Aesthetic mental and cervical reconstruction after severe acne inversa by using a bilateral pedicled expanded forehead flap.

Acne inverse (AI), also known as hidradenitis suppurativa, is characterized by inflammatory nodules, fistulating sinus tracts, and painful skin abscesses. The severe AI often produces disfiguring scars influenced in both the appearance and function, especially in the facial and cervical regions. There might be difficulties in the situation for surgical treatment. This report described a 26-year-old man with severe scarring contractures in the neck and mandible regions after a long-term AI treated successfully with a bilateral pedicled expanded forehead flap. With the achievement of mental cerv…

Arthrogryposis and congenital absence of the anterior cruciate ligament: a case report.

Arthrogryposis is a non-progressive disorder with congenitally soft tissue contractures affecting two or more joints. It can be sporadic, or inherited in an autosomal dominant or recessive fashion. Typically, in the upper limb, the shoulder is in adduction and internal rotation, the elbow is in extension with a lack of skin crease, and the wrist is in flexion with ulnar deviation. In the lower limb, the hip is in flexion, abduction and external rotation, the knee is contracted, with resistant clubfoot and congenital vertical talus. We report a case of a man and his son with a new form of autos…

Three-dimensional realisation of muscle morphology and architecture using ultrasound.

Two-dimensional B-mode ultrasound imaging and motion tracking were combined to generate three-dimensional reconstructions of the medial gastrocnemius. Architectural and morphological features of this muscle could be visualised. The length of the gastrocnemius belly was measured in normally (ND) developing children and in children with spastic diplegic cerebral palsy (SDCP) who had plantarflexion contractures. Using a random effects linear model we demonstrated that the gastrocnemius muscle bellies of children with SDCP were shorter than those of ND children (P = 0.001) even when corrected for …

Far lateral approach for foramen magnum lesions.

Twelve patients with lesions in the anterior or anterolateral regions of foramen magnum were treated through the far lateral approach. The patients presented with neck pain, dysesthesia, quadriparesis, numbness, respiratory distress, and spastic contractures. Most lesions were meningiomas and neurofibromas, with one patient each with a posterior inferior cerebellar artery aneurysm, neurenteric cyst, and chordoma. All mass lesions were excised totally and the aneurysm was clipped. Three patients had severe respiratory problems preoperatively and two of them died. The other patients made a satis…

Marden-Walker syndrome versus isolated distal arthrogryposis: evidence that both conditions may be variable manifestations of the same mutated gene.

In this report we present evidence that Marden-Walker syndrome and isolated distal arthrogryposis may be variable manifestations of the same entity. We describe the clinical and pathological findings in two affected siblings, the first two children of normal, non-consanguineous parents. The first child, a female, presented a typical Marden-Walker syndrome with Dandy Walker type CNS malformation, corpus callosum hypoplasia and enlarged ventricles. In the second pregnancy, echographic examination revealed joint contractures of the hands and feet. Fetopathological examination revealed a normoceph…

Microcephaly, characteristic facies, joint abnormalities, and deficient leucocyte chemotaxis: a further case of the syndrome of Say et al.

We report on a 13 year old boy with microcephaly, sloping forehead, prominent nose, scoliosis, and flexion contractures involving the elbows and knees. The patient showed severe mental and growth retardation. Since birth and up to the present he has suffered from multiple and varied infections. Immunological studies showed a marked decrease in leucocyte chemotaxis. Clinical and laboratory findings confirm the similarity of this case to the two brothers described by Say et al. We have not found any descriptions of similar patients. The purpose of this paper is to contribute to the phenotypic de…

Prenatal findings in chondrodysplasia punctata, tibia-metacarpal type.

Chondrodysplasia punctata, a skeletal dysplasia with craniofacial dysmorphism and joint contractures can occur with rhizomelia, mesomelia or both. The rhizomelic form is generally lethal, whereas one form of mesomelic chondrodysplasia punctata has been described that is associated with a presumably normal lifespan and intelligence. We describe a case of a fetus suspected prenatally of having rhizomelic chondrodysplasia punctata, who was subsequently diagnosed at 1.5 years of age to have the tibia-metacarpal form of chondrodysplasia punctata. The prenatal sonographic findings of second-trimeste…

Continuous spinal anesthesia for cesarean delivery in a patient with arthrogryposis multiplex congenita. A clinical report.

BACKGROUND AND OBJECTIVES: Arthrogryposis multiplex congenita (AMC) is a syndrome, diagnosed at birth, which presents with multiple joint contractures. Because this disease often progresses until there is dysfunction of multiple organ systems, it may have an impact on the anesthetic management. There are few anesthetic reports of this disease in the adult pregnant patient. METHODS: A patient with AMC who underwent elective cesarean delivery with continuous subarachnoid bupivacaine anesthesia. RESULTS: The patient delivered a healthy infant and recovered uneventfully from the anesthetic with …

Fetal seizures: further observations.

BACKGROUND: Previous reports of ultrasonographically diagnosed seizure activity in utero have described fetuses with obvious, gross, tonic-clonic movements of trunk and extremities. CASES: Three fetuses with akinesia deformation sequence are described: two with arthrogryposis multiplex congenita and one with Pena-Shokeir syndrome. Each was demonstrated by serial real-time ultrasound examinations to have joint contractures, absent fetal breathing motions, and lack of gross movements other than sonographically evident subtle seizure activity. CONCLUSION: Fetal seizure activity can be subtle wh…

Quantitative analysis of hip dysplasia in cerebral palsy: a study of radiographs and 3-D reformatted images.

