Peritoneal Dialysis in Patients Supported by Left Ventricular Assist Device.

INTRODUCTION: Terminal heart failure is often associated with end-stage kidney disease. Due to advantages concerning patient independence, peritoneal dialysis (PD) is an alternative to conventional hemodialysis treatment. As left ventricular assist device implantations continuously increase, data on combined PD and LVAD is rare. We present the first and largest cohort study on this exclusive patient cohort. METHODS: A retrospective study was conducted on patients who underwent LVAD implantation at a high-volume heart failure center from 2000 to 2024. Adverse events were analyzed according to

Immunomodulatory Effects of Pterocarpanquinone LQB-118 in Murine Peritoneal Macrophages.

BACKGROUND: Phagocytosis is an important function of macrophages. However, when it's dysregulated, it could compromise homeostasis. Thus, this study aimed to assess the inhibitory activity of pterocarpanquinone LQB 118 on murine macrophage phagocytosis. METHODS: We used peritoneal macrophages isolated from mice to evaluate the impact of LQB 118 (5 μM) on the modulation of phagocytic activity and possible action mechanism related: IL-12 (by ELISA), NO (by Griess reaction),ROS production (by flow cytometry), and intracellular signaling proteins (iNOS, P-Akt, P-mTOR, NF-κB, and P-NF-κB) (by flow

Surgical risk factors for technical survival of peritoneal dialysis catheters.

PURPOSE: Peritoneal dialysis has been demonstrated to be a cost-effective modality of dialysis treatment, providing a greater quality of life in comparison to hemodialysis. However, complications associated with the peritoneal dialysis catheter (PD catheter) can lead to increased patient morbidity and thus the necessity of PD catheter removal. While prior studies have identified patient-related risk factors, the impact of various surgical risk factors on technical survival is yet to be elucidated. METHODS: A retrospective, monocentric cohort study was conducted including all patients who unde

Baclofinding solutions: Baclofen-induced encephalopathy in a peritoneal dialysis patient.

Baclofen is a gamma-aminobutyric acid agonist that is commonly and widely used for the treatment of muscle spasticity. Given its predominant kidney excretion, patients with reduced kidney function are at particular risk of drug accumulation and toxicity, with neurotoxicity in the form of drowsiness, encephalopathy, seizures, and coma being the most reported clinical features. In addition to the importance of early identification of baclofen toxicity and drug discontinuation, dialysis can effectively accelerate baclofen elimination given its small molecule weight, and the relatively low volume

Scalable ion concentration polarization dialyzer for peritoneal dialysate regeneration.

A wearable artificial kidney (WAK) stands poised to offer dialysis treatment with maximal temporal and spatial flexibility for end-stage renal disease (ESRD) patients, while portability has not yet been achieved due to difficulties in portable purification. The ion concentration polarization (ICP), one of the nanoelectrokinetic phenomenon, has garnered substantial attention in the realm of portable purification applications, owing to its remarkable capacity for charge separation. In this work, scalable ICP dialyzer with 10,000-fold increase in throughput, was applied for peritoneal dialysate r

Unraveling Peritoneal Mesothelioma: A Case-Based Discussion on Diagnosis and Management.

Peritoneal mesothelioma is a rare and aggressive tumor, and its management remains poorly standardized in the literature. This case highlights the diagnostic and therapeutic challenges through a 56-year-old woman with no significant medical history or known asbestos exposure, who presented with progressive abdominal distension and postprandial vomiting. Imaging revealed diffuse nodular peritoneal thickening and large-volume ascites. A laparoscopic biopsy confirmed malignant epithelioid mesothelioma, supported by immunohistochemical positivity for cytokeratin 7 (CK7), calretinin, and Wilms tumo

Case Report: Psychogenic purpura in a uremic patient on peritoneal dialysis.

Psychogenic purpura (Gardner-Diamond syndrome) is a rare autoimmune vasculopathy characterized by the spontaneous onset of painful edema and infiltrative cutaneous lesions that rapidly develop into ecchymosis after severe psychological stress events. In this article, we report an 87-year-old female uremic patient who was admitted to the hospital with erythema and subcutaneous ecchymoses on the head and face following an Aedes mosquito sting. She was previously diagnosed with "toxic insect stings and skin bacterial infections" and was given anti-infective treatment by an outside hospital, which

Uncovering a peritoneal inclusion cyst after surgery: A case report.

