Incidence and Risk of Cytomegalovirus Infection After Intestine Transplantation.

BACKGROUND: Cytomegalovirus (CMV) is a major infectious complication after intestine transplantation, but data in this population remain limited. We evaluated the incidence, risk factors, and outcomes of clinically significant CMV infection (csCMVi) in intestine transplant recipients. METHODS: We performed a retrospective cohort study of adult and pediatric patients who underwent intestine transplantation between 2008 and 2022. csCMVi was defined as CMV disease or asymptomatic CMV DNAemia requiring treatment. Multivariable Cox regression identified predictors of csCMVi and its association wit

Recent advancements and limitations of intestinal organoids for clinical applications.

Colorectal cancer (CRC) is one of the leading causes of cancer-related deaths worldwide, and there has been a concerning rise in its incidence among younger populations. Although there have been significant advances in molecular characterization, translating research findings into effective therapeutic strategies for CRC management remains suboptimal. This challenge is largely attributed to the limitations of conventional preclinical models, such as two-dimensional or patient-derived tumor xenograft (PDX), which fail to recapitulate the complexity and heterogeneity of CRC tissues. Intestinal o

The coordinated actions of metformin in the intestine and liver improves hyperglycemia.

Metformin is the first-line oral anti-diabetic agent. Metformin concentrations in the intestine can reach up to 1.3 mM, while those in the portal vein are approximately 0.075 mM. It is unclear whether this metformin concentration difference contributes to metformin's antidiabetic effects. Here, we showed that high metformin concentrations upregulate G6PC expression through AMP-activated protein kinase (AMPK) activation to prevent glucose release in intestinal epithelial cells (IECs). The inhibition of mitochondrial activity by high metformin concentrations leads to drastically increased glucos

Crohn's Disease: From Immune Dysregulation to Intestinal Damage.

Crohn's disease (CD) is a long-lasting, recurring inflammatory bowel disease (IBD) marked by segmental, transmural inflammation that may affect any part of the gastrointestinal tract. Growing evidence suggests that CD does not originate from a single pathogenic mechanism, but instead results from the convergence of various biological disturbances that collectively disrupt intestinal homeostasis. Genetic alterations that weaken microbial detection, autophagy, and epithelial protection appear to establish initial vulnerabilities, enabling microbes to trigger exaggerated immune responses. This vu

In vitro culture of intestinal organoids for cell death assessments.

In vitro cultured organoids exhibit highly similar structures to their corresponding tissues and organs. Intestinal organoids derived from intestinal stem cells faithfully recapitulate the key characteristics of native intestinal epithelium, including morphological features, physiological functions, and personalized responses to specific stimuli. This protocol focuses on the isolation of intestinal crypts from murine intestinal tissues and the subsequent establishment of stable intestinal organoid cultures in vitro. Furthermore, propidium iodide (PI) staining coupled with high-content imaging

VGLL4 modulates Paneth cells and sustains intestinal homeostasis.

Paneth cells are defensive cells in the intestinal tract, which secrete niche factors and antimicrobial peptides (AMPs) to maintain the small intestinal stem cell niche and immune homeostasis. Here, we show that Vestigial-like family member 4 (VGLL4) plays a pivotal role in maintaining small intestinal homeostasis and in regulating Paneth cells. VGLL4 expression is downregulated in response to irradiation and DSS-induced colitis. Consistently, public datasets of human colitis show reduced VGLL4 expression. Loss of VGLL4 in the intestinal epithelium decreases Paneth cell numbers and AMPs produc

Hypoxia induces microtubule rearrangement in intestinal epithelial cells.

Microtubules are integral components of the highly regulated and dynamic cytoskeleton, which is vital for cell function and the maintenance of homeostasis. Microtubule disruption is associated with multiple disease states including colorectal cancer and neurodegenerative disorders. Although much is known about the mechanisms by which microtubules are regulated under physiologic conditions, the effect of pathological stimuli and how this contributes to disease progression is less clear. Hypoxia is a prominent microenvironmental feature of a range of pathological states including inflammation, i

NINJ1 as a novel biomarker of intestinal injury in necrotizing enterocolitis.

