A novel medical image segmentation network for colorectal polyp small targets and fuzzy boundaries.

OBJECTIVES: To address the challenges of complex feature variations and unclear boundary definitions between segmented targets and surrounding regions in medical images, a novel segmentation model based on Deformable Large Kernel Convolutional Attention (D-LKA) and Transformer is proposed. METHODS: The model first uses Vision Transformer as the encoder to enhance the ability to capture global information, overcoming the limitations of convolutional neural networks' receptive field. In the decoder, a D-LKA decoder with deformable large kernel convolution attention is used, allowing the model t

Spontaneous epidermal stimulation briefly and diffusely enhances skin spatial acuity in shooting athletes: Evidence from event-related potentials.

BACKGROUND: Tactile sensation is crucial because it provides information about objects during fine-motor actions. Skin stimulation has a wide range of effects, including analgesia, cognitive enhancement, and endocrine modulation. However, very few studies have investigated the effects of spontaneous epidermal stimulation (SES) on tactile sensation, and the underlying mechanisms remain unclear. METHODS: To explore the neural mechanisms of SES-induced enhancement of skin spatial acuity (SSA), we conducted 2 experiments on 40 healthy young shooters, combining the gate-control theory, heart rate

Chronic neurostimulation of splenic nerve enabled by hydrogel-bioelectronics for wireless electroceutical immunomodulation therapy.

Targeted modulation of the splenic nerve offers a selective alternative to vagus nerve stimulation, avoiding off-target activation of mixed vagal fibers. However, the small diameter and anatomical complexity of the splenic nerve pose great challenges for stable neural interfacing. Here, we report a splenic nerve wireless stimulator (SpNWS) built from stretchable and highly conductive hydrogel for chronic electroceutical immunomodulation therapy. The hydrogel-fiber neural electrodes conform to the geometries of splenic neurovascular bundles (SNVBs) and are secured using a bioadhesive approach,

Spatiotemporal consistency analysis of cerebral small vessel disease: an rs-fMRI study.

INTRODUCTION: Cerebral small vessel disease (SVD) affects older adults, but traditional approaches have limited the understanding of the neural mechanisms of SVD. This study aimed to explore the effects of SVD on brain regions and its association with cognitive decline using the four-dimensional (spatiotemporal) consistency of local neural activity (FOCA) method. METHODS: Magnetic resonance imaging data from 42 patients with SVD and 38 healthy controls (HCs) were analyzed using the FOCA values. A two-sample t test was performed to compare the differences in FOCA values in the brain between th

Noninvasive brain stimulation in autism: review and outlook for personalized interventions in adult patients.

Noninvasive brain stimulation (NIBS) has been increasingly investigated during the last decade as a treatment option for persons with autism spectrum disorder (ASD). Yet, previous studies did not reach a consensus on a superior treatment protocol or stimulation target. Persons with ASD often suffer from social isolation and high rates of unemployment, arising from difficulties in social interaction. ASD involves multiple neural systems involved in perception, language, and cognition, and the underlying brain networks of these functional domains have been well documented. Aiming to provide an o

The modulation of pain in reward processing is reflected by increased P300 and delta oscillation.

Pain elicits the desire for a reward to alleviate the unpleasant sensation. This may be a consequence of facilitated neural activities in the reward circuit. However, the temporal modulation of pain on reward processing remains unclear. We addressed this issue by recording electroencephalogram when participants received win or loss feedback in a simple gambling task. Pain treatment was conducted on 33 participants with topical capsaicin cream and on 33 participants with hand cream as a control. Results showed that pain generally increased the P300 amplitude for both types of feedback but did n

Neuroprotection in an Experimental Model of Multiple Sclerosis via Opening of Big Conductance, Calcium-Activated Potassium Channels.

