Intravenous sedation for dental patients with intellectual disability.

BACKGROUND: The poor quality of oral health care for people with intellectual disability (ID) has been recognized, and the strong fears about dental treatment suggested as a major reason for disturbances of visits to dentists by such patients. Intravenous sedation is a useful method for relieving the anxiety and fear of such patients about dental treatment, and is frequently essential for ID patients undergoing dental treatment. However, decision regarding the dose of sedative required to be administered for an adequate level of sedation is difficult because the effect of sedation cannot be ad

Pentobarbital vs chloral hydrate for sedation of children undergoing MRI: efficacy and recovery characteristics.

BACKGROUND: Chloral hydrate (CH) sedation for magnetic resonance imaging (MRI) is associated with significant failure rates, adverse events and delayed recovery. Pentobarbital (PB), reportedly produces successful sedation in 98% of children undergoing diagnostic imaging. This study compared the efficacy, adverse events and recovery characteristics of CH vs PB in children undergoing MRI. METHODS: With Institutional Review Board approval and written consent, children were randomly assigned to receive intravenous (i.v.) PB (maximum 5 mg x kg(-1) in incremental doses) or oral CH (75 mg x kg(-1))

Acute coronary syndrome induced by intravenous ephedrine in pregnant woman with normal coronaries.

Intravenous ephedrine administered during a C-section was observed to cause an acute coronary syndrome in a pregnant woman with normal coronaries. The patient developed sub-sternal chest pain, was noted to have 10 beats of non-sustained ventricular tachycardia, ST abnormalities were observed on her ECG and cardiac enzymes were elevated. The patient had normal coronary arteries by angiogram and during a one-year period of follow up no further cardiac events occurred.

Comparison of the effects of four anaesthetic agents on somatosensory evoked potentials in the rat.

Electrophysiological techniques provide an objective and non-invasive measure of neurological function. In order to undertake detailed evoked potential studies in rats on repeated occasions, it is necessary to find an appropriate anaesthetic agent which has minimal and reproducible effects on the parameters to be studied and also has a minimal effect on the general welfare of the animals. In this study we compared the effects of four common anaesthetic agents (ketamine-xylazine, medetomidine, isoflurane and fentanyl/fluanisone-midazolam) on somatosensory evoked potentials (SEPs) in rats follow

[Anesthetic managements of a patient with multiple sclerosis using propofol].

We experienced anesthetic management of a 45-yr-old female patient with a 12-yr history of multiple sclerosis who underwent orthopedic surgeries three times under general anesthesia. We chose rapid induction with propofol and maintained the anesthesia with nitrous oxide, oxygen, and sevoflurane. We monitored both core and peripheral temperatures to avoid the rapid increase of core temperature, which might worsen the symptoms of the disease. There is no other report of anesthesia using propofol as induction agent for a patient with multiple sclerosis. We succeeded in the satisfactory perioperat

[Anesthesia for a patient with von Willebrand disease and asthma].

An emergency appendectomy was performed in a patient with von Willebrand disease (vWD) and asthma. vWD is a hemostatic disorder in which the patient lacks von Willebrand factor, playing a critical role in the mediation of platelet adhesion. A 12-year-old boy with vWD underwent an appendectomy under general anesthesia. The patient had not previously received any medication for the treatment of vWD. His bleeding time was over 18 minutes. Confact F injection improved it to 6 minutes. The patient was given Confact F during the perioperative period and no abnormal hemorrhage occurred. Heat-treated

Effects of nitro-L-arginine on blood pressure and cardiac index in anesthetized rats: a pharmacokinetic-pharmacodynamic analysis.

PURPOSE: Nitric oxide synthase (NOS) inhibitors such as Nitro-L-arginine (L-NA) are being considered for the management of hypotension observed in septic shock. However, little information is available regarding the pharmacokinetic and pharmacodynamic properties of these agents. Our objective was to examine the relationships between L-NA plasma concentration and various hemodynamic effects such as cardiac index (CI), mean arterial pressure (MAP), and heart rate (HR) elicited by L-NA administration in rats. METHODS: L-NA was infused at doses between 2.5-20 mg/kg/hr in anesthetized rats over on

Methohexital vs midazolam/flumazenil anaesthesia during laryngoscopy under jet ventilation.

