Risk Factors for Massive Intraoperative Blood Loss During Posterior Spinal Instrumentation and Fusion in Children: A Retrospective Cohort Study.

Background: To investigate the risk factors for and prognostic implications of massive blood loss during posterior spinal instrumentation and fusion (PSIF) in pediatric patients with scoliosis. Methods: We retrospectively analyzed the electronic medical records of 460 children who underwent scheduled PSIF under general anesthesia between June 2021 and January 2024. Patients were grouped based on intraoperative blood loss: massive (estimated blood loss [EBL]/estimated blood volume [EBV] ≥ 30%) and nonmassive (EBL/EBV < 30%). Perioperative parameters were compared. Univariate and multivariate lo

Analgesic efficacy of an ultrasound-guided single puncture, dual injection, anterior approach targeting branches of the lumbosacral plexus versus lumbar epidural for unilateral lower limb orthopedic surgery: A randomized controlled study.

BACKGROUND AND AIMS: The traditional approach to combined lumbar and sacral plexus block require multiple injections with the patient in the lateral or prone position. We designed a study to compare the novel single puncture, dual injection, anterior approach targeting the branches of the lumbosacral plexus versus lumbar epidural for postoperative analgesia. MATERIAL AND METHODS: The present study enrolled 134 adult patients scheduled for unilateral lower limb orthopedic surgery under combined spinal epidural anesthesia without epidural activation. Study participants were randomly assigned to

Perioperative management of spinal cord stimulators in patients undergoing non-SCS spine surgery: A retrospective cohort study.

BACKGROUND: Spinal cord stimulation (SCS) use has expanded, so perioperative teams increasingly encounter these devices during unrelated spine surgery, where leads, anchors, and the generator may lie within or adjacent to the operative field. Real-world perioperative management of these devices remains uncharacterized. OBJECTIVE: To characterize documented perioperative SCS management in adults undergoing non-SCS spine surgery and to benchmark prespecified management elements against current practical guidance. METHODS: In this single-center retrospective pilot cohort study, we identified ad

Pursuing an Optimal Regional Analgesia Strategy for Total Knee Arthroplasty: A Double-Blind Randomized Controlled Study of Femoral Triangle, Adductor Canal, and Dual Subsartorial Blocks (FAD Trial).

Background Total knee arthroplasty (TKA) is often associated with intense postoperative pain, which can delay mobilization and hinder recovery. While motor-sparing blocks such as the femoral triangle block (FTB) and adductor canal block (ACB) are commonly used, both offer incomplete analgesic coverage. To overcome these limitations, the dual subsartorial block (DSB) was introduced as a procedure-specific, motor-sparing technique that combines and modifies FTB and ACB into a dual-injection approach for enhanced efficacy. Objective The objective of this study is to compare the analgesic efficacy

Comparison of perioperative analgesia between posterior stabilized and posterior cruciate retaining prostheses following total knee arthroplasty.

BACKGROUND: Posterior-stabilized (PS) and posterior cruciate retaining (CR) prostheses constitute the most widely used knee prostheses. This study aimed to determine the effects of PS and CR prostheses on postoperative analgesia following total knee arthroplasty (TKA) and provide a reliable clinical basis for optimizing postoperative analgesic effects. METHODS: Overall, 120 consecutive patients were enrolled in this study and classified into PS (n = 60) and CR (n = 60) groups according to the type of prosthesis used. All patients underwent TKA along with spinal anesthesia. All patients receiv

Clinical outcomes following elective lumbar spine surgery in patients living with dementia.

BACKGROUND CONTEXT: As the population ages and surgical techniques improve, more elderly patients with dementia are being considered for treatment of spinal disorders. However, the combined impact of procedural intensity and anesthesia exposure can exacerbate surgical risks, leading to more complex recoveries and postoperative morbidity. PURPOSE: To compare postsurgical outcomes of patients living with dementia who received elective lumbar spine surgery with a comparable group of patients without dementia. STUDY DESIGN/SETTING: Retrospective analysis using national Medicare claims. PATIENT

Total knee arthroplasty in patients with degenerative spine disease: does spinal fusion affect outcomes? A matched comparative analysis using a national database.

