The risk of endovascular thrombectomy in acute ischemic stroke patients with large vessel occlusions harboring unruptured intracranial aneurysms.

BACKGROUND AND PURPOSE: Endovascular thrombectomy (EVT) for acute ischemic stroke (AIS) with large vessel occlusions (LVO) has significantly improved over the past decade. Unruptured intracranial aneurysms (UIAs) can potentially increase the risk of hemorrhage during EVT procedures. Given that limited data exist on this topic, this study investigated the safety of EVT in AIS patients with LVO who also harbor UIAs. METHODS: We analyzed prospectively collected data on consecutive AIS patients treated with EVT at a comprehensive stroke center in Southeast China between 2016 and 2023. Digital sub

Vein of Galen Malformation with Pial Arteriovenous Fistula in a 12-month-old Child.

Pial arteriovenous fistulas are extremely rare vascular malformations of the brain (accounting for approximately 1.6% of vascular malformations of the brain) and are classified as Galenic and non-Galenic pial fistulas. We present a rare association of a mural-type vein of Galen malformation with a pial fistula involving the precentral branch of the right middle cerebral artery (MCA) in a 12-month-old infant. A 12-month-old preterm male infant, born vaginally with a delayed cry (birth weight 1.2 kg), presented with progressive macrocephaly, recurrent upper respiratory tract infections, developm

Case Report: Fatal massive hemoptysis secondary to pulmonary actinomycosis: a diagnostic and therapeutic challenge in a multidisciplinary approach.

BACKGROUND: Actinomyces infections tend to involve the head and neck, while lung infections are relatively rare, and fatal hemoptysis due to pulmonary actinomycosis is even rarer. CASE PRESENTATION: A 74-year-old male patient who presented with a cough with intermittent hemoptysis for 17 months was admitted to the hospital for the fourth time on June 24, 2024. At the first visit (15 months ago), a computed tomography (CT) scan of the chest revealed a high-density shadow in the right middle bronchus (suggesting a foreign body to be drained) and an infected lesion in the lower lobe of the right

Exploring the relationship between post-contrast acute kidney injury and different baseline creatinine standards: A retrospective cohort study.

OBJECTIVE: According to previous studies, the incidence of post-contrast acute kidney injury(PC-AKI) in diabetic is far higher than that in the general population. Therefore, we explored the relationship between the incidence of PC-AKI and different baseline serum creatinine (SCr) levels, and determined the relationship between PC-AKI and different types of contrast media (CMs), different doses of CM, and different examination methods in this specific population. MATERIALS AND METHODS: Patients with diabetes in whom CM was used between 2010 and 2020 at our institution were included. Participa

Unidentified bright objects of spleen on arterial phase CT: mimicker of splenic vascular injury in blunt abdominal trauma.

PURPOSE: We have described unidentified bright objects of spleen (UBOS), a hitherto undescribed entity, as hyperdense areas on arterial phase (AP) computed tomography (CT) seen in relation to splenic lacerations and are isodense to the normal parenchyma on portal venous phase with no correlate on digital subtraction angiography (DSA). UBOS mimic splenic vascular injuries like active contrast extravasation and pseudoaneurysm and need to be differentiated from them as it would have implications on patient management. We undertook this study to identify CT features of UBOS that can differentiate

A Comparison of Stent Implant versus Medical Treatment for Severe Symptomatic Intracranial Stenosis: A Controlled Clinical Trial.

BACKGROUND: Atherosclerotic stenosis of the major intracranial arteries is the most common cause of ischemic stroke. There are limited treatments for severe intracranial stenosis, and stent placement versus medical treatment remains controversial. The aim of this study was to compare functional outcomes of these two modalities in patients with severe symptomatic intracranial stenosis. METHODS: At a single center, between 2008 and 2011, patients with angiographically demonstrated severe (70-90%) symptomatic intracranial atherosclerosis were divided into two groups: group A, which received only

Aspirin resistance in patients with hemodynamic cerebral ischemia undergoing extracranial-intracranial bypass surgery.

BACKGROUND: Aspirin (acetylsalicylic acid, ASA) is the treatment of choice for prevention of vascular events in symptomatic steno-occlusive cerebrovascular disease (CVD). Cerebral revascularization using standard extracranial-intracranial (EC-IC) bypass surgery may be used to revert hemodynamic compromise. Aspirin is prescribed as standard medication in order to avoid bypass failure. Accumulating evidence of an increased risk of major adverse clinical events led to this study, in which we aimed to assess the prevalence of aspirin resistance and prothrombotic disorders among patients scheduled