In a randomised clinical study, two total intravenous anaesthesia techniques for microlaryngoscopic laser surgery were compared. After an induction dose of 100 mg methohexital, Group I received a maintenance infusion of 10 mg.min-1. In Group II anaesthesia was obtained by 15 mg midazolam followed by 0.1 mg.min-1 continuously and terminated by the injection of flumazenil. For analgesia 5 mg alfentanil were administered. Opiate-induced respiratory depression was antagonised by 0.08 to 0.12 mg naloxone. Prior to, during, and after surgery, adrenergic response was assessed by HPLC-analysis of bloo

Multiple sites for the cardiovascular actions of fentanyl in rats.

We studied the cardiovascular effects of intravenously administered fentanyl in normotensive rats anesthetized with pentobarbital and artificially ventilated. Fentanyl induced an immediate and short-lasting fall in blood pressure and heart rate by an action on opiate receptors localized at vagal nerve endings. Bilateral vagotomy suppressed these effects. The bradycardia, suppressed by bilateral vagotomy and reduced by previous administration of atropine, seemed to be due to a vagovagal reflex. Inhibition of the sympathetic outflow may also occur, because in pithed rats fentanyl failed to lower

Disoprofol and fentanyl for total intravenous anaesthesia.

Ninety patients undergoing a variety of surgical procedures were anaesthetised with a disoprofol infusion and increments of fentanyl. The first 40 patients constituted a preliminary dose finding study and the subsequent 50 patients a dose comparison study. The second 50 patients were alternately allocated to receive a maintenance infusion of disoprofol 150 or 200 micrograms/kg/minute. Fentanyl (1 microgram/kg) at induction was followed by further small increments when indicated. Maintenance dosage of 200 micrograms/kg/minute provided good operating conditions and rapid uncomplicated recovery i

Influence of piracetam and etiracetam compared to placebo, on rCBF and parameters of brain metabolism.

On occasion of a carotid angiography in man, regional cerebral bloodflow (rCBF) was measured before and after the injection of thiopental 5 mg/kg I.V. A significant decrease of rCBF was found after thiopental, in a previous study, due to a decrease of the brainmetabolism. To study the influence of piracetam and of etiracetam on this decrease of rCBF, 22 patients received intravenously the evening before the investigation 4 g piracetam or 4 g etiracetam or placebo, as well in the morning before the investigation and 30 minutes before the injection of thiopental. Besides the rCBF also other para

Effect of non-carbonic acidosis on total splanchnic perfusion and cardiac output during anaesthesia with O2-N2O-barbiturate-relaxant.

Seven dogs were anaesthetized using mebumal natrium-O2-N2O-gallamonijdidum. The PaCO2 was kept at a constant level by means of mechanical ventilation, and non-carbonic acidosis was induced with HCl infusion (0.3 normal). The arterial pH varied from 7.45 to 6.88. During this acidosis, a rising arterio-venous oxygen difference was observed, with an unchanged total oxygen consumption. The pulse fell, but the mean pressures in the right atrium and aorta were unchanged. The peripheral resistance rose by 50%, whereas the fall in cardiac output of 20% was non-significant (0.10 greater than P greater

Postoperative Pain Trajectories, Predictors of Severe Pain, and Satisfaction After Bariatric Surgery: A Prospective Multicenter Cohort Study.

Background Postoperative pain remains a clinical priority in bariatric surgery because of its impact on recovery, patient satisfaction, and postoperative complications. However, pain management after bariatric surgery is challenging because of inherent risks, such as respiratory complications, particularly in patients with obstructive sleep apnea (OSA). Current pain management strategies in bariatric surgery remain insufficient, with moderate-to-severe pain reported in 30-50% of patients within 24 hours postoperatively and up to 75% in those using intravenous (IV) patient-controlled analgesia
