Randomized controlled trial of high-dose sublingual immunotherapy to treat seasonal allergic rhinitis.

BACKGROUND: Seasonal allergic rhinitis is common and troublesome. Sublingual immunotherapy (SLIT) has been proposed as an alternative to injection immunotherapy and might offer some advantages if it were effective and practical in a community setting. OBJECTIVES: To assess the efficacy and side-effect profile of SLIT in patients with summer hay fever uncontrolled on current standard medication. To assess the feasibility of delivering SLIT in a United Kingdom general practice setting. METHODS: Double-blind, placebo-controlled study in 186 patients with severe summer hay fever identified from

Perceptions of Medical Malpractice Liability and Informed Consent Among Hospital Healthcare Workers: A Cross-Sectional Study.

Background and Objectives: Rising malpractice litigation and the strengthening of patient-rights legislation have changed the way hospital staff approach informed consent and legal risk. This study aimed to (i) assess the distribution of informed-consent knowledge, malpractice anxiety, and defensive practice across professional and departmental groups; (ii) identify independent predictors of high defensive practice; and (iii) evaluate experience-related moderation of the legal-literacy-anxiety association and predictor consistency across defensive-practice severity. Methods: We conducted a cro

Workarounds to Mandated Data Practices: Rethinking Digital Infrastructure in Child Welfare and Homeless-Serving Organizations.

Over the past decade, much of the infrastructure used for social service delivery in Canada has been digitized. Drawing on institutional ethnographic research practices, we undertook observational research, facilitated focus groups with twenty five key informants, and interviewed 121 leaders and workers in the child welfare and youth homelessness sectors to learn how data practices in these sectors are shaped by government-mandated infrastructure and tools associated with the adoption of digital information management systems in the public sphere. Despite promises that these new digital techno

Variation in duration of repeat prescriptions: a primary care cohort study in England.

BACKGROUND: Many patients receive repeat prescriptions for routine medications used to treat chronic conditions. Doctors typically issue repeat prescriptions with durations ranging from 28 to 84 days. There is currently no national guidance in England for the optimal prescription duration for routine medications. AIM: To evaluate current prescription durations for five common routine medications in England; explore and visualise geographical variation; and identify practice factors that are associated with shorter prescribing duration to inform policy making. DESIGN AND SETTING: A retrospect

[Diabetic Foot Status (DFS): A IWGDF-based classification for the assessment and management of diabetic foot disease in primary care].

Diabetic foot disease is a complication of diabetes mellitus, impacting morbidity, quality of life, and healthcare costs. Its clinical assessment is fragmented across multiple classification systems (SINBAD, IDSA), hindering multidisciplinary communication. The Diabetic Foot Status (DFS) is a new clinical classification based on the latest guidelines from the International Working Group on Diabetic Foot (IWGDF). It facilitates the classification of diabetic foot into three states: «Green,» «Amber,» and «Red,» also incorporating the degree of infection and the predominant origin of the lesion:

Diagnostic assessment for venous leg ulcers: Recommendations from the Wounds and Healing working group of the French Society of Vascular Medicine.

Venous leg ulcers (VLU) impair patients' quality of life and result in significant costs for society and is the consequence of chronic venous disease. There are no guidelines on the diagnostic assessment in leg ulcers, detailing the vascular examinations and the procedures for carrying them out in cases of VLU. Thus, the diagnostic evaluation is often heterogeneous and incomplete, leading to delays in the management and the healing. The Wounds and Healing group of the SFMV is proposing, through a review of the literature, to draw up recommendations for the diagnostic assessment of VLU in order

Management of Obstructive Sleep Apnea in the Infant and Newborn.

Obstructive sleep apnea in newborns and infants presents a unique challenge with distinct differences in sleep physiology, etiologies, and management compared to older children. The indications for and interpretation of polysomnography are less well defined in infants. There are also no broadly accepted clinical practice guidelines for treating sleep apnea in this age group. Etiologies include general causes of upper airway obstruction in infants such as laryngomalacia, micrognathia, and nasal obstruction in addition to adenotonsillar hypertrophy. Treatment strategies must be tailored to the s

Definitions and criteria for diabetes-related foot disease (IWGDF 2023 update).

