Evidence-Based Psychosocial Treatment for Individuals with Early Psychosis.

Coordinated specialty care (CSC) first-episode models are an evidence-based practice in the treatment of first-episode psychosis. Group, individual, and family therapies in CSC aim to help the client and family understand and cope with the experience of psychosis, promote symptomatic and functional recovery and improve quality of life, and support the pursuit of personally meaningful goals of the client. Common elements to these interventions include building a therapeutic alliance, recovery orientation, education, and skills training, which can be directed to a range of targets, including pro

[Allergies to non-betalactam antibiotics: a challenge in practice].

Allergies to non-betalactam antibiotics: a challenge in practice Abstract. The heterogeneous group of non-betalactam antibiotics can in part trigger very severe immunologically mediated hypersensitivity reactions. The risks are very differently distributed among the different representatives and a genetic predisposition to the development of Stevens-Johnson syndrome / toxic epidermal necrolysis or Drug rash with eosinophilia and systemic symptoms (DRESS syndrome) can be observed. Individual patient groups are particularly frequently affected, including patients with HIV or cystic fibrosis. In

Transforming responses: Exploring the treatment of substance-using African American women.

This article explores how intrapersonal and structural oppression may impact treatment and the recovery process of 23 self-identified African American women with histories of incarceration and substance use. Using a critical consciousness (CC) framework and content-based thematic analysis, researchers systematically coded and extracted themes and patterns from focus group data to evaluate how marginalizing processes-such as race-based discrimination-impact treatment, the therapeutic relationship, and service provision. Results indicate that participants' health and treatment were negatively im

[Follow-up protocol for newborns of birthweight less than 1500 g or less than 32 weeks gestation].

The mortality of children with a birthweight of less than 1500g or with a gestational age of less than 32 weeks (<1500<32) has decreased significantly in the last 20 years or so. Given the higher risk of disability in these children, follow-up after hospital discharge is considered essential. In this document, the Follow-Up Group of the Spanish Society of Neonatology, in collaboration with the Spanish Society of Paediatric Primary Care, propose a follow-up protocol specific for the<1500<32, which has as its aim to standardise the activities and evaluations according to good practice criteria.

Emergency Department Medical Clearance of Patients with Psychiatric or Behavioral Emergencies, Part 2: Special Psychiatric Populations and Considerations.

Patients who present to the emergency department (ED) with mental illness or behavioral complaints merit workup for underlying physical conditions that can trigger, mimic, or worsen psychiatric symptoms. However, there are wide variations in quality of care for these individuals. Psychiatry and emergency medicine specialty guidelines support a tailored, customized approach to patients. Our group has long advocated a dynamic comanagement approach for medical clearance in the ED, and this article summarizes best-practice approaches to the medical clearance of patients with psychiatric illness, t

Attachment in eating-disordered outpatients with and without borderline personality disorder.

This study aims at examining the attachment states of mind in 50 female outpatients with eating disorders compared with 50 matched control participants using the Adult Attachment Interview. Moreover, the differences in attachment states of mind among eating-disordered women with and without borderline personality disorders' diagnosis were explored. The results showed an over-representation of insecure-dismissing and unresolved states of mind in clinical group compared to controls. Patients with both diagnosis showed higher scores on involving anger and unresolved loss compared with those with

Orbital Imaging Manifestations of Neurocutaneous Syndromes Revisited.

Neurocutaneous syndromes or phakomatoses represent a heterogeneous group of multisystemic disorders involving structures of ectodermal origin. Characteristic ocular manifestations are described for individual entities that are often the first clues to the underlying diagnosis. However, opaque ocular media or involvement of retrobulbar orbit limits adequate clinical evaluation. This underlines the role of imaging, especially cross-sectional imaging modalities, such as computed tomography and magnetic resonance imaging, which offer a comprehensive evaluation of orbit and its contents. This revie

Aortic Valve Replacement With the Stentless Freedom SOLO Bioprosthesis: A Systematic Review.

