Testing the Reliability of BOLD-fMRI Motor Mapping in Patients with Cerebral Arteriovenous Malformations by Electric Cortical Stimulation and Surgery Outcomes.

OBJECTIVE: To test the reliability of blood oxygenation level-dependent functional magnetic resonance imaging (fMRI) in the primary hand motor cortex (M1) among patients with arteriovenous malformations (AVMs) by electric cortical stimulation (ECS) and surgery outcomes. METHODS: Forty-three patients with AVMs involving/adjacent to M1 underwent blood oxygen level-dependent fMRI (BOLD-fMRI) with repetitive finger-to-thumb opposition movements. The generated image sets were processed on the iPlan 3.0 workstation. A site-by-site comparison between the fMRI and ECS maps was performed with the aid

Ultrasound-accelerated, catheter-directed thrombolysis for inferior vena cava thrombosis after an orthotopic liver transplant.

Inferior vena cava thrombosis is a rare occurrence after an orthotopic liver transplant that is associated with a high rate of retransplant and mortality. There is no consensus regarding the optimal therapeutic strategy. Surgical management, including thrombectomy with revision of the cavocaval anastomosis, has been described. With the use of endovascular therapies, several minimally invasive approaches are available that are effective and avoid the high morbidity associated with reoperative surgery. We describe our successful experience using an approach after a liver transplant in which the

Histologic characterization of mobile and nonmobile carotid plaques detected with ultrasound imaging.

OBJECTIVES: Although mobile plaques in the carotid arteries detected by duplex ultrasound imaging are considered to cause unstable neurologic symptoms such as crescendo transient ischemic attack or progressive stroke, the histology of mobile plaques has not been sufficiently documented. This study examined the histopathologic features of mobile plaques of the carotid artery and compared the histopathology between mobile and nonmobile plaques. METHODS: Of 228 carotid plaques assessed by preoperative carotid ultrasound imaging, 21 (9.3%) were diagnosed as mobile symptomatic plaques. Of these, 1

Ruptured intrameatal AICA aneurysms--a report of two cases and review of the literature.

BACKGROUND: Aneurysms of the distal part of the anterior-inferior cerebellar artery (AICA) are rare. Most are located in the cerebellopontine angle close to the internal auditory meatus. To our knowledge, only 13 patients with the aneurysm located inside the internal auditory meatus have been reported in the literature. METHOD AND FINDINGS: We present two cases of ruptured intrameatal AICA aneurysms that were treated in our center in the past year. The locations and clinical manifestations are discussed and all 13 previously published cases are reviewed. CONCLUSION: Exposure and decompressio

Screw placement and osteoplasty under computed tomographic-fluoroscopic guidance in a case of advanced metastatic destruction of the iliosacral joint.

We present a case of combined surgical screw placement and osteoplasty guided by computed tomography-fluoroscopy (CTF) in a 68-year-old man with unilateral osteolytic destruction and a pathological fracture of the iliosacral joint due to a metastasis from renal cell carcinoma. The patient experienced intractable lower back pain that was refractory to analgesia. After transarterial particle and coil embolization of the tumor-feeding vessels in the angiography unit, the procedure was performed under general anesthesia by an interdisciplinary team of interventional radiologists and trauma surgeon

Axillary Artery Injury Associated with Proximal Humeral Fractures: Review of Long-Term Vascular, Orthopedic, and Neurologic Outcomes.

BACKGROUND: Fracture and dislocation of the proximal humerus is common. It is associated with a number of vascular, neurologic, and orthopedic complications; these include brachial plexopathy and avascular necrosis (AVN) of the humeral head. These complications are rare but can potentially cause severe disability to patients; however, they remain poorly described in the literature. To describe vascular, orthopedic, and neurologic outcomes after axillary artery repair, in patients with proximal humeral fractures or dislocations. METHODS: We conducted a retrospective review of patients admitted

[Multislice computed tomographic angiography in the assessment of central veins for endovascular treatment planning: comparison with phlebography].

The dysfunction of a vascular access for hemodialysis and its loss may depend on drainage difficulties of the superficial or deep venation due to hemodynamically significant stenosis or obstruction of a central vein, which generally involve the innominate-subclavian veins or superior vena cava. These alterations are often neglected due to their central and deep location; when there is hemodynamic compensation, they may remain asymptomatic. For these reasons every suspect clinical sign for central vein stenosis (gross arm syndrome or venous hypertension in an arteriovenous fistula) must not be

The critical role of the external carotid artery in cerebral perfusion of patients with total occlusion of the internal carotid artery.

AIM: The ipsilateral external carotid artery (ECA) can potentially provide an important collateral pathway for cerebral blood flow in the presence of occlusion or severe stenosis of the internal carotid artery (ICA), recovering up to 15% of the middle cerebral arterial flow. The aim of the study is to elucidate the role of ECA in cerebral flow of patients with total ICA occlusion. METHODS: Retrospective study of prospectively collected data of 139 patients with total ICA occlusions. The patients were divided to symptomatic and asymptomatic and were categorized in four subgroups according to t

Exchange-free single-pass gateway balloon-assisted neuroform atlas stenting for symptomatic high-grade intracranial atherosclerotic stenosis: clinical and angiographic outcomes.

BACKGROUND: Symptomatic high-grade intracranial atherosclerotic stenosis (sICAS) is associated with a high risk of early recurrent ischemic events despite optimized medical therapy. Conventional angioplasty-to-stent workflows typically require intracranial exchange maneuvers, which may increase procedural complexity and vessel manipulation in perforator-rich territories of the brain. We evaluated the safety and early angiographic outcomes of a standardized, exchange-free, single-pass Gateway balloon-assisted Neuroform Atlas stenting strategy. METHODS: We retrospectively analyzed consecutive a
