[SYNDACTYLY OF THE FINGERS: DIAGNOSIS, EVALUATION AND TREATMENT].

Syndactyly release is a surgical procedure aimed at restoring hand and finger function while maintaining the natural appearance of the interdigital web space (commissure) as much as possible. The timing of surgery depends on the complexity of the fusion, the patient's age, and their overall medical condition. The traditional technique is based on a zigzag incision and the use of skin grafts combined with local flaps. However, it is associated with a relatively high incidence of complications, such as web creep, hypertrophic scars, flap necrosis, and contractures. This article provides a genera…

A Successful Use of Electromyography-Based Neuromuscular Monitoring in a Patient With Cerebral Palsy: A Case Report.

Neuromuscular monitoring in patients with cerebral palsy (CP) is technically challenging due to limb contractures and muscle atrophy. We report a case of a 28-year-old male with CP who underwent surgery under general anesthesia. Intraoperative recovery from neuromuscular block was successfully monitored using an electromyography-based neuromuscular monitor. Due to finger deformities, the abductor digiti minimi muscle was selected for monitoring. The onset of rocuronium 0.65 mg/kg was delayed and its duration markedly prolonged. Adequate recovery of neuromuscular function was confirmed with a t…

A Lumbar Spinal Osteoid Osteoma Associated with Kyphoscoliosis, Head Drop, and Pelvic Obliquity: A Case Report.

CASE: A 19-year-old man presented with a progressive kyphoscoliosis deformity, pelvic obliquity, neck flexion, and bilateral lower limb contractures. Evaluation showed previously undiagnosed spinal osteoid osteoma. The patient underwent surgical resection with instrumentation of L3-L4. At the 2-year follow-up, the patient showed almost complete resolution of his complex spine deformity and improvement in radiological parameters. CONCLUSION: The current case described the most severe thoracolumbar deformity secondary to an osteoid osteoma of the lumbar spine in the literature. An intralesional…

Palmar Fascia Fibrosis in Dupuytren's Disease: A Narrative Review of Pathogenic Mechanisms and Molecular Insights.

Dupuytren's disease (DD) is a chronic fibroproliferative disorder of the palmar fascia, leading to disabling digital contractures and high recurrence after surgery. This narrative review highlights DD as a multifactorial condition in which genetic predisposition and cytogenetic instability converge with extracellular matrix remodeling, aberrant transforming growth factor β (TGF-β) and Wnt/β-catenin signaling, cytoskeletal stabilization and immune-inflammatory amplification. Epigenetic dysregulation further locks fibroblasts into a persistent myofibroblast state. Discrepancies between studies a…

Methotrexate-associated Peripheral Neuropathy on 18 F-FDG PET/CT and MRI Masquerading as Neurolymphomatosis.

A 53-year-old woman with primary central nervous system lymphoma with vitreal involvement presented with upper extremity weakness, pain, and contractures following methotrexate infusion. Electromyography showed mononeuropathies of the upper extremities, suspicious for neurolymphomatosis versus vasculitis. An 18 F-FDG PET CT indicated neurolymphomatosis, while an MRI of the bilateral forearms demonstrated intramuscular fluid collections, suspicious for abscesses or infarcts. Given the diagnostic dilemma, a right ulnar nerve biopsy was performed and showed findings most consistent with toxic/dru…

Anesthetic Management for a Child With a Newly Identified Mitochondrial Disease SLC25A46 Mutation: A Case Report.

SLC25A46 mutation is a newly recognized mitochondrial mutation causing neurological and muscular abnormalities. We describe a first-ever report of the anesthetic management of a seven-year-old boy with an SLC25A46 mutation during a major orthopedic procedure. The patient was nonverbal and presented with cerebral visual impairment, torticollis, and lower extremity contractures. Because of his new diagnosis of mitochondrial disease and history of delayed awakening after anesthesia, we performed general anesthesia with sevoflurane, a low-dose ketamine infusion, and small doses of fentanyl while a…

Establishment of a Beals syndrome patient-derived human induced pluripotent stem cell line HELPi001-A.

The human induced pluripotent stem cell line HELPi001-A was derived from peripheral blood mononuclear cells (PBMC) of a 35-year-old female Beals syndrome patient carrying a heterozygous FBN2c.728 T > C mutation. HELPi001-A were positive for pluripotent stem cell markers, had a normal karyotype and the ability to differentiate into cells representing all three germ layers. The patient not only demonstrated typical characteristics of Beals syndrome such as joint contractures and crumpled ears, but also demonstrated aortic dissection. HELPi001-A could serve as a platform for exploring the pathoge…

Recent Understandings of Biology, Prophylaxis and Treatment Strategies for Hypertrophic Scars and Keloids.

