The feasibility of integrating a home telehealth model for older persons living with hemodialysis.

BACKGROUND: In Thailand, there is a rapidly increasing population of older persons living with hemodialysis (OPLWH) for whom quality of life and clinical outcomes are their main focus. This study aims to assess the feasibility of an integrated home telehealth model on quality of life and laboratory parameters of OPLWH. METHODS: In this study, the second phase of a mixed methods exploratory sequential design was conducted using a repeated measures experimental design. Participants met the inclusion criteria, which included being an OPLWH at a single hemodialysis center of one hospital in Chian

[A model for integrating mental health care resources in the rural population of Mexico].

OBJECTIVE: The purpose of this project was to identify behaviors of health service utilization to solve mental health-related problems among rural inhabitants of Mexico. A model of pathways to mental health services was built. Based on this model, an integration-intervention model is proposed to help improve access to mental health services and their quality. MATERIAL AND METHODS: In 1997, in-depth interviews were conducted among 21 adults (9 men and 12 women) and seven healthcare providers, in a rural community in Jalisco, Mexico. Women's age ranged from 23 to 44 years; ten were married and

Assessing Eligibility for Anticancer Drug Health Insurance Reimbursement Using Large Language Models: Benchmark Development and Comparative Study.

BACKGROUND: Administrative costs in the health care system are driven in part by complex insurance eligibility determinations. Large language models (LLMs) are increasingly used for health insurance-related queries, yet their reliability for structured logical reasoning over coverage criteria has not been systematically evaluated. OBJECTIVE: This study aimed to develop a benchmark for anticancer drug reimbursement eligibility determination and evaluate whether LLMs can reliably perform eligibility verification. METHODS: We constructed a benchmark based on South Korea's National Health Insura

Machine Learning Model for Readmission Prediction of Patients With Heart Failure Based on Electronic Health Records: Protocol for a Quasi-Experimental Study for Impact Assessment.

BACKGROUND: Care for patients with heart failure (HF) causes a substantial load on health care systems where a prominent challenge is the elevated rate of readmissions within 30 days following initial discharge. Clinical professionals face high levels of uncertainty and subjectivity in the decision-making process on the optimal timing of discharge. Unwanted hospital stays generate costs and cause stress to patients and potentially have an impact on care outcomes. Recent studies have aimed to mitigate the uncertainty by developing and testing risk assessment tools and predictive models to ident

Conserved cellular signals and mechanisms accompany the reproductive shutdown of honey bee (Apis mellifera) queens for dispersal.

Temporal reproductive plasticity is a central life-history trait for most iteroparous animals, yet its underlying molecular mechanisms remain poorly understood. We utilize the reversible reproductive arrest experienced by honey bee queens during colony swarming to investigate how these reproductive specialists dynamically adapt their physiology. We demonstrate that pre-swarming nutritional restriction triggers a resource reallocation, whereby ovarian mass declines as flight muscle function is enhanced, providing empirical support for a flight-fecundity trade-off. This transition is accompanied

Community-engaged intervention mapping for developing the DIGNITY program: Supporting decision-making in aging and dementia for autonomy in rural nursing homes.

BACKGROUND: Person-centered care that honors individual preferences can improve the well-being of nursing home (NH) residents with Alzheimer's disease and related dementias (ADRD). However, preferences such as going outside independently are often restricted due to perceived safety risks. There is a critical need for strategies that help NH staff balance safety concerns with residents' autonomy. MATERIALS AND METHODS: We developed the Decision-making In aGing and demeNtIa for autonomy (DIGNITY) intervention using the Community-Engaged Intervention Mapping (CEIM) Model. Implementation of a Theory-Driven and Evidence-Based Posthospital Discharge Tobacco Cessation Pilot Program.

Background: The association of tobacco use and poor health outcomes is well documented. When tobacco users are hospitalized, some temporarily refrain from tobacco use with the aid of nicotine replacement therapy (NRT). However, most are unable to sustain cessation postdischarge. Objective: Investigate whether smoking cessation rates are higher for patients prescribed NRT compared with patients with no NRT (nNRT), 2 months postdischarge. Methods: Participants aged 19-65 years, who smoked ≥1 cigarette/day, were enrolled. Participants were randomly assigned to receive NRT versus nNRT. The primary

Integrated palliative care service for Parkinson's disease in the UK: impact on place of death.

OBJECTIVES: Patients with advanced Parkinson's disease and related disorders (PDRD) experience significant symptom burden, yet access to specialist palliative care (SPC) is often limited. The Bristol service uses existing resources to form a multidisciplinary meeting (MDM) attended by both movement disorders and palliative care specialists. This integrates primary and SPC and allows sharing of expertise to holistically address unmet palliative care needs. Here, we evaluate this model. METHODS: We conducted a retrospective review of case notes of patients discussed in the Parkinson's Disease P

Extending EPDS screening in perinatal consultation-liaison care: A pragmatic framework for risk assessment.

ObjectiveSymptom-based screening alone is insufficient for identifying perinatal mental health risk in general hospital settings. Although Edinburgh Postnatal Depression Scale (EPDS)-centred screening is embedded in Australian policy, psychosocial adversity and somatic illness may contribute to psychiatric morbidity that is not captured by symptom screening alone. This perspective proposes a consultation-liaison framework that augments, but does not replace, universal screening.Intimate partner violence and hyperemesis gravidarum are associated with substantial psychiatric morbidity that may n

The role of declining therapy volumes in skilled nursing facility outcomes: a mediation analysis.

INTRODUCTION: Significant declines in therapy provision in skilled nursing facilities (SNFs) followed the 2019 implementation of Medicare's Patient-Driven Payment Model (PDPM) and the onset of the COVID-19 pandemic, raising concerns about effects on patient outcomes. METHODS: Using Medicare fee-for-service claims and SNF assessment data from January 2018 through September 2021, we analyzed 3.5 million post-hospital SNF stays to assess whether changes in therapy volumes mediated changes in successful community discharge and 30-day hospital readmissions. RESULTS: Average total physical, occupa

Beyond diagnosis: Jordanian women's experiences in surgical decision-making for breast cancer.

PURPOSE: Breast cancer is still the most common malignancy among women worldwide. Although cultural values and family dynamics strongly shape treatment preferences, evidence on experiences of Jordanian women with early-stage breast cancer and factors influencing their surgical decisions is scarce. This study explores the experiences of Jordanian women with early-stage breast cancer and identifies contributing factors to surgical decisions. METHOD: A descriptive qualitative approach was employed, utilising semi-structured interviews with purposively selected participants (n = 15). RESULTS: Th

Implementation of Quality Improvement Measures for Bar-Coded Medication Administration.

INTRODUCTION: This quality improvement project was conducted in an emergency department composed of 17 beds. The unit's bar-coded medication administration scanning compliance rate was 68%. Nursing leadership reviewed patient safety reports, and the leading issue was medication administration compliance, thus necessitating a quality improvement intervention. In response to the bar-coded medication administration scanning issues, leadership formulated a strategy to address scanning failures and compliance. METHODS: Interventions were developed using the Plan-Do-Study-Act model, and these inclu
