Contribution to the validation of Italian version of Fear-Avoidance Beliefs Questionnaire.

BACKGROUND AIM: Propose further evidence for reliability and validity of Italian translation and adaptation of Fear Avoidance Beliefs Questionnaire, (FABQ) originally built-up by Waddel et al. (1993) METHODS: 250 participants, inpatient and outpatient, suffering from acute or chronic back-pain have been consecutively recruited from various Italian physiotherapy and functional rehabilitation centres. All participants were administered FABQ and QUALEFFO, to evaluate if avoiding behaviors can provoke disability independently from experimented pain. RESULTS: Varimax rotation, point out a 4 factor

Phobic anxiety symptom scores and incidence of type 2 diabetes in US men and women.

CONTEXT: Emotional stress may be a risk factor for type 2 diabetes (T2D), but the relation between phobic anxiety symptoms and risk of T2D is uncertain. OBJECTIVE: To evaluate prospectively the association between phobic anxiety symptoms and incident T2D in three cohorts of US men and women. DESIGN, SETTING AND PATIENTS: We followed 30,791 men in the Health Professional's Follow-Up Study (HPFS) (1988-2008), 68,904 women in the Nurses' Health Study (NHS) (1988-2008), and 79,960 women in the Nurses' Health Study II (NHS II) (1993-2011). Phobic anxiety symptom scores, as measured by the Crown-C

A clinically useful social anxiety disorder outcome scale.

Increasingly, emphasis is being placed on measurement-based care to improve the quality of treatment. Although much of the focus has been on depression, measurement-based care may be particularly applicable to social anxiety disorder (SAD) given its high prevalence, high comorbidity with other disorders, and association with significant functional impairment. Many self-report scales for SAD currently exist, but these scales possess limitations related to length and/or accessibility that may serve as barriers to their use in monitoring outcome in routine clinical practice. Therefore, the aim of

Cardiovascular activity in blood-injection-injury phobia during exposure: evidence for diphasic response patterns?

Exposure to feared stimuli in blood-injection-injury (BII)-phobia is thought to elicit a diphasic response pattern, with an initial fight-flight-like cardiovascular activation followed by a marked deactivation and possible fainting (vasovagal syncope). However, studies have remained equivocal on the importance of such patterns. We therefore sought to determine the prevalence and clinical relevance of diphasic responses using criteria that require a true diphasic response to exceed cardiovascular activation of an emotional episode of a negative valence and to exceed deactivation of an emotional

Familiarity with and affective responses to foods in 8-11-year-old children. The role of food neophobia and parental education.

The present study investigated whether the familiarity with and affective responses to foods are predicted by the individual trait food neophobia and by parental education in school children. The cross-sectional data collection involved children (N=208, 8- and 11-year-old) from Helsinki, and their parents. A questionnaire assessing children's food neophobia with Food Neophobia Scale (FNS), and familiarity with and affective responses to 36 foods was completed by parents. Children with low FNS were familiar with a larger number of foods than those with high FNS. High FNS was associated with low

Determinants and trajectory of phobic anxiety in patients living with an implantable cardioverter defibrillator.

OBJECTIVE: The implantable cardioverter defibrillator (ICD) is the gold standard therapy to prevent life-threatening arrhythmias. Phobic anxiety predicts ventricular arrhythmia in coronary heart disease patients, but little is known about phobic anxiety in ICD patients. This study aimed to identify determinants and the course of phobic anxiety in ICD patients. PATIENTS: 140 outpatients living with an ICD (mean age 56±14 years, 66% men). MAIN OUTCOME MEASURES: Phobic anxiety was assessed with the Symptom Checklist-90 Revised at a mean of 27±21 months (range 3-109) post-ICD placement (baseline

Clinical staging model applied to young people presenting with social anxiety.

AIM: Although the use of illness-staging models in clinical medicine has proved particularly useful, the concept has not been widely applied in mental health. Here, we apply a clinical staging framework to a population of help-seeking young people presenting with social anxiety. The goal was to provide a detailed description of common clinical stage of those presenting for treatment of social anxiety, and to delineate the associations between symptom type, severity and clinical stage. METHODS: The results of a structured clinical interview along with background clinical information formed the

[Psychoeducation leads to a reduction in fear of spiders in 8- to 9-year-old children - a pilot study].

OBJECTIVE: In Western societies spiders are among the least liked of all arthropods, eliciting feelings of fear and disgust. The clinical manifestation of this fear - spider phobia - is a common anxiety disorder. In most cases the disorder has an early onset in childhood. The symptoms show a chronic course and can persist into adulthood if not treated. Etiological models emphasize the role of modeling and negative information transmission for the acquisition of the disorder. Even though powerful psychotherapeutic methods exist, referral to treatment is rather uncommon for children. Often spont

More reasons to be straightforward: findings and norms for two scales relevant to social anxiety.

The validity of both the Social Interaction Anxiety Scale and Brief Fear of Negative Evaluation scale has been well-supported, yet the scales have a small number of reverse-scored items that may detract from the validity of their total scores. The current study investigates two characteristics of participants that may be associated with compromised validity of these items: higher age and lower levels of education. In community and clinical samples, the validity of each scale's reverse-scored items was moderated by age, years of education, or both. The straightforward items did not show this pa

Combined cognitive bias modification treatment for social anxiety disorder: a pilot trial.

BACKGROUND: Cognitive Bias Modification (CBM) is a promising treatment for Social Anxiety Disorder (SAD). However, previous randomized trials have not systematically examined the combination of CBM for attention (CBM-A) and interpretation (CBM-I) or the credibility and acceptability of these protocols. METHODS: We conducted a randomized, double-blind placebo-controlled trial (N = 32) to examine the efficacy of a CBM treatment called Attention and Interpretation Modification (AIM) for SAD. AIM comprised eight, twice weekly computer sessions with no therapist contact. During AIM, participants (

The moderated effects of video feedback for social anxiety disorder.

