Severe Renal Phenotype Across A Multigenerational Tuberous Sclerosis Complex (TSC) Family.

BACKGROUND: Tuberous sclerosis complex (TSC) is an autosomal dominant genetic disorder characterized by the formation of hamartomas in the brain, kidney, and heart, along with other complex clinical manifestations, including TSC-associated neuropsychiatric disorder (TAND). Development of genotype-phenotype correlations within TSC can aid clinicians in providing prognostic data and improve clinical management. We present here a multigenerational family who has a pathogenic variant in TSC2 displaying a severe renal phenotype. METHODS: A 23-year-old Caucasian male (Patient 1) was determined to h

Prader-Willi syndrome as a neurogenetic model for psychosis and obsessive-compulsive disorder: A review of clinical, behavioral, and biological insights.

Prader-Willi syndrome (PWS) is a complex neurodevelopmental disorder classically defined by hyperphagia and obesity. However, its profound psychiatric phenotype offers a unique genetic framework for understanding major mental illnesses. This review positions PWS as a potentially informative biological model for psychosis and obsessive-compulsive disorder (OCD), bridging the gap between 15q11-q13 imprinting defects and neural circuit dysfunction. We synthesize evidence demonstrating that psychosis in PWS is not a uniform trait but is disproportionately linked to the maternal uniparental disomy

Recurrent Dystrophic Calcification and Squamous Cell Carcinoma in a Cultured Keratinocyte Scar 35 Years After a 60% Total Body Surface Area Flame Burn.

Dystrophic calcification of burn scars is a rare but disabling late complication that can result in chronic ulceration and increase the risk of squamous cell carcinoma. A male patient sustained burns covering 60% of his total body surface in 1989 and received a combination of split-thickness autografts, paternal homografts, and cultured keratinocyte sheet grafts. The first ulcer appeared in 2004 and recurrent lower limbs ulcerations continued intermittently over the following 2 decades. Imaging and histopathology revealed subcutaneous calcium deposits and 3 distinct well-differentiated squamou

Gates of hardened steel: Bruce Springsteen's Mansion on the Hill and the father-son relationship.

This essay contributes to the medical humanities by using Bruce Springsteen's 1982 song 'Mansion on the Hill' from his album Nebraska to interrogate the clinical aetiology of the 'paternal void' and generational emotional inheritance. Grounded in rigorous psychoanalytical scholarship, the analysis synthesises the relational theories of D W Winnicott and more contemporary advances by Jessica Benjamin, Christopher Bollas and André Green to decode the song's architectural and narrative metaphors. The distant, hardened steel gates of the mansion represent the emotional distance of the father rather

Genetic diversity of the Y-chromosome in domestic animals: from evolutionary insights to functional implications.

The Y-chromosome, a hallmark of male-specific inheritance, holds immense importance in domestic animal genetics. It harbors the Sex-determining Region Y (SRY) gene, which initiates testis development and male differentiation. Due to its strict paternal inheritance and minimal recombination outside the pseudoautosomal regions (PARs), the Y-chromosome is a stable and reliable marker for studying male lineage, phylogeography, and patrilineal evolution in domestic species. Despite these constraints, advances in sequencing technologies and molecular tools (like Y-STRs and Y-SNPs) have enabled in-de

The impact of mercury exposure on male reproduction: Mechanistic insights.

Mercury is a pervasive environmental toxin with significant negative effects on human health. In occupational settings, incidents such as the Minamata and Niigata disease in Japan and the large-scale methylmercury poisoning in Iraq have highlighted the severe health impacts of mercury exposure. It is widely accepted that all forms of mercury including methylmercury and mercuric chloride have the potential to induce toxic effects in mammals, and there is increasing concern about the impact of environmentally relevant levels of mercury on reproductive functions. This review summarizes current kn

Predictors of the impact of rights- and recovery-oriented training for mental health professionals.

