An Underrecognized Cause of Recurrent Spontaneous Pneumothorax: Birt-Hogg-Dubé Syndrome With Isolated Pulmonary Manifestation.

Birt-Hogg-Dubé (BHD) syndrome is a rare autosomal dominant disorder characterized by a classic triad of cutaneous fibrofolliculomas, bilater … and GPNMB excluded lymphangioleiomyomatosis. Given the characteristic cyst pattern, germline whole-exome sequencing identified a pathogenic heterozygous FLCN frameshift variant, confirming the diagnosis. Because of the lifetime risk of renal malignancy, early recognition is important and genetic testing is recommended when BHD is suspected.

[Pulmonary disease caused by Mycobacterium abscessus in an infant: A case report and literature review].

Pulmonary infection caused by Mycobacterium abscessus is rare in children without underlying pulmonary disease, especially in infants. A 3-month-old female infant was admitted to the Third Xiangya Hospital of Central South University on July 29, 2023. Cough was her only clinical manifestation, and chest computed tomography revealed multiple patchy and mass-like high-density opacities in both lungs. Metagenomic next-generation sequencing of bronchoalveolar lavage fluid confirmed the diagnosis of Mycobacterium abscessus pulmonary disease. Further evaluation for immunodeficiency and whole-exome s…

Coronoidectomy for Alleviating Restricted Mouth Opening in Masticatory Muscle Tendon-Aponeurosis Hyperplasia: Integration With Genetic Mutation Analysis.

Masticatory muscle tendon-aponeurosis hyperplasia (MMTAH) is often misdiagnosed as temporomandibular joint disorders, hypertrophic masticatory muscles, or congenital maxillomandibular dysplasia due to overlapping clinical manifestations. This case study is aimed at elucidating a novel genetic association in MMTAH by reporting a patient with pathognomonic features, including chronic limited mouth opening, bilateral coronoid process elongation, tendon hyperplasia in the masseter and temporalis muscles, and concomitant fatty degeneration. Crucially, whole-exome sequencing of peripheral blood iden…

Variable Intrafamilial Cardiac Phenotype Segregating With a TBX20 Missense Variant in the Putative Transcriptional Activation Domain.

Non-syndromic atrioventricular canal (AVC) occurs in ∼1/1000 livebirths and most of them are considered sporadic cases or the result of multifactorial inheritance. We report on a fetus diagnosed with non-syndromic AVC, whose mother showed interatrial septal tissue redundancy and slight ascending aortic ectasia. Trio-based exome sequencing (ES), performed during the pregnancy, detected the maternally inherited rare c.946A > G p.(Thr316Ala) missense variant in the TBX20 gene as the reasonable cause of this condition. Segregation analysis extended to the grandmother, previously diagnosed with iso…

Hereditary Sensory and Autonomic Neuropathy Type 2: A Case Report and a Review of the Literature.

We report a case of hereditary sensory and autonomic neuropathy presenting with childhood-onset symmetric distally predominant limb hypoesthesia to tactile, thermal, and painful stimuli. Exome sequencing identified a homozygous pathogenic variant in the with-no-lysine (K) kinase 1 (WNK1), lysine deficient protein kinase 1 gene. The clinical, electrophysiological, and genetic findings confirmed a diagnosis of hereditary sensory and autonomic neuropathy type 2A (HSAN 2A). This case highlights the importance of genetic confirmation in the evaluation of early-onset neuropathies, especially when th…

Biallelic COL4A2 Variants Associated With Brain Small Vessel Disease and Brain Malformations.

Deleterious variants in COL4A2, encoding type IV collagen's alpha-2 chain, cause heterogeneous cerebrovascular and developmental brain malformations. While many dominant variants are known, biallelic changes are rarely reported. We reported two severe cases: Case #1, an aborted fetus with cerebral calcifications, hemorrhages, periventricular leukomalacia, and cerebellar disruption; and Patient #2, a 2-year-old girl with neurodevelopmental impairment, cortical malformations (frontal schizencephaly, polymicrogyria), and reduced white matter volume. Exome sequencing identified a homozygous missen…

High-grade Serous Neoplasm of Fallopian Tube Mimicking a Low-grade Lesion.

Low-grade and high-grade serous neoplasms are generally considered to be separate entities with distinct morphologic features, pathogenesis and clinical behavior. However, rare tumors defy this dualistic classification. Herein, we describe an incidental serous neoplasm confined to the fallopian tube with both low-grade and high-grade features, including ciliated cells and low proliferative rate, along with variable nuclear pleomorphism and an aberrant p53 cytoplasmic expression pattern by immunohistochemistry. Whole exome sequencing revealed a subclonal TP53 splice mutation, supporting this to…

Pediatric intestinal pseudo-obstruction found in 3-year-old male with Rett-related mutation of methyl-CpG binding protein 2.

A 3-year-old male with chronic abdominal distention, constipation, and severe malnutrition is diagnosed with pediatric intestinal pseudo-obstruction (PIPO) after extensive evaluation that excluded mechanical, malabsorptive, metabolic, inflammatory, and infectious causes. Aside from speech delay, he has a normal neurologic exam. Whole exome sequencing reveals a pathogenic methyl-CpG binding protein 2 (MECP2) variant, suggesting atypical Rett syndrome. Management includes promotility agents and a gastrostomy tube with cyclic feedings of peptide-based formula, leading to resolution of symptoms. T…

A Novel LRP6 Mutation Highlights the Role of Wnt Signaling in Tooth Agenesis.

