Neurophysiologic dysfunction in hypomelanosis of Ito: EEG and evoked potential studies.

To evaluate neurological involvements in hypomelanosis of Ito (HI), we conducted electroencephalographic and evoked potential studies in 3 children with HI. Patient 1 presented with the West syndrome, and was severely handicapped. Patient 2 had mild mental retardation. Patient 3 appeared neurologically normal, but chromosome analysis showed a 46,XX/46,XX,12p + mosaic karyotype. EEGs of Patient 1 initially showed asymmetric hypsarrhythmia, and later diffusely increased fast waves, occipital dominant alpha rhythm plus multifocal spikes. Patients 2 and 3 had minimal and no EEG abnormalities, resp

Ocular pigmented spots and eyelid myxomas.

Ophthalmic abnormalities are part of a recently recognized syndrome that includes myxomas, spotty pigmentation, and endocrine overactivity. The complex is familial with mendelian dominant mode of inheritance. The 63 patients (27 males and 36 females) we have identified with this complex had the following ophthalmic abnormalities: facial and eyelid lentigines in 44 patients (70%); pigmented lesions on the caruncle or conjunctival semilunar fold in 17 patients (27%); and eyelid myxomas in ten patients (16%). Because these findings usually preceded the most serious component of the syndrome, card

Sunscreens: topical and systemic approaches for protection of human skin against harmful effects of solar radiation.

This review deals with topical and systemic approaches for protection of human skin against the harmful effects of solar radiation. Two concerns about the deleterious effects of sun exposure involve: (1) acute effects (e.g., sunburn and drug-induced phototoxicity) and (2) potential long-term risks of repeated sun exposures leading to development of solar elastosis, keratoses, induction of both nonmelanoma and melanoma skin cancer, and alteration of immune responses and functions. Action spectra of normal and abnormal reactions of human skin to acute and chronic effects of solar radiation are p

Fenestrated sheen macular dystrophy. A new autosomal dominant maculopathy.

A new, slowly progressive, autosomal dominant macular dystrophy was found in five patients from three generations of a family, with follow-up for long as 27 years. The mild functional abnormalities of this dystrophy were roughly correlated with age; in the sixth decade of life, paracentral scotomatous areas were noted. The earliest ophthalmoscopic findings were present in a 4-year-old and consisted of a yellowish refractile sheen with red fenestrations within the sensory retina of the macula. By the third decade, an annular zone of hypopigmentation of the retinal pigment epithelium (RPE) appea

Effect of radiation injury on the growing face.

The literature on the effect of ionizing radiations on developing and rapidly growing tissue has been reviewed, and the effect especially on children who received radiation therapy in the Clinic for Maxillo-Facial and Plastic Surgery of the University of Dusseldorf for various tumours of the head and neck presented. Our findings indicate that we do not yet know the minimal irradiation dosage that may cause growth retardation in infants and children. It has therefore been suggested that surgery should be more frequently considered in the treatment of childhood tumours, since this caused less si

Congenital circumscribed hypomelanosis: a characterization based on electron microscopic study of tuberous sclerosis, nevus depigmentosus, and piebaldism.

Subcellular defects of hypomelanosis in tuberous sclerosis (TS) (28 subjects) were compared by light and electron microscopy with oThere forms of congenital circumscribed hypomelanosis that occur in nevus depigmentosus (ND) (8 subjects) and in piebaldism (PB) (4 subjects), respectively. On the light microscopic level in both TS and ND, the population density of functioning melanocytes was normal but each perikaryon was small, and dopa activity was decreased. On the ultrastructural level, the hypomelanotic skin and hair of TS were associated with a decrease in the synthesis, melanization, and s

A new formula for depigmenting human skin.

