The International Network for Evaluating Outcomes of very low birth weight, very preterm neonates (iNeo): a protocol for collaborative comparisons of international health services for quality improvement in neonatal care.

BACKGROUND: The International Network for Evaluating Outcomes in Neonates (iNeo) is a collaboration of population-based national neonatal networks including Australia and New Zealand, Canada, Israel, Japan, Spain, Sweden, Switzerland, and the UK. The aim of iNeo is to provide a platform for comparative evaluation of outcomes of very preterm and very low birth weight neonates at the national, site, and individual level to generate evidence for improvement of outcomes in these infants. METHODS/DESIGN: Individual-level data from each iNeo network will be used for comparative analysis of neonatal

Hypertensive Disorders of Pregnancy and Breastfeeding Among US Women.

IMPORTANCE: Hypertension contributes to US maternal-infant morbidity and mortality, with potential attenuation from breastfeeding. Little is known regarding breastfeeding outcomes among mother-infant dyads exposed to hypertensive disorders of pregnancy (HDP). OBJECTIVE: To quantify the extent to which HDP is associated with never breastfeeding and the time to breastfeeding cessation among postpartum women in the US. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used nationally representative data from the Centers for Disease Control and Prevention Pregnancy Risk Assessment Mo

Impact of postpartum maternal fever or hypothermia on newborn and early infant illness and death in Southwestern Uganda.

BACKGROUND: Deaths occurring during the neonatal period contribute close to half of under-five mortality rate (U5MR); over 80% of these deaths occur in low- and middle-income countries (LMICs). Poor maternal antepartum and perinatal health predisposes newborns to low birth weight (LBW), birth asphyxia, and infections which increase the newborn's risk of death. METHODS: The objective of the study was to assess the association between abnormal postpartum maternal temperature and early infant outcomes, specifically illness requiring hospitalisation or leading to death between birth and six weeks

Factors associated with exclusive breastfeeding by maternal HIV status: a population-based survey in Kenya.

BACKGROUND: Exclusive breastfeeding (EBF) in the first six months remains low globally, despite known benefits of lower morbidity and mortality among breastfed infants. It is important to understand factors associated with breastfeeding to support optimal breastfeeding practices, particularly in settings with a high burden of HIV. METHODS: We analyzed data from a population-level survey of mother-infant pairs attending 6-week or 9-month immunizations at 141 clinics across Kenya. Primary outcomes included maternal report of (1) EBF at 6-week visit, defined as currently feeding the infant breas

Case Report: Novel rare mutation c.6353C > G in the ABCA12 gene causing harlequin ichthyosis identified by whole exome sequencing.

BACKGROUND: Harlequin ichthyosis (HI) is a severe rare genetic disease that mainly affects the skin. Neonates with this disease are born with thick skin and large diamond-shaped plates covering most of their bodies. Affected neonates lose the ability to control dehydration and regulate temperature and are more susceptible to infections. They also face respiratory failure and feeding problems. These clinical symptoms are factors associated with high mortality rates of neonates with HI. Until now, there are still no effective treatments for HI patients and most patients die in the newborn period

Prenatal Exposure to Snus Alters Heart Rate Variability in the Infant.

INTRODUCTION: Maternal use of smoked tobacco during pregnancy causes significant morbidity and mortality in the human infant including alterations in autonomic control with increased risk of sudden infant death syndrome. We hypothesized that maternal snus (smokeless tobacco) use during pregnancy affects autonomic cardiac regulation in the infant, as measured by heart rate variability (HRV) and the low frequency and high frequency ratio (LF/HF ratio). METHODS: A prospective observational study of 56 infants of women who used snus (n = 23) or cigarettes (n = 13) during pregnancy versus tobacco-

Vertical Transmission of Type B Coxsackieviruses: A Systematic Review Emphasising Clinical Evidence and Seriousness of a Neglected Issue.