OBJECTIVE: This study examines the factors influencing the effectiveness of a rights- and recovery-based training program for mental health professionals. METHODS: From an initial pool of 643 professionals providing informed consent, 218 (33.9%) attended the training activity and completed the first follow-up assessment. Effectiveness was evaluated using a scale measuring beliefs and attitudes toward service users. Potential predictors included gender, age, experience, ideological orientation, education, profession type, personal or close experience with mental distress, and training satisfac

The Impact of the Parental Patterns of Morbidity and Comorbidity in the Cross-Generational Transmission of Risk for Major Depression and Alcohol Use Disorder.

To further understand the inter-relationship of the familial transmission of major depression (MD) and alcohol use disorder (AUD), we examine, via a multivariable Cox proportional hazards model, risks for AUD and MD in 1,244,516 individuals born in Sweden from 1970 to 1990 to intact mother-father pairs as a function of parental diagnoses of MD and/or AUD. Across the nine possible mating types, we see both direct transmission (MD → MD, AUD → AUD) and also, less strongly, indirect transmission: MD → AUD and AUD → MD. Risks in offspring accumulate with multiple affected parents, which reveals the

De novo mutation of the RET proto-oncogene revealing multiple endocrine neoplasia type 2A: a sporadic case from Western Algeria.

BACKGROUND: Multiple endocrine neoplasia type 2A (MEN2A) is a rare autosomal dominant disorder associated with RET proto-oncogene mutations. Early diagnosis is absolutely critical for managing and preventing aggressive forms of medullary thyroid carcinoma (MTC) and associated tumours. Our aims are to confirm the clinical diagnosis and to detect, in relatives, those who bear the familial mutation in order to benefit from medical management before the first symptoms appear. CASE PRESENTATION: We report a sporadic case of MEN2A in a 34-year-old woman from western Algeria. She presented with bila

Immunoglobulin therapy for infertility and the role of immune cells in pregnancy success: An extensive investigation and update.

In the United States, roughly one out of every eight couples, or 7.5 million women, experience challenges related to conceiving or maintaining a pregnancy. The body's immune response is vital during pregnancy. T cells, natural killer (NK) cells, B cells, and macrophages (MQ) are immune cells in the female reproductive tract. They are in charge of maintaining tissue homeostasis and regulating the immune system's response to invasive pathogens. Failure to regulate these immune cells might result in inflammation, which reduces fertility. The immune system modulation of pregnancy loss has been stu

Familial confounding in the associations between maternal health and autism.

Evidence suggests that maternal health in pregnancy is associated with autism in the offspring. However, most diagnoses in pregnant women have not been examined, and the role of familial confounding remains unknown. Our cohort included all children born in Denmark between 1998 and 2015 (n = 1,131,899) and their parents. We fitted Cox proportional hazard regression models to estimate the likelihood of autism associated with each maternal prenatal ICD-10 diagnosis, accounting for disease chronicity and comorbidity, familial correlations and sociodemographic factors. We examined the evidence for

Risk factors predisposing children to transient hypogammaglobulinemia of infancy.

Introduction: Transient hypogammaglobulinemia of infancy (THI) is a heterogeneous disorder characterized by reduced serum immunoglobulin G (IgG) levels in early infancy. Objective: This study aimed to identify potential risk factors associated with THI. Methods: Children with THI and normoglobulinemic healthy children were compared by using a questionnaire that addressed possible risk factors. Results: In total, 108 participants were enrolled, 54 patients with THI and 54 healthy controls. The median age at diagnosis of the patients with THI was 17 months (range, 4-38 months), and 40 (74.1%) we

Exploring Lymphocyte Immunotherapy as a Treatment for Recurrent Miscarriage: A Case Study of Immune System Modulation and Positive Clinical Pregnancy Rate.

This case study reviews the use of Lymphocyte Immunotherapy (LIT) in a 28-year-old female patient who had a history of recurrent first-trimester miscarriages and failed intracytoplasmic sperm injection (ICSI) cycles. She had good ovarian reserve, normal semen analysis, and negative autoimmune tests but experienced four early miscarriages, which suggested an immune-mediated issue. LIT was chosen as a treatment option to address the possibility of immune system dysregulation contributing to pregnancy loss. LIT is the process of infusing the husband's lymphocytes to promote maternal tolerance of

Investigation of a mouse model of Prader-Willi Syndrome with combined disruption of Necdin and Magel2.

