Routine use of intravenous acetaminophen safely enhances pain control after minimally invasive hepatectomies: a retrospective cohort study.

BACKGROUND: Effective postoperative pain management is crucial after minimally invasive hepatectomy (MIH) to promote recovery, and multimodal analgesia strategies are used to reduce opioid requirements and improve outcomes. Acetaminophen is commonly included as part of multimodal therapies for postoperative pain management. However, the safety and efficacy of acetaminophen for postoperative analgesia in MIH remains unestablished due to its hepatic metabolism. This study aimed to evaluate the safety and efficacy of routine intravenous acetaminophen administration following MIH. METHODS: The da

Percutaneous cervical cordotomy for cancer-related pain: national data.

OBJECTIVES: Percutaneous cervical cordotomy (PCC) is an interventional ablative procedure in the armamentarium for cancer pain treatment, but there is limited evidence to support its use. This study aimed to assess the effectiveness and safety of PCC. METHODS: Analysis was undertaken of the first national (UK) prospective data repository of adult patients with cancer undergoing PCC for pain treatment. The relationship between pain and other outcomes before and after PCC was examined using appropriate statistical methods. RESULTS: Data on 159 patients' PCCs (performed from 1 January 2012 to 6

Perioperative pain management: an update for obstetrician-gynecologists.

The opioid epidemic in the United States is unprecedented and continues to worsen. Many opioid abusers obtain their pills through legitimate prescriptions, directly or indirectly, from a medical provider. While practitioners have a responsibility to treat pain, it is now becoming clear that aggressive opioid prescription practices contribute to an epidemic of abuse. The medical community has to balance the unintended consequences of opioid misuse and abuse with the need to provide adequate pain control. Additionally, providers are being held accountable by the legal system and professional org

Psychological interventions for acute pain after open heart surgery.

BACKGROUND: This is an update of a Cochrane review previously published in 2014. Acute postoperative pain is one of the most disturbing complaints in open heart surgery, and is associated with a risk of negative consequences. Several trials investigated the effects of psychological interventions to reduce acute postoperative pain and improve the course of physical and psychological recovery of participants undergoing open heart surgery. OBJECTIVES: To compare the efficacy of psychological interventions as an adjunct to standard care versus standard care alone or standard care plus attention c

Acute Responses of Strength and Running Mechanics to Increasing and Decreasing Pain in Patients With Patellofemoral Pain.

CONTEXT: Patellofemoral pain (PFP) is typically exacerbated by repetitive activities that load the patellofemoral joint, such as running. Understanding the mediating effects of changes in pain in individuals with PFP might inform injury progression, rehabilitation, or both. OBJECTIVE: To investigate the effects of changing pain on muscular strength and running biomechanics in those with PFP. DESIGN: Crossover study. SETTING: University research laboratory. PATIENTS OR OTHER PARTICIPANTS: Seventeen participants (10 men, 7 women) with PFP. INTERVENTION(S): Each participant completed knee pa

Calcaneal Osteotomies.

Calcaneal osteotomies are surgical procedures performed to correct a hindfoot deformity or realign the calcaneus in conditions that include pes planus and pes cavus. These joint-sparing procedures offer the ability to realign the calcaneus, enhancing foot biomechanics, redistributing forces, and reducing pain and dysfunction. Among the most common types of calcaneal osteotomies are the medial slide, lateral slide, and Dwyer calcaneal osteotomy, each with distinct indications, advantages, and surgical considerations.

Technique for arthroscopic rotator cuff repair.

Modern arthroscopic tools and techniques have allowed surgeons to operate on a wide variety of injuries using procedures less invasive than traditional open methods. For shoulder surgery in general, and rotator cuff repairs specifically, methods now yield a similar footprint as open procedures with several advantages, including reduced tissue trauma, postoperative pain, swelling, and concern about the deltoid attachment, which should lead to good outcomes.

[Laparoscopic transhiatal oesophagectomy: our experience].

The authors present their initial experience with minimally invasive surgery of the oesophagus. Two emblematic cases of benign and malignant oesophageal pathology, treated by laparoscopic transhiatal oesophagectomy, are reported. The surgical procedure is described in detail and compared with other techniques reported in the literature. Its advantages in terms of postoperative pain and morbidity are stressed. The role of the procedure in neoplastic diseases of the oesophagus is still debated, above all with regard to the accuracy of the mediastinal node dissection.

Arthroscopic removal of painful Bristow hardware.

Complications of metallic hardware about the shoulder have been well documented in the literature. This case report presents a recreational athlete who presented to our office with a painful shoulder 4 years after a Bristow procedure. Radiographic imaging confirmed contact of the humeral head on the Bristow screw. In these unusual situations, arthroscopic evaluation, as this case suggests, should be considered. The patient's hardware was removed arthroscopically, obviating an open procedure and hospitalization.

