Adherence to geriatric assessment-based recommendations in older patients with cancer: a multicenter prospective cohort study in Belgium.

BACKGROUND: In the general older population, geriatric assessment (GA)-guided treatment plans can improve overall survival, quality of life and functional status (FS). In GA-related research in geriatric oncology, studies mainly focused on geriatric screening and GA but not on geriatric recommendations, interventions and follow-up. The aim of this study was to investigate the adherence to geriatric recommendations and subsequent actions undertaken in older patients with cancer. PATIENT AND METHODS: A prospective Belgian multicenter (N = 22) cohort study included patients ≥70 years with a mali

Obstetric Care Consensus No. 4 Summary: Periviable Birth.

Approximately 0.5% of all births occur before the third trimester of pregnancy, and these very early deliveries result in the majority of neonatal deaths and more than 40% of infant deaths. A recent executive summary of proceedings from a joint workshop defined periviable birth as delivery occurring from 20 0/7 weeks to 25 6/7 weeks of gestation. When delivery is anticipated near the limit of viability, families and health care teams are faced with complex and ethically challenging decisions. Multiple factors have been found to be associated with short-term and long-term outcomes of periviable

Minnesota's Early Experience with Medical Home Implementation: Viewpoints from the Front Lines.

BACKGROUND: Evidence is evolving about the impact of patient-centered medical homes (PCMHs) on important outcomes in primary care. Minnesota has developed its own PCMH certification process, envisioned as an all-payer initiative with an emphasis on patient-centeredness, which may add unique experiences and outcomes to the national discussion. OBJECTIVE: We aimed to identify the facilitators and barriers encountered by nine diverse primary care practices selected from the first 80 to achieve PCMH certification in Minnesota. DESIGN: This was a qualitative analysis of semi-structured, in-person

Remote and digital services in UK general practice 2021-2023: the Remote by Default 2 longitudinal qualitative study synopsis.

BACKGROUND: Remote services (in which the patient and staff member are not physically colocated) and digital services (in which a patient encounter is digitally mediated in some way) were introduced extensively when the COVID-19 pandemic began in 2020. We undertook a longitudinal qualitative study of the introduction, embedding, evolution and abandonment of remote and digital innovations in United Kingdom general practice. This synoptic paper summarises study design, methods, key findings, outputs and impacts to date. OVERVIEW OF THE STUDY AND KEY FINDINGS: From September 2021 to December 202

A Screening Model for Latent Tuberculosis Infection Among Household Contacts of Patients With Tuberculosis Based on the CyTB Test and an Android App: Protocol for a Development and Implementation Study.

BACKGROUND: Latent tuberculosis infection (LTBI) remains a major barrier to tuberculosis elimination, particularly in community settings where household contacts (HHCs) of patients with active tuberculosis are at increased risk of infection and progression to disease. Current tests for LTBI have practical limitations that restrict their use in field and primary care settings. Antigen-based skin testing, combined with digital tools for induration measurement, may improve the accuracy and delivery of HHC screening under routine program conditions. OBJECTIVE: This study aims to develop and evalu

Touch-Based Partner Yoga for Gay, Bisexual, Transgender, and Queer Men in a Community Wellness Setting: Protocol for a Mixed Methods Program Evaluation of "The Studio".

BACKGROUND: Leisure-time physical activity (LTPA) is a well-established contributor to physical, psychological, and social well-being worldwide. Human touch also plays a vital role in life course health, yet opportunities for safe, consensual touch are often limited, particularly in LTPA settings. For gay, bisexual, transgender, and queer (GBTQ) men, barriers to affirming LTPA spaces can make it particularly difficult to access such benefits. In response, community-based approaches that integrate touch are needed, alongside systematic evaluations of such strategies. "The Studio" (pseudonym), a

Social Transfers for Exclusive Breastfeeding in Brazil: Protocol for a Randomized Controlled Trial.

BACKGROUND: According to the World Health Organization's infant and young child feeding guidelines, infants should be exclusively breastfed for the first 6 months of life. Despite public health campaigns to increase exclusive breastfeeding (EBF) rates, socioeconomic inequities persist among low-income breastfeeding mothers, especially in countries with large wealth and health gaps, such as Brazil. Social transfer programs are initiatives that provide financial support to individuals or households to improve their well-being and reduce financial burdens. These may be conditional, requiring reci

Exploring how and why Care Groups work to improve infant feeding practices in low- and middle-income countries: a realist review protocol.

