Pulmonary considerations in the immunocompromised patient.

The compromised patient who presents to the emergency department with pulmonary complaints is becoming a common occurrence. An immunocompromised state can result from a disease process such as HIV or from medications used to prevent graft rejection in solid organ recipients or to treat conditions such as collagen vascular disease. The emergency department physician should be familiar with the more common complications that can afflict this unique patient group. This article addresses the presentation, evaluation, and treatment of the more common pulmonary complications that can occur in solid

Role of immunotyping in chronic lymphocytosis: review of the natural history of the condition in 145 adult patients.

We investigated the clinical value of immunotyping in 145 consecutive adult patients with absolute or relative lymphocytosis: 132 (91%) had B-cell lymphocytosis, 5 (4%) had T-cell lymphocytosis, 2 (1%) had hairy cell leukemia, and 6 (4%) had reactive lymphocytosis. Of the five patients with T-cell lymphocytosis, four were best categorized as having T gamma-chronic lymphoproliferative disease and had an indolent clinical course. Of the 132 patients with B-cell lymphocytosis, 121 (92%) had B-cell chronic lymphocytic leukemia (B-CLL), and 11 (8%) had small cleaved ("lymphosarcoma") cell leukemia.

Regulatory T-cell deficiency leads to features of autoimmune liver disease overlap syndrome in scurfy mice.

INTRODUCTION: Scurfy mice have a complete deficiency of functional regulatory T cells (Treg) due to a frameshift mutation in the Foxp3 gene. The impaired immune homeostasis results in a lethal lymphoproliferative disorder affecting multiple organs, including the liver. The autoimmune pathology in scurfy mice is in part accompanied by autoantibodies such as antinuclear antibodies (ANA). ANA are serological hallmarks of several autoimmune disorders including autoimmune liver diseases (AILD). However, the underlying pathogenesis and the role of Treg in AILD remain to be elucidated. The present st

[Thoracic lymphadenopathy due to vascular transformation of lymph node sinuses associated with upper limb edema in a chronic hemodialysis patient with congestive heart failure].

BACKGROUND: Vascular transformation of lymph node sinuses (VTLS) is a rare disorder characterized by transformation of lymph node sinuses into endothelium-lined capillary-like channels. This phenomenon was originally discovered by accident whilst examining regional lymph nodes draining cancer. However, it has been found in association with other conditions associated with lympho venous congestion and distension, such as congestive heart failure (CHF) or even lymphoadenopathy alone. CASE REPORT: We describe the clinical case of a male dialysis patient with CHF (secondary to ischemic-hypertensi

Chediak-Higashi syndrome.

PURPOSE OF REVIEW: Chediak-Higashi syndrome, a rare autosomal recessive disorder, was described over 50 years ago. Patients show hypopigmentation, recurrent infections, mild coagulation defects and varying neurologic problems. Treatment is bone marrow transplant, which is effective in treating the hematologic and immune defects, however the neurologic problems persist. The CHS1/LYST gene was identified over 10 years ago and homologous CHS1/LYST genes are present in all eukaryotes. This review will discuss the advances made in understanding the clinical aspects of the syndrome and the function

Lymphomatoid Papulosis Type B in a Patient with Crohn's Disease Treated with TNF-Alpha Inhibitors Infliximab and Adalimumab.

Dear Editor, Lymphomatoid papulosis (LP) is a chronic, recurrent, usually self-limited papulonecrotic or papulonodular skin disease, which belongs to the group of primary cutaneous CD30+ lymphoproliferative disorders (1). Three main histological subtypes of LP have been recognized: type A (histiocytic), type B (mycosis fungoides-like), and type C (anaplastic large cell lymphoma-like). Recently, new histologic LP variants classified as type D (CD8-positive, cytotoxic form) and type E (angioinvasive form) have also been described. The etiology of LP has not been determined to date (2-4). Herein …

Diagnostic criteria for NK cell large granular lymphocyte leukemia: validation through a multicentric international study.

Natural killer (NK) large granular lymphocytic leukemia (LGLL) is a rare lymphoproliferative disorder lacking definitive clonality markers, complicating diagnosis and distinction from reactive NK cell expansions. We previously proposed an NK clonality score with high diagnostic accuracy, but a subset of patients remained unclassified. In this multicenter international study, we refined and validated updated diagnostic criteria using independent training (n = 78) and validation (n = 57) cohorts from 3 national registries (United States, Italy, and France). The revised framework integrates NK sc

Interleukin-6 levels decrease following rituximab treatment but increase following tocilizumab treatment.

