Efficacy and safety of solriamfetol for excessive daytime sleepiness in narcolepsy and obstructive sleep apnea: a systematic review and meta-analysis of clinical trials.

INTRODUCTION: Excessive daytime sleepiness (EDS) is the inability to maintain wakefulness and alertness during the major waking episodes of the day, with sleep occurring unintentionally or at inappropriate times. Solriamfetol is a selective norepinephrine-dopamine reuptake inhibitor approved for EDS. This review was done to assess the efficacy and safety of solriamfetol in patients with EDS in narcolepsy or OSA. METHODS: A systematic search of the electronic database was conducted for relevant studies. Any randomized controlled trial with outcome measures on the efficacy or safety of solriamf

State-dependent changes in the upper airway assessed by multidetector CT in healthy individuals and during obstructive events in patients with sleep apnea.

OBJECTIVE: To determine whether airway narrowing during obstructive events occurs predominantly at the retropalatal level and results from dynamic changes in the lateral pharyngeal walls and in tongue position. METHODS: We evaluated 11 patients with severe obstructive sleep apnea (OSA) and 7 healthy controls without OSA during wakefulness and during natural sleep (documented by full polysomnography). Using fast multidetector CT, we obtained images of the upper airway in the waking and sleep states. RESULTS: Upper airway narrowing during sleep was significantly greater at the retropalatal lev

Assessment of the relationship between metabolic syndrome and obstructive sleep apnea in male drivers of Shahroud city in 2018: a cross sectional study.

BACKGROUND: Metabolic syndrome involves a set of metabolic risk factors that directly increases the risk of atherosclerotic cardiovascular disease. Physical inactivity due to driving can increase the risk of metabolic syndrome. It is also known that sleep disorders (sleep apnea) can result in MetS. Driving in Iran is considered a very popular but risky occupation, so paying attention to this profession is of special importance. Therefore, the researchers aimed to investigate the association between sleep disorders and metabolic syndrome in drivers in Shahroud city in 2018. METHODS: This cross

Single Nucleotide Polymorphisms in Adiponectin Gene Are Not Directly Associated with Increased Risk of Obstructive Sleep Apnea Syndrome in a Chinese Han Population.

PURPOSE: This study aims to test the possible correlation between single nucleotide polymorphisms (SNPs) in the adiponectin gene and increased risk of obstructive sleep apnea syndrome (OSAS) in a Chinese Han population. MATERIALS AND METHODS: A total of 266 subjects were enrolled into the study to detect 9 SNPs in the adiponectin gene. Multivariate unconditional logistic regression analysis, adjusted for gender and age, was used to estimate the associations of these SNPs with OSAS risk. RESULTS: No evidence of a direct association was observed between these SNPs and the risk of OSAS in the C

Coexistence of Obstructive Sleep Apnea and Superior Vena Cava Syndromes Due to Substernal Goitre in a Patient With Respiratory Failure: A Case Report.

INTRODUCTION: Substernal goiter may rarely cause superior vena cava syndrome (SVCS) owing to venous compression, and cause acute respiratory failure due to tracheal compression. Obstructive sleep apnea syndrome (OSAS) may rarely occur when there is a narrowing of upper airway by edema and vascular congestion resulting from SVCS. CASE PRESENTATION: We presented the clinical course and treatment of acute respiratory failure (ARF) developed in a patient with SVCS and OSAS due to substernal goiter. After treatment of ARF with invasive mechanical ventilation, weaning and total thyroidectomy were s

The prevalence of obstructive sleep apnea-hypopnea syndrome-related symptoms and their relation to airflow limitation in an elderly population receiving home care.

BACKGROUND: Both airflow limitation and obstructive sleep apnea-hypopnea syndrome (OSAHS)-related symptoms are most prevalent in the elderly population. Previous studies revealed significant associations between OSAHS-related symptoms and obstructive airway diseases in the general population. However, other studies showed that the frequency of OSAHS-related symptoms in patients with obstructive airway diseases decreases after the age of 60 and older. AIMS: To investigate the prevalence of OSAHS-related symptoms (snoring, breathing pauses, and excessive daytime sleepiness [EDS]) and their rela

Multidimensional neuropsychological heterogeneity in obstructive sleep apnea: deep clinical phenotyping in a single-center cohort, population proteomic context, and depressive-burden risk stratification.

UNLABELLED: Obstructive sleep apnea (OSA) is associated with substantial cognitive and affective burden, yet conventional respiratory severity does not fully explain this heterogeneity. We investigated whether neuropsychological vulnerability in OSA shows differentiated clinical association patterns, whether cognition, depression, and anxiety share general-population proteomic associations, and whether a separate depressive-burden model can support pragmatic risk stratification. METHODS: We conducted a clinically anchored study integrating a single-center clinically phenotyped adult OSA cohor

Midface Advancement Surgery and Its Effects on Upper Airway Function and Obstructive Sleep Apnea Status in Children With Syndromic Craniosynostosis: A Systematic Review.

BACKGROUND: Syndromic craniosynostosis (SC) is a craniofacial developmental disorder characterized by the premature fusion of one or more cranial sutures. This early fusion leads to irregular skull shapes and, in severe cases, to complications, including respiratory problems. SC was reported to be associated with more than 150 syndromes, among them Apert, Crouzon, and Pfeiffer are the most common. Individuals with SC often experience OSA and airway obstruction. Surgical interventions, including midface advancement, address the deformities of these complications. OBJECTIVE: This study aims to

Pre- and Post-treatment Changes in Systemic Inflammatory Biomarkers After Oral Appliance Therapy for Obstructive Sleep Apnea: A Systematic Review and Meta-Analysis.

Systemic inflammation often accompanies obstructive sleep apnea (OSA), along with higher chances of heart-related metabolic issues. Although oral appliances serve as an accepted option instead of continuous positive airway pressure (CPAP), their influence on markers of systemic inflammation lacks clear evidence. The aim of this systematic review and meta-analysis was to assess changes in systemic inflammation markers following oral appliance therapy (OAT) among individuals diagnosed with OSA. Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) standards, a sys
