A case of acute traumatic subluxation of the extensor carpi ulnaris tendon as it passes through the sixth compartment of the wrist is described. The literature suggests that acute injuries can be treated for 6 weeks in a long arm cast, with the forearm in pronation and slight radial deviation and dorsiflexion, but there are no case reports described. The findings in this case suggest that for optimal results, surgical repair of the retaining fibro-osseous sheath of the extensor carpi ulnaris is preferable over the nonoperative cast technique.

Information on exposure and reaction to sunlight, together with the history of exposure to fluorescent lighting, was elicited by postal questionnaire. Case-control analysis from 58 patients with malignant melanoma, 182 matched controls and 151 unmatched controls showed associations between malignant melanoma and (i) bouts of painful sunburn, (ii) reaction of untanned skin to sunlight, and (iii) number of moles (on right forearm), whereas no association could be found with exposure to fluorescent light at work.

A case of partial sacral agenesis resulting in mixed upper and lower motor neurogenic bladder dysfunction is reported. Although the patient also has a concomitant congenital absence of one forearm she is being managed successfully on intermittent clean self-catheterization, illustrating the value of this modality in the management of neurogenic bladder dysfunction and that it can quite easily be managed by a young woman with only one functional hand. A review of the literature on this uncommon condition is also presented.

A case history is presented of a 50-year-old epileptic receiving massive dose of phenobarbital and small doses of dilantin. Following a minor injury to her forearm a "brown tumor" of hyperparathyroidism and osteomalacia, both of which responded to vitamin D.A review of the literature is given; this is the second case reported which documents hyperparathyroidism under such circumstances, and the first to show radiographs of such changes. Radiologists should be aware of the possibility of these changes in patients on long-term anticonvulsants.

During a 17-year period, 33 injuries produced radiographic abnormalities in 26 members of a professional football team. Sites of injury were correlated with playing position and protective equipment worn at the time of injury. Fractures of multiple transverse processes of the lumbar spine are reported in offensive halfbacks and offensive ends. Two linebackers and one center sustained fractures of the mid-forearm. Several related reports of the morbidity and mortality of football injuries are also discussed.

The Galeazzi fracture dislocation, an uncommon forearm fracture in adults, becomes even rarer when occurring bilaterally, with limited studies documenting this condition. Here, we report the case of a 57-year-old female who presented to our emergency room with bilateral Galeazzi fracture dislocations. The patient underwent bilateral open reduction and internal fixation of the radial fractures, along with stabilization of the distal radioulnar joints. Follow-up monitoring over 16 months postoperatively showed positive outcomes without complications.

Lateral elbow tendinopathy (LET) is an overuse injury of the common extensor tendon that occurs in active people. Modifiable risk factors include repetitive movements of the elbow, forearm, and hand, and tasks like handling heavy tools, which may overload the tendon. The clinical course of LET varies considerably. Some people experience a single, brief episode of symptoms, whereas others have persistent or recurring episodes of LET. Eccrine porocarcinoma (EPC) is a rare malignant sweat gland tumor that accounts for approximately 0.005% of all cutaneous carcinomas. It favors the lower extremities. Only 3% of EPCs are on the hand, and only 6 cases occurring specifically on fingers have been previously documented. However, we met a patient with EPC presenting the primary lesion on the left thumb and an extensive cutaneous metastasis on the left forearm. Pathologic findings of axillary lymph nodes confirmed lymphatic metastasis.

BACKGROUND/AIM: Hirayama disease, a rare neurological disorder, is characterized by progressive, asymmetric muscle atrophy of the forearm and hand in young male patients.
CASE REPORT: We noted two cases of Hirayama disease that appeared in the right upper limb and identified the cervical neutral and flexion position magnetic resonance imaging (MRI) characteristics.
CONCLUSION: In the flexion posture, the cervical MRI allows early diagnosis of Hirayama disease and should be considered in patients with the typical clinical symptoms.

Volar dislocation of the second and third carpometacarpal joint is extremely rare and most often caused by high-energy trauma. In this case a 15-year-old boy fell on his hand while playing soccer. The injury was undiagnosed for three months, perhaps due to an ipsilateral forearm fracture. He was treated with open reduction and Kirchner wire fixation for nine weeks. At two years follow-up he had a painfree hand with a 20% reduction in grip strength.

A 51-year-old female developed urticarial lesions of her right forearm which progressed into transient edema and subcutaneous swelling. Later a small infiltrated subcutaneous nodule also appeared and was removed in toto. Histopathological examination revealed the presence of Dirofilaria repens. This worm is the cause of an endemic zoonosis in the Mediterranean area. In the past decade many cases have been reported worldwide, but the condition appears rare in Hungary and skin findings have not been described.

Recurrent massive epistaxes due to heriditary haemorrhagic telangectasia were managed in two patients by ablation of the mucosa of the nasal and maxillary cavities and resurfacing with radial forearm fasciocutaneous free flaps. Both patients had undergone multiple procedures attempting to control the epistaxes prior to the definitive procedures described. Both patients remained epistaxis free postoperatively and retained their nasal airways. Olfactory function was preserved in the second patient due to preservation of the superior turbinate and cribriform olfactory mucosa.

Sporotrichoid cutaneous leishmaniasis (CL) was diagnosed in a Filipino man who had worked in Saudi Arabia for 2 years. Two primary lesions-one on the forearm and one on the abdomen-were characterized by "satellite" papules and subcutaneous nodules extending proximally in a sporotrichoid pattern. Leishmania organisms were found in both primary lesions and a subcutaneous nodule. There was no evidence of systemic involvement. Recognition that Old World CL may disseminate through lymphatic channels has diagnostic and therapeutic importance.

A 24-year-old female gymnast had a 3-month history of bilateral forearm pain. A Tc-99m MDP bone scan demonstrated focally increased activity in the radial shafts on blood pool and delayed images, characteristic of fatigue fractures. Fatigue fractures commonly occur in the lower extremities. Upper extremity fatigue fractures, in contrast, are uncommon and usually involve the humerus or ulna. Fatigue fracture of the radial shaft from gymnastic exercise has not been previously reported.

Two subjects currently undergoing covert sensitization treatment were asked to imagine alternately the covert scene and a pleasant imagery scene while EMGs from the corrugator and forearm extensor muscle sites were continuously monitored. Visual inspection of the data suggests that corrugator EMG can reliably discriminate the covert-sensitization scene from the pleasant imagery and from resting baseline conditions. Facial EMG may have potential for objectively studying imagery and its role in covert sensitization and other imagery-based treatments.
