Early or later prophylactic INSURE in preterm infants of less than 30 weeks' gestation.

We aimed to determine whether an early prophylactic INSURE strategy combined with early nasal continuous positive airway pressure (nCPAP) treatment could decrease the subsequent need for mechanical ventilation (MV) compared to the administration of surfactant prophylaxis later, at the 15th minute after birth, combined with early nCPAP. Infants born at <30 weeks' gestation were randomized to receive surfactant prophylaxis immediately or at the 15th minute after birth. All infants received 100 mg/kg poractant alpha, were ventilated with a T-piece resuscitator and were extubated to nCPAP if they

Risk factors associated with the need for a tracheostomy in extremely low birth weight infants.

In an attempt to determine the risk factors associated with the need for a tracheostomy in extremely low birth weight (ELBW) infants, a retrospective, case control study was conducted (each infant with a tracheostomy [case] was matched to two controls). Medical records were reviewed for patients' characteristics, risk factors for tracheostomy and outcome. During the study period (June 1996 to Dec 2010), 934 ELBW infants were admitted to our institution and nine infants had a tracheostomy and were matched to 18 controls. There were no differences in birth weight (BW) and gestation age (GA) betw

Neonatal outcomes of extremely preterm infants from the NICHD Neonatal Research Network.

OBJECTIVE: This report presents data from the Eunice Kennedy Shriver National Institute of Child Health and Human Development Neonatal Research Network on care of and morbidity and mortality rates for very low birth weight infants, according to gestational age (GA). METHODS: Perinatal/neonatal data were collected for 9575 infants of extremely low GA (22-28 weeks) and very low birth weight (401-1500 g) who were born at network centers between January 1, 2003, and December 31, 2007. RESULTS: Rates of survival to discharge increased with increasing GA (6% at 22 weeks and 92% at 28 weeks); 1060

Nasal CPAP or intubation at birth for very preterm infants.

BACKGROUND: Bronchopulmonary dysplasia is associated with ventilation and oxygen treatment. This randomized trial investigated whether nasal continuous positive airway pressure (CPAP), rather than intubation and ventilation, shortly after birth would reduce the rate of death or bronchopulmonary dysplasia in very preterm infants. METHODS: We randomly assigned 610 infants who were born at 25-to-28-weeks' gestation to CPAP or intubation and ventilation at 5 minutes after birth. We assessed outcomes at 28 days of age, at 36 weeks' gestational age, and before discharge. RESULTS: At 36 weeks' gest

A review of the status of magnesium and related minerals in the sudden infant death syndrome (SIDS).

Sudden infant death syndrome (SIDS) is the sudden death of an infant under 1 year of age that remains unexplained after a thorough case investigation, including performance of a complete autopsy. Despite recent reductions in the SIDS rate attributed to placing the baby to sleep on his back, SIDS remains the most common cause of infant mortality between 1 month and 1 year of age in developed countries. This review concerns the current state of a hypothesis (1972) that magnesium deficiency, which causes sudden death in young mammals, is the major cause of SIDS. Numerous investigators have compar…

Interactions between maternal smoking and other prenatal risk factors for sudden infant death syndrome (SIDS).

In numerous investigations, maternal smoking increases the risk of sudden infant death syndrome (SIDS). In the present study we investigated whether prenatal risk factors for SIDS modify the effect of maternal smoking on SIDS mortality. We analysed data from a population-based cohort study (222 cases, 260,604 infants at risk) within the Westphalian Perinatal Inquiry in Germany between 1990 and 1994. In the stratified analysis, smoking was classified into non-smoking, moderate (1-10 cigarettes/d) and heavy smoking (> 10 cigarettes/d). Multiplicative interactions between smoking and other prenat

Newborn Early Warning Systems in Screening and Detection of Neonatal Emergencies: A Bibliometric Analysis.

BACKGROUND: Neonatal mortality remains a major global health challenge, particularly in low- and middle-income countries (LMICs), where preventable causes account for a significant proportion of deaths. Delayed recognition of clinical deterioration contributes to adverse outcomes, highlighting the importance of reliable monitoring tools. Newborn Early Warning Systems (NEWS) have been developed to support early detection of physiological instability and have demonstrated effectiveness in neonatal care settings. AIM: This study aimed to map the global research landscape on NEWS by analysing pub

Temporal and spatial trends in isolated gastroschisis: a population-based study in a middle-income setting.