Big conductance calcium-activated (BK) channel openers can inhibit pathologically driven neural hyperactivity to control symptoms via hyperpolarizing signals to limit neural excitability. We hypothesized that BK channel openers would be neuroprotective during neuroinflammatory, autoimmune disease. The neurodegenerative disease was induced in a mouse experimental autoimmune encephalomyelitis model with translational value to detect neuroprotection in multiple sclerosis. Following the treatment with the BK channel openers, BMS-204253 and VSN16R, neuroprotection was assessed using subjective and

Transcranial direct current stimulation (tDCS), a treatment option for tinnitus in profound hearing loss.

A 45-year-old man presented with a history of sudden sensory neural hearing loss and severe tinnitus in his left ear. Audiological investigations revealed a profound hearing loss on his left ear and mild conductive hearing loss on his right. Tinnitus pitch and loudness were matched to a 4 kHz narrow-band noise at 50dBHL and subjective tinnitus questionnaires revealed that he had a catastrophic handicap (grade IV). Traditional audiological treatment approaches (tinnitus maskers, hearing aid and sound therapy) that stimulate the cochlea to induce cortical reorganisation were futile. Hence, a top

A medical cost estimation with fuzzy neural network of acute hepatitis patients in emergency room.

Taiwan is an area where chronic hepatitis is endemic. Liver cancer is so common that it has been ranked first among cancer mortality rates since the early 1980s in Taiwan. Besides, liver cirrhosis and chronic liver diseases are the sixth or seventh in the causes of death. Therefore, as shown by the active research on hepatitis, it is not only a health threat, but also a huge medical cost for the government. The estimated total number of hepatitis B carriers in the general population aged more than 20 years old is 3,067,307. Thus, a case record review was conducted from all patients with diagno

I finally see what you see: Parkinson's disease visual hallucinations captured with functional neuroimaging.

BACKGROUND: Functional neuroimaging studies have described alterations in neural activation in PD patients with chronic hallucinations. These studies have not, however, captured neural activation patterns during an actual hallucinatory event. The objective of this work was to investigate neuroanatomical substrates active during visual hallucinations in a patient with Parkinson's disease (PD). METHODS: We conducted an event-related functional magnetic resonance imaging (fMRI) case-study examination of a 66-year-old male PD patient with stereotypic, chronic, and frequent visual hallucinations.

Maternal PCMT1 gene polymorphisms and the risk of neural tube defects in a Chinese population of Lvliang high-risk area.

Protein-L-isoaspartate (D-aspartate) O-methyltransferase 1 (PCMT1) gene encodes for the protein repair enzyme L-isoaspartate (D-aspartate) O-methyltransferase (PIMT), which is known to protect certain neural cells from Bax-induced apoptosis. Previous study has shown that PCMT1 polymorphisms rs4552 and rs4816 of infant are associated with spina bifida in the Californian population. The association between maternal polymorphism and neural tube defects is still uncovered. A case-control study was conducted to investigate a possible association between maternal PCMT1 and NTDs in Lvliang high-risk

Cranial base approaches to inaccessible intracranial tumors.

PURPOSE OF REVIEW: Craniotomy created through the base of the skull has improved exposure of many types of extraaxial tumors and thus enhanced both tumor control and preservation of neural function. The purpose of this article is to review recent advances in this emerging field. RECENT FINDINGS: Use of microscopes and endoscopes has allowed these procedures to become progressively less invasive. Electrophysiological monitoring has enhanced neural identification and preservation. The increasingly documented efficacy of stereotactic radiation for certain tumor types (e.g. meningioma, schwannoma

The development of psychopathy.

The current review focuses on the construct of psychopathy, conceptualized as a clinical entity that is fundamentally distinct from a heterogeneous collection of syndromes encompassed by the term 'conduct disorder'. We will provide an account of the development of psychopathy at multiple levels: ultimate causal (the genetic or social primary cause), molecular, neural, cognitive and behavioral. The following main claims will be made: (1) that there is a stronger genetic as opposed to social ultimate cause to this disorder. The types of social causes proposed (e.g., childhood sexual/physical abu

Audit of clinical judgements and actions in an audiological investigation of school entrant children screened positive for hearing loss.

