Cutting edge: NLRP12 controls dendritic and myeloid cell migration to affect contact hypersensitivity.

Nucleotide-binding domain leucine-rich repeat (NLR) proteins are regulators of inflammation and immunity. Although first described 8 y ago, a physiologic role for NLRP12 has remained elusive until now. We find that murine Nlrp12, an NLR linked to atopic dermatitis and hereditary periodic fever in humans, is prominently expressed in dendritic cells (DCs) and neutrophils. Nlrp12-deficient mice exhibit attenuated inflammatory responses in two models of contact hypersensitivity that exhibit features of allergic dermatitis. This cannot be attributed to defective Ag processing/presentation, inflamma

Erythroderma in children.

Although erythroderma is a well-recognized entity in the adult age groups and has been studied by various authors, there is a paucity of studies on erythroderma in the pediatric age group. It poses a greater challenge to the dermatologist and pediatrician because of its potential life threatening nature. In a study conducted by us in a large Indian hospital to delineate the causes of neonatal and infantile erythroderma, the causes identified were infections (40%), ichthyosiform erythroderma (25%), atopic dermatitis (15%), infantile seborrheic dermatitis (10%) and unidentified (10%). In another

Molecular mechanisms of leukocyte trafficking in T-cell-mediated skin inflammation: insights from intravital imaging.

Infiltration of T cells is a key step in the pathogenesis of the inflammatory skin diseases atopic dermatitis, allergic contact dermatitis and psoriasis. Understanding the mechanisms of T cell recruitment to the skin is therefore of fundamental importance for the discovery and application of novel therapies for these conditions. Studies of both clinical samples and experimental models of skin inflammation have implicated specific adhesion molecules and chemokines in lymphocyte recruitment. In particular, recent studies using advanced in vivo imaging techniques have greatly increased our unders

Risk factors for contact allergy to nickel - results of a multifactorial analysis.

To quantify the independent impact of potential risk factors for nickel contact allergy (NCA), a multifactorial Poisson regression analysis of standardized anamnestic and patch test data (with nickel sulfate, 5% in petrolatum) was performed, comprising 74 940 patients assessed in the 33 German and Austrian contact dermatitis units of the Information Network of Departments of Dermatology (IVDK) between 1992 and 2000. NCA was observed in 15.5% patients. Female sex was the strongest risk factor (prevalence ratio 3.74, 95% CI: 3.51-3.98). Risk increased monotonically and significantly with decreas

Angry back syndrome is often due to marginal irritants: a study of 17 cases seen over 4 years.

17 patients with angry back syndrome were identified over a 4-year period (1994-97). 10 such patients were classified as multiple reactors and 7 as exacerbations of atopic eczema. The 10 multiple reactors reacted most commonly to medicaments, rubber chemicals, fragrances, methylchloroisothiazolinone/methylisothiazolinone, colophonium and nickel. A final diagnosis of relevant allergic contact dermatitis was made in 8 of the 10 patients with multiple reactions. Relevant contact allergens were rubber chemicals, plants, metals and preservatives, all of which can be marginal irritants. None of the

[Significance and possibilities of histopathologic diagnosis in breed-specific skin diseases].

An increasing number of punch biopsies is submitted for histopathology. Results are often disappointing for the veterinary pathologist as well as the referring veterinarian. Inappropriate timing of sampling, selection and preparation of the biopsy site, type of biopsy, biopsy technique and fixation as well as incomplete histories often contribute to insufficient diagnosis and interpretation of skin lesions. The introduction of this review gives an overview of the requirements for a satisfying evaluation of skin lesions and the limits of diagnostic histopathology on skin samples, especially pun

Critically appraised topic on adverse food reactions of companion animals (7): signalment and cutaneous manifestations of dogs and cats with adverse food reactions.

BACKGROUND: Outside of pruritus, there is no clear consensus on the nature and prevalence of cutaneous manifestations of adverse food reactions (AFRs) in dogs and cats. RESULTS: We searched two databases on August 7, 2018, for articles reporting detailed data on the signalment and clinical signs of at least one dog or cat with a cutaneous AFR (CAFR). We identified 233 and 407 citations from which were selected 32 articles reporting original information. Twenty-two articles included data on 825 dogs with CAFRs. The reported age of onset varied from less than one to 13 years of age; a beginning

Contact dermatitis to white petrolatum.

A 31-year-old Caucasian woman presented with a 20-year history of presumed atopic dermatitis. She complained of severe pruritus and the presence of extensive patches of erythema and scale. Her previous treatments included: multiple topical corticosteroids, tacrolimus 0.1% ointment, pimecrolimus 1% cream, and cyclosporine with no improvement of her symptoms. Her past medical history was unremarkable and she was on no other oral medications, including over-the-counter products. On physical examination, multiple erythematous, scaly patches were present on the chest, abdomen, back, and upper extrem

Topical mometasone. A review of its pharmacological properties and therapeutic use in the treatment of dermatological disorders.

