Aminoglycosides-Related Ototoxicity: Mechanisms, Risk Factors, and Prevention in Pediatric Patients.

Aminoglycosides are broad-spectrum antibiotics largely used in children, but they have potential toxic side effects, including ototoxicity. Ototoxicity from aminoglycosides is permanent and is a consequence of its action on the inner ear cells via multiple mechanisms. Both uncontrollable risk factors and controllable risk factors are involved in the pathogenesis of aminoglycoside-related ototoxicity and, because of the irreversibility of ototoxicity, an important undertaking for preventing ototoxicity includes antibiotic stewardship to limit the use of aminoglycosides. Aminoglycosides are fund

Retinoic acid degradation shapes zonal development of vestibular organs and sensitivity to transient linear accelerations.

Each vestibular sensory epithelium in the inner ear is divided morphologically and physiologically into two zones, called the striola and extrastriola in otolith organ maculae, and the central and peripheral zones in semicircular canal cristae. We found that formation of striolar/central zones during embryogenesis requires Cytochrome P450 26b1 (Cyp26b1)-mediated degradation of retinoic acid (RA). In Cyp26b1 conditional knockout mice, formation of striolar/central zones is compromised, such that they resemble extrastriolar/peripheral zones in multiple features. Mutants have deficient vestibular

Galvanic vestibular stimulation: a novel modulatory countermeasure for vestibular-associated movement disorders.

Motion sickness or kinetosis is the result of the abnormal neural output originated by visual, proprioceptive and vestibular mismatch, which reverses once the dysfunctional sensory information becomes coherent. The space adaptation syndrome or space sickness relates to motion sickness; it is considered to be due to yaw, pith, and roll coordinates mismatch. Several behavioural and pharmacological measures have been proposed to control these vestibular-associated movement disorders with no success. Galvanic vestibular stimulation has the potential of up-regulating disturbed sensory-motor mismatc

A new animal model for Ménière's disease.

CONCLUSION: A new murine model for the study of Ménière's disease has been developed by treatment with both lipopolysaccharide (LPS) and aldosterone. Induction of vestibular dysfunction in the hydropic animal model may entail additional stress such as reduced inner ear blood flow, and sudden acute changes in endolymph volume and/or pressure. OBJECTIVE: The purpose of this study was to develop a more suitable animal model, showing closer resemblance to the pathophysiological process in Ménière's disease. MATERIALS AND METHODS: Adult CBA/J mice were treated by intratympanic injection of LPS, i

Long-term exposure to noise impairs cortical sound processing and attention control.

Long-term exposure to noise impairs human health, causing pathological changes in the inner ear as well as other anatomical and physiological deficits. Numerous individuals are daily exposed to excessive noise. However, there is a lack of systematic research on the effects of noise on cortical function. Here we report data showing that long-term exposure to noise has a persistent effect on central auditory processing and leads to concurrent behavioral deficits. We found that speech-sound discrimination was impaired in noise-exposed individuals, as indicated by behavioral responses and the mism

Promotion of noise-induced hearing loss by chemical contaminants.

Recent studies have underscored the ability of a wide range of chemical agents to potentate noise-induced hearing loss. Given the ubiquitous nature of noise exposure particularly in many work settings, the high rate of noise-induced hearing loss, the limited degree to which auditory function can recover following damage to the inner ear, and the disparate chemical structures that appear capable of impairing hearing, this issue appears to have great public health significance. A compendium of chemicals known to potentiate noise induced hearing loss is presented along with a hypothesis that migh

[Effects of Cisplatin on Auditory Function in Children with cancer. Otoacoustic Emission Evaluation].

This study was done to ascertain prospectively whether distortion product-evoked otoacoustic emissions (DP-EOAE) might detect changes in specific frequencies damaged in inner ear function earlier, before they become permanent after cisplatinum exposure for cancer treatment in children. Sixteen children treated with cisplatinum for various types of cancer were repeatedly evaluated after each chemotherapy session; results were compared to 44 controls. We observed a progressive damage in auditory function. In second assessment 50% of DP-EOAE studies were abnormal; in the third study, 66% were abn

In vitro and animal studies of sodium thiosulfate as a potential chemoprotectant against carboplatin-induced ototoxicity.

When carboplatin (cis-diammine-1,1-cyclobutane-dicarboxylato-platinum) delivery to brain tumors is optimized with osmotic blood-brain barrier disruption (BBBD), high frequency hearing loss can result. Treatment with sodium thiosulfate (STS) blocked carboplatin cytotoxicity against the LX-1 human small cell lung carcinoma cell line in vitro. STS decreased carboplatin-induced ototoxicity in a guinea pig model, as determined by electrophysiological measurements and analysis of inner ear outer hair cell numbers. Protection was found when STS was administered up to 8 h subsequent to carboplatin but

Treatment with high doses of loop diuretics in chronic renal failure.

In the late 60s, A. Heidland and his collaborators were amongst the first to characterize the renal action of frusemide in preterminal renal failure by giving quantitative information on dose requirements, fractional Na and K excretion, effects on renal haemodynamics, and by recognizing reversible inner ear dysfunction as the major dose-limiting side-effects. These early studies have been largely reconfirmed. They have been extended by more clearly characterizing (a) altered pharmacokinetics of frusemide, e.g. the importance of altered transepithelial transport in the proximal tubule and intra

Indirect immunofluorescence in the investigation of rapidly progressive sensorineural hearing loss and Menière's disease.