Despite initially positive results, video feedback for social anxiety has never been shown to reduce social anxiety in a controlled experiment with diagnosed participants, and only once with undiagnosed participants. Previous studies arguably did not detect such an effect because of limited assessment of anxiety and potential moderators. We tested video feedback with cognitive preparation among treatment-seeking participants with a primary diagnosis of social anxiety disorder. In Session 1, participants gave an extemporaneous speech and either received the intervention or not. In Session 2, 6-

Phobic spider fear is associated with enhanced attentional capture by spider pictures: a rapid serial presentation event-related potential study.

The early posterior negativity (EPN) reflects early selective visual processing of emotionally significant information. This study explored the association between fear of spiders and the EPN for spider pictures. Fifty women completed a Spider Phobia Questionnaire and watched the random rapid serial presentation of 600 neutral, 600 negatively valenced emotional, and 600 spider pictures (three pictures per second). The EPN was scored as the mean activity in the 225-300-ms time window at lateral occipital electrodes. Participants with higher scores on the phobia questionnaire showed larger (i.e.

Gaze avoidance in social phobia: objective measure and correlates.

Gaze aversion could be a central component of the physiopathology of social phobia. The emotions of the people interacting with a person with social phobia seem to model this gaze aversion. Our research consists of testing gaze aversion in subjects with social phobia compared to control subjects in different emotional faces of men and women using an eye tracker. Twenty-six subjects with DSM-IV social phobia were recruited. Twenty-four healthy subjects aged and sex-matched constituted the control group. We looked at the number of fixations and the dwell time in the eyes area on the pictures. Th

Detection and distraction effects for threatening information in social phobia and change after treatment.

This work examines differences in the detection and distraction by social-threat-related information between a social phobia group (SP; N=33) and a normal control group (NC; N=32). The change obtained after psychological treatment is also studied for the SP group. A paper-and-pencil visual search task is used, in which the emotional valence of the "target" (social threat, physical threat, and neutral words) and "distractor" (social threat, physical threat, neutral, and nonsense words) verbal stimuli is manipulated. Results indicate that there are no differences in the detection of social-threa

Dynamic liquid association: complex learning without implausible guidance.

Simple associative networks have many desirable properties, but are fundamentally limited by their inability to accurately capture complex relationships. This paper presents a solution significantly extending the abilities of associative networks by using an untrained dynamic reservoir as an input filter. The untrained reservoir provides complex dynamic transformations, and temporal integration, and can be viewed as a complex non-linear feature detector from which the associative network can learn. Typically reservoir systems utilize trained single layer perceptrons to produce desired output r

Change processes in residential cognitive and interpersonal psychotherapy for social phobia: a process-outcome study.

The purpose of this study was to test cognitive and interpersonal models for improving social phobia. Eighty patients with social phobia were randomized to 10-week residential cognitive (RCT) or residential interpersonal psychotherapy (RIPT). They completed process measures every Thursday and a sub-outcome measure every Monday. The ratings were analyzed with mixed models. Weekly changes in the process variables derived from the cognitive model (self-focus, estimated probability and estimated cost of negative social events, safety behaviors) predicted subsequent weekly changes in social anxiety

Fear of blood/injection in healthy and unhealthy adults admitted to a teaching hospital.

Blood/injury phobia is one of the specific phobias. The aim of this study was to determine the fear of injection and blood in patients and healthy people. This study was carried out at Tepecik Hospital, Clinical Biochemistry Laboratory and Blood Center. Data were collected from 1500 adults who agreed to participate in the study (237 patients with chronic diseases and 1263 healthy people) during the period from January 2003 to February 2005. All participants completed two self-administered questionnaires (17-item Symptom Questionnaire and 20-item Blood/Injection Fear Scale) after giving blood s

A confirmatory factor analysis of specific phobia domains in African American and Caucasian American young adults.

The current study investigated factors related to specific phobia domains and differences in patterns among African American and Caucasian American adults. Subjects were 100 African Americans and 121 Caucasian Americans who completed the Fear Survey Schedule--Second Edition (FSS-II). Fears related to specific phobia domains were first examined, with frequencies differing between African American and Caucasian American samples on three of the six specific phobia domains. A confirmatory factor analysis was conducted to determine the patterns of specific phobias among the African American sample.

Behavioral avoidance and self-reported fainting symptoms in blood/injury fearful individuals: an experimental test of disgust domain specificity.

This study examined the specificity of disgust in predicting avoidance in blood/injury (BI) phobia. Participants high (n=38) and low (n=46) in BI fear completed measures of disgust across multiple domains and severity of BI-related fear. They then completed three randomly presented behavioral avoidance tasks (BATs) that consisted of exposure to a 15'' severed deer leg (BI task), a live spider (spider task), and a 'contaminated' cookie (cookie task). Fainting symptoms associated with each BAT were recorded as well. When controlling for gender and BI fear group membership, mutilation disgust con

Naltrexone renders one-session exposure therapy less effective: a controlled pilot study.

In vivo exposure has become the gold standard treatment for specific phobia. The endogenous opioid system is one mechanism proposed to explain why exposure provides such quick and effective treatment for specific phobia. The effect of naltrexone on fear and avoidance behavior was investigated among 15 specific phobia participants who received exposure treatment. Participants were randomly assigned to receive naltrexone, placebo, or no drug prior to attending one-session exposure treatment. Mixed effects regression results revealed that across time, the naltrexone group tolerated significantly