Idiopathic multicentric Castleman disease (iMCD) is a rare, gradually progressive lymphoproliferative disorder thought to arise from chronic hypercytokinemia, with IL-6 playing a central role in disease manifestations. C-reactive protein (CRP) levels are widely used as a surrogate marker of IL-6 activity in clinical practice. Although anti-IL-6 therapies, such as tocilizumab, are established treatments for iMCD, spurious elevation of circulating IL-6 levels after IL-6 pathway blockade has been reported, complicating biomarker interpretation. In contrast, changes in IL-6 and CRP levels followin

Ectopic Epstein-Barr virus infection in T cells identifies reduced therapeutic responsiveness and immune dysfunction after allogeneic haematopoietic stem cell transplantation.

OBJECTIVES: Epstein-Barr virus (EBV) reactivation after allogeneic haematopoietic stem cell transplantation (allo-HSCT) is typically monitored by plasma viral load, but this fails to capture cellular reservoirs. Lineage-specific testing, although usually performed in high-risk patients, may identify biologically distinct disease states. We aimed to evaluate the clinical and biological significance of lineage-specific EBV quantification especially ectopic EBV infection in T cells in allo-HSCT recipients with EBV reactivation. METHODS: We analysed 144 allo-HSCT recipients with EBV reactivation

A rare case of unicentric Castleman disease in the posterior mediastinum: case report and integrated literature review.

BACKGROUND: Castleman disease (CD) is a complex lymphoproliferative disorder characterized by nonspecific clinical manifestations and imaging features. When occurring within the mediastinum, it is easily confused with other diseases, posing diagnostic challenges. We report a case of unicentric hyaline vascular Castleman disease (UCD) located in the posterior mediastinum. This case report is supplemented by a retrospective analysis of the literature, aiming to enhance understanding and improve diagnostic accuracy for this condition. CASE SUMMARY: This paper provides a detailed account of the c

Prognostic impact of cancer-directed surgery in primary pulmonary lymphoma: a population-based, propensity score-matched analysis.

BACKGROUND: Primary pulmonary lymphoma (PPL) is a rare lymphoproliferative disorder with unclear optimal management strategies. The prognostic impact of cancer-directed surgery (CDS) remains controversial, with existing evidence limited to small institutional series yielding conflicting conclusions. This study aimed to evaluate the association between CDS and survival outcomes in patients with PPL using a large population-based cohort. METHODS: Patients diagnosed with PPL between 2000 and 2014 were identified from the Surveillance, Epidemiology, and End Results (SEER) database. Propensity sco

Thymic Carcinoma Presenting as Diffuse Bone Marrow Infiltration Mimicking Lymphoma.

Thymic epithelial tumors (TETs) represent the most common primary neoplasms of the anterior mediastinum, yet thymic carcinoma (TC) stands out as an exceptionally rare and biologically aggressive subtype. Unlike thymomas, TCs exhibit marked cytologic atypia, early invasive potential, and a high predisposition for extrathoracic metastasis. We report the case of a 59-year-old male who presented with a seven-month history of progressive, refractory lumbar pain and functional limitation. Initial clinical and laboratory evaluation revealed normocytic anemia, leukocytosis, elevated inflammatory marke

Kimura Disease: Retrospective Analysis of 53 Cases and Three Mepolizumab-Responsive Cases.

BACKGROUND: Kimura disease (KD) is a rare, chronic lymphoproliferative disorder, which is believed to be driven by a Th2 immune response, though its exact pathogenesis remains unclear. Despite various treatment approaches, no standardized therapy has been established. This study investigated the clinical features of 53 KD patients and evaluated the therapeutic potential of mepolizumab in three cases. METHODS: A retrospective age-stratified analysis was performed on 53 patients diagnosed with KD at two tertiary medical centers in China, covering the period from January 2014 to December 2024. W

Simultaneous total internal biliary diversion during liver transplantation for progressive familial intrahepatic cholestasis type 1: Standard of care?

Patients post liver transplant (LT) with progressive familial intrahepatic cholestasis type 1 (PFIC-1) often develop progressive graft steatohepatitis, intractable diarrhea, and growth failure. A total internal biliary diversion (TIBD) during an LT may prevent or reverse these adverse events. Children with PFIC-1 who underwent an LT at our institute were divided into 2 groups, A and B based on the timeline where we started offering a TIBD in association with LT. Pre-LT parameters, intraoperative details, and posttransplant complications like graft steatosis and diarrhea were also analyzed betw

Risks of malignancies related to disease-modifying antirheumatic drugs in rheumatoid arthritis: a pharmacovigilance analysis using the FAERS database.

OBJECTIVES: Over the years when disease-modifying antirheumatic drugs (DMARDs) have been used in rheumatoid arthritis patients, reports of malignancies have emerged. This study aims to investigate the association between malignancies and DMARDs by using data extracted from the Food and Drug Administration Adverse Event Reporting System (FAERS). METHODS: FAERS data (January 2019 to December 2023) were reviewed. For each drug-event pair, the disproportionality analysis was conducted to evaluate the risk of malignancy. Multivariate logistic regression was implemented to mitigate potential biases
