Complex genetic dependencies among growth and neurological phenotypes in healthy children: Towards deciphering developmental mechanisms.

The genetic mechanisms of childhood development in its many facets remain largely undeciphered. In the population of healthy infants studied in the Growing Up in Singapore Towards Healthy Outcomes (GUSTO) program, we have identified a range of dependencies among the observed phenotypes of fetal and early childhood growth, neurological development, and a number of genetic variants. We have quantified these dependencies using our information theory-based methods. The genetic variants show dependencies with single phenotypes as well as pleiotropic effects on more than one phenotype and thereby po

Intrauterine Growth Restriction Due to Gestational Diabetes: From Pathophysiology to Diagnosis and Management.

Intrauterine growth restriction (IUGR) represents a condition where the fetal weight is less than the 10th percentile for gestational age, or the estimated fetal weight is lower than expected based on gestational age. IUGR can be caused by various factors such as maternal, placental or fetal factors and can lead to various complications for both the fetus and the mother, including fetal distress, stillbirth, preterm delivery, and maternal hypertension. Women with gestational diabetes are at an increased risk of developing IUGR. This article reviews the different aspects of gestational diabetes

Recent progress in understanding the pathogenesis of fetal and neonatal alloimmune thrombocytopenia.

Fetal and neonatal alloimmune thrombocytopenia (FNAIT) occurs in c. 1 in 1000 births and is caused by maternal antibodies against human platelet alloantigens that bind incompatible fetal platelets and promote their clearance from the circulation. Affected infants can experience bleeding, bruising and, in severe cases, intracranial haemorrhage and even death. As maternal screening is not routinely performed, and first pregnancies can be affected, most cases are diagnosed at delivery of a first affected pregnancy. Unlike its erythrocyte counterpart, Haemolytic Disease of the Fetus and Newborn, t

Fast-scan magnetic resonance imaging in fetal visualization.

Magnetic resonance imaging has potential as an imaging technique in obstetrics. Its application has been limited by the amount of image degradation that occurs as a result of fetal movement during the long imaging times. By use of a fast-scan imaging technique good images are obtained of fetal anatomy in 3 to 14 seconds per image, depending on the setting used. Different organs could be visualized by varying the setting. Magnetic resonance imaging complements the role of ultrasonography in fetal visualization. It also allows clear fetal imaging in such high-risk situations as maternal obesity

Management and outcomes of pregnancies complicated by human B19 parvovirus infection: a prospective study.

During a large statewide outbreak of fifth disease in Connecticut in 1988, 39 pregnant women were identified who had serologic evidence of recent human B19 parvovirus infection. The patients were followed up prospectively with targeted fetal ultrasonographic examinations to detect signs of fetal hydrops. Of these 39 pregnant women, 37 had healthy infants and two patients had miscarriages. None of the fetuses developed hydrops. We propose that pregnant women exposed to B19 parvovirus be tested for evidence of IgG and IgM B19-specific antibodies and that targeted fetal ultrasonography be conside

Maternal dietary fatty acids and their roles in human placental development.

Fatty acids are essential for feto-placental growth and development. Maternal fatty acids and their metabolites are involved in every stage of pregnancy by supporting cell growth and development, cell signaling, and modulating other critical aspects of structural and functional processes. Early placentation process is critical for placental growth and function. Several fatty acids modulate angiogenesis as observed by increased tube formation and secretion of angiogenic growth factors in first-trimester human placental trophoblasts. Long-chain fatty acids stimulate angiogenesis in these cells v

Simple autopsy method for analysis of complex fetal cranial malformations.

A new radiography technique for examining second trimester abortions permits an accurate analysis of growth and maturation of the fetal skull. Reference material for normal development has been previously published. The potential of the method is demonstrated with three examples: thanatophoric dysplasia, Arnold-Chiari malformation, and anencephaly.

Atypical manifestations of congenital parvovirus B19 infection.

Parvovirus B19 infection in pregnancy is associated with fetal anemia, hydrops and fetal death. We report two unusual manifestations of vertical parvovirus B19 infection. The first patient developed hydrops as consequence of myocarditis with involvement of sino-atrial node. The other had pleural effusion reactive to the hepatic localization of the virus.

The Vortex shunt: Short-term feasibility studies in the fetal lamb model of lower urinary tract obstruction and pleural effusion.

BACKGROUND: The suboptimal shunt design of the currently available vesico-amniotic and thoraco-amniotic shunts is likely at tributed to the risk of shunt failure, either by displacement, kinking, or obstruction, resulting in treatment failure and the need for additional fetal intervention. OBJECTIVE: To evaluate the feasibility of ultrasound-guided insertion, drainage, and short-term dislodgement risks of the novel Vortex shunt in fetal lambs with surgically induced lower urinary tract obstruction (LUTO) and pleural effusion. STUDY DESIGN: LUTO was surgically created in eight fetal lambs at

Supplementing desflurane with intravenous anesthesia reduces fetal cardiac dysfunction during open fetal surgery.

OBJECTIVE: To lower the incidence and severity of fetal cardiovascular depression during maternal fetal surgery under general anesthesia. AIM: We hypothesized that supplemental intravenous anesthesia (SIVA) with propofol and remifentanil would lower the need for high-dose inhalational anesthesia and provide adequate maternal depth of anesthesia and uterine relaxation. SIVA technique would minimize prolonged fetal exposure to deep inhalational anesthetics and significant intraoperative fetal cardiovascular depression. BACKGROUND: Fetal hypoxia and significant fetal hemodynamic changes occur d

An intelligent, compact wearable pressure-strain combo sensor system for continuous fetal movement monitoring.

