Mapping cortical asymmetry and complexity patterns in normal children.

This study reports the first comprehensive three-dimensional (3D) maps of cortical patterns in children. Using a novel parametric mesh-based analytic technique applied to high-resolution T1-weighted MRI scans, we examined age (6-16 years) and gender differences in cortical complexity (the fractal dimension or complexity of sulcal/gyral convolutions) and asymmetry of 24 primary cortical sulci in normally developing children (N=24). Three-dimensional models of the cerebral cortex were extracted and major sulci mapped in stereotaxic space. Given the documented age-related changes in frontal lobe

[Clinical study on air in epidural hematomas].

Air in epidural hematoma has previously been reported by some authors. With the advent of CT scan, the presence of air in epidural hematomas is not uncommon findings. In our series, 27 of 78 (34.6%) cases with acute epidural hematoma had air bubbles in epidural hematoma on CT scan. Acute epidural hematoma was located in the temporal in 18, frontal in 3, occipital in 2, parietal in one and posterior fossa in 3 cases. The air entrance was thought to be mastoid air cells in 13, open fracture in 5, frontal sinus in 3, sphenoid sinus in 3, and unknown in 3 cases. No patients in our series developed

Refractory Temporal Gelastic Seizure: A Case Report.

Gelastic seizures are rare epileptic events characterized by sudden, unprovoked bursts of laughter that are typically associated with hypothalamic hamartomas but may also arise from cortical epileptogenic foci. We report the case of an eight-year-old boy born prematurely at 28 weeks' gestation who presented with a two-year history of recurrent hypermotor seizures, loss of consciousness, behavioral arrest, falls, and frequent episodes of inappropriate laughter. Video electroencephalography demonstrated epileptiform activity consistent with left temporal lobe epilepsy with mild diffuse encephalo…

Instruction-induced modulation of the visual stream during gesture observation.

Although gesture observation tasks are believed to invariably activate the action-observation network (AON), we investigated whether the activation of different cognitive mechanisms when processing identical stimuli with different explicit instructions modulates AON activations. Accordingly, 24 healthy right-handed individuals observed gestures and they processed both the actor's moved hand (hand laterality judgment task, HT) and the meaning of the actor's gesture (meaning task, MT). The main brain-level result was that the HT (vs MT) differentially activated the left and right precuneus, the

Comparing three neuropsychological subgrouping approaches in subjective and mild cognitive impairment from a naturalistic multicenter study.

Subjective cognitive impairment (SCI) and mild cognitive impairment (MCI) are two clinical groups with an increased risk to develop dementia, but they are highly heterogeneous. This study compared three different approaches to subgroup SCI and MCI patients and investigated their capacity to disentangle cognitive and biomarker heterogeneity. We included 792 patients from the MemClin-cohort (142 SCI and 650 MCI). Biomarkers included cerebrospinal fluid measures of beta-amyloid-42 and phosphorylated tau, as well as visual ratings of medial temporal lobe atrophy and white matter hyperintensities o…

Spontaneous Intracranial Hemorrhage Concurrent With Subarachnoid and Subdural Hemorrhages: Report of a Rare Case.

Spontaneous intracerebral hemorrhage (SICH) is a rare occurrence in the temporal lobe, and its coexistence with other intracranial bleeding types such as subdural hemorrhage (SDH) and subarachnoid hemorrhage (SAH) is infrequently documented. Typically, SICH is managed conservatively without surgical intervention. In this case report, we present an unusual case of SICH in the temporal lobe, characterized by bleeding extending beyond the brain parenchyma into the subarachnoid and subdural spaces. Our approach involved tubular hematoma evacuation (surgical approach). Literature reports propose th…

Seizure freedom after laser amygdalohippocampotomy guided by bilateral responsive neurostimulation in pediatric epilepsy: illustrative case.

BACKGROUND: For patients with difficult-to-lateralize temporal lobe epilepsy, the use of chronic recordings as a diagnostic tool to inform subsequent surgical therapy is an emerging paradigm that has been reported in adults but not in children. OBSERVATIONS: The authors reported the case of a 15-year-old girl with pharmacoresistant temporal lobe epilepsy who was found to have bitemporal epilepsy during a stereoelectroencephalography (sEEG) admission. She underwent placement of a responsive neurostimulator system with bilateral hippocampal depth electrodes. However, over many months, her respo…

Comparison of CSF neurofilament light chain, neurogranin, and tau to MRI markers.

INTRODUCTION: We determined whether cerebrospinal fluid (CSF) neurofilament light (NfL), neurogranin (Ng), and total-tau (t-tau) differentia … ated with decreasing microstructural integrity and WMH. Higher t-tau was associated with decreasing temporal lobe and Alzheimer's disease (AD) meta region of interest (ROI) cortical thickness. There was no association between Ng and any MRI measure. CSF Aβ42 interacted with Ng for declines in temporal lobe and AD meta ROI cortical thickness and cingulum FA. DI …

Beyond the hippocampus: The role of parahippocampal-prefrontal communication in context-modulated behavior.