Cerebral palsy patients (31 hips) were evaluated using radiographic and three-dimensional (3-D) images to quantify hip anatomy. The 3-D images overcome distortions caused by joint contractures. Changes were more pronounced in the non-ambulators and characterized by shallow sockets with increased neck-shaft angles. These hips tended to subluxate in a posterior-superior direction and most had defects in the femoral heads. Ambulators had increased femoral anterversion but other hip parameters tended to improve with age. The 3-D measures of roof steepness and socket depth were found to correlate s…

[Indications for arthrodesis of the knee in Africa (apropos of 24 cases)].

29 knees arthrodesis was performed in 24 patients at the Orthopaedic Department in Dakar. The indications were: post-operative complications following poliomyelitis knee contractures in 11 cases, knee fusion following septic arthritis in 9 cases. The miscellaneous indications were tuberculosis of the knee (5 cases), post traumatic injuries (3 cases) and 1 case of knee osteo-arthritis. The authors pointed out the nature and diversity of those different indications and, now uncommon in developed countries, where knee arthroplasty is primary indication, whereas, arthrodesis is indicated for failu…

[A variant of the epilepsia partialis continua syndrome in a child with hypoplasia of the internal carotid artery].

The authors describe a case of a 13-year-old adolescent. Being seven years old, he developed the epileptic syndrome associated with persistent focal and secondary generalized fits refractory to the treatment. Later the clinical picture became very suggestive of the syndrome Epilepsia partialis continua. In the course of the illness, the patient developed the diencephalic syndrome associated with arterial hypertension and early puberty, right-handed central hemiparesis, contractures, partial motor aphasia, and a decrease of the intellect. Computer-aided axial tomography revealed progressive dev…

Familial myopathy with scapulohumeral distribution, rigid spine, cardiopathy and mitochondrial abnormality.

A 37-year-old woman with scapulohumeral muscular atrophy, rigid spine and cardiopathy is reported. Muscle weakness, advanced atrioventricular block and contractures at the neck, elbows and ankles had occurred during her childhood. An autosomal dominant mode of inheritance was suggested because her mother, sister and brother had the same disorder. Pleomorphic mitochondria had accumulated in the subsarcolemmal space of the skeletal muscle. There was no evident enzyme defect in the mitochondrial electron transport system. Although the clinical features had some similarity with those of Emery-Drei…

The Washington Regimen: rehabilitation of the hand following flexor tendon injuries.

This article describes the use of the "Washington Regimen" of early controlled motion in the rehabilitation of flexor tendon injuries of the hand. This regimen is derived from a combination of Kleinert's controlled active extension with rubber-hand passive flexion, Duran's controlled passive techniques, and the modification of the Kleinert orthosis that uses a palmar pulley system. Based on results of clinical investigations, this regimen of early controlled motion appears effective in inhibiting peritendinous scarring, joint contractures, and other complications that commonly occur secondary …

Congenital midline cervical cleft.

We present a case of congenital midline cervical cleft (CMCC), a sporadic lesion with a predilection for white females. It characteristically occurs in the midline ventral neck and consists of a cephalad skin tag, a mucosal surface, and a caudal sinus. There is an associated submucosal fibrous cord and interwoven bundles of skeletal muscle. This is occasionally tethered to the mandible and/or sternum causing neck contractures. CMCC is usually isolated, but rare cases have been associated with midline clefts of the tongue, lower lip, mandible, and sternum. The embryogenesis is unclear but argum…

Hip replacement for adults with unreduced congenital dislocation. A new surgical technique.

Total hip replacement for adults with unreduced congenital dislocation presents a difficult problem because soft-tissue contractures usually prevent sitting at the normal anatomical level. Extensive soft-tissue division or a high-level acetabulum leads to reduced function and poor fixation of the components. We describe a new technique for hip replacement in such cases. The shortened abductors and flexors are released proximally and excision of the upper third of the ilium allows them to be repaired without tension, while providing bone graft to reconstruct the acetabular roof. We report 12 su…

Myopathy in Addison's disease.

Since the first description of primary adrenocortical insufficiency by Thomas Addison in 1855 several large series of patients with Addison's disease have been published. The common signs and symptoms include: weakness, hyperpigmentation, weight loss, gastrointestinal complaints, and hypotension. It is rare for patients with Addison's disease to present with musculoskeletal symptoms including flexion contractures, hyperkalaemic neuromyopathy, Guillain-Barré syndrome, migratory myalgia, sciatica-like pain, and low back pain. Myopathy has not been previously described in Addison's disease. Herei…

Resection arthroplasty with Swanson's implant for posttraumatic stiffness of proximal interphalangeal joints.

Proximal interphalangeal joint resection arthroplasty with Swanson's implant should be considered as a simplified new hinge joint with a simplified shorter but isometric extensor system which will work if the flexor system is intact. The procedure is indicated in cases of severe, ill-tolerated, fixed joint contractures. Functional improvement with restoration of some range of motion can be expected. Patients should be aware that they will not regain a complete range of motion at the PIP joint, there will be limited involvement of the distal interphalangeal joint, and there may be some discomfo…
