Image-guided navigation in oral and maxillofacial surgery.

Image-guided surgery is the logical extension of imaging as it integrates previously acquired radiological or nuclear medicine images with the operative field. In conventional image-guided surgery, a surgeon uses a surgical instrument or a pointer to establish correspondence between features in the preoperative images and the surgical scene. This is not ideal because the surgeon has to look away from the operative field to view the data. Augmented reality guidance systems offer a solution to this problem but are limited by deformation of soft tissues. Real-time intraoperative imaging offers a

A case of fatal respiratory failure after surgery for advanced supraglottic laryngeal carcinoma.

Pulmonary lymphangitic carcinomatosis (PLC) is an uncommon type of pulmonary metastasis from laryngeal carcinoma. We encountered a case of fatal respiratory failure after surgery for supraglottic laryngeal carcinoma with multiple cervical lymph node metastases. On day 25 postoperatively, the patient suddenly developed fever and noted slowly progressive difficulty in breathing. Laboratory data, radiological findings, and the clinical course of this patient were strongly suggestive of PLC. The patient refused fiberoptic bronchoscopy and glucocorticoid therapy, leading to rapid deterioration due

[Preservation of laryngeal function in surgical treatment of hypopharyngeal carcinoma].

OBJECTIVE: To study of the feasibility, the surgical techniques and the results of laryngeal function preservation in surgical treatment of hypopharyngeal carcinoma. METHODS: A retrospective review of 305 (279 males, 26 females, ages ranging from 14 to 77 years) cases with malignant neoplasms of the hypopharynx that were treated from 1978 to 1996 were accomplished. Of the 305 (stage I, 6; II, 12; III, 82; IV, 205) cases, 234 were originated from pyriform sinus, 21 from postcricoid, 35 from posterior pharyngeal wall and 15 from superior hypopharynx. Two hundred and eighty-five patients receive

The prepuce free flap: dissection feasibility study and clinical application of a super-thin new flap.

The aim of this study was to develop a free flap of the male prepuce. First, a dissection feasibility study was performed in eight male cadavers. Dissection proved feasible, and the mean surface area of the prepuce, when folded out, was 46.7 cm2. The mean pedicle length was 15 cm, with a mean diameter of the inferior external pudendal artery at its origin of 1.2 mm and a mean diameter of the inferior external pudendal vein at its origin of 1.9 mm. Next, a viability study was performed by isolating a prepuce free flap during male-to-female sex reassignment procedures. The flap as well as the re

Traumatic and spontaneous extracranial internal carotid artery dissections.

Seventy patients with spontaneous and 21 with traumatic extracranial internal carotid artery dissections were studied clinically and angiographically with mean follow-ups of 64 (spontaneous group) and 40 months (traumatic group). Sixty percent of the patients in the spontaneous group and 71% in the traumatic group also had follow-up angiograms. In traumatic dissections aneurysms were common, significantly fewer aneurysms resolved or became smaller and fewer stenoses resolved or improved, whereas more stenoses progressed to occlusion. Traumatic dissections were more likely to leave the patients

Stage T3 squamous cell carcinoma of the glottic larynx treated with surgery and/or radiation therapy.

This is a retrospective analysis of 68 patients with Stage T3 squamous cell carcinoma of the glottic larynx treated with surgery and/or irradiation from March 1965 to April 1981 at the University of Florida. Follow-up ranged from 2 to 17.5 years. Initial local-regional control of disease above the clavicles (i.e., prior to salvage attempts) by treatment technique for patients at risk for greater than or equal to 2 years was 11/19 (58%) with irradiation alone, 5/7 (71%) with preoperative irradiation and surgery, 8/9 (89%) with surgery and postoperative irradiation, and 18/23 (78%) with surgery

[The free jejunal graft for the reconstruction of the hypopharynx and cervical esophagus].

The free jejunal transfer for primary reconstruction of the cervical oesophagus after laryngo-pharyngectomy was described more than 20 years ago. Its popularity as a surgical method has increased in recent years with improvement in microsurgical techniques. Tumour resection and preparation of the jejunal graft are performed synchronously by two surgical teams. After suturing the graft into the defect the mesenteric vessels are usually anastomosed to the superior thyroid pedicle. In this series four patients are presented, in three there were no complications, in the fourth a fistula developed

Flap reconstruction in major surgery of the head and neck.