Multiple paradigms indicate that the physical environment can influence spontaneous and learned behavior. In rodents, context-dependent behavior is putatively supported by the prefrontal cortex and the medial temporal lobe. A preponderance of the literature has targeted the role of the hippocampus. In addition to the hippocampus proper, the medial temporal lobe also comprises parahippocampal areas, including the perirhinal and postrhinal cortices. These parahippocampal areas directly connect with multiple regions in the prefrontal cortex. The function of these connections, however, is not well…

Regional age-related atrophy after screening for preclinical alzheimer disease.

Brain atrophy occurs in aging even in the absence of dementia, but it is unclear to what extent this is due to undetected preclinical Alzheimer disease. Here we examine a cross-sectional cohort (ages 18-88) free from confounding influence of preclinical Alzheimer disease, as determined by amyloid PET scans and three years of clinical evaluation post-imaging. We determine the regional strength of age-related atrophy using linear modeling of brain volumes and cortical thicknesses with age. Age-related atrophy was seen in nearly all regions, with greatest effects in the temporal lobe and subcorti…

CD38 and MGluR1 as possible signaling molecules involved in epileptogenesis: A potential role for NAD+ homeostasis.

In spite of long-term intensive scientific research efforts, there are still many issues concerning the mechanisms of epileptogenesis and epilepsy to be resolved. Temporal lobe, in particular hippocampus, is vulnerable to epileptogenic process. Herein, electrical kindling model of temporal lobe were analyzed using proteomic approach. A dramatic decrease in nicotinamide adenine dinucleotide (NAD+) level was exhibited during the kindling procedure in hippocampus. After stage 3, high CD38 expression was detected by qPCR, nano-liquid chromatography-tandem mass spectrometry (nano-LC-MS/MS) and west…

Deviance detection is the dominant component of auditory contextual processing in the lateral superior temporal gyrus: A human ECoG study.

Auditory contextual processing has been assumed to be based on a hierarchical structure consisting of the primary auditory cortex, superior temporal gyrus (STG), and frontal lobe. Recent invasive studies on mismatch negativity (MMN) have revealed functional segregation for auditory contextual processing such as neural adaptation in the primary auditory cortex and prediction in the frontal lobe. However, the role of the STG remains unclear. We obtained induced activity in the high gamma band as mismatch response (MMR), an electrocorticographic (ECoG) counterpart to scalp MMN, and the components

Therapeutic evaluation of a patient with ruptured intracranial aneurysm without subarachnoid hemorrhage by CT imaging: a case report.

BACKGROUND: The majority of ruptured intracranial aneurysms are combined with subarachnoid hemorrhage, but patients with only intracerebral hematoma without any subarachnoid hemorrhage are extremely rare. CASE PRESENTATION: The patient was hospitalized due to sudden dizziness combined with slurred speech. The patient showed considerable decreased physical activity without any nuchal rigidity. Head CT showed hematoma in the left temporal lobe, and the shape of hematoma was extremely irregular. MRI indicated the absence of any vascular malformations. The patient was diagnosed with middle cerebr…

Neuropsychological and neuroimaging changes in preclinical Alzheimer's disease.

Alzheimer's disease (AD) is a common, devastating form of dementia. With the advent of promising symptomatic treatment, the importance of re … research suggest that volume loss and cerebral blood flow or metabolic changes, particularly in the temporal lobe, may be detected before the onset of dementia. There exist several markers of a preclinical period of AD, in which specific cognitive and biochemical changes precede the clinical manifestations. The preclinical indicators of AD reflect early comprom …

No effect of hippocampal lesions on perirhinal cortex-dependent feature-ambiguous visual discriminations.

Previous studies have shown that perirhinal cortex lesions in monkeys impair visual discriminations with a high degree of "feature ambiguity," a property of visual discriminations that can emerge when features are a part of both rewarded and unrewarded stimuli. The effects of damage to the hippocampus on these perirhinal-dependent feature-ambiguous tasks are, however, unknown. Prominent theories of medial temporal lobe function predict similar effects of perirhinal cortex and hippocampal lesions on cognitive tasks. In contrast, our hypothesis is that perirhinal cortex, and not the hippocampus,…

Gabapentin-lactam, a close analogue of the anticonvulsant gabapentin, exerts convulsant activity in amygdala kindled rats.

The cyclic GABA analogue gabapentin (GBP), which recently has been marketed for treatment of epilepsy, is particularly effective against complex-partial seizures as occurring in temporal lobe epilepsy. In the present study, we compared the effects of GBP and its lactam analogue (GBP-L) in the amygdala kindling model of temporal lobe epilepsy. In fully kindled rats, GBP (50 mg/kg and 100 mg/kg i.p.) dose-dependently increased the threshold for focal seizures and inhibited the progression from focal to generalized seizures. This effect was not associated with any marked adverse effects. In contr…

[Cytomegalovirus fetal infection. Porencephaly with polymicrogyria in a 15-year-old boy].

A boy, born after 41 weeks of gestation, presented with splenomegaly, microcephaly and chorioretinitis accompanied by immaturity signs. His mother was in good health but her previous pregnancy had been aborted owing to rubella. Laboratory data, including serological and virological evidence, confirmed the diagnosis of fetal cytomegalovirus infection. CT scan indicated a large cyst in the left temporal lobe and periventricular calcifications. At about 8 months of age, convulsions were noticed which were not controlled effectively by medication. There was spastic rigidity without significant psy…
