An update on treatment of the anxiety disorders.

The recently discovered high prevalence of the anxiety disorders, as well as studies demonstrating their strong association with psychophysiologic symptoms, will probably increase referrals for these disorders to psychiatric clinicians. The authors review developments since 1983 in the psychopharmacologic and psychologic-behavioral treatment of the anxiety disorders. They find a growing body of empirical evidence that a variety of pharmacologic and cognitively oriented behavioral therapies are effective treatments. Less evidence is available for the utility of psychodynamic therapies, although

Robust Estimation of Respiratory Variability Uncovers Correlates of Limbic Brain Activity and Transcutaneous Cervical Vagus Nerve Stimulation in the Context of Traumatic Stress.

OBJECTIVE: Variations in respiration patterns are a characteristic response to distress due to underlying neurorespiratory couplings. Yet, no work to date has quantified respiration pattern variability (RPV) in the context of traumatic stress and studied its functional neural correlates - this analysis aims to address this gap. METHODS: Fifty human subjects with prior traumatic experiences (24 with posttraumatic stress disorder (PTSD)) completed a ∼3-hr protocol involving personalized traumatic scripts and active/sham (double-blind) transcutaneous cervical vagus nerve stimulation (tcVNS). Mediating Effect of Depression and Anxiety on the Relationship Between Grief Reaction and Post-Traumatic Stress Symptoms in Lebanese Volunteers.

OBJECTIVE: The Lebanese people have endured through several disasters, such as the Beirut explosion, coronavirus pandemic, and cholera outbreak. However, volunteers who have tried to overcome such national disasters develop emotional stress after witnessing people's death. This study was conducted to explore whether volunteers' depression or anxiety mediates the relationship between grief response and post-traumatic stress disorder (PTSD) symptoms. METHODS: We conducted an anonymous online survey study and collected responses from 270 volunteers in Lebanon from March 20-26, 2023. Demographic

Neuropsychiatric complications of traumatic brain injury.

Traumatic brain injury (TBI) may be defined as any extracranial mechanical force to the brain that results in any period of loss of consciousness, any loss of memory for events immediately before or after the event, or any alteration in mental status at the time of the event. The major causes are automobile accidents, falls, sporting injuries, and assaults. Many soldiers returning from combat in Afghanistan and Iraq have also experienced TBI. This article provides an overview of the neuropsychiatric complications of TBI, including impairment of consciousness, posttraumatic amnesia, cognitive d

Learning based PTSD symptoms in persons with specific learning disabilities.

Specific learning disorders (SLD) persist into adulthood. Persons with SLD frequently experience emotional and social difficulties. Following qualitative descriptions of individuals with SLD who experienced learning, as traumatic, we hypothesized that individuals reporting SLD would report higher levels of learning-based post-traumatic-stress-disorder (PTSD) symptoms. In Study 1 (N = 216), participants responded to questionnaires concerning SLD and learning-based PTSD. A separate sample (N = 43) was queried about adjustment disorder symptoms. Study 2 (N = 176) examined if current psychological

Involvement of protein kinase C beta1-serotonin transporter system dysfunction in emotional behaviors in stressed mice.

Selective serotonin reuptake inhibitors are the first-line antidepressants for treating major depressive and post-traumatic stress disorders. These inhibitors directly bind to the serotonin transporter (SERT). Protein kinase C (PKC) is a key regulator of SERT functions as it can attenuate SERT activity through phosphorylation and its subsequent internalization. However, whether PKC-regulated SERT functions are involved in emotional impairment in a mouse model of stress remains unclear. Using a mouse model of swim-induced stress, we investigated whether the PKC-SERT system is involved in emotio

Early childhood PTSD and its correlates: a population-based study from Brazil.

Post-traumatic stress disorder (PTSD) in preschoolers is increasingly recognized, yet population-based studies remain scarce, especially in contexts of high urban violence and social vulnerability. We conducted the Brazilian Preschool Mental Health Study in Embu das Artes, a high-risk urban area, enrolling 1,292 children from public preschools and their caregivers. Standardized face-to-face interviews assessed PTSD symptoms (YCPS for children, PCL-C for parents), socioemotional development (ASQ: SE-2), and sociodemographic variables. Associations were examined using logistic regression models.

Trauma and Pediatric Primary Care.

Children with traumatic stress are more likely to seek and receive care from pediatricians than mental health specialists. Therefore, it is vital that pediatricians are equipped with the knowledge and tools to identify and support these children using trauma-informed approaches to care. In this article, we describe approaches to trauma screening, highlighting the Care Process Model for Pediatric Traumatic Stress as a flexible, evidence-based example of a comprehensive approach to screening and responding to trauma in pediatric primary care. Child and adolescent psychiatrists can play a critica

An Investigation of a Community-Based Phase-I Online Group Trauma Intervention.

This study examined the feasibility of online group therapy for trauma survivors. Community-based participants (N = 178; 64% women; mean age = 43) self-referred for an eight-week psychoeducational, skills-based trauma therapy group, which was offered via secure video conferencing. Participants were diverse in terms of socio-economic status and ethnic identity. Participants completed the PCL-5 at registration (T0), pre-treatment (T1), and post-treatment (T2). T0-T1 reflected a waitlist period; T1-T2 represented the treatment phase. Mixed effects models showed significant overall PCL-5 reduction

Investigational drugs in PTSD.

Post-traumatic stress disorder (PTSD) remains one of psychiatry's most challenging disorders, common, disabling, and biologically complex. Despite decades of research, only sertraline and paroxetine are FDA-approved, both with modest efficacy. Even first-line trauma-focused psychotherapies leave up to half of patients with persistent symptoms. However, advances in neurobiology are reframing PTSD as a disorder of maladaptive stress circuitry, neuroplasticity, and memory reconsolidation, opening new therapeutic possibilities. This comprehensive review examines current PTSD pharmacotherapy, emerg

The central importance of expressive suppression to posttraumatic stress symptoms: A network analysis.

