OBJECTIVES: We compared 2 different protocols of early mobilization with a protocol of delayed mobilization in patients with simple radial head fractures (B2.1 type of Orthopaedic Trauma Association Classification). An attempt to correlate certain characteristics of the radial head fractures with outcome was made.
DESIGN: Prospective randomized comparative study.
SETTING: Level 1 trauma center.
PATIENTS/PARTICIPANTS: One hundred eighty consecutive patients were randomly allocated into 3 different protocols.
INTERVENTION: In the first group, immediate mobilization of the elbow joint was app…

INTRODUCTION: Various studies have found that 6.6 to 31% of supracondylar elbow fractures in children have nerve-related complications. One-third of these are cases of anterior interosseous nerve (AIN) palsy that usually result in a deficit of active thumb and index flexion. The goal of this cadaver study was to describe the course of the AIN to achieve a better understanding of how it may get injured.
MATERIALS AND METHODS: On 35 cadaver specimens, the median nerve and its collateral branches destined to muscles were dissected at the elbow and forearm levels. The distance at which the variou…

This study examines 32 patients operated for radial head fractures, mean age 48.7 years (range 26-76 years), Mason type II in 25 cases and Mason type III with 3 main fragments in 7 cases. Fractures were surgically reduced and fixed with Herbert's (12 cases) or Osteomed (20 cases) cannulated screws. The latter have a head that is <1 mm high and are only threaded distally. Both types of screw provide compression across the fracture. In Mason type II cases, the mean MEPS score was 98.4 and the DASH score 6.8. In Mason type III cases, MEPS was 99.3 and DASH 9.5. We believe that fixation can be car

Biceps or triceps ruptures are rare but can cause a significant disability. Surgical repair has become the preferred method of treatment for the complete rupture, but the decision when to treat partial tears is less clear. Reconstruction of the tendon is the preferred method when patients have a delayed presentation.

This study aimed to test the usefulness and validity of two versions commonly quoted in the literature of the Milch classification of paediatric supracondylar mass fractures. Anteroposterior and lateral elbow radiographs of 10 consecutive acute paediatric lateral condylar mass fractures requiring open surgical reconstruction were presented to six observers in order to assess inter- and intraobserver reliability of both versions. Accuracy of classification was compared with intraoperative findings. Observers agreed with the operative findings in only 50% of cases. There was poor interobserver (…

Radial head arthroplasty is indicated for displaced comminuted radial head fractures that cannot be managed reliably with open reduction and internal fixation and that have an associated elbow dislocation. Replacement also is indicated in patients with comminuted radial head fractures that have or are likely to have a disruption of the medial col-lateral, lateral collateral, or interosseous ligaments. Biomechanical studies have demonstrated that metallic implants restore elbow stability similar to the native radial head. The early and midterm clinical experience with metallic radial head arthr…

Congenital radial head dislocation is the most common congenital elbow abnormality. Patients generally remain asymptomatic until adolescence and, at that time, may benefit from radial head resection. Open reduction and ligament reconstruction may offer advantages over late radial head resection if performed before the age of 2 years. Further long-term studies are needed to determine if open reduction and ligament reconstruction are helpful.

Family physicians often are required to evaluate patients who present with acute skeletal trauma. The first of this two-part series discusse … of significant force. Multiple views should be obtained in adults with a suspected fracture of the elbow. Complications in fractures of the wrist are strongly related to the location of the fracture.

Fracture-separation of the distal humeral epiphysis in neonates is difficult to diagnose radiologically because the cartilaginous epiphysis is radiolucent. We report a case in which fracture-separation of the distal humeral epiphysis in a neonate was diagnosed with the help of ultrasonography, which provided a clear delineation of the injury.

The use of diagnostic and therapeutic arthroscopy has increased considerably in the past decade. Initially applied only to the knee joint, arthroscopy now has an established place in the treatment of shoulder, elbow and ankle disorders. Its use in the hand and the wrist is also expanding.

The author has examined 165 children with FHCH. A method of surgical treatment of FHCH has been worked out and applied in 37 children; its essence consists in the open reposition of the fractures and the fixation of the fragment of the head with an original construction compression device which provides the possibility of making early active movements preventing rigidity in the elbow joint. The method does not envisage the necessity of fixation of the extremity with a plaster bandage and reduces the period of clinical and general treatment (by 18 and 14 days respectively). The economic damage …

Rotation of the distal fracture fragment in the supracondylar fracture is a contributing factor in the development of the cubitus varus deformity. Computed tomography (CT) is superior to conventional radiography in the assessment of the position of fracture fragments and was used to study 20 patients with supracondylar fractures after reduction. These fractures were followed up with radiography until healed. As determined from CT scans, rotation of the distal fracture fragment of greater than 10 degrees resulted in an abnormal Baumann angle and a cubitus varus deformity in all patients. Use of

A 19-year-old healthy bodybuilder presented to the emergency department with gradually worsening pain in both his upper arms and shoulders and inability to fully flex his elbows. The haematological investigations revealed a markedly raised Creatinine Kinase (74,400 U/L) and myoglobinuria. The patient required an emergency surgical decompression of the pectoral and the anterior and posterior compartments of arms of both upper limbs with secondary closure after 48 h. The patient had an uneventful post-op and recovery of his functions with some initial restriction of full flexion of his left elbo…

A 16-year-old teenager presented himself with a swollen left elbow, with no associated vascular-nerve complications. The standard radiography was without abnormalities. The echography showed the presence of an oblong vascularized formation occupying the posterior part of the elbow. The magnetic resonance imaging (MRI) showed a hyper vascularized lesion developing at the expense of the brachial triceps muscle with an intermediate signal intensity on the sequences weighted in T1 and a hyper signal in T2. The anatomopathological study of the initial biopsy and of the tumor part concluded with a c…

Coccidioidomycosis is a deep mycosis caused by the fungus Coccidioides immitis. Disseminated disease can affect any part of the body. Coccidioidal synovitis is a rare manifestation of musculoskeletal coccidioidomycosis requiring aggressive treatment. We report a case of a 68 year old man who presented to our centre in Chennai with pain and restriction of movement of the elbow of 4 months duration. After being investigated, he was subjected to a synovectomy following which he was diagnosed to have coccidioidal synovitis of the elbow joint by histology and culture. He was treated with itraconazo…
