Eighteen of 62 children diagnosed with Langerhans' cell histiocytosis at the Children's Hospital of Pittsburgh (Pa) between 1970 and 1986 demonstrated ear and temporal bone involvement. In six children, such otologic disease was their sole presenting manifestation. Common signs and symptoms included aural discharge, postauricular swelling, and conductive hearing loss. The otologic findings in these children, if not investigated properly, could easily be attributed to acute or chronic infectious ear disease. Computed tomography with contrast enhancement proved to be particularly valuable as a d

By 1986 the central analysis team of this on-going multinational survey had received a total of 10,682 cases for analysis and had accepted 10,320. In all, some 26 centres in 17 countries, involving over 200 doctors, had participated in this survey. A common protocol was used for data collection; around 98% of all possible data was recorded (using precirculated definitions) and analysed via a computer-aided system in Leeds, England. The construction and format of a series of computer-aided decision-support and teaching programs has been described in an earlier (1982) report. These programs are

Imaging techniques used for the care of cutaneous melanoma patients have greatly changed over the past century, from symptom-driven radiography toward a multimodality framework integrated for staging, directing surgery, and systemic therapy, and surveillance. Historically, clinical evaluation and skin exams have been the tenets of melanoma diagnosis and staging. In recent years, noninvasive imaging, such as dermoscopy, total-body photography and reflectance confocal microscopy, has expanded the diagnostic toolset for primary melanoma detection. Concurrently, several imaging techniques have bee

Molecular dynamics (MD) simulation is a computational technique based on Newtonian mechanics and statistical physics that tracks molecular motion and ligand-target recognition at atomic resolution. In recent years, with advances in computational resources and continuous optimization of simulation algorithms, the application of MD simulations in traditional Chinese medicine (TCM) research and development has expanded steadily. Because TCM involves chemically diverse constituents, multiple targets, metabolic transformation, and pathway-level regulation, its mechanisms are difficult to interpret

Traditional fall detection methods face significant limitations in complex environments, particularly under occlusion and poor lighting conditions. To address these challenges and enhance the detection accuracy in intelligent real-time monitoring systems, this study proposes an optimized BMR-YOLO framework based on the YOLOv8n architecture. Our approach introduces four strategic improvements to effectively overcome environmental constraints. First, we enhance the backbone network by incorporating a BiFormer vision transformer with dual-layer routing attention, enabling dynamic allocation of co

Brain-computer or brain-machine interface technology allows humans to control machines using their thoughts via brain signals. In particular, these interfaces can assist people with neurological diseases for speech understanding or physical disabilities for operating devices such as wheelchairs. Motor-imagery tasks play a basic role in brain-computer interfaces. This study introduces an approach for classifying motor-imagery tasks in a brain-computer interface environment, which remains a challenge for rehabilitation technology using electroencephalogram sensors. Methods used and developed for

Breast cancer liver metastases (BCLM) are usually unresectable and difficult to treat with systemic chemotherapy. A major reason for chemotherapy failure is that BCLM are typically small, avascular nodules, with poor transport and fast washout of therapeutics from surrounding capillaries. We have previously shown that nanoalbumin-bound paclitaxel (nab-PTX) encapsulated in porous silicon multistage nanovectors (MSV) is preferentially taken up by tumour-associated macrophages (TAM) in the BCLM microenvironment. The TAM alter therapeutic transport characteristics and retain it in the tumour vicin

With increasing use of left ventricular assist devices (LVAD) it is critical to devise strategies to optimize LVAD speed while controlling mean arterial pressure (MAP) and flow according to patient physiology. The complex interdependency between LVAD speed, MAP, and flow frequently makes optimization difficult under clinical conditions. We propose a method to guide this procedure in silico, narrowing the conditions to test clinically. A computational model of the circulatory network that simulates HF and LVAD support, incorporating LVAD pressure-flow curves was applied retrospectively to anony

Traumatic brain injury (TBI) is one of the injuries that can bring serious consequences if medical attention has been delayed. Commonly, analysis of computed tomography (CT) or magnetic resonance imaging (MRI) is required to determine the severity of a moderate TBI patient. However, due to the rising number of TBI patients these days, employing the CT scan or MRI scan to every potential patient is not only expensive, but also time consuming. Therefore, in this paper, we investigate the possibility of using electroencephalography (EEG) with computational intelligence as an alternative approach

KEY POINTS: Reduced computational models are used to test effects of loss of inhibition to the Kölliker-Fuse nucleus (KFn). Three reduced computational models that simulate eupnoeic and vagotomized respiratory rhythms are considered. All models exhibit the emergence of respiratory perturbations associated with Rett syndrome as inhibition to the KFn is diminished. Simulations suggest that application of 5-HT1A agonists can mitigate the respiratory pathology. The three models can be distinguished and tested based on their predictions about connections and dynamics within the respiratory circuit