Neovascular glaucoma associated with ocular hypoperfusion secondary to carotid artery disease.

Three cases of neovascular glaucoma associated with ocular hypoperfusion secondary to carotid artery disease are reported. In two cases, anastomosis of the internal and external carotid artery systems via a superficial temporal artery to the middle cerebral artery (Yasargil procedure) resulted in resolution of the rubeosis, relief of the ocular pain and improvement of the circulation of the globe. The third case, which did not have this procedure, resulted in a blind eye with severe rubeotic glaucoma.

[Diagnosis of carbohydrate malabsorption].

After a review of causes and symptoms of sugar malabsorption and the usual diagnostic methods, the application is described of a recently developed procedure with high specificity and sensitivity: the hydrogen breath test. Examination of a large number of children shows that its sensitivity is higher than that of the procedures used so far, that lactose malabsorption is present in over 30% of the children with recurrent abdominal pain and/or diarrhoea; that in contrast to the prevailing opinion, malabsorption of sucrose in children is rare.

Patient-Reported Outcomes Following Two-Tined Three-Site Conventional Radiofrequency Ablation of Genicular Nerves for Chronic Knee Pain.

BACKGROUND: Genicular nerve radiofrequency ablation (GNRFA) has emerged as an effective therapy for chronic knee pain. Although some studies have evaluated its effectiveness, there is limited real-world data on the long-term patient-reported outcome (PRO) data for GNRFAs. OBJECTIVE: This study aimed to evaluate PROs following conventional GNRFAs performed using two-tined electrodes and the classical three-site technique targeting the superior medial, superior lateral, and inferior medial genicular nerves. METHODS: We performed a retrospective chart review to gather data from consecutive GNRF

Basivertebral Nerve Ablation in a Patient With Ankylosing Spondylitis: A Case Report.

BACKGROUND: Lower back pain is a symptom of ankylosing spondylitis (AS), stemming from arthritic changes within the sacroiliac joints, lumbar facet joints, and between the vertebral bodies. Most commonly, interventional techniques targeting the sacroiliac or facet joints have been used to mitigate AS-related pain. CASE REPORT: In this case report, we describe a patient with AS and axial lower back pain who experienced insufficient and temporary relief with these first-line procedures. Furthermore, magnetic resonance imaging (MRI) showed degenerative changes to vertebral endplates suggesting a

A survey of postoperative pain treatments and unmet needs.

More than 300 million patients undergo surgery worldwide each year. Pain associated with these procedures is associated with short- and long-term negative sequelae for patients, healthcare providers, and healthcare systems. The following chapter is a review of the reality of postoperative pain management in everyday clinical routine based on survey- and registry-derived data with a focus on care in adults. Between 30% and up to 80% of patients report moderate to severe pain in the days after surgery. Structures, processes, and outcomes vary widely between hospitals and indicate gaps between ev

Headache attributed to carotid or vertebral artery pain.

Headache or pain in the face or neck attributed to the carotid or vertebral artery was not recognized as a special type of pain until the concept of carotidynia occurred in the 1960s. Carotidynia has long been assumed as an entity until modern imaging techniques showed that pain localized in the carotid region could most often be related to injury of the artery or other symptomatic causes. These causes include headache due to arterial dissection, which has been described as a true pain originating from the vessel structure itself and which has also been attributed to a comorbidity of migraine

Retained Endovascular Catheter Causing Erosive Cyst Formation of the Cuboid After Embolization: A Case Report.

CASE: This report describes a 20-year-old woman with aggravating pain and swelling in the left foot a year after lower extremity angiography and embolization for a vascular lesion. Despite successful embolization, the patient continued to experience worsening pain, swelling, and hypersensitivity, which was diagnosed as complex regional pain syndrome. Imaging revealed a retained microcatheter with embolization material causing a significant cystic lesion in the cuboid with cortical erosion. Surgical intervention was performed including removal of the foreign material, debridement, and viable ce

Cryoneurolysis of Intercostal Nerves for Postherpetic Neuralgia: A Case Report.

Cryoneurolysis is a procedure that has been shown to be efficacious in managing pain in various settings. Multiple studies have demonstrated the benefit of cryoneurolysis; however, the literature on managing postherpetic neuralgia with cryoneurolysis is scarce. The current treatment options available include transdermal lidocaine patches, capsaicin, non-steroidal anti-inflammatory drugs, nerve blocks, antidepressants, anticonvulsants, and opioids. Pain-relieving treatments such as opioids can have long-term detrimental effects on a patient or be ineffective for long-term pain relief, so there