BACKGROUND: The need for total knee arthroplasty (TKA) and spinal fusion (SF) for degenerative spine disease (DSD) is increasing. However, it is still unknown if prior SF for DSD impacts outcomes following TKA. This study aims to fill this gap by comparing the risk of complications and revisions in patients undergoing TKA with DSD between patients with and without SF. METHODS: This study is a retrospective review of the PearlDiver Mariner Database between 2010 and 2020. On the basis of whether or not patients had had prior SF, the patients undergoing TKA were divided into two groups: patients

Minimally Invasive Adhesiolysis for the Incarcerated Gravid Uterus: A Case Report of Early Second Trimester Intervention.

Incarceration of the gravid uterus is a rare obstetric complication in which the retroverted uterus becomes trapped in the pelvis, often resulting in maternal and fetal morbidity if not recognized and managed early. We present the case of a 36-year-old multigravida at 15 weeks' gestation with no prior abdominal surgeries who initially presented with urinary retention and was diagnosed with an incarcerated gravid uterus. Multiple attempts at manual reduction under spinal and general anesthesia were unsuccessful. Intraoperative transabdominal ultrasound demonstrated a live intrauterine pregnancy

Postoperative complications, length of stay, and discharge disposition following single-level anterior lumbar interbody fusion in elderly and octogenarian patients.

BACKGROUND: Anterior lumbar interbody fusion (ALIF) has become a widely accepted treatment for degenerative lumbar spine pathologies, with increasing prevalence due to its effectiveness in restoring lumbar lordosis and improving spinal balance. This study aims to evaluate postoperative complications, length of stay (LOS), and discharge disposition following ALIF across different age groups. METHODS: A total of 92,800 weighted cases of patients aged 50 and older underwent single-level ALIF in the National Inpatient Sample (NIS) from 2016 to 2020. Patients were stratified into age cohorts (50-6

Impact of Activity-Based Training on Bowel Function in a Rat Model of Spinal Cord Injury.

Significant bowel-related issues after spinal cord injury (SCI) that affect morbidity and quality of life (QOL) include diminished bowel motility, loss of sphincter control, gastric ulcers, autonomic dysreflexia, pain, diarrhea, constipation, and fecal incontinence. Clinical diagnoses and research in humans have largely relied on anorectal manometry (ARM) procedures to increase understanding of the functional effects of SCI on colorectal motility and defecation physiology. Recent pre-clinical rodent studies have also used ARM to further our understanding of bowel-related dysfunctions post-SCI.

Percutaneous auricular nerve stimulation (neuromodulation) for the treatment of pain: A proof-of-concept case report using total joint arthroplasty as a surrogate for battlefield trauma.

BACKGROUND: There are few effective pain treatments following trauma on the battlefield other than opioids, which are limited by respiratory depression. Ultrasound-guided percutaneous peripheral nerve stimulation ("neuromodulation") has been proposed as an analgesic, but requires physician-level skills, advanced equipment, and an hour to administer. In contrast, percutaneous auricular neuromodulation may be placed by a medic in the field under nonsterile conditions in a few minutes, theoretically provides analgesia for any anatomic location, has no side effects, and no significant risks. An Integrated Physiotherapeutic Approach With Virtual Reality in Schizophrenic Patients With Ipsilateral Femoral Shaft and Intertrochanteric Fractures: A Case report.

Femoral shaft fracture occurs through the diaphyseal region of the femur. The intertrochanteric (IT) fracture is an extracapsular fracture that occurs through the proximal metaphyseal area of the femur. The ipsilateral femoral shaft and IT fractures happen in rare cases following high-energy trauma. These fractures are difficult to manage due to their complexity. They are usually managed surgically and require prompt physiotherapeutic management postoperatively. The postoperative complications involve pain, stiffness, reduced muscle strength, deep vein thrombosis, muscular weakness, and atroph
