Streptomycin was injected subcutaneously into guinea pigs (n = 12) with unilateral endolymphatic hydrops. In the unoperated ears, sensory cells were degenerated at the basal ends of all cochleas and to varying extents in the vestibular sense organs. Recurrent middle ear disease may carry a high probability of complications, and there is no doubt of the necessity of revision surgery for either cholesteatoma or tumor. The vascular permeability of the middle ear mucosa of chinchillas was examined by light and electron microscopy using horseradish peroxidase (HRP). Two kinds of experimental otitis media were used: serous otitis media (SOM) produced by eustachian tube obstruction and purulent otitis media (POM) produced by inoculation with Streptococcus pneumoniae type 7F. Between February, 1985 and February, 1987, 49 eyes of 47 patients underwent epikeratoplasty for one of the following indications: adult aphakia (21 eyes; 20 patients), pediatric aphakia (10 eyes; 10 patients), myopia (7 eyes; 6 patients), and keratoconus (11 eyes; 11 patients). 1. Rabbit ears were single-pass perfused with a cell-free medium. Arterial pressure, oxygen consumption, and lactate production increased with flow rate. 2. Methyl salicylate was hydrolysed with an apparent Vmax of 1.5 nmol/min per cm2 surface area, a rate about 25 times greater than after arterial administration. 3. Estimation of the Km was not possible, due to oedema developing at arterial concentrations greater than 100 microM methyl salicylate. 4. In many eukaryotic cells G-proteins play a key role in signal transduction through outer cell membranes. To study this pathway in the auditory organ of mammals we examined tissue preparations from the stria vascularis and the organ of Corti from the guinea pig inner ear. The activity of adenylate cyclase was measured by stimulation at the site of the enzyme, the hormone receptors and the modulating G-proteins. The responses of the mouse ear to heat alone, X-rays alone or X-rays combined with heat were measured at 10 months after initial X-ray treatments (19 Gy or 10 X 3.8 Gy), which caused similar acute reactions. To determine if the cochlear implant can enable sound detection in children with a congenitally deformed cochlea, we reviewed warble-tone thresholds in five ears of four children in the implant clinical trials program. Of the five ears, there were two common cavity deformities, two cochlear hypoplasias, and one incomplete partition. The kinetics of penicillin diffusion in serum and middle ear effusions (MEE) was studied in experimentally induced serous otitis media (SOM) and purulent otitis media (POM) after systemic injection of procaine-penicillin G. A higher rate of diffusion of penicillin into the middle ear cavity was found in POM than in SOM. The time to reach peak concentrations in MEE was also shorter in POM. Elimination of penicillin from the middle ear cavity was slower in SOM than in POM.

We have continued to employ transmastoid labyrinthectomy since reporting our results for the first 15 patients.
