Effect of supraspinatus tendon repair technique on the infraspinatus tendon.

Supraspinatus tendon tears are common and often propagate into larger tears that include the infraspinatus tendon, resulting in loss of function and increased pain. Previously, we showed that the supraspinatus and infraspinatus tendons mechanically interact through a range of rotation angles, potentially shielding the torn supraspinatus tendon from further injury while subjecting the infraspinatus tendon to increased risk of injury. Surgical repair of torn supraspinatus tendons is common, yet the effect of the repair on the infraspinatus tendon is unknown. Since we have established a relations

The value of intraoperative neuromonitoring combined with high-definition endoscopy in the operation of brachial plexus schwannoma.

BACKGROUND: To investigate the value of intraoperative neuromonitoring (IONM) combined with high-definition endoscopy in surgical treatment of brachial plexus schwannoma. METHODS: A retrospective analysis was conducted on twenty patients diagnosed with brachial plexus schwannoma from January 2020 to December 2024. All cases were treated surgically with IONM combined with high-definition endoscopy. Intraoperative and postoperative nerve function were assessed to evaluate the value of this combined approach during surgery. RESULTS: All twenty patients underwent complete intracapsular tumor res

Musculoskeletal ultrasound abnormalities in professional rugby union players in the United States.

This cross-sectional observational study aimed to characterize the prevalence of musculoskeletal sonographic abnormalities among players (N = 23) in a Major League Rugby team during preseason medical evaluations. Bilateral ultrasound examinations were performed on the acromioclavicular (AC) joints, quadriceps, patellar, and Achilles tendons. Players most commonly reported prior injuries to the ankle/foot, leg, and shoulder. Ultrasound findings revealed a high prevalence of abnormalities across all examined sites: 65.2% in the AC joints, 70.6% in the quadriceps tendons, 100% in the patellar ten

Delayed primary flexor tendon repair in zone II injuries: results of using WALANT and controlled true active motion.

Early repair of flexor tendon injuries is ideal, but delays are common. We studied the outcomes of flexor tendon repairs delayed from 5 days to 6 months and carried out under wide-awake local anaesthesia with no tourniquet (WALANT). Twenty-four patients (29 fingers) who underwent primary flexor tendon repair on zone II using a four- to six-strand core suture technique, followed by controlled early active motion therapy. Clinical assessments, including total active motion (TAM) and Disabilities of the Arm, Shoulder and Hand, were made 6, 8 and 12 weeks after operation. All outcomes improved sig

Tube posterior tibial artery flow-through free flap for salvaging upper limbs with contaminated soft tissue defects and arterial injuries.

The tube posterior tibial artery flow-through free flap technique is proposed for salvaging upper limbs with severely contaminated or infected soft tissue defects and major arterial injuries. Between January 2016 and February 2024, six patients were treated using this method. This is a two-stage approach involving initial restoration of blood supply via a tube posterior tibial artery flow-through free flap, followed by delayed soft tissue coverage utilizing the unfolded tube flap after a series of wound debridements and vacuum sealing drainage treatments. All upper limbs and flaps completely s

Cervical spine fracture dislocation with mild neurological deficits.

Cervical fracture dislocation often leads to neurological deficits, manifesting with sensory and motor symptoms, which may persist even after surgical intervention. We presented two cases with mild neurological deficits following such injuries. In Case 1, the patient presented with left-hand numbness 1 month after a car accident. Despite this, he exhibited full muscle power in all four limbs. Cervical spine MRI revealed a posterior column tear, facet fracture dislocation and spinal cord indentation at the C5/C6 level. The patient underwent open reduction with anterior and posterior fixation. A

Transforaminal Migration of K-Wire From Clavicle to Cervical Spinal Canal: A Case Report and Review of Literature.

INTRODUCTION: Kirshner wires are commonly used for orthopedic surgical fixation for shoulder injuries, particularly around the acromioclavicular joint. They are versatile, cheap, and minimally invasive in stabilizing bone fragments. However, there have been cases of secondary migration of K-wires from the clavicle to surrounding tissues, particularly around the cervical spine. CASE REPORT: We present a case of a 60-year-old female with left-sided neck pain and radiation to the left upper limb, a K-wire was found to have migrated through the C7-T1 foramina left side into the spinal canal poste…

Superficial Branch of the Radial Nerve Versus Sural Nerve Grafting After Traumatic Adult Brachial Plexus Injury.

The purpose of this study was to compare two sources of nerve graft for brachial plexus reconstruction: the denervated superficial branch of the radial nerve (SBRN) and the sural nerve. Ninety-seven patients who underwent brachial plexus reconstruction with denervated SBRN nerve (24 patients with 24 grafts) or with sural nerve grafting (73 patients with 83 nerve grafts) were included. The two groups were compared with respect to postoperative muscle reinnervation, disabilities of the arm, shoulder, and hand (DASH) scores. In the SBRN group, only four (17%) of the nerve grafts provided grade II

A case of seat belt-induced small bowel rupture and Chance fracture accompanied by elevated serum amylase.