Continuous fetal movement monitoring in late pregnancy may improve fetal wellbeing and pregnancy outcomes. While fetal movements can be visualized with ultrasound, it is intermittent and limited to clinical settings. Inertial measurement units may enable at-home fetal monitoring but usually require a large-footprint, multisensor design. Here, we report smart, compact wearable pressure-strain combo sensors continuously monitoring fetal movements through maternal abdominal skin motions. In the 2D and 3D artificial abdomen systems, our octagonal-shaped gold nanowire-based strain sensor served as

Fetal centers and the role of the neonatologist in complex fetal care.

As prenatal imaging and genetic diagnostic techniques developed, clinicians knew earlier and with greater accuracy of the extent and severity of fetal anomalies. This, coupled with an acute awareness of high rates of death or devastating neonatal morbidities in some cases, drove efforts to create innovative fetal interventions. However, with advances in neonatal quaternary care, infants with even the most complex congenital anomalies now have a substantially greater chance of survival. But many still require highly coordinated intensive care from the moment of delivery, have lengthy and compli

Fetal acidosis in prolonged pregnancy cannot be attributed to cord compression alone.

Fetal heart rate abnormalities associated with prolonged pregnancy have been attributed to umbilical cord vulnerability rather than placental insufficiency. Although intrapartum fetal heart rate patterns indicative of umbilical cord compression are common beyond 41 weeks' gestation, fetal intolerance to labor develops only in a subset of such patients. To test the hypothesis that suboptimal placental function contributes to reduced amniotic fluid volume and, ultimately, to fetal intolerance to labor, we prospectively collected biochemical and clinical data on 112 prolonged pregnancies. Data an

Cell-based Noninvasive Prenatal Testing (cbNIPT)-A Review on the Current Developments and Future Prospects.

Considering the diagnostic limitations of cfDNA-based noninvasive prenatal testing (NIPT), scientists have long been interested in isolating and analyzing rare intact fetal and trophoblast cells from maternal blood or endocervical samples to diagnose fetal genetic conditions. These cells may be scarce and difficult to isolate, but they are a direct source of pure fetal genetic material. In this review, we summarize the history of cell-based NIPT, present an updated review on its current developments, evaluate its genetic diagnostic potential, and discuss its future prospects for clinical use.

Mosaic partial trisomy 17q2.

Examination of an infant born after prenatal diagnosis of mosaic partial trisomy 17q2 showed the unique phenotypic features of this chromosomal abnormality, that is, frontal bossing, large mouth, brachyrhizomelia, and hexadactyly. Amniocentesis was performed because of polyhydramnios and ultrasound diagnosis of fetal craniofacial dysmorphology and rhizomelic shortening of the limbs. Chromosomal mosaicism was restricted to fetal tissue and amniotic fluid cells. The placental chromosomal complement was normal, suggesting that the abnormality developed after differentiation of embryonic and troph

Fetal hyperinsulinism in rhesus isoimmunization.

Analyses of peritoneal ascitic fluid obtained prior to intrauterine transfusion show that some babies with severe rhesus isoimmunization develop raised insulin levels up to a month before delivery. The glucose content of fetal ascitic fluid is usually only about 10 mg. per 100 ml. less than the glucose content of maternal plasma and there is no evidence that this relation is influenced by the fetal or maternal insulin level. The electrolyte content of fetal ascitic fluid is very similar to that of maternal plasma, but fluid from babies with hyperinsulinism has an unusually high calcium content

Dissecting the contributions of the peripheral chemoreflex and myocardial hypoxia to fetal heart rate decelerations in near-term fetal sheep.

Brief repeated fetal hypoxaemia during labour can trigger intrapartum decelerations of the fetal heart rate (FHR) via the peripheral chemoreflex or the direct effects of myocardial hypoxia, but the relative contribution of these two mechanisms and how this balance changes with evolving fetal compromise remain unknown. In the present study, chronically instrumented near-term fetal sheep received surgical vagotomy (n = 8) or sham vagotomy (control, n = 11) to disable the peripheral chemoreflex and unmask myocardial hypoxia. One-minute complete umbilical cord occlusions (UCOs) were performed ever

Sex-differentiated placental methylation and gene expression regulation has implications for neonatal traits and adult diseases.

Sex differences in physiological and disease traits are pervasive and begin during early development, but the genetic architecture of these differences is largely unknown. Here, we leverage the human placenta, a transient organ during pregnancy critical to fetal development, to investigate the impact of sex in the regulatory landscape of placental autosomal methylome and transcriptome, and its relevance to health and disease. We find that placental methylation and its genetic regulation are extensively impacted by fetal sex, whereas sex differences in placental gene expression and its genetic …

Maternal gut microbiota influences immune activation at the maternal-fetal interface affecting pregnancy outcome.

Preeclampsia is a leading cause of morbidity and mortality in pregnant women, affecting 5-8% of gestations worldwide. Its development is influenced by maternal immune abnormalities, metabolic disorders, and gut dysbiosis. In this study, we show that gut dysbiosis in pregnant C57BL/6J dams leads to increased fetal resorption, impaired placental development and altered vascularization. These adverse outcomes are associated with key pathological features of preeclampsia, including hypoxia, endoplasmic reticulum (ER) stress and reduction in uterine natural killer (NK) cell numbers. Furthermore, gu

Endocrine disruptors and pregnancy.

BACKGROUND/AIM: Sensitivity to endocrine disruptors is higher in early life. Endocrine disruptor chemicals can be passed from pregnant women to their babies through the placenta or breast milk during lactation, leading to long-term and potentially permanent adverse effects. MATERIALS AND METHODS: This review evaluates the effects of endocrine-disrupting chemicals (EDCs) on pregnancy by summarizing findings from experimental and observational studies. Exposure routes, reproductive outcomes, fetal development implications, and potential preventive strategies are analyzed. RESULTS: Exposure to …
