[Free anterolateral thigh flap for reconstruction of head and neck defects].

OBJECTIVE: To explore the anterolateral thigh (ALT) free flap in which the donor site is less noticeable and the morbidity is minimal for the reconstruction of head and neck defect. METHODS: A retrospective review was performed of 8 ALT flaps reconstructions for head and neck tumor ablation defects between Dec. 2003 and May 2005. Recipient sites were subdivided into pharyngeal wall (n = 3), oral tongue (n = 2), buccal mucosa (n = 1), base of tongue (n = 1) and skull base (n = 1). The operative technique of ALT flap was described. RESULTS: All 8 free flaps survived. No complication was observ…

The magnetic resonance imaging profile of occult intracranial violations as a result of sinus surgery.

INTRODUCTION: Intracranial injury is a well-described complication of sinus surgery. Although such complications are typically associated with catastrophic morbidity, the neurological sequelae may be subtle and even unrecognized during the postoperative period. Magnetic resonance imaging (MRI), which can distinguish intracranial hemorrhage and injury, may provide a means for the accurate assessment of atypical presentations of occult intracranial complications. OBJECTIVE: To describe the MRI profile of occult intracranial complications that result from unrecognized skull base violation during…

Asymmetry of the sphenoid bone and its suitability as a reference for analyzing craniofacial asymmetry.

The purposes of this study were to evaluate the asymmetry of the sphenoid bone and to determine its suitability as a reference for analyzing asymmetry of the skull. Thirty-seven dry skulls from India were divided into group A (n = 18), with a right-left length discrepancy of less than 2 mm for both the external acoustic meatus-frontozygomatic suture and external acoustic meatus-subspinale, and group B (n = 19), with a right-left length discrepancy of more than 2 mm for either of these 2 parameters. The skulls were then examined with regard to the percentage of asymmetry of the sphenoid bone, t

Facial nerve repair by interposition nerve graft: results in 22 patients.

Resection of tumors of the posterior cranial base may incorporate a segment of the facial nerve because of tumor infiltration, or may result in unplanned nerve injury. Immediate repair of the facial nerve by resuture or with an autogenous nerve graft is highly desirable to ensure optimal recovery of facial function. Twenty-four patients who underwent extensive surgery of the posterior skull base and facial nerve reconstruction were studied. Of these, 12 patients had preoperative facial weakness and 3 had facial palsy. All patients underwent graft reconstruction from the subarachnoid or labyrin…

[The vomer and its role in the transfer of pressure in the masticatory system].

Dimensions of certain parts of the orofacial skeleton and vomer were obtained by a bioanthropologic measurement for which native preparations of whole and parasagitally sawed up skulls were used. The application of a harmonic frame of the skull and the main dimensions of the vomer allowed the construction of an ideal model of the vomer using Araldit B. The photoelasticimetric procedure and pressure load to the model were used for the determination of possible pressure tensions, if any, occurring during mastification, namely the share of the vomer in the transfer of these tensions to the base o

Mind the gap: natural cleft palates reduce biting performance in bats.

Novel morphological traits pose interesting evolutionary paradoxes when they become widespread in a lineage while being deleterious in others. Cleft palate is a rare congenital condition in mammals in which the incisor-bearing premaxilla bones of the upper jaw develop abnormally. However, ∼50% of bat species have natural, non-pathological cleft palates. We used the family Vespertilionidae as a model and linear and geometric morphometrics within a phylogenetic framework to (1) explore evolutionary patterns in cleft morphology, and (2) test whether cleft morphological variation is correlated wit

Estimation of skull table thickness with clinical CT and validation with microCT.