Of 117 children attending an aural clinic after screening positive by one or both of two tests for hearing loss at school entry, 43 (36.8%) were either seen again for review or listed for surgery. Clinical findings which were significantly associated with either being reviewed or listed for surgery, as opposed to being discharged, were the presence of conductive or sensory neural hearing loss, pronounced negative pressure, reduced compliance volume or absent stapedial reflexes on impedance testing and the presence of an abnormal tympanic membrane associated with effusion on otoscopy. This popu

Prelinguistic vocalizations of hearing-impaired and normally hearing subjects: a comparison of consonantal inventories.

Phonetic transcriptions of 48 babbling samples from 11 normally hearing subjects, aged 4-18 months, and 39 samples from 14 hearing-impaired (HI) subjects, aged 4-39 months, were analyzed to determine the inventory of consonantal phones for each recording session. Group comparisons revealed that inventories of the hearing-impaired subjects were generally smaller than those of the hearing infants and that they evidenced little change in size prior to 20 months. Analysis of place and manner of articulation of supraglottal phones in the inventories showed that, compared with inventories of the nor

An intelligent computational framework combining neural networks with complex Pythagorean fuzzy FUCA for airspace capacity evaluation and traffic forecasting.

The contemporary aviation system demands intelligent airspace capacity assessment and traffic prediction, which are essential towards maximizing operational efficiency, safety, and strategic planning. But there are few studies that combine predictive learning with complex fuzzy decision-making, and all three, which are nonlinear traffic dynamics, complex-valued expert uncertainty, and complex decision-making. To overcome uncertainty, vagueness, and complex expertise evaluations that are part of airspace management, this paper suggests a new hybrid framework that integrates both neural networks

Utilizing Reinforcement Learning to Overcome the Challenge of Muscle-Specific EMG Placements for Musculoskeletal Modeling.

OBJECTIVE: Electromyography (EMG)-driven musculoskeletal (MSK) models are widely used in biomechanics and rehabilitation, but they require muscle-specific (MSp) EMG recordings, which can be difficult to obtain from surface electrodes (sEMGs). This study aims to develop a novel framework to drive a MSK model using non-muscle-specific (NMSp) EMG recordings. METHODS: The key innovation is a reinforcement learning (RL)-based solution that can train a policy to estimate MSp excitations for MSK model control inputs without precise sEMG placement. To validate the method, ten able-bodied individuals

Consistent cortico-basal ganglia-cerebellar activation in early motor sequence learning: a systematic review and meta-analysis.

OBJECTIVE: This study aims to systematically investigate activation in brain regions during early-stage motor sequence learning (MSL) and to identify convergent neural activation patterns, thereby advancing our understanding of the neural mechanisms underlying MSL. METHODS: This meta-analysis integrated functional magnetic resonance imaging (fMRI) data from 12 studies (219 participants) using the serial reaction time task (SRTT) paradigm, applying anisotropic effect-size seed-based d mapping (AES-SDM) to identify common brain activation regions underlying MSL. After the primary meta-analysis,

Post-stroke sarcopenia as a neuromuscular disorder: a mini review of neural mechanisms and clinical implications.

BACKGROUND: Post-stroke sarcopenia represents a distinct neuromuscular disorder characterized by progressive muscle loss and functional decline. Unlike age-related sarcopenia, stroke-induced muscle wasting involves specific neural mechanisms originating from central nervous system injury. OBJECTIVE: To systematically review the neural mechanisms underlying post-stroke sarcopenia and synthesize evidence on its clinical implications, diagnostic approaches, and therapeutic interventions. METHODS: A mini review of the literature was conducted, synthesizing evidence from key studies addressing ne

The neural correlates of consciousness: A systematic review and meta-analysis of the visual awareness negativity and P3b.

Isolating the neural correlates of consciousness is a paramount challenge in neuroscience. In electroencephalographic research, two event-related potentials have been identified as candidate markers of visual awareness: the visual awareness negativity (VAN) and the P3b (or late positivity). To rigorously evaluate their validity as neural correlates of consciousness, we conducted the first systematic review and meta-analysis of these signals, presented as a registered report. We assessed their necessity and sufficiency through meta-analysis of "seen" effect sizes (where consciousness was presen
