Rocky Mountain spotted fever vaccine: a regional need.

Because the killed Rickettsia vaccines have failed to provide protection against Rocky Mountain spotted fever, we approached the problem of protective immunity to Rickettsia rickettsii with a closely related, live, less pathogenic spotted fever Rickettsia. Guinea pigs vaccinated with R conorii produced antibodies to spotted fever group rickettsiae and did not have fever or other signs of illness after challenge with R rickettsii. A vaccine against Rocky Mountain spotted fever would be useful in some medical practices in the southeastern United States because of the life-threatening nature of t…

Rocky Mountain spotted fever: a warm weather problem.

Rocky Mountain spotted fever occurs primarily in the warm weather months of April through September in the southeastern and south central United States. It is transmitted by the bite of ticks infected with the etiologic Rickettsia. This article reviews the epidemiology, clinical manifestations, diagnosis and therapy of Rocky Mountain spotted fever. Emphasis is placed on the dilemmas of early diagnosis and the necessity of early and often empirical antibiotic treatment of suspected cases.

Viral haemorrhagic fever surveillance in Kenya, 1980-1981.

Following two cases of Marburg virus disease in Kenya in 1980, viral haemorrhagic fever surveillance was undertaken in western Kenya. Over a 21-month period investigations, including virus isolation attempts, patient and contact serology, visits to areas where suspected cases occurred, interviewing family members and neighbours of suspected cases and following up any additional illnesses in these areas, were carried out. During the study two cases were found that were likely to have been Ebola haemorrhagic fever based on rising antibody titres or positive serology in contacts. Diagnoses of hep…

[Mediterranean boutonneuse fever. Apropos of 154 recent cases].

The Mediterranean spotted fever is always present in the south of France. The actual incidence is unknown. The disease appears in summer. The diagnostic is based on the association of fever, "black spot" and exanthema and/or a seroconversion. Indirect immunofluorescence is the most used technique. Some patients have severe complications: neurologic, cardiovascular, renal, thrombopenia. These cases look like Rocky Mountain Spotted Fever. The treatment is based on tetracycline.

Left ventricular dysfunction in Rocky Mountain spotted fever.

Rocky Mountain spotted fever is a serious infectious disease that continues to occur with increasing incidence in various areas of the United States. A high case-fatality ratio (11%) persists in spite of availability of adequate chemotherapeutic agents, and the cardiac involvement may play an important role in the final outcome of some cases. To assess the effect of the disease on left ventricular function 13 patients underwent M-mode echocardiographic examination on admission to a children's hospital. Increased left ventricular dimension was found in 3 patients and decreased left ventricular …

[Malignant form of Mediterranean boutonneuse fever. 6 cases].

Six cases of severe Mediterranean boutonneuse fever are reported. The clinical diagnosis, based on the presence of summer fever with an erythematous rash plus an escharr in three patients, was confirmed by serology (microagglutination in 1 case, indirect immunofluorescence in 4). The symptoms resembled those of Rocky Mountain spotted fever associating, as they did, a purpuric rash with neurological signs (impaired consciousness in all 6 cases, convulsion in 2), respiratory symptoms (5) and digestive signs (diarrhoea in 1 case, enlargement of the liver in 4). Laboratory examinations showed thro…

Rat-bite fever in animal research laboratory personnel.

A laboratory animal technician experienced undulating fever, chills, and myalgia 3 days after he was bitten by a laboratory rat. The clinical symptoms subsided with antibiotic therapy, but recurrent fever, malaise, and joint pain occurred when therapy was discontinued. Streptobacillus moniliformis was cultured from the patient's blood.

Azoospermia caused by typhoid fever. A case report.

A case of a typhoid fever patient with azoospermia is reported. Before typhoid fever, the semen analyses indicated an oligozoospermia (6.4 million/ml). Degree of fever up to 41.2 degrees C during 10 days was enough to suppress spermatogenesis, resulting in an azoospermic state. After the fever ceased, the spermatogenesis recovered and sperm production functioned normally after +/- 3 months. The situation corresponds with the time needed for spermatogenesis as previously reported.

[Salmonella dublin enteric fever in two compromised hosts].

Salmonella (S) dublin is a rare cause of human enteric fever. We studied 2 compromised hosts, one of them being treated for lymphocytic leukemia and the other for chronic lymphocytic leukemia. Both had recurrent enteric fever caused by S. dublin. Both strains were resistant to chloramphenicol and to ampicillin and the patients were treated by trimethoprim-sulfamethoxazole to which the organism was sensitive. Investigations failed to discover an extraintestinal localisation and stool cultures remained negative after antibiotic treatment. One patient had a relapse of enteric fever at every relap…

Laboratory outbreak of Q fever acquired from sheep.

In April and May, 1981, an outbreak of Q fever occurred in a university department which used gravid sheep for fetal respiratory studies. During the subsequent investigation 91 people were studied and 28 were found to have complement-fixing antibodies to the phase II antigen of Coxiella burneti in their serum. Symptoms compatible with C. burneti infection occurred in 14 of the seropositive patients. The majority of symptomatic infections could be related to the delivery of twin fetuses in one laboratory, but some people remote from this incident who were exposed to sheep in other parts of the …

Q fever hepatitis: clinical manifestations and pathological findings.

This report describes the clinical manifestations and pathological findings in 5 patients with serologically diagnosed acute Q fever. Each patient presented with headache, malaise, spiking fever, and hepatitis. Percutaneous biopsy of the liver in 4 patients revealed granulomatous changes with many lesions containing a dense fibrin ring surrounding a central lipid vacuole. Biopsy of the bone marrow in the fifth patient revealed similar abnormalities. These lipogranulomas should be considered characteristic of Q fever.
