Fetoscopic placement of a transurethral stent for intrauterine obstructive uropathy.

PURPOSE: Fetal urinary obstructive uropathy and consecutive oligohydramnios result in a poor outcome. Usually renal insufficiency and life threatening lung hypoplasia have developed at term. We report a case of in utero fetal cystoscopy and successful placement of a transurethral vesico-amniotic Double-J (Medical Engineering Corp., New York, New York) stent. Indications, results and the potential benefits of different techniques are discussed. MATERIAL AND METHODS: A 36-year-old woman (primipara) was evaluated at week 26 due to a male fetus with bilateral hydronephrosis, massive distended bla

The fetal and labor outcome of 102 positive contraction stress tests.

A total of 5447 contraction stress tests (CST) were performed on 3246 high-risk patients during pregnancy. Positive tests occurred in 102 patients (3.1%), 59 of whom had primary cesarean sections. Labor was induced in 40 patients (39.2%), and 17 developed cardiotocographic evidence of fetal distress. In 2 patients labor was terminated due to prolapse of the umblilical cord, and the remaining 21 patients did not develop fetal distress. Fetal distress was more common in patients with proved intrauterine growth retardation (IUGR) and where the acceleration patterns were not noted during the CST.

Additional value of advanced neurosonography and magnetic resonance imaging in fetuses at risk for brain damage.

OBJECTIVE: To assess the additional value of fetal multiplanar (axial, coronal and sagittal) neurosonography and magnetic resonance imaging (MRI) to that of the standard axial ultrasound planes in diagnosing brain damage in fetuses at high risk. METHODS: This was a prospective, multicenter, observational study. Women were eligible for participation if their fetus was at risk for acquired brain anomalies. Risk factors were congenital infection, alloimmune thrombocytopenia, fetal growth restriction, trauma during pregnancy, fetal hydrops, monochorionic twins and prior ultrasound finding suggest

Fetal MRI Neuroradiology: Indications.

Fetal MRI is a safe, noninvasive examination of the fetus and placenta, a complement to ultrasonography. MRI provides detailed CNS evaluation, including depicting parenchymal architecture and posterior fossa morphology, and is key in prenatal assessment of spinal dysraphism, neck masses, and ventriculomegaly. Fetal MRI is typically performed after 22 weeks gestation, and ultrafast T1 and T2-weighted MRI sequences are the core of the exam, with advanced sequences such as diffusion weighted imaging used for specific questions. The fetal brain grows and develops rapidly, and familiarity with gest

Successful in utero transesophageal pacing for severe drug-resistant tachyarrhythmia.

Sustained fetal tachyarrhythmia can evolve into a life-threatening condition in 40% of cases when hydrops develops, with a 27% risk of perinatal death. Several antiarrhythmic drugs can be given solely or in combination to the mother to achieve therapeutic transplacental concentrations. Therapeutic failure could lead to progressive cardiac insufficiency and restrict therapeutic options to either elective delivery or direct fetal administration of antiarrhythmic drugs, which may increase the risk of death. We report for the first time successful fetal transesophageal pacing to treat a hydropic f

New terminology for adverse events of fetal therapy: Re-evaluation of the thoraco-amniotic shunting in a Japanese study.

AIM: Since fetal therapy has been newly developed, objective evaluation of adverse events (AE), all harmful events that are not always related to the procedures, has not yet been adequately reported. We established new terminology and tried to re-evaluate it based on the Japanese prospective thoraco-amniotic shunting (TAS) study. METHODS: From the literature, all complications that occurred with all fetal therapies were identified as a basis for developing the terminology. Grading was set from 0 to 5. Grade 3 was defined as the need for invasive treatment, such as surgery. Grade 4 was defined

EXIT procedure for fetal mediastinal teratoma with large pericardial effusion: a case report with review of literature.

BACKGROUND: Large mediastinal teratomas in the fetus are rare and can present with direct compression of intrathoracic structures as well as pericardial and pleural effusions. Mediastinal fetal teratoma may be detected as a mass in the chest during a routine prenatal ultrasound. Because of the pressure on mediastinal structures it may result in non-immune fetal hydrops (NIFH) and polyhydramnios. The development of hydrops may lead to fetal demise. Timely obstetric and/or surgical intervention is important to improve survival in this patient population. Case review: We report a case of a large

Fetal brain injury following prolonged hypoxemia and placental insufficiency: a review.

It is well-established that severe, acute episodes of hypoxemia can damage the brain before birth, but the effects of more sustained hypoxemia are less well understood. We have used fetal fetal sheep in a series of studies aimed at determining the effects of prolonged hypoxemia, induced by placental insufficiency of differing severity and duration, on fetal brain structure. Restriction of placental, and hence fetal, growth by carunclectomy caused impaired development of neural processes and connections in the hippocampus, cerebellum, and visual cortex; neuronal migration and neuronal numbers d

Structural and haemovascular aspects of placental growth throughout gestation in young and aged mares.

This investigation was carried out to study the equine placenta from early gestation to near term, with special reference to morphological changes associated with the development of the vasculature of the fetal component of the microcotyledons. Pregnant uteri were removed post mortem from five Thoroughbred mares between 110 and 309 days of gestation, two of which were aged, multiparous animals suffering age-related degenerative changes in their endometrium (endometrosis), while the other three were young, and had primigravid normal healthy uteri. Pieces of endometrium with placenta attached we

Role of Ultrasonography in the Evaluation of Normal Developmental Pattern of Fetal Cerebral Sulci Between 18 and 32 Weeks of Gestational Age.