Group B Coxsackieviruses (CV-B), like other enteroviruses, are essentially transmitted from person to person through the faecal-oral route. However, vertical transmission from mother to infant, through transplacental passage or inhalation of contaminated cervical swabs during delivery, can also occur. This underestimated route of CV-B transmission has been associated with serious consequences, mainly abortion, stillbirth, congenital malformations, as well as serious morbidity and mortality in the newborns. Based on a systematic review of clinical observations, this paper explains the mechanism

Low-Dose Aspirin Prophylaxis to Prevent Preeclampsia.

Hypertensive disorders in pregnancy, including pre-eclampsia, are among the leading causes of maternal and infant morbidity and mortality in the United States and globally. Although evidence supporting the benefits of low-dose aspirin for the prevention of preeclampsia has been building for nearly four decades, many women with high-risk pregnancies still do not receive this potentially life-saving medication. In this column, I review the evidence for professional organization recommendations, the gaps between these recommendations and the use of low-dose aspirin, and the implications for resea

Environmental Contaminants and Congenital Heart Defects: Focus on PFAS, PAHs, and Other Emerging Exposures.

BACKGROUND: Congenital heart defects (CHDs) are the most prevalent birth defects, contributing significantly to infant morbidity and mortality. While genetic factors account for a subset of CHDs, environmental exposures during critical periods of cardiac development are increasingly recognized as potential contributors. CONTENT: This review synthesizes current evidence linking per- and polyfluoroalkyl substances, polycyclic aromatic hydrocarbons, and other environmental contaminants to CHDs. We discuss epidemiological findings, biological mechanisms, exposure assessment methodologies, and fut

Prenatal Diagnosis, In Utero Management, and Perinatal Outcomes of Congenital Diaphragmatic Hernia.

Congenital diaphragmatic hernia (CDH) is a complex malformation complicating 0.02% to 0.03% of live births. The etiology of CDH is often multifactorial. Sonographic screening can aid in the detection of ~70% of CDH cases. Sonographic measures, that is, lung-to-head ratio, can also be utilized to predict the severity of fetal/neonatal disease and guide fetal eligibility for prenatal interventions, such as the FETO procedure. The FETO procedure has provided improved overall morbidity and mortality among fetuses with severe left-sided CDH, although limited data on its role in the treatment of rig

Neuro-meningeal tuberculosis: A pediatric case report and review of the literature.

Pediatric neuro-meningeal tuberculosis leads to high rates of mortality and morbidity. Prompt diagnosis and initiation of treatment are challenging; imaging findings play a key role in establishing the presumptive diagnosis. General brain imaging findings are well reported; however, specific data on cerebral vascular and spinal involvement in children are sparse. We report the case of a 10-month-old infant who presented to the emergency department with an acute febrile meningeal syndrome, complicating a prior delay in motor development. A brain MRI, including sections through the thorax, revea

ACAD9 treatment with bezafibrate and nicotinamide riboside temporarily stabilizes cardiomyopathy and lactic acidosis.

Pathogenic ACAD9 variants cause complex I deficiency. Patients presenting in infancy unresponsive to riboflavin have high mortality. A six-month-old infant presented with riboflavin unresponsive lactic acidosis and life-threatening cardiomyopathy. Treatment with high dose bezafibrate and nicotinamide riboside resulted in marked clinical improvement including reduced lactate and NT-pro-brain type natriuretic peptide levels, with stabilized echocardiographic measures. After a long stable period, the child succumbed from cardiac failure with infection at 10.5 months. Therapy was well tolerated. P

Life-threatening periocular pseudomonal necrotizing fasciitis in an immunocompetent infant.

PURPOSE: Necrotizing fasciitis, a severe soft tissue bacterial infection, is uncommon in the periocular region due to its rich blood supply. This report highlights a rare case in an immunocompetent infant. OBSERVATIONS: A 10-month-old immunocompetent female exhibited fever, irritability, and right eyelid swelling post-fall. Despite initial treatment with cephalexin for presumed cellulitis, her condition rapidly deteriorated, suggesting necrotizing fasciitis. She stabilized after receiving broad-spectrum intravenous antibiotics and aggressive surgical debridement but later required orbital exe

A rare case of Fournier's gangrene developing from colon perforation in an infant: A case report.