Peritoneal inclusion cysts (PICs) are benign, multilocular fluid-filled lesions that predominantly affect women of reproductive age, often arising after abdominal surgery or chronic inflammation. We present the case of a 30-year-old woman with a history of laparotomy for a myometrial mass, who developed severe lower abdominal pain and distension 5 months postsurgery. Initial ultrasound revealed a large intraperitoneal fluid collection measuring 10 cm, with debris and septations. MRI further characterized the lesion as a peritoneal inclusion cyst, showing enhancing septations and no solid compo

Postoperative adhesion formation: the role of peritoneal macrophages and targeting therapy.

BACKGROUND: Postoperative adhesion (PA) is the most common complication of abdominal or pelvic surgery, resulting in various complications, including small bowel obstruction, secondary female infertility, chronic abdominal pain and even death. Unfortunately, there are few effective methods for the prevention and treatment of it. Previous studies confirmed that coagulation response, inflammation, fibrosis and fibrinolysis played a central role in PA formation and a variety of cells, such as monocytes, macrophages, fibroblasts, neutrophils and mesothelial cells participated in this process. Peri

Impact of leptin on postoperative peritoneal adhesion formations in colorectal resection.

PURPOSE: Postoperative peritoneal adhesion formation (PAF) is a distressing complication. Leptin, secreted by fat tissues, may be associated with PAF. However, the relationships between PAF and leptin are unclear. This study investigated the impact of leptin on PAF after colorectal tumor resection. METHODS: A cecum cauterization mouse model was used to investigate PAF and its association with leptin. Mice adhesion score (AS) was evaluated on day 7 after treatment. The clinical study included patients who underwent elective colorectal resection with temporary ileostomy between April 2021 and D

CT and MR in peritoneal malignancies: pearls and pitfalls at preoperative examination.

Peritoneal malignancies (PM) are defined as the spread of malignant epithelial cells in the peritoneal cavity. Until the recent past, the prognosis was considered extremely poor, and the treatment options had only palliative intent. Currently, new locoregional treatments have radically changed the outcome. CT is pivotal in PM diagnosis, staging, surgical planning, and determining therapeutic decisions. MRI should be evaluated in a preoperative setting for the evaluation of mesentery, serosal, and in any cases of contraindication of CT with contrast medium, while in the restaging clinical setti

Distinct Immunosuppressive Tumor Microenvironment in Gastric Cancer With Peritoneal Metastasis.

PURPOSE: Immunotherapy combined with chemotherapy is the standard palliative treatment for gastric cancer. However, peritoneal metastases are often resistant to immunotherapy, underscoring the need to better understand the tumor immune microenvironment (TIME). In this study, we aimed to comprehensively analyze the TIME in peritoneal metastases of gastric cancer. MATERIALS AND METHODS: Paired single-cell suspensions from malignant ascites and peripheral blood mononuclear cells (PBMCs) were obtained from 27 patients with gastric cancer for multicolor fluorescence-activated cell sorting (FACS) a

Laparoscopic Excision of a Giant Parietal Peritoneal Lipoma: A Case Report.

Lipomas are benign tumours derived from adipocytes, commonly found across the body, especially on the trunk and limbs. Although typically small, rare cases involve giant lipomas larger than 10 cm, often causing clinical symptoms. We report a case of a giant parietal peritoneal lipoma in a 45-year-old obese woman presenting with right upper abdominal pain. Imaging revealed a large, well-defined fatty mass in the abdominal wall, which was surgically excised laparoscopically. The specimen measured 18x14 cm and weighed 984 gm, with histology confirming a benign lipoma. The procedure was uncomplica

Molecular and Functional Characterization of the Peritoneal Mesothelium, a Barrier for Solute Transport.

Peritoneal dialysis (PD) is an increasingly needed, life-maintaining kidney replacement therapy; efficient solute transport is critical for patient outcome. While the role of peritoneal perfusion on solute transport in PD has been described, the role of cellular barriers is uncertain, the mesothelium has been considered irrelevant. We calculated peritoneal blood microvascular endothelial surface area (BESA) to mesothelial surface area (MSA) ratio in human peritonea in health, chronic kidney disease, and on PD, and performed molecular transport related gene profiling and single molecule localiz

Impact of peritoneal dialysis on weight gain in a patient with anorexia nervosa.

Anorexia nervosa (AN) is an eating disorder characterized by restriction of energy intake leading to a significantly low body weight, and intense fear of gaining weight. Severe electrolyte changes such as hypokalemia and hypophosphatemia; and alterations in water metabolism such as hyponatremia and edema, can occur in patients with AN. Hypokalemia and chronic volume depletion may lead to acute kidney injury (AKI) and chronic kidney disease (CKD). There are few reports of patients with AN who require maintenance dialysis. Therefore, it is unclear whether hemodialysis (HD) or peritoneal dialysis
