Ignition times of endotracheal tubes used in laser surgery have been determined in room air with oxygen flowing through the tube. Using uncuffed endotracheal tubes in children, we measured the oxygen level around the endotracheal tube at the vocal cords and found that it closely approximates the inspired oxygen concentration. We then tested Rusch red rubber and polyvinyl chloride endotracheal tubes while varying the external oxygen concentration. Mucoid impaction of tracheobronchial tree is a common condition which may complicate tracheostomy. Here we describe a case of a 72-year-old man, status after tracheostomy, who presented to the hospital with an acute upper airways obstruction and respiratory arrest due to a tracheal cast. The cast was successfully dislodged by the Nd-Yag laser.

In seven patients with hereditary haemorrhagic teleangiectasia (HHT) only one patient had lasting benefit from treatment with the flashlamp-pulsed dye laser. In seven other patients who had received other treatment before, none had any observable effect of this laser. The flashlamp-pulsed dye laser therefore does not seem to be effective in the treatment of nasal teleangiectasias.

OBJECTIVE: The purpose of this study was to assess the efficacy of the treatment of unruptured tubal ectopic pregnancies by the use of carbon dioxide laser laparoscopy.
STUDY DESIGN: A series of 125 consecutive ectopic pregnancies were treated laparoscopically; the tubal pregnancy was removed by a laparoscopic laser technique. Between May 1988 and December 1990, 855 patients were submitted to HPV-hybridisation. There is little objective long-term follow-up comparing laser therapy with intubation for palliation of malignant dysphagia. In a prospective, nonrandomized two-center trial 43 patients treated with the neodymium:yttrium-aluminum-garnet laser were compared with 30 patients treated by endoscopic intubation; the two groups were comparable for mean age and tumor position, length, and histology. Either transscleral cyclophotocoagulation with the contact neodymium:YAG (Nd:YAG) laser or cyclocryotherapy (CCT) was performed in rabbits to compare the efficacy of these procedures in reducing intraocular pressure (IOP) as well as the histologic changes they induced. The difference in mean maximum reduction of IOP in the two groups was statistically insignificant, but the Nd:YAG laser irradiation, using 2.5 J, destroyed less tissue than the CCT.

Twenty-four partial nephrectomies were performed in 18 piglets using either the combination Nd:YAG laser technique (contact and noncontact) or a steel scalpel. Additional haemostasis was attempted with ligatures. Blood loss, operation time, and number of ligatures were compared in the two treatment groups. Twelve piglets had a two-week follow-up. The mean blood loss with the laser was 72 +/- 10 g and 83 +/- 13 g with the steel scalpel (no significant difference). Twenty-one eyes of eighteen patients with clinically uncontrolled open angle glaucoma, 3 to 49 months after the initial laser treatment underwent repeat argon laser trabeculoplasty. Retreatment was successful in eleven eyes (52%). Ten eyes (48%) has marked increases in intraocular pressure, necessitating urgent filtering surgery. Repeat argon laser trabeculoplasty was effective for these patients who show a prolonged response to their initial treatment.

The persistence of facial telangiectasis and erythema in patients with rosacea frequently presents a major cosmetic problem. It may also contribute to relapses of papular and pustular lesions. Bilateral vocal cord paralysis is a rare but major complication of thyroid surgery. Since 1983, patients with bilateral cord paralysis have been managed in this department using the carbon dioxide laser. Six patients with bilateral cord paralysis secondary to thyroid surgery underwent partial cordectomy and arytenoidectomy. Proximal tubal obstruction is encountered in 33% of women with tubal factor infertility. Traditional microsurgery and the most recent transcervical tuboplasty have achieved 20% to 30% intrauterine PR. Thirty-eight neodymium yttrium aluminium garnet (Nd-YAG) laser sessions have been performed in 26 patients under local anaesthesia. The majority of patients had recurrences of inoperable non-small-cell lung cancer after radiotherapy. Overall 240 patients with chemical burns of the esophagus were examined. Local endoscopic treatment with the aid of lasers was applied in 180 victims. The efficacy of different types of laser radiation was compared in terms of the times of the healing of the esophageal wall.