Fournier's gangrene, a rare and life-threatening soft tissue infection affecting the genitalia and perineum, results from various microorganisms. This rapidly progressing necrotizing fasciitis yields higher mortality and morbidity rates. We report a case of a 1-month-old male infant with Fournier's gangrene due to perforation transverse colon complicated with septic shock and pneumonia also accompanied by patent processus vaginalis. Radiological findings of pneumonia and pneumoperitoneum were exhibited. Early diagnosis and management are demanded to mitigate life-threatening and improve the pr

Gestational weight gain in uncomplicated twin pregnancies by pre-pregnancy body mass index.

In 2009, the Institute of Medicine provided provisional guidelines for gestational weight gain (GWG) in twin gestations, citing limited research. We examined GWG by pre-pregnancy body mass index in a retrospective cohort of twin births uncomplicated by infant mortality, preterm birth, or low birth weight from Massachusetts in 2006-2017 (N = 273). Median (inter-quartile range [IQR]) GWG was 20.4 kg (IQR: 17.2-25.9 kg) for women who were normal weight pre-pregnancy, 21.3 kg (IQR: 17.2-25.9 kg) for women with overweight pre-pregnancy, and 13.6 kg (IQR: 8.6-20.9 kg) for women with obesity pre-preg…

The role of RAS mutations in MLL-rearranged leukaemia: A path to intervention?

Childhood acute lymphoblastic leukaemia (ALL) with MLL rearrangement (MLL-r) is an aggressive disease still associated with a high mortality rate. Recent investigations have identified co-operating mutations in the RAS pathway and although the functional consequences of these mutations are not yet fully understood, aberrant regulation of RAS pathway signalling at both transcriptional and protein levels is observed. Studies investigating the efficacy of specific inhibitors of this pathway, e.g. MEK-inhibitors, have also achieved encouraging results. In this context, this mini-review summarizes

Intrauterine growth restriction - part 1.

Intrauterine growth restriction (IUGR) is a major and silent cause of various morbidity and mortality for the fetal and neonatal population. It is defined as a rate of fetal growth that is less than normal for the growth potential of that specific infant. The terms IUGR and small for gestational age (SGA) are often used interchangeably, although there exists subtle differences between the two. IUGR/SGA is an end result of various etiologies that includes maternal, placental and fetal factors and recently added genetic factors too, also contribute to IUGR. In this review article we will cover t

The First Report of a Pregnancy in a Patient with Purine Nucleoside Phosphorylase Deficiency.

BACKGROUND: The cause of primary immunodeficiency has expanded to nearly 200 distinct disorders. An improved understanding of these disorders has resulted in decreased morbidity and mortality with reciprocal improved life expectancy. Obstetricians should have knowledge of primary immunodeficiency, as more women with these disorders will reach reproductive age. CASE: 21-year-old G1P0 with purine nucleoside phosphorylase (PNP) deficiency delivered a viable infant vaginally at 37 weeks. Although the patient's diagnosis and pregnancy placed her at increased risk for infection, she remained asympt

Committee Opinion No. 639: The Importance of Vital Records and Statistics for the Obstetrician-Gynecologist.

Information from vital records is critical to identify and quantify health-related issues and to measure progress toward quality improvement and public health goals. In particular, maternal and infant mortality serve as important indicators of the nation's health, thereby influencing policy development, funding of programs and research, and measures of health care quality. Accurate and timely documentation of births and deaths is essential to high-quality vital statistics. This Committee Opinion describes the process by which births, maternal deaths, and fetal deaths are registered; the challe…

Does in utero exposure to Illness matter? The 1918 influenza epidemic in Taiwan as a natural experiment.

This paper tests whether in utero conditions affect long-run developmental outcomes using the 1918 influenza pandemic in Taiwan as a natural experiment. Combining several historical and current datasets, we find that cohorts in utero during the pandemic are shorter as children/adolescents and less educated compared to other birth cohorts. We also find that they are more likely to have serious health problems including kidney disease, circulatory and respiratory problems, and diabetes in old age. Despite possible positive selection on health outcomes due to high infant mortality rates during th…
