPOSE: In absence of nodal metastases or aggressive features, thyroid lobectomy (TL) should be preferred over total thyroidectomy (TT) for 1-4 cm unifocal, papillary thyroid carcinoma (PTC). However, occult, despite non-microscopic (≥2 mm), nodal metastases may be present in clinically node-negative (cN0) PTC. METHODS: Among 4216 thyroidectomies for malignancy (2014-2023), 110 TL plus ipsilateral central neck dissection (I-CND) were scheduled for unifocal cT1b/small cT2 (≤3 cm) cN0 PTCs. Frozen section examination (FSE) of removed nodes was performed: when positive, completion thyroidectomy…

Basaloid squamous carcinoma of mandible.

Basaloid squamous carcinoma is a rare distinct histologic variant of squamous cell carcinoma of the head and neck region. Basaloid squamous carcinoma is more aggressive and has a poorer prognosis, although histologically, it is associated with squamous cell carcinoma and squamous atypia. The usual site of occurrence for basaloid squamous carcinoma is the base of the tongue, hypopharynx (pyriform sinus), and the supraglottic larynx (Wain et al. Hum Pathol 1986;17:1158-1166). We are reporting an unusual case of basaloid squmous carcinoma of the mandible with no secondaries and features of sarcom

Minimally invasive esophagectomy.

Minimally invasive esophagectomy can be safely performed in selected cases in centers specializing in minimally invasive esophageal surgery.Potential benefits include lessened physiologic insult, with decreased hospital stay and a more rapid recovery to full activity. Drawbacks include the cost of the disposable instrumentation and the steep learning curve. As thoracic surgeons continue to acquire expertise with this procedure, improved results may be expected. Prospective trials with longer follow-up will be required to confirm any advantages of MIE over conventional approaches. Open surgical