PURPOSE: Computational fluid dynamics have paradigm shifting potential in understanding the physiological flow of fluids in the human body. This translational branch of engineering has already made an important clinical impact on the study of cardiovascular disease. We evaluated the feasibility and applicability of computational fluid dynamics to model urine flow.
MATERIALS AND METHODS: We prepared a computational fluid dynamics model using an idealized male genitourinary system. We created 16 hypothetical urethral stricture scenarios as a test bed. Standard parameters of urine such as pressu

Patient-specific biomechanical modeling of the cardiovascular system is complicated by the presence of a physiological pressure load given that the imaged tissue is in a pre-stressed and -strained state. Neglect of this prestressed state into solid tissue mechanics models leads to erroneous metrics (e.g. wall deformation, peak stress, wall shear stress) which in their turn are used for device design choices, risk assessment (e.g. procedure, rupture) and surgery planning. It is thus of utmost importance to incorporate this deformed and loaded tissue state into the computational models, which im

BACKGROUND: Reliability of computational models for cardiovascular investigations strongly depends on their validation against physical data. This study aims to experimentally validate a computational model of complex congenital heart disease (i.e., surgically palliated hypoplastic left heart syndrome with aortic coarctation) thus demonstrating that hemodynamic information can be reliably extrapolated from the model for clinically meaningful investigations.
MATERIALS AND METHODS: A patient-specific aortic arch model was tested in a mock circulatory system and the same flow conditions were re-

BACKGROUND: Difficulties associated with implementing gene therapy are caused by the complexity of the underlying regulatory networks. The forms of interactions between the hundreds of genes, proteins, and metabolites in these networks are not known very accurately. An alternative approach is to limit consideration to genes on the network. Steady state measurements of these influence networks can be obtained from DNA microarray experiments. However, since they contain a large number of nodes, the computation of influence networks requires a prohibitively large set of microarray experiments. Fu

OBJECTIVE: Our goal was to compare the patterns of brain injury detected by computed tomography, conventional MRI (T1- and T2-weighted sequences), and diffusion-weighted MRI in a cohort of term newborns with neonatal encephalopathy studied uniformly with all 3 modalities on the third day of life.
METHODS: Term newborns (> or =36 weeks' gestation) admitted to our center with neonatal encephalopathy were scanned with computed tomography, MRI, and diffusion-weighted MRI at 72 (+/-12) hours of life (n = 48). Each modality was scored independently of the other with previously validated scoring sys

BACKGROUND: Maxillary sinusitis may develop from the extension of periodontal disease. In this case, reconstructed three-dimensional images from multidetector spiral computed tomographs were helpful in evaluating periodontal bony defects and their relationship with the maxillary sinus.
METHODS: A 42-year-old woman in good general health presented with a chronic deep periodontal pocket on the palatal and interproximal aspects of tooth #14. Probing depths of the tooth ranged from 2 to 9 mm, and it exhibited a Class 1 mobility. Radiographs revealed a close relationship between the root apex and

OBJECTIVE: Thoracoabdominal injuries commonly occur as a result of motor vehicle crashes. In order to design occupant protection systems that reduce risk of injury, researchers are using a variety of tools, including computational human body models. Though research has been conducted to provide morphological and volumetric data for the thoracoabdominal cavity of the average male, there is currently an interest in developing models for a wider range of occupants. One particular cohort of interest is the small female by stature and weight because of their use in restraint system development. Geo

BACKGROUND: The clinical translation of AI-assisted surgical navigation and digital twin modeling is hindered by the absence of a standardized, reusable anatomical spatial reference framework, as current one-dimensional or two-dimensional localization methods for the Segond fracture bed inadequately represent three-dimensional bony spatial relationships for intelligent navigation systems.
OBJECTIVE: To meet the needs of AI- and digital twin-driven precision orthopedics, this study constructs a CT-based three-dimensional coordinate system of the knee using tibial landmarks, applies a normaliza

OBJECTIVES: Transformer-based models for clinical prediction using longitudinal electronic health record (EHR) data are often developed with large feature sets and long patient histories under the assumption that more data improves performance. However, high-dimensional inputs and long sequences increase computational burden, potentially limiting scalability. We evaluated how feature dimensionality and sequence length affect predictive performance, calibration, risk stratification, and computational efficiency in EHR prediction.
MATERIALS AND METHODS: Using longitudinal EHR data from adults w