Being aware of the conservation of antigenicity, we performed indirect immunofluorescence on non fixed non decalcified frozen hamster's cochlea. Once killed, the heads of the hamsters were immediately dipped into liquid nitrogen and then stored at -20 degrees C until cutting. The sections were done at the same temperature, with Tissue-Tek embedding the heads to avoid breaking, using a sharp tungsten knife. The immunofluorescence itself was performed with the sera of the patients which were revealed through donkey fluorescein-conjugated antiserum to human immunoglobulins. For counterstaining, w

Sensorineural hearing loss associated to gonadal dysgenesis in sisters: Perrault's syndrome.

Genetic alterations that involve the inner ear and other systems may present well-defined clinical differences. We present two oriental sisters, age 34 and 26 years, with a picture of primary amenorrhea and "infantile" uterus associated with progressive hearing loss begun in infancy in the older sister, and after 20 years of age in the younger sister. The gynecologic evaluation showed uterine hypoplasia and gonadal dysgenesis in both sisters. The audiologic study showed bilateral sensorineural hearing loss with a flat-type curve in both cases, with an average threshold of 500, 1000 and 2000 Hz

Vascular mechanisms in Meniere's disease. Theoretical considerations.

Unimpeded venous drainage of the vestibular organs via the paravestibular canaliculus (PVC) vein is crucial to inner ear fluid mechanisms. With increased venous pressure, insufficient drainage may result in endolymphatic hydrops, unless collateral veins develop. Different mechanisms cause venous insufficiency in different cases of Meniere's. One suggested mechanism is alteration of the intermediate endolymphatic sac (IES) microcirculation with decreased regulation of blood held within subepithelial capillaries of the sac. Increased blood may thus drain into the veins, with increased venous pre

Hereditary deafness in children: diagnosis and a family report.

A family with hereditary deafness is presented. The mother and ten of her children suffer sensorineural hearing loss. The maternal grandparents and four uncles and aunts were all hard of hearing. Clinical examination revealed no accompanying gross anomalies. In one child, Mondini deformity is the cause of deafness. In the rest of the family isolated membranous labyrinth deformity is suggested. The methods of early diagnosis are discussed, among which polytomography proved the most reliable for diagnosing Mondini and Michel deformities. We demonstrated how early fitting of hearing aids is essen

Test Electrode-Based Estimation of Achievable Insertion Depth in Cochlear Implantation.

Processing the optimal length of electrode arrays for cochlear implants (CIs) is vital for achieving maximum effectiveness, and the results tend to differ between the preoperative radiological estimations and the depth reached at the time of surgery. This study evaluates the feasibility of using a flexible insertion test electrode with colored depth markers to determine the practically achievable electrode insertion depth before CI electrode array placement. The study was conducted at a tertiary center and included patients with inner ear anomalies, reimplantation cases, and profound deafness

Diagnosis and management of bilateral and multiple semicircular canal dehiscence: a case report.

Semicircular canal dehiscence (SCCD) is a rare inner ear disease caused by an anatomical defect in the bony covering of the semicircular canal (SCC). This condition most commonly affects the superior semicircular canal, and less frequently involves multiple canals in one or both ears. Although the clinical and physiological features of a single SCCD are well known, there are only a few reported cases of multiple semicircular canal dehiscences. Thus, their clinical and physiological characteristics require further investigation. We present the case of a 59-year-old male patient of Moroccan ethn

A Japanese Case of Lenz-Majewski Syndrome With a Novel PTDSS1 Variant.

BACKGROUND: Lenz-Majewski syndrome (LMS) is a rare genetic disorder characterized by osteosclerosis, intellectual disability, characteristic facies, and distinct craniofacial, dental, cutaneous, and distal-limb anomalies. Mutations in the PTDSS1 gene, which encodes one of the phosphatidylserines (PS) synthase enzymes, PSS1, have been identified as causative in LMS patients. These mutations make PSS1 insensitive to feedback inhibition by PS levels. METHODS: Whole genome sequence (WGS) was performed on a patient with congenital cutis laxa and her parents. PS synthase activity was analyzed in PT

Feingold syndrome with GJB2 variants.

Congenital hearing loss is the most common birth defect, with genetic factors implicated in 50 % of prelingual cases. GJB2 variant, causing up to 50 % of autosomal recessive non-syndromic hearing loss, typically show stable hearing profiles and favorable cochlear implant (CI) outcomes. In this case, however, the atypical clinical course prompted further evaluation. A 3-year-6-month-old girl, born at 35 weeks presented with profound bilateral hearing loss detected by auditory brainstem response. The patient had normal tympanic membranes, angulated ears, microcephaly, short stature, narrow palpe

Audiological Profile of Patients with Vitiligo.

INTRODUCTION: Vitiligo is one of the most common causes of hypopigmentation; it occurs when melanin production is impaired due to the loss of melanocytes, which are present throughout the skin, in the epidermis, and in hair follicles, and are also seen in the mucosa, uvea, and several structures of the inner ear. Scientists have investigated the association between vitiligo and sensorineural hearing loss (SNHL); some believe there is a connection, while others disagree. OBJECTIVE: To evaluate the relationship between vitiligo and SNHL. METHODS: The present observational study was conducted f