This systematic review examined the clinical and hemodynamic performance of the stentless Freedom SOLO (Sorin Group, Milan, Italy) aortic bioprosthesis. The occurrence of postoperative thrombocytopenia was also analyzed. The Freedom SOLO is safe to use in everyday practice, with short cross-clamp times, and postoperative pacemaker implantation is notably lower. Valvular gradients are low and remain stable during short-term follow-up. Thrombocytopenia is more severe than in other aortic prostheses; however, this is without clinical consequences. Within a few years, the 15-year follow-up of this

Italian survey of neonatal respiratory support strategies.

Although the management of respiratory distress syndrome (RDS) in preterm infants has been characterized by significant progress in recent years, it is difficult to translate the research results into clinical practice. Previous surveys have demonstrated that in some areas, the current management of RDS does not reflect evidence from randomized trials. Therefore, the Pulmonology Study Group of the Italian Society of Neonatology decided to perform a similar survey in Italy with the aim of identifying possible aspects of respiratory management of preterm infants with RDS that merit improvement,

Use of light-cured resin to manufacture occlusal splints: report of two cases.

This article presents selected cases of patients with functional disorders of the stomatognathic system. This group of patients had a need to made different types of removable occlusal splints. In the past, occlusal appliances were made mostly using self-cured acrylate materials, which for many years had no replacements. The rapid development of dental materials technology led to creation of thermo-formable materials and resins, which can successfully replace traditional acrylic materials in daily clinical practice. A practical application of light-cured resin in the fabrication of the occlusa

"Our college is changing": women's college student affairs administrators and transgender students.

Student affairs administrators at contemporary American women's colleges are charged with supporting the personal, intellectual, and relational growth of students. A small but vocal group of students who identify as transgender and who are living as male, genderqueer, or transitioning to live as male, have emerged as a visible subpopulation in highly selective women's colleges. In this qualitative, phenomenological study of the perceptions of student affairs administrators (n = 31), three participant profiles-ambivalent, supporter, and advocate-emerged. Implications for student affairs practic

Incidence of oronasal haemorrhage in infancy presenting to general practice in the UK.

The frequency of oronasal haemorrhage in infancy was estimated from two national GP research databases (6% UK population). When a case was identified, other presentations in the child over the first year were available from one dataset. In the first year haemoptysis is rare. In contrast, epistaxis (7-20 cases of per 10,000 infants) was 10 times more common, and 14.3% of these infants had an injury at some other point in infancy, (four times greater than the general population). In general practice epistaxis may herald other trauma presentations, implying that such infants may be part of a high

Bovine spongiform encephalopathy and Creutzfeldt-Jakob disease: background and implications for nursing practice.

Creutzfeldt-Jakob disease belongs to a group of rapidly progressive, fatal neurological disorders. Several cases of iatrogenic Creutzfeldt-Jakob disease (CJD) have been reported in the literature after neurosurgery and corneal grafting. The total number of people affected by CJD is unknown at this time, and it will remain unknown for some time to come. High infectious risks classification includes the brain, spinal cord, and eyes. Transmissible spongiform encephalopathy (TSE) prions are resistant to steam sterilization, dry heat, ethylene oxide gas, and chemical disinfection. Prevention of pri

Psychopathology in older adults with severe and profound mental retardation.

The prevalence of psychopathology in younger and older adults with severe and profound mental retardation was examined. Participants were evaluated by direct-care staff using the Diagnostic Assessment for the Severely Handicapped (DASH), which measures the frequency, duration, and severity of symptoms associated with 13 major psychiatric disorders derived from the DSM-III-R. The frequency of disorders was comparable for younger and older adults. Age group differences favoring the older adults were revealed in the analyses of duration and severity on 4 of the DASH subscales. Implications of the

Harbinger of plague: a bad case of gay bowel syndrome.

In 1976, a group of physicians in private proctologic practice in New York City coined the illness "Gay Bowel Syndrome" in reference to a constellation of gay male anorectal disorders. Through analysis of biomedical discourse and popular media, it is apparent that Gay Bowel Syndrome is an essentialized category of difference that is neither gay-specific, confined to the bowel, nor a syndrome. The use and diagnosis of Gay Bowel Syndrome must be abandoned before it further lends itself to the formation of social policies and governing practices that seek to force gay male bodies into positions o
