BACKGROUND: In contrast to the wide and successful use of valved aortic and pulmonary homografts, the non-valved prolongation of the thoracic aorta for the repair of some complex congenital heart diseases has rarely been described. We present here our experience with the use of descending aorta and aortic arch homografts as non-valved conduits for the surgical repair of complex cardiac malformations in 8 patients.
METHODS: One atriopulmonary conduit replacement four extraatrial cavopulmonary connections, and one intraatrial cavopulmonary repair were achieved by means of a homograft of the des

Our approach to the treatment of valvular heart disease, and mitral valve disease in particular, has been deeply modified by the experience acquired since the introduction of cardiac surgery and the technical advances in this field in the last decade and by new methods of investigative and interventional cardiology. In pure severe mitral regurgitation alone, the treatment of choice is reconstructive surgery. In the experience of our group, 191 patients out of 342 operated since 1970 for this type of valve lesion were referred for reconstructive surgery. Taking the results into consideration (7

Valvular heart disease (VHD) is a clinically important diagnosis, with significant associated morbidity and mortality. Multiple imaging modalities exist to characterize valvular and associated cardiac anatomy. Cardiovascular magnetic resonance (CMR) has emerged as a comprehensive noninvasive imaging modality for VHD. With use of well-established, standardized imaging sequences, CMR can accurately and precisely diagnose valvular structural abnormalities, assess severity of regurgitant and stenotic lesions, and potentially define patient prognosis. This article reviews the clinical applications

Infective endocarditis (IE) following transcatheter valvular interventions, including transcatheter aortic valve implantation and transcatheter edge-to-edge repair, is an uncommon but life-threatening complication associated with major diagnostic and therapeutic challenges. As indications for structural interventions continue to expand, the burden of device-related IE is expected to increase. This expert opinion, jointly developed by the Association of Valvular Heart Disease and the Association of Cardiovascular Intervention of the Polish Cardiac Society, summarizes contemporary evidence regar

AIMS: Transcatheter mitral valve replacement (TMVR) is a promising therapeutic solution to treat high-risk patients with severe mitral regurgitation (MR) contraindicated to surgery. Optimal selection of patients who will benefit from the procedure is of paramount importance. We aimed to investigate factors associated with TMVR screening.
METHODS AND RESULTS: From November 2016 to July 2018, we examined conditions associated with TMVR screening success in patients referred to the two French heart valve clinics with the greatest TMVR experience. Among a total of 40 consecutively screened patien

Paravalvular leak (PVL) is a serious complication of surgical valve replacement, often affecting elderly, multimorbid, high-risk patients. The risk of surgical intervention is often prohibitive in these cases, and so percutaneous PVL closure emerged as a feasible and effective management strategy, with a low complication rate. Specific devices for closure of PVL's are currently not widely available, and so PVLs are closed using generic vascular closure devices, which may result in residual paravalvular regurgitation or even closure device displacement. Although rare, late displacement of the c

OBJECTIVES: Current nonpharmacological therapies for symptomatic patients with hypertrophic obstructive cardiomyopathy (HOCM), including septal myectomy and alcohol septal ablation (ASA), carry significant risks for serious cardiac conduction abnormalities. We present a review of the currently available published data regarding the novel use of the relatively low-risk MitraClip® system in the treatment of symptomatic patients.
METHODS: Data were collected from 4 separate studies on the use of the MitraClip on 15 symptomatic HOCM patients with systolic anterior motion (SAM) of the mitral valve

Symptomatic stenosis of the aortic valve has recently been an important clinical issue. On one hand the number of patients who need surgical treatment of valve failure is increasing. On the other hand their general state of health and concomitant diseases determine that they are disqualified from surgery. The trasncutaneus technique of replacement of the aortic valve is for these patients a promising alternative. Therefore TAVI procedures have been developed and the number of performed transcatheter aortic valve replacements is still increasing. Although it would appear unlikely that TAVI coul

Nocardia causes several types of infection in both immunocompetent and immunocompromised hosts, and may affect any organ. Endocarditis due to infection by Nocardia species is rare and has a poor prognosis. To our knowledge, after a literature review, only 12 cases of nocardial endocarditis have been reported, most treated with valve replacement. We report the case of a patient with prosthetic valve endocarditis due to Nocardia species, who was successfully treated with antimicrobial chemotherapy only (trimethoprim-sulfamethoxazole), instead of valve replacement.