BACKGROUND: Necrotizing enterocolitis (NEC) remains a lethal gastrointestinal emergency in neonates with an elusive etiology. Ninjurin-1 (NINJ1) mediates plasma membrane rupture during lytic cell death, yet its role in NEC remains undefined. This study investigated the clinical relevance of NINJ1 in NEC progression and evaluated its utility as a biomarker for disease severity. METHODS: We conducted a prospective observational study quantifying serum NINJ1 levels via ELISA in 68 neonates with NEC and 42 gestational age-matched controls without infection. The NEC cohort was stratified by treatme

Indomethacin Fails to Increase Intestinal Permeability in Healthy Volunteers.

INTRODUCTION: Indomethacin is often used experimentally to induce intestinal hyperpermeability, enabling evaluation of interventions targeting barrier function. METHODS: We conducted a randomized, double-blind, placebo-controlled study (NCT05538247) in healthy volunteers to assess whether a supplement could mitigate indomethacin-induced hyperpermeability. Participants received 150 mg/d of indomethacin for 6 days, either before or during placebo/supplement administration. Permeability was measured using 13 C-mannitol and lactulose urinary excretion. RESULTS: Contrary to expectations, indometh

Intestinal microenvironment-modulating nanomedicines for inflammatory bowel disease.

Inflammatory bowel diseases present a highly restrictive intestinal microenvironment for drug delivery, in which epithelial injury, chronic inflammation, and microbial dysbiosis collectively impair transport, retention, and therapeutic efficacy. Excessive reactive oxygen species (ROS), mucus barrier disruption, altered luminal acidity, hydrogen sulfide accumulation, and abnormal enzymatic activity destabilize therapeutic cargos and limit access to inflamed tissues. These pathological features have motivated the development of nanocarriers designed either to directly modulate biochemical abnorm

Impact of enteric neuronal loss on intestinal cell composition.

Hirschsprung disease (HSCR) is a congenital disorder characterized by the absence of an enteric nervous system (ENS) in the distal gut. While the ENS is critical for normal gut function, its broader role in maintaining intestinal homeostasis remains underexplored. Using single-cell RNA sequencing, we investigated the impact of ENS loss on gut composition in wildtype and ret mutant (HSCR model) zebrafish. Significant alterations were identified, including increases in immune cells and shifts in epithelial and extracellular matrix (ECM)-producing cell populations. Immune dysregulation was highli

Surgical Management of Intestinal Obstruction: A Prospective Observational Study.

BACKGROUND: Intestinal obstruction remains one of the most common surgical emergencies encountered in tertiary care settings globally, carrying significant morbidity and mortality if not promptly diagnosed and managed. The etiological spectrum, clinical presentation, and optimal surgical strategies vary considerably between small bowel and large bowel obstruction (LBO) and are further influenced by the patient's comorbidity burden, nutritional status, and oncological history. This study was undertaken to systematically evaluate the surgical management of intestinal obstruction at a tertiary ca

The immune anatomy of the small intestine facilitates regulatory responses.

The intestinal immune system monitors the body's largest interface with the environment. It must prevent adverse responses to ingested material, while protecting itself from harm, constraining the inflammatory potential of gut microbiota, and detecting pathogens. Gut immune anatomy and distribution of its microbiota may help explain how it manages these roles. The lymphatics of the jejunum and proximal ileum are anatomically independent of the rest of the gut. Mesenteric lymph nodes in these lymphatics specialize in regulatory functions. This proximal region of the small intestine, with the gr

Regional organization of nutrient absorption across the small intestine.

The small-intestinal epithelium must simultaneously enable functions that are spatially compartmentalized along the length of the organ: nutrient absorption and protection against environmental insults. Although regional differences within the intestine have been recognized for centuries, only now has a comprehensive framework emerged to define this organization at molecular, cellular and functional levels. In this Review, we highlight the epigenetic and transcriptional features of small-intestinal zonation across longitudinal and apical-basal axes, and we examine the molecular mechanisms that

Mapping mesenchymal diversity in the human small intestine and organoids.

The organization of diverse mesenchymal populations during human small intestinal development is critical for tissue architecture and function yet remains poorly defined. Here, to construct a comprehensive, tissue-scale map of the developing human small intestine at single cell resolution, we leveraged single-cell RNA-sequencing data to build a Xenium spatial transcriptomics gene panel covering the cell diversity of the human small intestine. We defined five subpopulations occupying discrete anatomical locations within the lamina propria and submucosa-the subepithelial cells, lamina propria fi
