Unique Variant of NOD2 Pediatric Granulomatous Arthritis With Severe 1,25-Dihydroxyvitamin D-Mediated Hypercalcemia and Generalized Osteosclerosis.

Pediatric granulomatous arthritis (PGA) refers to two formerly separate entities: autosomal dominant Blau syndrome (BS) and its sporadic phenocopy early-onset sarcoidosis (EOS). In 2001 BS and in 2005 EOS became explained by heterozygous mutations within the gene that encodes nucleotide-binding oligomerization domain-containing protein 2 (NOD2), also called caspase recruitment domain-containing protein 15 (CARD15). NOD2 is a microbe sensor in leukocyte cytosol that activates and regulates inflammation. PGA is characterized by a triad of autoinflammatory problems (dermatitis, uveitis, and arthr

The Effect of Debridement of Coexisting Partial Ligament Injuries on Outcomes Following Arthroscopic Osteosynthesis for Minimally Displaced Scaphoid Nonunions.

PURPOSE: Partial intercarpal ligament injuries can coexist with scaphoid nonunions. However, whether these injuries should be debrided simultaneously when scaphoid nonunions are treated is unclear. The purpose of this study was to compare union rates and clinical outcomes after arthroscopic management of scaphoid nonunions, in which coexisting partial ligament injuries were, or were not, simultaneously debrided. METHODS: This retrospective study included 46 patients with scaphoid nonunions and coexisting partial intercarpal ligament injuries who underwent arthroscopy-guided bone grafting and

[Two cases of rheumatoid arthritis developed after polymyositis].

We report two cases of rheumatoid arthritis (RA) who later had developed after polymyositis (PM). The first patient was 64-year old male who experienced muscular weakness of the four limbs in proximity 10 years ago. He was diagnosed as PM because of the elevated serum CK and the myogenic pattern of EMG, and his symptoms were improved by treatment with corticosteroid. He started to complain polyarthralgia 2 years ago, followed by interstitial pneumonia, pleuritis and skin ulcer. He was admitted because of exacerbated polyarthralgia, multiple subcutaneous nodules, skin eruption and fever. The le

The Proximal Humeral Ossification System Improves Assessment of Maturity in Patients with Scoliosis.

BACKGROUND: We recently developed a classification system to assess skeletal maturity by scoring proximal humeral ossification in a similar way to the canonical Risser sign. The purpose of the present study was to determine whether our system can be used to reliably assess radiographs of the spine for modern patients with idiopathic scoliosis, whether it can be used in combination with the Sanders hand system, and whether the consideration of patient factors such as age, sex, and standing height improves the accuracy of predictions. METHODS: We retrospectively reviewed 414 randomized radiogra

Modified non-transarticular K-wire external fixation versus traditional transarticular pinning for bony mallet finger: a prospective comparative cohort study.

BACKGROUND: Traditional transarticular distal interphalangeal joint (DIPJ) pinning for bony mallet fingers risks stiffness and iatrogenic cartilage injury. This study compared a modified non-transarticular K-wire external fixator with traditional transarticular pinning for surgically treated bony mallet finger. METHODS: This single-center prospective nonrandomized comparative cohort study included 120 adults with acute bony mallet finger. Sixty patients underwent traditional transarticular DIPJ pinning, and 60 underwent modified non-transarticular K-wire external fixation. The primary outcome

Isolated scaphoidectomy for type II SLAC and SNAC wrists: retrospective case-series at long-term follow-up.

BACKGROUNDS: Stage II scapholunate advanced collapse (SLAC) and Scaphoid non-union advanced collapse (SNAC) wrists are currently treated either by scaphoidectomy and Four Corner Fusion or by Proximal Row Carpectomy. In our retrospective study, isolated scaphoidectomy demonstrated clinical non-inferiority when compared to standard procedures and could be supplemented by a Four Corner Fusion only at a later time if needed. METHODS: Between 2006 and 2011, 13 patients / 14 wrists (mostly males, manual workers) underwent the surgical procedure. 8 patients were affected by SNAC and 5 by SLAC. The a

Clinical, radiological and dynamic CT results of scapholunate intercarpal ligamentoplasty for scapholunate dissociation.

A clinical, radiological and four-dimensional computed tomography (4DCT) assessment of the outcomes of scapholunate intercarpal ligamentoplasty (SLICL) was done with a minimum follow-up of 2 years. Twenty-nine patients (23 men and 6 women) with a mean age of 40 years (22-57) who had chronic scapholunate dissociation were treated with the SLICL procedure. There were 18 cases of dynamic instability and 11 of static instability. The patients were evaluated with a mean follow-up of 61 months (24-94). SLICL significantly reduced pain and increased grip strength and wrist function. On radiographs, t

Simultaneous juxta-epiphyseal proximal phalanx fracture with flexor tendon entrapment in a child: a case report and review of literature.

Juxta-epiphyseal/Salter-Harris fractures are the most common hand fractures in children and the proximal phalanx is involved in most cases. In the absence of soft-tissue interposition, these growth plate injuries are simple to reduce and are stable. However, in some cases, flexor tendon entrapment could be present. We report on an 11-year-old girl who sustained a fall onto her outstretched hand with subsequent injuries in her long, ring, and small fingers. Plain radiographs showed a severely displaced juxta-epiphyseal proximal phalanx fracture in her ring finger associated with mildly displace

Hematoma and distraction arthroplasty for thumb basal joint osteoarthritis: minimum 6.5-year follow-up evaluation.

PURPOSE: Although hematoma and distraction arthroplasty has been found to be successful in the treatment of thumb basal joint arthritis in the short term, questions about its efficacy in the long term have been raised. The goal of the present study was to evaluate the results of this procedure in patients at least 6.5 years after surgery. METHODS: Twenty-two thumbs from 22 patients from a single surgeon's practice were entered into a prospective single-arm study for surgical treatment of basal thumb arthritis. Treatment consisted of piecemeal excision of the entire trapezium and 5 weeks of K-

The natural history of an untreated isolated scapholunate interosseus ligament injury.

The natural history of an untreated isolated scapholunate interosseus ligament injury remains unclear, although it is commonly assumed that patients continue to suffer with pain, stiffness and weakness of the wrist and ultimately develop secondary osteoarthritis (SLAC wrist). In this study, we evaluated the clinical condition of 11 patients with an arthroscopically proven interosseus scapholunate ligament injury, but without any radiological signs of either DISI deformity or scapholunate gapping, who had declined further treatment at an average follow-up of 7 years. Whilst there was on going p

Digit replantation. Analysis of 163 replantations in an 11 year period.

From July 1965 to June 1976, 163 digits in 107 patients were replanted, and 145 digits survived-an 89 per cent success rate. Critical analyses were made of the causes of failures, complications, bone union, sensory recovery, secondary reconstructive procedures, and the final outcome. The majority of the patients were satisfied with their replanted digits, and no reamputation because of uselessness or causalgic pain of the replanted part has been necessary.