Prader-Willi syndrome (PWS) is a multigenic disorder caused by the loss of 7 contiguous paternally expressed genes. Mouse models with inactivation of all PWS genes are lethal. KO mouse models for each candidate gene have been generated, but they lack the functional interactions between PWS genes. Here, we revealed an interplay between Necdin and Magel2 PWS genes and generated a mouse model (named Del Ndn-Magel2 mice) with a deletion including both genes. A subset of Del Ndn-Magel2 mice showed neonatal lethality. Behaviorally, surviving mutant mice exhibited sensory delays during infancy and al

Protective mechanisms of zinc and/or selenium supplementation against BPA-induced male and offspring reproductive toxicity: Insights from multiomics analysis.

Bisphenol A (BPA), a common environmental endocrine disruptor, has been shown to adversely affect male reproductive function. However, the mechanisms of reproductive impairment in both BPA-exposed males (F0) and their unexposed offspring (F1) remain unclear. Furthermore, there are currently no reports on that paternal Zinc (Zn) and/or Selenium (Se) supplementation ameliorate BPA-induced and transgenerational reproductive damage in F0 and F1, respectively. This study aimed to address these scientific issues by integrating transcriptomics and metabolomics. Our results revealed that BPA induced r

Laparoscopic Adrenal Surgery in an Adolescent Boy in the Caribbean With Malignant Hypertension Secondary to Pheochromocytoma: A Case Report and Literature Review.

Malignant hypertension is defined as extremely elevated blood pressure (BP) with or without target organ damage. It can be either hypertensive urgency, characterized as malignant hypertension without any clear evidence of immediate organ damage, or hypertensive emergency with proof of new or worsening target-organ damage. The modern definition of hypertensive emergencies without retinopathy would be based on the presence of acutely raised BP with injury to at least three target organs. Pheochromocytoma is the rarest cause of malignant arterial hypertension, characterized by the classic triad o

The HRAS Variant c.175G>A (p.Ala59Thr) Causes a Predominantly Ectodermal Phenotype Lacking Classic Costello Syndrome Features.

Costello syndrome (CS) is a rare dominant HRAS RASopathy characterized by curly hair, cardiac abnormalities, craniofacial anomalies, and developmental delay. HRAS codon 58, 59, and 60 variants are associated with milder phenotypes. We describe a three-generation family with a previously unreported heterozygous HRAS variant c.175G>A (p.Ala59Thr) causing a predominantly ectodermal phenotype. Exome and Sanger sequencing were used for genetic analysis. Dermatological and cardiac evaluations were performed, including skin biopsy and hair sample microscopy. Biallelic variants in DNAH11 cause male infertility with asthenozoospermia in a Chinese non-consanguineous family: A case report.

RATIONALE: Asthenozoospermia (AZS), a leading cause of male infertility, frequently results from genetic defects that disrupt flagellar assembly. Mutations in the DNAH11 gene can cause abnormal dynein arm structures, leading to AZS and impaired male fertility. However, current evidence regarding DNAH11 mutations in male infertility remains limited, warranting further exploration of their pathogenic mechanisms. PATIENT CONCERNS: A 35-year-old Chinese male presented with primary infertility and AZS. DIAGNOSES: Whole-exome sequencing was performed on the proband to identify the potential geneti

Case Report: Lethal neonatal hypertrophic cardiomyopathy from compound heterozygous MYBPC3 variants.

INTRODUCTION: Bi-allelic pathogenic variants in MYBPC3 cause a rare and lethal neonatal form of hypertrophic cardiomyopathy (HCM) that often evades detection during routine prenatal screening. We report a comprehensive investigation of such a case to highlight the clinical utility of postmortem molecular diagnosis. METHODS: A two-month-old infant died from sudden-onset acute heart failure. We performed a full forensic autopsy with detailed histological examination and conducted trio-based whole-exome sequencing (WES) on the proband and parents to identify the genetic etiology. RESULTS: Postm
