Two children with acute rheumatic fever had concurrent liver disease which obscured their diagnosis. One patient had evidence of viral hepatitis and the other apparently had liver toxicity from high doses of aspirin. The differential diagnosis of arthritis with liver disease must include acute rheumatic fever.

To compare rickettsial infectivity and seropositivity rates against spotted fever group (SFG) rickettsiae, ticks and wild mammals were collected from three areas where Rickettsia rickettsii was thought to be enzootic in Connecticut during 1978-1979, and from four additional sites (with no reported human cases) between 1976 and 1979. Cases of rickettsiosis of all kinds have reduced to mere sporadic episodes in the last thirty years all over the Mediterranean. But in the last five years, some regions of Italy (Lazio, Liguria, Sicily, Sardinia) have registered an extraordinary epidemiological event, the endemoepidemic expansion of boutonneuse fever which was previously only seen in about thirty cases a year. After a review of the literature on heart damage in rickettsiosis, the data from a clinical and ecgraphic investigation of 17 patients suffering from boutonneuse fever are reported. There was little cardiovascular involvement and inconstant hypotension but two heart complications are described, one myocardial ischaemia which cleared up a few days after the end of the diseases, and an atrial fibrillation that lasted two days. Stress is laid on the need for constant ecgraphic monitoring, even in this minor rickettsiosis.

During an epizootic of Rift Valley fever in South Africa in 1974/1975, mainly affecting sheep and cattle, a large number of human cases occurred. The majority of the patients, mostly farmers, farm labourers and veterinary surgeons, acquired the infection while handling the carcasses of animals which had died of Rift Valley fever. Because myocarditis has been noted frequently as present at necropsy in fatal Rocky Mountain spotted fever (RMSF) and ECG alterations may also accompany the disease, we studied the pathogenesis of myocarditis in RMSF. Development of cellular immunoreactivity to Salmonella typhi and Salmonella paratyphi-A was studied by the leukocyte migration inhibition test in 9 patients with typhoid fever and in 2 patients with paratyphoid fever. Cellular reactivity could be demonstrated from the first days of the disease in all the subjects. The most pronounced migration inhibition was observed during the febrile period. It is suggested that specific cellular reactivity may play a pathogenetic role in typhoid fever.

Nasal sensitivity to rye grass pollen allergens was evaluated by provocation testing in patients with hay fever due to grass pollen using measurements of nasal airways resistance (NAR), a reproducible system for delivery of allergen, and stringent criteria for allergen storage.