Hypertrophic scars and keloids are fibroproliferative disorders that may arise after any deep cutaneous injury caused by trauma, burns, surgery, etc. Hypertrophic scars and keloids are cosmetically problematic, and in combination with functional problems such as contractures and subjective symptoms including pruritus, these significantly affect patients' quality of life. There have been many studies on hypertrophic scars and keloids; but the mechanisms underlying scar formation have not yet been well established, and prophylactic and treatment strategies remain unsatisfactory. In this review, …

Needle Aponeurotomy for Dupuytren Disease.

Needle aponeurotomy is an effective, minimally invasive treatment for metacarpophalangeal and interphalangeal joint contractures caused by Dupuytren disease. Multiple joints and digits can be safely treated in 1 session. Needle aponeurotomy is more cost-effective and has a significantly lower complication rate compared with open fasciectomy and collagenase injections. Recurrence rates are higher compared with open fasciectomy and collagenase injections. Patient satisfaction rates are high following needle aponeurotomy; the single clinic visit required and the minimal downtime after treatment a…

Superficial morphea: case report, look-alikes, pathogenesis, and treatment.

Superficial morphea, a rare variant of morphea, is characterized by hypopigmented to hyperpigmented skin lesions located predominantly in a symmetric fashion at intertriginous sites. These patches and plaques typically lack the significant induration, contractures, and atrophy seen in other subtypes of morphea. Histologic examination is key for accurate diagnosis considering the number of similar conditions which may clinically mimic superficial morphea. Herein, we present a case of a 25-year-old woman who re-presented for consultation in our clinic after gradual progression of her skin lesion…

Novel FKBP10 Mutation in a Patient with Osteogenesis Imperfecta Type XI.

Osteogenesis imperfecta (OI) is a set of clinically and genetically heterogeneous disorders with autosomal dominant, recessive and X-linked inheritance patterns. The aim of this study was to describe a novel genetic abnormality in a case of OI type XI with mild joint contractures, kyphoscoliosis, muscular atrophy, progressively deforming and multiple bone fractures in a consanguineous Iranian family. Based on the phenotype, investigation of two candidate genes, CRTAP (OI type VII) and FKBP10 (OI type XI) detected a novel homozygous frameshift mutation in the FKBP10 gene. This finding can be us…

Melorheostosis mimicking synovial osteochondromatosis.

Melorheostosis is an uncommon, sporadic, sclerosing bone lesion that may affect the adjacent soft tissues. It has been associated with many entities such as osteopoikilosis, soft tissue vascular malformations, bone and soft tissue tumors, nephrotic syndrome, segmental limb contractures, osteosarcoma, desmoid tumor, and mesenteric fibromatosis. Synovial osteochondromatosis is a benign neoplasia of the hyaline cartilage presenting as nodules in the subsynovial tissue of a joint or tendon sheath. The intra-articular extension of melorheostosis mimicking synovial osteochondromatosis has not been r…

A novel mutation expands the genetic and clinical spectrum of MYH7-related myopathies.

MYH7 mutations are an established cause of Laing distal myopathy, myosin storage myopathy, and cardiomyopathy, as well as additional myopathy subtypes. We report a novel MYH7 mutation (p.Leu1597Arg) that arose de novo in two unrelated probands. Proband 1 has a myopathy characterized by distal weakness and prominent contractures and histopathology typical of multi-minicore disease. Proband 2 has an axial myopathy and histopathology consistent with congenital fiber type disproportion. These cases highlight the broad spectrum of clinical and histological patterns associated with MYH7 mutations, a…

Farber lipogranulomatosis with predominant joint involvement mimicking juvenile idiopathic arthritis.

The case of a 10-year-old boy with Farber lipogranulomatosis with predominant joint involvement, subacute, laryngeal and tongue granulomas, microcytic anemia, elevated ESR and CRP, is presented. The boy had no signs of CNS and internal organ involvement. The disease manifested at 6 months; at 11 months the boy had widespread granulomatous polyarthritis with contractures, and juvenile idiopathic arthritis (JIA) was suggested. All antirheumatic therapies failed. Immunologic assessment revealed elevated serum interleukin-1β, increased T-helper, NK and CD25-positive cells, and circulating immune c…