We report herein our experience with a 10-year-old girl who incurred injuries consistent with seat belt syndrome. The patient was involved in a low-speed motor vehicle accident while restrained in a three-point belt (shoulder and lap belts). She complained of abdominal pain and developed tenderness and abdominal guarding. On admission, enhanced computed tomography (CT) demonstrated a fluid collection in the pelvis and a fractured vertebra in the lumbar spine. The following day, her C-reactive protein and serum amylase levels were elevated and plain CT demonstrated free air in the left side of

Pathological findings identified during the posterior approach to the spinal accessory nerve after high-energy trauma.

The spinal accessory to suprascapular nerve transfer is a key procedure for restoring shoulder function in upper brachial plexus injuries and is typically undertaken via an anterior approach. The anterior approach may miss injury to the suprascapular nerve about the suprascapular notch, which may explain why functional outcomes are often limited. In 2014 we adopted a posterior approach to enable better visualization of the suprascapular nerve at the notch. Over the next 6 years we have used this approach for 20 explorations after high-energy trauma. In 7/20 we identified abnormalities at the l

A Case of Fatal Stanford Type A Aortic Dissection Caused by a Traffic Accident with Low Energy Impact.

A seemingly healthy 84-year-old female pedestrian was mildly bumped by a car, and she hit her left shoulder, flank, and lower legs when she fell down on a street. She was conscious and stable when transferred to an emergency room. She had no sign of any major injuries except minor lacerations on her legs. Repeated evaluations including chest X-ray, ECG, and focused assessment with sonography for trauma did not reveal any abnormal findings. While waiting for discharge, she started having mild chest discomfort. Unexpectedly, the third echocardiogram showed mild pericardial effusion, and a CT wit

[Anal incontinence and obstetrical anal sphincter injuries, epidemiology and prevention].

Our main objectives were to identify risk factors, methods for early diagnosis, and prevention of obstetric anal sphincter injuries (OASIs), using a literature review. The main risk factors for OASIs are nulliparity, instrumental delivery, posterior presentation, median episiotomy, prolonged second phase of labor and fetal macrosomia. Asian origin, short ano-vulvar distance, ligamentous hyperlaxity, lack of expulsion control, non-visualization of the perineum or maneuvers for shoulder dystocia also appear to be risk factors. There is a risk of under-diagnosis of OASIs in the labor ward. Experi

A Role for Postoperative Negative Pressure Wound Therapy in Multitissue Hand Injuries.

In this study, we compared outcomes in patients with acute hand injury, who were managed with or without negative pressure wound therapy (NPWT) after reconstructive surgery. All of the patients who sustained acute and multitissue injuries of the hand were identified. After reconstructive surgery, a conventional dressing was applied in Group 1 and NPWT was applied in Group 2. The dressing and NPWT were changed every 3 days. The mean age and Hand Injury Severity Scoring System score of both groups were not significantly different. Disabilities of the Arm, Shoulder, and Hand (DASH) scores were ev

A Review on Functional Electrical Stimulation in Tetraplegic Patients to Restore Hand Function.

The purpose of this collective review is to describe a new form of functional electrical stimulation called neuroprosthesis. This unique technology has been devised to produce lateral pinch and palmar grasp in persons with C5 and C6 motor level spinal cord injuries. This neuroprosthesis includes external as well as implanted components. First, a receiver is surgically implanted into the patient's chest above a pectoralis major muscle. The receiver stimulator is then connected to 8 surgically implanted epimysial or intramuscular electrodes. Restoration of upper extremity function can greatly im

Complex transradial carpal dislocation in a Professional Pianist: Case Report.

Carpal fracture-dislocation associated to distal radius fractures is an uncommon injury of the wrist. Clinical assessment, instrumental diagnosis and treatment are all challenges for the surgeon. In addition, the prognosis in high-functioning patients is nearly always poor. The authors describe an early diagnosis of scapholunate dissociation and joint capsule tear associated with radial styloid and triquetral fractures in a 39-year-old professional piano player. A dorsal approach was used to reduce and fixate the fracture with k-wires, and to repair soft-tissue injuries with a Titanium micro-a

Clavicle shaft fractures: are children little adults?

Most clavicle shaft fractures in children are nondisplaced, minimally shortened or angulated, and are well treated with nonoperative management. Indications for operative fixation include open fractures and impending open fractures. Relative indications for operative fixation include fractures in multi-trauma patients, floating shoulder injuries, comminuted fractures, and shortened fractures in older adolescents. Controversies exist in preferred methods of fixation and include superior or anteroinferior plating, locked or unlocked plating, and intramedullary fixation. Retrospective studies of …

Double semi-tubular plating of clavicle using a piggyback technique--an alternative way of treating clavicle mid-shaft fractures in young patients.

The majority of clavicle fractures unite uneventfully. Indications for operative surgery are limited, with high rates of complications. We describe our results with an alternative cost effective method of treating these injuries in young patients. It is a retrospective series of 20 patients treated between January 2005 and January 2009 with a "piggy back" technique using double plating with two 3.5 mm drill hole semitubular plates. All patients were assessed with DASH (Disability of Arm, Shoulder and Hand) score at last review. Average time to union was 12.5 weeks. The average DASH score was 8