Mometasone, a synthetic 16 alpha-methyl analogue of beclomethasone, is classified as a 'potent' glucocorticoid for dermatological use. It is available as 0.1% cream, ointment and lotion formulations for the treatment of patients with inflammatory glucocorticoid-responsive dermatoses. In patients with atopic dermatitis, the effect of mometasone 0.1% applied once daily over 2 to 3 weeks were similar to those of other glucocorticoids of similar potency, such as betamethasone dipropionate 0.05% twice daily and methylprednisolone aceponate 0.1% once daily. Mometasone 0.1% was significantly superior

[Clinical evaluation of the efficacy and tolerability of the ''NoAll Bimbi Pasta Trattante'' barrier cream in napkin dermatitis].

AIM: Napkin dermatitis or diaper dermatitis is one of the most frequent cutaneous disease in the pediatric age. The most common type of napkin dermatitis is the irritant contact form, which is frequently complicated by an overinfection with bacteria or Candida albicans. METHODS: In this paper, 25 children (7 males and 18 females), aged between 1 month and 4 years and affected by napkin dermatitis, were treated with a barrier cream containing zinc oxide, vitamin E, panthenol (NoAll Bimbi Pasta Trattante, PSN Rottapharm, Milan, Italy). In 20 patients, a diagnosis of irritant contact diaper derm

Nicotinamide and the skin.

Nicotinamide, an amide form of vitamin B3, boosts cellular energy and regulates poly-ADP-ribose-polymerase 1, an enzyme with important roles in DNA repair and the expression of inflammatory cytokines. Nicotinamide shows promise for the treatment of a wide range of dermatological conditions, including autoimmune blistering disorders, acne, rosacea, ageing skin and atopic dermatitis. In particular, recent studies have also shown it to be a potential agent for reducing actinic keratoses and preventing skin cancers.

[Filaggrin gene mutations are frequent and increase the risk of skin disease].

Filaggrin proteins are important for the epidermal differentiation and skin barrier function. About 10% of Europeans carry loss-of-function mutations within the filaggrin gene complex. Mutations are strongly associated with ichthyosis vulgaris and atopic dermatitis as well as dry and fissured skin. No overall association with psoriasis vulgaris has been identified. The filaggrin gene discovery has been a major breakthrough for dermatology in terms of diagnostics and prognostics, but it has not yet been translated into therapeutic advances.

[Inpatient behavioral medicine in chronic skin diseases].

Behavioral medicine is a newer approach in the management of various diseases. In the last years, special programs have been developed for atopic dermatitis, psoriasis vulgaris and other chronic skin diseases. An increase in the patient's self-control could be achieved by optimizing the patient's skills in dealing with their skin disease. The article summarizes the principles and procedures of in-patient behavioral medicine, especially the strategies employed by dermatological training groups and psychotherapeutical groups to help the patient deal with their skin disorder.

[Isolated ACTH deficiency and primary thyroid insufficiency in an atopic patient: hypophysitis?].

6 years after surgery for epidermoid lung cancer, a 58-year-old man presented with atopic dermatitis and a high level of IgE, primary hypothyroidism of autoimmune origin, slight hyperprolactinemia and adrenal insufficiency of pituitary origin. ACTH levels were low and unresponsive to CRH. There was no deficiency of the other pituitary hormones and the MRI of the pituitary was normal. The other instances of autoimmune thyroiditis associated with ACTH deficiency are reviewed. The etiology of the deficiency is discussed in the context of a possible hypophysitis.

Advances in Relationship Between Alcohol Consumption and Skin Diseases.

Throughout history, alcohol consumption has been an integral part of human culture. Alcohol consumption, alcoholism in particular, influences the onset and progression of liver diseases, neurological disorders, and multiple types of cancer. However, the role of alcohol consumption in influencing skin diseases has often been overlooked. In this review, we present the progress of research investigating the effects and potential mechanisms of action of alcohol consumption on acne, rosacea, psoriasis, atopic dermatitis, melanoma, and non-melanoma skin cancer.

Dupilumab and alopecia: A Janus effect.

Atopic dermatitis (AD) and alopecia areata (AA) are two common skin diseases that can coexist in the same patient, sharing some pathogenetic mechanisms such as the overexpression of interleukin 4 (IL4) and interleukin 13 (IL13). Recently, cases of AA healing during treatment with anti-IL4/IL13 monoclonal antibody dupilumab (D) for AD were reported. However, cases of appearance of AA have been described during D treatment too. We report two patients that experienced these two opposite effects of D.

Fournier gangrene associated with hyper IgE syndrome (Job syndrome).

We report a case of a 32-year-old man with hyper IgE syndrome (Job syndrome) who developed Fournier gangrene due to infectious multiple atheromas of the scrotal skin that progressed to the right groin and thigh. The patient required surgical debridement and subsequent skin grafting. This is a rare case of Fournier gangrene associated with hyper IgE syndrome (Job syndrome). When a patient without diabetes mellitus has repeated infections and atopic-like dermatitis, Job syndrome should be considered.

[Helicobacter pylori and skin diseases--a (still) intact myth?].

Helicobacter pylori plays a key role in the aetiology of peptic ulcer, gastric cancer and gastric MALT-lymphoma. Based on a number of reports, a possible relationship of Helicobacter pylori infection to a variety of different dermatoses has been suggested, including urticaria, rosacea, acne-rosacea, atopic dermatitis, alopecia areata, Sjögren's syndrome, Schönlein-Henoch purpura, and Sweet syndrome. Larger case-control studies, however, do not confirm this relationship. Therefore, Helicobacter pylori eradication therapy cannot be generally recommended in these dermatoses.