The challenging restoration of form and function in radical surgery of the head and neck demands sound concepts and proper execution in reconstruction. Flaps are most frequently employed in reconstruction, following major surgery because of their rich blood supply and cosmetic superiority. Of the many types of flaps known, the midline forhead flap, temporoforehead flap, mastoid-occipital flap, tongue flap, and deltopectoral flap have enjoyed most popularity. The design and application of these flaps are discussed. Our techniques in reconstruction of the oral cavity, hypopharynx, esophagus, nos

Treatment of oral carcinoma with selective therapy for the individual patient.

A series of 121 patients with carcinoma of the lower oral cavity seen at the British Columbia Cancer Institute during the years 1963 to 1969 has been reviewed. We have evidence of improved treatment results over a previous series of patients treated between 1953 and 1962. The results are as satisfactory as other reported treatment policies in which either surgery or radiation is used exclusively. Careful follow-up of all patients is essential. When recurrence or metastatic disease is recognized early, further treatment can still result in cure. The criteria for case selection of the individual

Long-term effect of postoperative radioactive iodine therapy on parathyroid function in patients with differentiated thyroid cancer.

OBJECTIVE: The study aimed to assess the impact of postoperative radioactive iodine (RAI) therapy on parathyroid function in patients who un … ficantly higher calcium levels at one month postoperatively (p = 0.031) and lower rates of CLND and neck dissection. No significant differences in biochemical parameters were found at the five-year follow-up, except for one patient who developed normocalcemic hyperparathyroidism after RAI. CONCLUSION: High-dose RAI therapy may lead to transient decreases in calc …

Surgical resources in advanced thyroid cancer treatment with aerodigestive tract invasion.

BACKGROUND: Despite papillary thyroid cancer (PTC) excellent prognosis, 10-15% of patients may present aggressive local behaviour. We present two cases with different aerodigestive tract invasion partners in which two reconstructions were used, out of all the surgical resources we have planned preoperatively [1-4]. METHODS: Case 1: 57-year-old woman with asymmetric goitre and a 60mm nodule (Bethesda-VI). CT showed suspected involvement of aero-digestive tract. Endobronchial ultrasound (EBUS) showed no tracheal invasion. Per oral endoscopic-US confirmed transmural oesophageal involvement. Surg

A Combined Microsurgical and Endovascular Approach to Giant Paraclinoid Aneurysm: 2-Dimensional Operative Video.

Giant paraclinoid aneurysm remains a treatment challenge because of their complex anatomy and surgical difficulties stems frequently from a calcified or atherosclerotic aneurysmal neck and compression of the optic pathways.1-9 To improve exposure, facilitate the dissection of the aneurysm, assure vascular control, reduce brain retraction and temporary occlusion time, and enable simultaneous treatment of possible associated aneurysms, we combined the cranio-orbital zygomatic (COZ) approach9 with endovascular balloon occlusion of the internal carotid artery (ICA) and suction decompression of the

[Blister-like Aneurysm Originating from the Anterior Communicating Artery:A Case Report].

Blister-like aneurysms(BLA)are morphologically thin-walled fragile structures that are associated with a high risk of intraoperative rupture. BLAs presumably originate from an arterial dissection and often stem from a non-branching site on the supraclinoid segment of the internal carotid artery. We report a rare case of a patient who was successfully treated for a ruptured BLA originating from the anterior communicating artery(Acom). A 60-year-old woman presented with severe headache and loss of consciousness. Computed tomography(CT)showed diffuse subarachnoid hemorrhage(SAH), which was observ

Retrosigmoid Approach for Vestibular Schwannoma.

Vestibular schwannoma is a benign tumor that affects 3% of the population, but accounts for 85% of tumors occurring at the cerebellopontine angle (CPA). In this case, we present a 48-year-old female with history of cholesteatoma on the right and chronic suppurative otitis media on the left who presented with an 18 month history of bilateral hearing loss, worse on the right. Investigations revealed a right sided vestibular schwannoma measuring 1.6 cm in diameter. Audiogram revealed an AAO-HNS (American Academy of Otolaryngology-Head and Neck Surgery) class C hearing on the right and class B on