Complete depigmentation of the noraml skin of adult male blacks was procured by the daily application for five to seven weeks of a formula consisting of 0.1% tretinoin, 5.0% hydroquinone, 0.1% dexamethasone, and hydrophilic ointment. Depigmentation was not attainable when any one of the components was omitted. The formula was therapeutically effective in treatment of melasma, ephelides, and postinflammatory hyperpigmentation. Senile lentigines were resistant to this therapy.

Fibrous dysplasia.

Three cases of monostotic fibrous dysplasia of the cranial bones are presented. Several theories of the etiology of this uncommon condition are put forward and the relationship between monostotic and polyostotic fibrous dysplasia, Albright's disease, and hyperparathyroidism is examined. The difficulty in diagnosis and the ways they may present to the ear, nose and throat surgeon are discussed. It is suggested that conservative surgery is the best mode of treatment for maxillary or temporal lesions. However, ethmoid lesions, particularly when the optic foramen or base of skull are eroded, may n

Clinical features in 27 patients with Angelman syndrome resulting from DNA deletion.

We report the clinical features in 27 Australasian patients with Angelman syndrome (AS), all with a DNA deletion involving chromosome 15(q11-13), spanning markers from D15S9 to D15S12, about 3 center dot 5 Mb of DNA. There were nine males and 18 females. All cases were sporadic. The mean age at last review (end of 1994) was 11 center dot 2 years (range 3 to 34 years). All patients were ataxic, severely retarded, and lacking recognisable speech. In all patients, head circumference (HC) at birth was normal but skewed in distribution, with 62 center dot 5% at the 10th centile. At last review HC w

Generalized melanoses and nonmelanotic pigmentations of the head and neck.

The dentist is in a strategic position to evaluate early color changes of the head and neck. Certain oral and perioral changes of pigmentation are amenable to early diagnosis. Systemically acquired generalized melanoses and nonmelanotic color changes should be periodically reviewed by all examining physicians and dentists. The multiple forms of acanthosis nigricans are described, and the triad of Albright's syndrome is compared with the findings of Peutz-Jegher's syndrome, multiple basal cell nevi, neurofibromatosis, and Addison's disease. The spectrums of changes seen in abnormal nonmelanotic

[Clinical observation of implant-natural tooth supported fixed partial denture for replacement of missing teeth in 86 patients].

Glaucoma represents a major cause of vision loss throughout the world. Primary open-angle glaucoma, the most common form of glaucoma, is a chronic, progressive disease often, though not always, accompanied by elevated intraocular pressure (IOP). In this disorder, retinal ganglion cell loss and excavation of the optic nerve head produce characteristic peripheral visual field deficits. Patients with normal-tension glaucoma present with typical visual field and optic nerve head changes, without a documented history of elevated IOP. A variety of secondary causes, such as pigment dispersion syndrom

A novel deletion in the BLOC1S6 Gene Associated with Hermansky-Pudlak syndrome type 9 (HPS-9).

BACKGROUND: Hermansky-Pudlak Syndrome (HPS), a rare autosomal recessive disorder, is characterized by oculocutaneous albinism, bleeding diathesis, and sometimes severe lung problems and inflammatory bowel disease. Symptoms include skin and hair pigmentation variations, along with visual impairments. Variants in eleven genes encoding protein complexes essential for membrane trafficking and intracellular endosomal transport pathways underlie various recognized HPS subtypes. This study focuses on HPS-9, a subtype of Hermansky-Pudlak Syndrome caused by a variant in the BLOC1S6 gene, which is a sub

Arthroscopic Management of Pigmented Villonodular Synovitis of the Hip in Children and Adolescents.

BACKGROUND: Pigmented villonodular synovitis (PVNS) is a benign proliferative synovial disorder most commonly described to affect the knee in adults. Literature describing PVNS in the pediatric population is limited to 2 small case series and a handful of single-patient case reports. Within these studies, only 2 patients with PVNS of the hip are described. PURPOSE: To describe the presentation, management, and outcomes of a single-center series of pediatric patients with PVNS of the hip treated with arthroscopic synovectomy. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A retros