Brain injuries resulting from motor vehicle crashes (MVC) are extremely common yet the details of the mechanism of injury remain to be well characterized. Skull deformation is believed to be a contributing factor to some types of traumatic brain injury (TBI). Understanding biomechanical contributors to skull deformation would provide further insight into the mechanism of head injury resulting from blunt trauma. In particular, skull thickness is thought be a very important factor governing deformation of the skull and its propensity for fracture. Current computed tomography (CT) technology is l

Artificial Intelligence (AI)-driven prototype for sex estimation from cranial Computed Tomography (CT) images: A multi-regional analysis of the orbits, mastoid process, and foramen magnum in a Thai population.

Sex estimation from skeletal remains is a fundamental step in forensic anthropology. However, traditional morphometric methods relying on linear measurements often suffer from inter-observer variability and limited diagnostic power. This study aimed to develop and validate an artificial intelligence (AI)-driven prototype using an object detection framework to estimate sex gender from Cranial Computed Tomography (CT) images in a Thai population. 3D reconstructed images of the skull focusing on three anatomical regions-Orbit, Mastoid Process, and Foramen Magnum-were utilized. Two AI models, one

Olfactory Groove Meningioma Presenting With Visual Decline and Gait Disturbance: A Case Report.

Olfactory groove meningiomas (OGMs) are typically indolent neoplasms arising from the anterior skull base and frequently attain a considerable size prior to detection because early symptoms are often vague and nonspecific. Common clinical features include personality or behavioral alterations, diminished or absent olfaction, and gradually worsening visual impairment, which are often incorrectly attributed to psychiatric conditions or neurodegenerative processes, leading to delays in diagnosis. We report the case of a 55-year-old female who presented with a seven-month history of progressive bi…

Impact of Orbital Rim Osteotomy on Surgical Freedom in Transorbital Endoscopic Approaches: A Cadaveric Study and Novel Methodology.

PURPOSE: Transorbital neuroendoscopic surgery (TONES) is an emerging minimally invasive skull-base approach. The addition of an orbitotomy may increase the size of the surgical corridor, but this measurement has not previously been quantified. METHODOLOGY: Six fresh-frozen cadaveric specimens (11 orbits) underwent bilateral superior eyelid crease incisions and periorbital dissection to the meningo-orbital band and superior orbital fissure. This was followed by bilateral sphenoidotomy to expose the trigeminal ganglion and temporal dura. A custom surgical probe with an Augmented Reality, Univer

Man with posterior fossa tumors 15 years apart.

A 48-year-old man with a history of hypertension, peripheral vascular disease and a 50-pack-year history of smoking presented with new onset vertigo, tinnitus, diplopia and ataxia in January 1978. CT scan showed a radiolucent defect in the left cerebellar hemisphere with a possible mural nodule. In ensuing months, he experienced worsening symptoms with a corresponding increase in lesion size on re-imaging. Months later, a left posterior fossa craniotomy was performed and revealed a single cystic lesion containing copious amounts of straw-colored fluid and a single mural nodule in the inferior

Prevalence and natural course of craniocervical junction anomalies during growth in patients with osteogenesis imperfecta.

Pathology in the craniocervical junction is a serious complication of osteogenesis imperfecta (OI). Our aim was to analyze the prevalence and natural course of craniocervical junction anomalies in patients with OI during growth. In a one-center retrospective study, we analyzed lateral skull radiographs and midsagittal magnetic resonance images of 76 patients with either type I, III, or IV OI. The material included longitudinal series of 31 patients. In total, 150 patient images taken at ages 0 to 39 years were analyzed and compared with age-matched control data. Craniocervical anomalies were o

Contribution of the vertebral artery to cerebral circulation in the rat snake Elaphe obsoleta.

Blood supplying the brain in vertebrates is carried primarily by the carotid vasculature. In most mammals, cerebral blood flow is supplemented by the vertebral arteries, which anastomose with the carotids at the base of the brain. In other tetrapods, cerebral blood is generally believed to be supplied exclusively by the carotid vasculature, and the vertebral arteries are usually described as disappearing into the dorsal musculature between the heart and head. There have been several reports of a vertebral artery connection with the cephalic vasculature in snakes. We measured regional blood flo