OBJECTIVE: To estimate the prevalence and neonatal mortality of isolated gastroschisis, assess temporal trends, identify risk factors for neonatal death, and describe its spatial distribution in a middle-income setting. METHODS: This population-based study included all live births in São Paulo State, Brazil, from 2004 to 2017. Cases of isolated gastroschisis were identified from live birth and death certificates. Temporal trends in prevalence and neonatal mortality (0-27 days) were analyzed using Prais-Winsten regression to estimate annual percent change (APC). Maternal and neonatal factors a

Predictors of perinatal mortality in emerging regions of Ethiopia: Evidence from EDHS 2016.

BACKGROUND: Perinatal mortality rate is one of indictors used to measure the quality of obstetric and pediatric services globally. Compared to developed settings, perinatal mortality rate is higher in low-income countries, indicating societal inequities in health care and a scarcity of prenatal services. It is responsible for roughly 42% of all stillbirth in Sub-Saharan Africa, and 41% of newborn death globally. Despite Ethiopia's efforts to reduce perinatal mortality by improving the quality of care for maternal and child health, perinatal mortality rate is still very high, and as to our sear

Progress Toward Achieving and Sustaining Maternal and Neonatal Tetanus Elimination - Worldwide, 2000-2022.

Tetanus remains a considerable cause of mortality among undervaccinated mothers and their infants following unhygienic deliveries, especially in low-income countries. Strategies of the maternal and neonatal tetanus elimination (MNTE) initiative, which targets 59 priority countries, include strengthening antenatal immunization of pregnant women with tetanus toxoid-containing vaccines (TTCVs); conducting TTCV supplementary immunization activities among women of reproductive age in high-risk districts; optimizing access to skilled birth attendants to ensure clean deliveries and umbilical cord car

A descriptive analysis of the coverage of newborn care services among women who delivered in health facilities in 17 sub-Saharan African countries.

BACKGROUND: Sub-Saharan Africa (SSA) has seen an increase in facility-based births over the years. However, the region has the world's highest newborn mortality rate (42% in 2019). Quality care around the time of birth can avert these deaths. This study examined the newborn care interventions given to women who gave birth in health facilities in 17 countries in SSA. METHODS: A cross-sectional population-based study was conducted. We used data from the most recent Demographic and Health Surveys (DHS) conducted in 17 sub-Saharan African countries. We analysed a weighted sample of 226,706 women

Uterocervical angle versus cervical length in the prediction of spontaneous preterm birth in women with history of spontaneous preterm birth: a prospective observational study.

Источник пометил публикацию как отозванную

BACKGROUND: Preterm delivery is a leading cause of neonatal mortality and morbidity. History of spontaneous preterm birth is the greatest risk factor for another preterm delivery. So, every effort should be made to prevent the recurrence of preterm delivery in this vulnerable group. This study aimed to evaluate the predictive ability of the anterior uterocervical angle and cervical length in preterm birth. PATIENTS AND METHODS: This was a prospective cohort study that included 70 patients with a history of spontaneous preterm birth. Ultrasound measurements of cervical length and anterior uter

Divergent age patterns of under-5 mortality in south Asia and sub-Saharan Africa: a modelling study.

BACKGROUND: Understanding the age pattern of under-5 mortality is essential for identifying the most vulnerable ages and underlying causes of death, and for assessing why the decline in child mortality is slower in some countries and subnational areas than others. The aim of this study is to detect age patterns of under-5 mortality that are specific to low-income and middle-income countries (LMICs). METHODS: In this modelling study, we used data from 277 Demographic and Health Surveys (DHSs), 58 Health and Demographic Surveillance Systems (HDSSs), two cohort studies, and two sample-registrati

Does nuchal cord at birth increase the risk for cerebral palsy?

BACKGROUND: Nuchal cord is a common finding in pregnancy. It is unclear whether a nuchal cord at birth causes birth asphyxia and raises the risk for developing cerebral palsy of the offspring. AIM: To evaluate the incidence of cerebral palsy in children born with and without nuchal cord. STUDY DESIGN: A population-based cohort analysis including all singleton deliveries over >20 years at a single tertiary medical center was conducted. The incidence of cerebral palsy in children up to 18 years of age was evaluated. Kaplan-Meier survival curve was used to compare cumulative incidence between t

Secular trend, seasonality and effects of a community-based intervention on neonatal mortality: follow-up of a cluster-randomised trial in Quang Ninh province, Vietnam.

BACKGROUND: Little is know about whether the effects of community engagement interventions for child survival in low-income and middle-income settings are sustained. Seasonal variation and secular trend may blur the data. Neonatal mortality was reduced in a cluster-randomised trial in Vietnam where laywomen facilitated groups composed of local stakeholders employing a problem-solving approach for 3 years. In this analysis, we aim at disentangling the secular trend, the seasonal variation and the effect of the intervention on neonatal mortality during and after the trial. METHODS: In Quang Nin