The Bentall procedure is a surgical repair of an ascending aortic or aortic root aneurysm in combination with aortic valve disease. Less commonly, it is used to repair aortic dissection affecting the aortic root and valve. During the procedure, a composite aortic valve graft is used to replace the proximal ascending aorta and aortic valve. The procedure is performed through a median sternotomy during cardiopulmonary bypass. In this modification of the original procedure, coronary artery circulation is maintained by removing a full-thickness "button" of aorta surrounding the coronary ostia, mak

Failure of anticoagulant therapy in a patient with a St. Jude heart valve in the mitral position is reported. After 8 years of optimal anticoagulation at an International Normalized Ratio between 2.5 and 3.5, a floating thrombus was discovered by chance during transthoracic echocardiography and then surgically removed. A study for congenital or acquired thrombophilia disclosed a positivity for activated protein C resistance, due to the presence of factor V Leiden. The intensity of warfarin anticoagulation was increased to provide more protection for the patient.

A 68-year-old woman with severe chronic obstructive pulmonary disease, aortic valvular insufficiency, and diffuse thoracic aortic aneurysm underwent aortic valve replacement and separate Dacron graft replacement of the ascending aortic and arch aneurysms using the elephant trunk technique. She was discharged on the tenth postoperative day. Five months later, she underwent endovascular stent-graft repair of the descending thoracic aortic aneurysm. She recovered uneventfully, and was discharged on the third postoperative day. Follow-up computed tomography at 6 months demonstrated exclusion of al

Cardiac prosthetic valves have not yet reached the point of reliability and durability to be used for other than remedial purposes. In mitral valve disease, the prime targets are patients in whom medical treatment could not successfully reverse progressive clinical disability, or in whom less drastic mitral valve operations have failed. In aortic stenosis, the prime targets are patients with substantial cardiac symptoms. In aortic regurgitation, earliest indications of disability provide the timing for valve replacement. In this lesion, prophylactic operations in the absence of symptoms are of

BACKGROUND: Postoperative thrombocytopenia is frequently observed after both surgical aortic valve replacement (SAVR) and transcatheter aortic valve implantation (TAVI). However, contemporary real-world data directly comparing the magnitude and temporal pattern of platelet decline between these two interventions remains limited.
METHODS: Consecutive patients with severe aortic stenosis who underwent SAVR with a bioprosthesis or TAVI at a single tertiary center between 2017 and 2023 were retrospectively analyzed. Demographic, clinical, echocardiographic, and laboratory data were collected. Pla

OBJECTIVES: To critically appraise the design of the ongoing multicentre, randomized START YOUNG trial comparing transcatheter aortic valve implantation (TAVI) with bioprosthetic surgical aortic valve replacement (SAVR) in patients aged 65-75 years with symptomatic severe aortic stenosis (AS).
METHODS: Publicly available trial information (ClinicalTrials.gov: NCT06861361) was reviewed, and the design was benchmarked against the minimum expected methodological standards for generating practice-changing evidence in patients with very long life expectancy (LE) following SAVR. The appraisal domai

BACKGROUND AND AIMS: Transcatheter aortic valve implantation (TAVI) has become a viable treatment option for patients with severe aortic valve stenosis across a broad range of surgical risk. The Nordic Aortic Valve Intervention (NOTION) trial was the first to randomize patients at lower surgical risk to TAVI or surgical aortic valve replacement (SAVR). The aim of the present study was to report clinical and bioprosthesis outcomes after 10 years.
METHODS: The NOTION trial randomized 280 patients to TAVI with the self-expanding CoreValve (Medtronic Inc.) bioprosthesis (n = 145) or SAVR with a b