Emotion regulation plays a role in the development of posttraumatic stress symptoms (PTSS). Most research in this area has focused on how people regulate their emotions, a process called self-emotion regulation (self-ER). Self-ER strategies are well-established predictors of PTSS. However, people also regulate their emotions with the help of others, a process called social emotion regulation (social ER). Relative to self-ER, social ER is understudied, and its associations with PTSS and self-ER are unclear. Therefore, we used network analysis to examine the associations among self-ER, social ER

The development of an integrative surf therapy program for trauma survivors: A conceptual model and case illustration.

Psychological trauma can fundamentally disrupt an individual's sense of safety and bodily autonomy, straining existing mental health resources and calling for innovative therapeutic approaches. This article introduces "Tools from the Waves," an integrative surf therapy program designed to enhance emotional resilience, lower the risk of developing PTSD, and help survivors manage or reduce their current symptoms. The 15-week intervention integrates therapeutic surfing with psychoeducational guidance, and group therapy. Therapeutic surfing functions as a "bottom-up" somatic intervention, utilizin

Trajectories of complex posttraumatic stress disorder symptoms change in phase-based treatment, prolonged exposure, and skills-training.

International Classification of Diseases (11th ed.; ICD-11) Complex Posttraumatic Stress Disorder (CPTSD) is a new diagnostic category consisting of PTSD and disturbances of self-organization (DSO) symptoms. This study compares the trajectories of ICD-11 PTSD and DSO symptoms change in three treatments: a phase-based intervention, STAIR Narrative Therapy (SNT), Prolonged Exposure (PE), and skills-training in affect and interpersonal regulation (STAIR). Ninety-two adult patients with ICD-11 CPTSD following childhood abuse were randomly assigned to treatment conditions. Piecewise latent curve mo

Trauma and resilience among non-displaced in the early phase of the war in Ukraine.

This study investigates how civilians who remained in war-affected suburbs of Kyiv following the 2022 outbreak of war in Ukraine cope with trauma and build resilience under conditions of prolonged insecurity. Using a mixed-methods design, we combine standardized assessments of post-traumatic stress symptoms (PCL-5) with in-depth qualitative interviews conducted in suburbs of Kyiv, a region largely neglected in empirical research. Nineteen interviews were analyzed using qualitative content analysis. Two contrasting case vignettes, one with the highest and one with the lowest PTSD symptom load,

Hybrid type 1 randomized controlled trial of technology-assisted stepped care behavioral health intervention for caregivers and children following pediatric traumatic injury.

Pediatric traumatic injury (PTI) has substantial physical and emotional health impacts, sometimes resulting in hospitalization and/or the development of posttraumatic stress disorder (PTSD) and depression in children. Caregivers are a key source of support for children following PTI; however, many caregivers experience their own distress and psychological reactions following injury. The CAARE (Caregivers' Aid to Accelerate Recovery after pediatric Emergencies) model is a technology-assisted, stepped care intervention to support the emotional and behavioral recovery of preadolescent children ho

Birth Trauma and Postpartum Childbirth-related Posttraumatic Stress Disorder: A Narrative Review of Pathology, Risk Factors, and Practice Recommendations.

IMPORTANCE: Trauma and childbirth-related posttraumatic stress disorder (c-PTSD) are underrecognized possible consequences of birth and obstetric care that may affect maternal well-being, parent-infant bonding, and future reproductive decisions. Despite a growing body of research, gaps remain in the implementation of robust screening, timely diagnosis, and trauma-informed practices to prevent and address c-PTSD. OBJECTIVE: Review our current understanding of psychological birth trauma and c-PTSD, emphasizing provider roles in prevention, identification, and management. EVIDENCE ACQUISITION:

Interpersonal Victimization and Post-Traumatic Stress Among Transgender and Gender Expansive People: A Systematic Review.

BACKGROUND: Transgender and gender expansive (TGE) people experience high rates of interpersonal victimization, which has been linked to high rates of post-traumatic stress disorder (PTSD, a highly disabling and under-studied mental illness among TGE people). This systematic review identifies, classifies, critically appraises, and synthesizes the peer-reviewed literature describing the association between interpersonal victimization and post-traumatic stress among TGE people. This review collates what is known about the associations between victimization and PTSD among TGE people and makes rec

[A qualitative study of the experience of interactive guidance therapy following a traumatic childbirth].

This study explores the experience of interactive guidance therapy with video feedback following childbirth-related trauma (PTSD-A), an underrecognized condition that affects the mother-child relationship. Through semi-structured interviews with nine mothers from the Evagit study (Evaluation of Interactive Guidance for Perinatal Trauma), thematic analysis reveals that this approach provides a safe framework for revisiting the trauma, strengthening mother-infant interactions, and supporting the development of maternal self-esteem.

Posttraumatic stress symptom reduction after gender-affirming medical interventions: A longitudinal study.

Transgender and gender expansive (TGE) people experience high levels of posttraumatic stress disorder (PTSD). Gender-affirming medical interventions (GAMIs) may be helpful in reducing PTSD symptoms. This study examined whether GAMIs were associated with reduced PTSD symptom severity longitudinally and whether the association was mediated by gender minority stress (GMS) and depressive symptoms. TGE participants (N = 2,514) from a national cohort study were included if they attained at least one GAMI at one or more points during 2019-2022. Two longitudinal models were run, adjusted for time and