Non-syndromic tooth agenesis (TA) is a rare developmental disorder that impairs oral function, systemic health, psychological well-being, and quality of life. The Wnt signaling pathway plays a central role in TA pathogenesis, and emerging evidence implicates that the low-density lipoprotein receptor-related protein 6 (LRP6) is involved in autosomal-dominant inheritance of TA. In this study, whole-exome sequencing (WES) of a Chinese family uncovered a novel missense mutation (c.692C>T, p.T231M) in LRP6. Sanger sequencing validated this variant, which was identified as a de novo mutation in the …

Dopa-responsive dystonia and paroxysmal dystonic attacks associated with ATP1A3 gene variant.

An 18-year-old man had episodes of severe generalised dystonia, from aged 7 months and becoming progressively more frequent. He also had gradually developed interictal limb dystonia. He was initially diagnosed with paroxysmal kinesigenic dyskinesia but he did not improve with several medications. A levodopa trial led to levodopa-induced dyskinetic movements. However, a lower titration of 25 mg of levodopa two times per day substantially improved his motor features and quality of life. Laboratory investigations and MR scans of the brain were unremarkable. Whole-exome sequencing identified a pat…

A novel homozygous splicing mutation in AK7 causes multiple morphological abnormalities of sperm flagella in patients from consanguineous Pakistani families.

Multiple morphological abnormalities of the flagella (MMAF) represent a severe form of sperm defects leading to asthenozoospermia and male infertility. In this study, we identified a novel homozygous splicing mutation (c.871-4 ACA>A) in the adenylate kinase 7 (AK7) gene by whole-exome sequencing in infertile individuals. Spermatozoa from affected individuals exhibited typical MMAF characteristics, including coiled, bent, short, absent, and irregular flagella. Transmission electron microscopy analysis showed disorganized axonemal structure and abnormal mitochondrial sheets in sperm flagella. Im…

Structural and functional insights into a novel homozygous missense pathogenic variant in CUL7 identified in consanguineous Pakistani family.

3M syndrome is a rare genetic familial disorder characterized by short stature, growth retardation, facial dysmorphism, skeletal abnormalities, fleshy protruding heels, and normal intelligence, caused by mutations in the CUL7, OBSL1 and CCDC8 genes. In the present study, a novel homozygous missense variant of CUL7 (NP_001161842.1, c.4493T > C, p.L1498P) has been identified in a consanguineous Pakistani family by whole exome sequencing. In silico structural evaluation, molecular docking and simulation studies of mutant CUL7 provides substantial evidence about its crucial role in the progression…

Expanding the genotype-phenotype spectrum in SCN8A-related disorders.

BACKGROUND: SCN8A-related disorders are a group of variable conditions caused by pathogenic variations in SCN8A. Online Mendelian Inheritance in Man (OMIM) terms them as developmental and epileptic encephalopathy 13, benign familial infantile seizures 5 or cognitive impairment with or without cerebellar ataxia. METHODS: In this study, we describe clinical and genetic results on eight individuals from six families with SCN8A pathogenic variants identified via exome sequencing. RESULTS: Clinical findings ranged from normal development with well-controlled epilepsy to significant developmental …

Clinical Presentation and Genetic Analysis of Neonatal 11β-Hydroxylase Deficiency Induced by a Chimeric CYP11B2/CYP11B1 Gene.

In terms of prevalence, 11β-hydroxylase deficiency (11β-OHD), a common form of congenital adrenal hyperplasia, closely follows 21-hydroxylase deficiency. 11β-OHD has been attributed to diminished enzymatic activity owing to CYP11B1 gene variants, mainly encompassing single nucleotide variations and insertions-deletions. The involvement of chimeric CYP11B2/CYP11B1 genes in 11β-OHD has rarely been reported. We conducted a genetic investigation on a male infant with generalized pigmentation and abnormal steroid hormone levels. Whole-exome sequencing revealed a heterozygous variant in CYP11B1 inhe…

A Novel ANO1 Gene Variant is Associated with Intestinal Dysmotility Syndrome Masquerading as Hirschsprung Disease: A Case Report.

Anoctamin 1 (ANO1)-related intestinal dysmotility syndrome (OMIM: 620045) is an extremely rare disorder with only 2 cases reported in the medical literature. We present the clinical scenario of a 2-month-old male infant that presented to our center with diarrhea, vomiting, and abdominal distension. Routine investigations did not yield a clear diagnosis. Whole-exome sequencing showed a novel homozygous nonsense ANO1 pathogenic variant (c.1273G>T) with a protein alternation of p.Glu425Ter that fits the patient's phenotype. Sanger sequencing revealed the same ANO1 variant in both parents in a het…

Human genetics and molecular genomics of Chiari malformation type 1.

Chiari malformation type 1 (CM1) is the most common structural brain disorder involving the craniocervical junction, characterized by caudal displacement of the cerebellar tonsils below the foramen magnum into the spinal canal. Despite the heterogeneity of CM1, its poorly understood patho-etiology has led to a 'one-size-fits-all' surgical approach, with predictably high rates of morbidity and treatment failure. In this review we present multiplex CM1 families, associated Mendelian syndromes, and candidate genes from recent whole exome sequencing (WES) and other genetic studies that suggest a s…

Coffin-Siris Syndrome: Case Series of Three Patients and a Novel ARID2 Variant.

Coffin-Siris syndrome (CSS) is a rare congenital disorder characterized by coarse facial features, intellectual disability or developmental delay, and aplasia or hypoplasia of the tips of the fifth finger and/or toes. Mutations in genes affecting the switch/sucrose non-fermenting ATP-dependent chromatin remodeling complex are reported to cause CSS. Here, we describe three CSS patients. Two girls aged 3 and 2 years old presented with global developmental delay, poor growth, and a dysmorphic face. Whole-exome sequencing (WES) was performed and they were diagnosed with CSS due to heterozygous fra…
