In situ cephalic vein bypasses from axillary to the brachial artery after catheterization injuries.

The need to bypass to the brachial artery is rare. Over a five-year period, 16 patients had suffered iatrogenic post-catheterization injuries of the upper extremity. We have performed 16 bypasses, in 16 patients, mean age was 65 years (range 47-75), to the brachial artery originating from an artery proximal to the shoulder joint. In all cases, the axillary artery was the donor artery. All bypasses were created by using the cephalic vein with the in situ technique and distal anastomoses were made to a distance-free section of brachial artery. No operative mortality, neurological complications o

Reconstruction of a missed posterior locked shoulder fracture-dislocation with bone graft and lesser tuberosity transfer: a case report.

INTRODUCTION: Posterior shoulder fracture-dislocation is a rare emergency condition with poor prognosis when there is a delay in diagnosis and presence of associated injuries. CASE PRESENTATION: We present a case of a neglected four-part fracture-dislocation of the proximal humerus in a 34-year-old Greek woman. Except from the substantially displaced and comminuted tuberosity fractures, an anterolateral defect of approximately 50% of the articular surface was apparent. Open reduction of the humeral head was followed by reconstruction of the proximal humerus with allograft impaction, transfer

Functional electrical stimulation in tetraplegic patients to restore hand function.

The purpose of this collective review is to describe a new form of functional electrical stimulation called neuroprosthesis. This unique technology has been devised to produce lateral pinch and palmar grasp in persons with C5 and C6 motor level spinal cord injuries. This neuroprosthesis includes external as well as implanted components. First, a receiver is surgically implanted into the patient's chest above a pectoralis major muscle. The receiver stimulator is then connected to 8 surgically implanted epimysial or intramuscular electrodes. Restoration of upper extremity function can greatly im

Outcomes of surgical treatment of brachial plexus injuries using nerve grafting and nerve transfers.

Between 1993 and 1998, 32 male patients with brachial plexus injuries were surgically treated. Eighteen interfascicular grafting and 71 extraplexal neurotization procedures were performed separately or in combination. Donor nerves were the intercostals, spinal accessory, phrenic, contralateral C7, and cervical plexus, in order of frequency. Patients were followed for a minimum of 24 (average, 35) months. Biceps function was best following grafting the musculocutaneous nerve itself, or neurotization with the phrenic nerve (100 percent grade 4), followed by neurotization with the intercostals (8

Superior detachment of a glenoid labrum variant resembling an incomplete discoid meniscus in a wheelchair ambulator.

Symptoms of instability and painful clunking in the shoulder were found associated with superior detachment of an incomplete discoid glenoid labrum and biceps origin in a 25-year-old woman. The patient was paraplegic below the T4 level secondary to resection of a spinal cord astrocytoma at age of 17. Symptoms began after 3 years of wheelchair use and repetitive transfer injuries. On arthroscopic examination, only the central surface of the articular glenoid cartilage was visible because of labral coverage. The entire labrum was meniscoid without distinguishable variation. The superior labrum w

Mechanisms of brachial plexus lesions.

The main causes of brachial plexus palsies are traction, due to extreme movements, and heavy impact. In downward traction of the arm and forcible widening of the shoulder-neck angle the lesion will occur in the upper roots and trunk. Forcible upward traction will cause avulsion of T1 and C8. The most violent trauma will result in a lesion at all levels. Rupture of the cords and/or individual infraclavicular nerves will be produced by traction and/or forcible widening of the scapulohumeral angle. Vascular structures are subjected to the same mechanism and injuries of these structures give infor

[Three-dimensional computerized tomography in trauma surgery. A case presentation].

Three-dimensional images of bone structures can easily be reconstructed from computed tomography data. The technique and the advantages of contemplating reconstructions of bone defects in different directions are described. A special software programme allows to combine all data which are given by the standard CT. The slice diameter is two millimetres. The reconstruction shows bone surfaces in all required views. In traumatic and orthopaedic surgery 3-D-CT is useful in the analysis, detection and preoperative planning of comminuted bone injuries of the spine, pelvis, knee, shoulder and the cal

CT arthrography and arthroscopy in chronic glenohumeral joint instability.

The authors evaluate the value of CT arthrography in glenohumeral joint instability. A study was conducted on a group of 16 patients with recurrent dislocation of the shoulder. All patients underwent CT arthrography and arthroscopy. The diagnostic accuracy of CT arthrography was rated for different types of lesions on the basis of arthroscopic confirmation of its findings. Our results, which include a statistical analysis, showed a diagnostic accuracy of 97.3% in Bankart lesions and 100% in Hill-Sachs lesions and loose bodies. Dilation of the subscapular bursa and injury of the glenoid labrum-

[Peroperative vascular accidents during orthopedic surgery. Apropos of 55 cases].