BACKGROUND: Within our inquiry into the implementation of breastfeeding policy in Malawi, Care Groups have been mentioned as a means to improve maternal and child health and nutrition outcomes. The 'Care Group model' is an approach primarily used in international development settings, whereby social and behaviour changes are promoted through supported peer-to-peer (mostly mother-to-mother) knowledge sharing. The aim of most Care Groups is to promote improved infant nutrition, improve hygiene and increase the number of children who are fully vaccinated and exclusively breastfed for the first 6

Factors influencing preference for short-term over long-term contraceptives among women in northern Uganda. A mixed-methods study.

INTRODUCTION: Modern contraceptives have contributed to reduction of unintended pregnancy among the women of reproductive age (15 to 49) globally. Use of long-term reversible contraceptives is low (13%) in Uganda yet they are cost-effective. This study aimed to identify factors influencing preference of short-term reversible contraceptives over long-term methods among women of reproductive age attending the family planning clinic at Lira Regional Referral Hospital (LRRH). METHOD: We performed a mixed-methods cross-sectional study, in which structured interviews were administered to 220 family

The implementation of safer drug consumption facilities in Scotland: a mixed methods needs assessment and feasibility study for the city of Edinburgh.

BACKGROUND: Scotland currently has amongst the highest rates of drug-related deaths in Europe, leading to increased advocacy for safer drug consumption facilities (SDCFs) to be piloted in the country. In response to concerns about drug-related harms in Edinburgh, elected officials have considered introducing SDCFs in the city. This paper presents key findings from a feasibility study commissioned by City of Edinburgh Council to support these deliberations. METHODS: Using a multi-method needs assessment approach, we carried out a spatial and temporal analysis of drug-related data in Edinburgh

Development of a Virtual Transition Program for Adolescents and Young Adults With Type 1 Diabetes: Three-Stage User-Centered Design Study.

BACKGROUND: Adolescents and young adults (AYAs) with type 1 diabetes (T1D) face a heightened risk of care gaps and preventable complications during the transition from pediatric to adult care. AYAs from low-income families are more likely to experience poor outcomes. Care guidelines for T1D now recommend a transition program beginning several years prior to anticipated independence. However, existing transition programs often require substantial resources, limiting their scalability and accessibility. OBJECTIVE: We used a human-centered design approach to develop and refine a low-cost, virtua

U.S. Primary Care Clinic Staff Attitudes Toward Providing Medication Abortion.

INTRODUCTION: We assessed pre-intervention staff attitudes toward providing medication abortion (MAB) in 10 clinics participating in a program to adopt MAB into primary care. METHODS: In 2024, staff MAB "champions" from 10 organizations in eight states administered our online survey to staff at clinics preparing to introduce MAB (N = 888). Eligibility criteria included any staff who could directly or indirectly be involved in a patient MAB visit and did not self-identify as champions. Surveys queried on staff perceptions of implementation acceptability, belonging in primary care, readiness fo

Use of single-item scales in the evaluation of German digital health applications (DiGAs): Scoping review protocol.

INTRODUCTION: German Digital Health Applications (DiGAs) are reimbursable, patient-facing digital interventions within the German healthcare system that must demonstrate positive healthcare effects. Outcome measurement in DiGA evaluations is influenced by both methodological considerations and feasibility requirements in real-world care settings. In practice, these evaluations may be affected by factors such as limited patient adherence, high dropout rates, and variability in real-world data quality, which can complicate outcome data collection. Single-item scales may offer a low-burden altern

Governance frameworks and decision-making models for intensive care units in low- and middle-income countries: a scoping review protocol.

INTRODUCTION: Intensive care units (ICUs) operate at the intersection of advanced technology, complex ethical decision-making and resource-intensive care. While many high-income countries have developed institutional governance mechanisms-such as Clinical Ethics Committees, structured triage policies and formal palliative care consultation pathways-to support ethical deliberation and accountability in ICUs, the applicability, structure and reported outcomes of such frameworks in low- and middle-income countries (LMICs) remain unclear and dispersed across disciplines. OBJECTIVE: This scoping r