Background Brain development is a crucial process of intrauterine life and can be readily visualized on ultrasonography. This study aims to visualize developmental patterns of various fetal cerebral sulci using ultrasonography between 18 and 32 weeks of gestation. Sulci are best visualized on images that are taken perpendicular to their expected course of development. Initially, they appear as small dot/dimple on the brain surface and later develop into a V-shaped indentation and finally deepen to form notch and echogenic line into the brain forming a Y-shaped configuration. Material and metho

Identifying placental dysfunction in women with reduced fetal movements can be used to predict patients at increased risk of pregnancy complications.

Maternal perception of fetal movements has historically been used to indicate fetal wellbeing, and has been used with varying success in recent years to identify those pregnancies at increased risk of stillbirth, and other placental pathologies. We present a hypothesis that links reduced fetal movements (RFM) to fetal growth restriction (FGR) and stillbirth through placental dysfunction, and suggests the possibility that this can allow development of a reliable method to identify those women experiencing RFM who are at increased risk of adverse outcome. Reduced fetal movement is thought to rep

Recurrence rates of arrhythmias during pregnancy in women with previous tachyarrhythmia and impact on fetal and neonatal outcomes.

In women with heart disease, sustained arrhythmias can result in an increased risk to the mother and fetus. The purpose of this study was to determine the recurrence rates of arrhythmias during pregnancy in women with cardiac rhythm disorders and examine the impact on fetal and neonatal outcomes. Women with tachyarrhythmias before pregnancy who underwent obstetric care at the Toronto General and Mount Sinai Hospitals from 1990 to 2002 were included. The recurrence rates of arrhythmias were calculated. A multivariate logistic model was used to identify predictors of fetal complications. Seventy

Gefitinib for EGFR mutated lung adenocarcinoma during pregnancy: A case report with 5-Year Follow-Up.

We report the case of a 37-year-old pregnant woman at 19 weeks of gestation who was diagnosed with a stage M1b EFGRmut lung adenocarcinoma, in 2019. After a right upper and middle bilobectomy (T3N1 and R0 pathological status) and a complete cerebellar metastasis resection, she was treated with gefitinib, which offered her a prolonged recurrence-free survival without any negative consequence for the fetal development or the future development of her now 5- year- old child.

Rudimentary horn pregnancy associated with pre-eclampsia.

The unusual association of pre-eclampsia with a pregnancy in the rudimentary horn of the uterus in a multigravid patient is reported. The infrequent development of rudimentary horn pregnancy beyond the twentieth week of gestation may explain the rarity of this complication. The possible effect of rapid distension of the rudimentary horn with exposure of the myometrial surface to overwhelming fetal antigenic proteins may be of relevance in the development of pre-eclampsia in this case.

Induction of Fetal Hydrocephalus Followed by Ventriculoamniotic Shunting and End of Study Analysis in the Pregnant Sheep Model.

INTRODUCTION: As part of an evidence-based reassessment of ventriculoamniotic shunting for fetal aqueductal stenosis, we developed an animal model for the induction of hydrocephalus, placement of a ventriculoamniotic shunt, and end of study analysis in fetal sheep. METHODS: We made multiple revisions to both the animal model and the shunt over time. Regarding the animal model, we found that induction of hydrocephalus at 85 days gestation, shunt placement at 100 days gestation, and study end at 135 days gestation generated the most reproducible results. CONCLUSION: The purpose of this manuscr

Failure to detect fetal obstructive uropathy by second trimester ultrasound.

The prenatal diagnosis, management and outcome of an hereditary obstructive uropathy is presented. Serial ultrasonic assessment of the fetal urinary tract was carried out from the early second trimester onward. Slight bilateral hydronephrosis as a first sign of obstructive uropathy was only established as late as 30 weeks of gestation. On the basis of weekly ultrasound scans, a conservative approach was adopted. In view of fetal maturity, labour was induced at 36 weeks resulting in the vaginal delivery of a male infant with moderate bilateral hydronephrosis. Neonatally, anuria developed due to

Antenatal diagnosis of thalassemia: a review.

The overall results of the WHO International Registry for the Prenatal Monitoring of Hereditary Anemias are summarized in Tables 11 and 12. Comparison of data regarding adequacy of samples, and freedom from errors and from fetal losses show that the chance of a couple obtaining a useful result is greater than 90%, whether fetal flood is sampled because the fetus is at risk for a hemoglobinopathy or another disease, or fetal DNA is obtained for globin gene analysis. Each approach serves as a model for the others. Fetal blood sampling technology has been improved because of the need for entirely

Precision Unveiled in Unborn: A Cutting-Edge Hybrid Machine Learning Approach for Fetal Health State Classification.

PURPOSE: Understanding and categorizing fetal health is an influential field of research that profoundly impacts the well-being of both mother and child. The primary desire to precisely examine and cure fetal disorders during pregnancy to enhance fetal and maternal outcomes is the driving force behind the classification of fetal health. Fetal cardiac abnormalities (structural or functional) need immediate doctor attention, and their early identification and detection in all stages of pregnancy can help doctors with the timely treatment of the mother and the unborn child by enabling appropriate

Eicosanoid Biosynthesis in Male Reproductive Development: Effects of Perinatal Exposure to NSAIDs and Analgesic Drugs.

Increasing rates of infertility associated with declining sperm counts and quality, as well as increasing rates of testicular cancer are contemporary issues in the United States and abroad. These conditions are part of the Testicular Dysgenesis Syndrome, which includes a variety of male reproductive disorders hypothesized to share a common origin based on disrupted testicular development during fetal and neonatal stages of life. Male reproductive development is a highly regulated and complex process that relies on an intricate coordination between germ, Leydig, and Sertoli cells as well as oth