The functional or vital prognosis may be affected by the rare vascular accidents resulting from orthopedic operations. During a 10 year period (1970-1980), 55 vascular lesions were observed in 40 patients, and were either arterial (40 cases) or venous (15 cases) in type. Vascular injuries in 9 cases were due to spinal operations (cervical, lumbosacral or herniated disc arthrodesis), in 7 cases to shoulder operations (recurrent dislocation, excision of first rib), in 9 cases to hip surgery (prosthesis, plate and screws, fractured acetabulum), and in 15 cases to operations on the lower limbs (os

[Injuries on vaulting].

OBJECTIVE AND METHOD: Using an online questionnaire, the injuries of 649 acrobats on horseback were recorded (636 female, 13 male, average age 20.8 years, average sports time 12.1 years). 64% of the athletes practised their sport in the upper and 34% in the lower performance classes. RESULTS: 62.7% of the athletes hurt themselves on vaulting at least once during their sports time. Most injuries occured at the lower limbs (57.5%), particularly on foot (31.2%) and knee (22.2%). At the upper limbs (26.1%), injuries were most often registered at hands (8.4%) and shoulders (6.0%). 16.4% of the ath

Combined nerve transfers for repair of the upper brachial plexus injuries through a posterior approach.

The upper brachial plexus injury leads to paralysis of muscles innervated by C5 and C6 nerve roots. In this report, we present our experience on the use of the combined nerve transfers for reconstruction of the upper brachial plexus injury. Nine male patients with the upper brachial plexus injury were treated with combined nerve transfers. The time interval between injury and surgery ranged from 3 to 11 months (average, 7 months). The combined nerve transfers include fascicles of the ulnar nerve and/or the median nerve transfer to the biceps and/or the brachialis motor branch, and the spinal a

Preoperative hyperglycemia within 24 hours of surgery is associated with adverse outcomes after rotator cuff repair.

BACKGROUND: Rotator cuff repair (RCR) is a common orthopedic procedure that relies on proper tendon healing for good outcomes. While hyperglycemia is known to worsen outcomes in other surgeries, its effects on RCR remain underexplored. We investigated whether preoperative hyperglycemia is associated with increased complication rates following RCR using a large, multi-institutional database. METHODS: Using the TriNetX Research Network, we identified patients who underwent primary RCR between May 2005 and May 2024. Patients were divided into two cohorts based on preoperative glucose within 24 h

Critical Predictors of the Outcomes of Rotator Cuff Arthroscopic Repair.

Objective  To evaluate the functional outcomes after rotator cuff arthroscopic repair in an adult population and to establish risk factors for poor prognosis in this procedure. Methods  This retrospective cohort study analyzed medical records of 302 shoulders that underwent arthroscopy for rotator cuff repair between October 1, 2014, and December 31, 2017. Patients were included if they had a complete rotator cuff injury and met other specific criteria. Surgical procedures were performed consistently, and postoperative care involved a structured rehabilitation protocol. The primary outcome mea

Limited incision plating of midshaft clavicle fractures: a case series of 1,038 patients.

BACKGROUND: Clavicle fractures are a common injury in young and active patients. Nonoperative management has historically been preferred; however, recent studies have challenged this paradigm with evidence of lower nonunion rates and improved functional outcomes following surgical treatment. This article describes a limited incision approach to clavicle plating and reports long-term functional and radiographic outcomes for a large cohort of operative midshaft clavicle fractures. METHODS: A prospectively maintained database was used to identify 1,038 patients who underwent surgical treatment f

Chronic Adaptation of the Coracohumeral Space and Subscapularis Tendon in Professional Baseball Pitchers.

BACKGROUND: Pitchers frequently experience anterior shoulder pain, possibly associated with coracohumeral impingement; however, whether the coracohumeral distance (CHD) and/or subscapularis tendon adapt chronically (bilateral difference) due to pitching, and whether clinical measures are associated with CHD and subscapularis tendon organization have not been evaluated in professional pitchers. HYPOTHESIS: The authors hypothesized that dominant arm CHD would be smaller than the nondominant arm, dominant subscapularis tendon would have increased spatial frequency (ie, be more disorganized), and

Handrim kinetics and quantitative ultrasound parameters for assessment of subacromial impingement in wheelchair users with pediatric-onset spinal cord injury.

BACKGROUND: Most manual wheelchair users with pediatric-onset spinal cord injury (SCI) will experience shoulder pain or pathology at some point in their life. However, guidelines for preservation of the upper limb in children with SCI are limited. RESEARCH QUESTION: What are the relationships between manual wheelchair handrim kinetics and quantitative ultrasound parameters related to subacromial impingement in individuals with pediatric-onset SCI? METHODS: Subacromial impingement risk factors including supraspinatus tendon thickness (SST), acromiohumeral distance (AHD), and occupation ratio