Multiple disciplines are involved in the management of diabetes-related foot disease and a common vocabulary is essential for clear communication. Based on the systematic reviews of the literature that form the basis of the International Working Group on the Diabetic Foot (IWGDF) Guidelines, the IWGDF has developed a set of definitions and criteria for diabetes-related foot disease. This document describes the 2023 update of these definitions and criteria. We suggest these definitions be used consistently in both clinical practice and research, to facilitate clear communication with people wit

Evaluation and Treatment of Psychogenic Nonepileptic Seizures.

Patients with psychogenic nonepileptic seizures (PNES) represent a distinct, challenging group among those with functional neurologic symptom (conversion) disorders and involve a complex set of symptoms and comorbidities, best evaluated and treated by a multidisciplinary team of clinicians. Multidisciplinary, collaborative care is becoming more common, using evidence-based treatment. Outpatient neurology clinics at sites not currently treating these patients hold potential for providing such a model of care, with coordination of services. Best practice care should encourage the integration of

Expert consensus on the bone repair strategy for osteoporotic fractures in China.

Osteoporotic fractures, also known as fragility fractures, are prevalent in the elderly and bring tremendous social burdens. Poor bone quality, weak repair capacity, instability, and high failure rate of internal fixation are main characteristics of osteoporotic fractures. Osteoporotic bone defects are common and need to be repaired by appropriate materials. Proximal humerus, distal radius, tibia plateau, calcaneus, and spine are common osteoporotic fractures with bone defect. Here, the consensus from the Osteoporosis Group of Chinese Orthopaedic Association concentrates on the epidemiology, c

Optimizing Expansion Clinic for Patients With Magnetic Controlled Growth Rods.

In 2017, members of the Pediatric Spine Study Group reported that 80% of new growth-friendly implants inserted were magnetically controlled growing rods (MCGRs). MCGRs are usually expanded more often than every 6 months, as was general practice with traditional growth rods. The growing volume of expansion visits is managed variably at different centers. Although there is existing research comparing MCGRs with traditional systems for efficacy and complications, there is limited literature regarding how expansion clinics are managed. This article reviews current practices such as scheduling, exp

Combining clinical, radiological and genetic approaches to pneumothorax management.

Familial spontaneous pneumothorax (FSP) accounts for 10% of primary spontaneous pneumothoraces. Appropriate investigation of FSP enables early diagnosis of serious monogenic diseases and the practice of precision medicine. Here, we show that a pneumothorax genetics multidisciplinary team (MDT) can efficiently diagnose a range of syndromic causes of FSP. A sizeable group (73.6%) of clinically unclassifiable FSPs remains. Using whole genome sequencing we demonstrate that most of these cases are not known monogenic disorders. Therefore, clinico-radiological assessment by an MDT has high sensitivi

Hepcidin Is a Reliable Marker of Iron Deficiency Anemia in Newly Diagnosed Patients with Inflammatory Bowel Disease.

RESULTS: There was a high statistically significant difference between IBD patients and controls in levels of hepcidin (P < 0.01). Namely, serum hepcidin levels were significantly higher in the control group. There was no statistically significant correlation of serum hepcidin with CRP, Mayo score, or CDAI, respectively (P > 0.05). However, we have found a statistically significant negative correlation of sTfR and TIBC with hepcidin (P < 0.01). CONCLUSION: Results of our study suggest that hepcidin is a reliable marker of IDA in patients with IBD, and it could be used in routine clinical prac

Optimizing Medication Treatment of Opioid Use Disorder During COVID-19 (SARS-CoV-2).

: The COVID-19 health crisis joined, rather than supplanted, the opioid crisis as the most acutely pressing threats to US public health. In the setting of COVID-19, opioid use disorder treatment paradigms are being disrupted, including the fact that methadone clinics are scrambling to give "take-home" doses where they would typically not. The rapid transition away from in-person examination, dosing and group therapy in an era of social isolation calls for adjustments to clinical practice, including emphasizing patient-provider communication, favoring new inductees on buprenorphine and leveragi
