Prenatal coke: what's behind the smoke? Prenatal cocaine/alcohol exposure and school-age outcomes: the SCHOO-BE experience.

Despite media reports and educators' concerns, little substantive data have been published to document or refute the emerging reports that children prenatally exposed to cocaine have serious behavioral problems in school. Recent pilot data from this institution have indeed demonstrated teacher-reported problem behaviors following prenatal cocaine exposure after controlling for the effects of prenatal alcohol use and cigarette exposure. Imperative in the study of prenatal exposure and child outcome is an acknowledgement of the influence of other control factors such as postnatal environment, se

A realist synthesis of integrated palliative care and oncology: Synopsis.

BACKGROUND: Existing evidence demonstrates the benefits of integrated palliative care for people with cancer and their carers, for improved symptom burden, quality of life and appropriate healthcare resource use. The integration of palliative care and oncology has the potential to improve the quality of life and is recommended by international guidelines. However, it is not yet consistent practice. There are many approaches to integration, but it is unclear what works, for whom and in what contexts to achieve the best possible outcomes for people with cancer, carers and healthcare systems. OB

Future horizons: Innovation, aging, and equity.

Precision medicine is on the verge of transforming the treatment of neurodegenerative diseases (NDDs) like Alzheimer's disease (AD) and Parkinson's disease (PD), in response to the intricate interactions of genetic, epigenetic, environmental, and lifestyle factors underlying disease heterogeneity. As the world's aging populations grow, with dementia cases expected to double by 2040 and the costs amounting to over €130 billion a year in Europe alone, there is an urgent need for novel strategies to stem the socioeconomic costs of NDDs. Conventional "one-drug-fits-all" strategies that depend on l

Bariatric metabolic surgery.

According to the WHO, the worldwide prevalence of obesity body mass index (BMI) 30 kg/m² nearly doubled between 1980 and 2008, with 10% of men and 14% of women and a total of more than half a billion adults (aged >20 years old) being classed as obese. At least 2.8 million people die each year worldwide as a result of being overweight or obese, usually from the inevitable related comorbidities. It has been reported that approximately 65% of the worlds population inhabits countries where overweight and obesity are responsible for higher mortality than underweight. The recently published Interdis

Co-development, initial evaluation and implementation of a digital health intervention (SmartADHD) for young people (aged 16-30) with attention deficit hyperactivity disorder, including optimisation for sustainable delivery via health and social care settings: protocol for a mixed-methods study.

INTRODUCTION: Attention deficit hyperactivity disorder (ADHD) is a neurodevelopmental condition affecting 5–7% of children/adolescents and 2–5% of adults. It is characterised by inattention, hyperactivity and impulsivity, impacting functioning across multiple domains with linked social and economic burden. Despite effective treatments that reduce the risk of negative outcomes, the UK is currently failing to provide equitable access to healthcare for people with ADHD. Young people with ADHD are at a vulnerable life-stage, facing multiple transitions such as moving educational settings, starting

Mapping and analysis of the knowledge graph on the current status and key issues in global research on Norovirus infections in children over the past 28 years.

BACKGROUND: Norovirus (NoV) is a major pathogen responsible for acute gastroenteritis and outbreaks of diarrhea in humans. Children, due to their underdeveloped immune systems and higher risk of exposure, constitute a population with a relatively concentrated incidence of infection and disease burden. In recent years, research on NoV infections in children has been growing globally; however, there remains a lack of systematic quantitative analysis regarding the evolution of research themes, models of international collaboration, and future development directions in this field. Therefore, this

Degraded trabecular bone score and periodontitis progression: 5-year cohort with increased risk in postmenopausal women.

BACKGROUND: No longitudinal evidence exists linking the trabecular bone score (TBS), a novel index of bone quality, to periodontitis. This 5-year retrospective cohort study explored the relationship between TBS and periodontitis progression in the Electrical Generating Authority of Thailand (EGAT) population, with a focus on postmenopausal women. METHODS: Baseline mean TBS values of 617 participants aged 30-80 years were assessed from dual-energy x-ray absorptiometry (DXA) images of L1-L4 lumbar spines and classified as normal, partially degraded, or degraded TBS groups. At baseline and at 5-

Clinician experiences of organisational compassion in healthcare: a qualitative study.

BACKGROUND: While compassion is widely recognised as an essential component of high-quality patient care, the compassion needs of clinicians often go unrecognised and unmet. Clinicians face multifaceted sources of workplace suffering, both sources inherent to working with the sick and avoidable sources due to healthcare systems and leadership challenges. Organisational compassion, defined as the continuous and systematic identification, prevention and alleviation of sources of suffering for healthcare workers, offers a paradigm shift in mitigating and preventing clinician suffering and burnout

The genetic landscape and phenotypic spectrum of GAA-FGF14 ataxia in China: a large cohort study.

BACKGROUND: An intronic GAA repeat expansion in FGF14 was recently identified as a cause of GAA-FGF14 ataxia. We aimed to characterise the frequency and phenotypic profile of GAA-FGF14 ataxia in a large Chinese ataxia cohort. METHODS: A total of 1216 patients that included 399 typical late-onset cerebellar ataxia (LOCA), 290 early-onset cerebellar ataxia (EOCA), and 527 multiple system atrophy with predominant cerebellar ataxia (MSA-c) were enrolled. Long-range and repeat-primed PCR were performed to screen for GAA expansions in FGF14. Targeted long-read and whole-genome sequencing were perfo

General insights on obstacles to dog vaccination in Chad on community and institutional level.

Domestic dogs are responsible for 95% of all human rabies cases worldwide and continue to be the main reservoir for this fatal virus in African and Asian countries. Interrupting the spread of the disease in the domestic dog population is therefore necessary for long-term, sustainable rabies control. Chad has been recognized as a rabies-endemic country since 1961, but no national control strategy is in place to date and dog vaccination coverage is very low. This qualitative, descriptive study aims to describe the main barriers to dog vaccination on both the community and the institutional level

Conversational Agents as Mediating Social Actors in Chronic Disease Management Involving Health Care Professionals, Patients, and Family Members: Multisite Single-Arm Feasibility Study.

BACKGROUND: Successful management of chronic diseases requires a trustful collaboration between health care professionals, patients, and family members. Scalable conversational agents, designed to assist health care professionals, may play a significant role in supporting this collaboration in a scalable way by reaching out to the everyday lives of patients and their family members. However, to date, it remains unclear whether conversational agents, in such a role, would be accepted and whether they can support this multistakeholder collaboration. OBJECTIVE: With asthma in children representi

Experts' Needs Assessment and Evaluation of Mobile Apps for Children of Parents With Mental Illness: Mixed Methods Study.

BACKGROUND: Children of parents with mental health illness (COPMI) have the right to receive preventive interventions to avoid developing their own mental health and socioeconomic problems. However, access to interventions varies widely within health care and social services depending on their geographical location in Sweden. In this regard, mobile health (mHealth) interventions for COPMI provide a pathway for more equitable and sustainable preventive support. OBJECTIVE: This project aims to (1) map and assess the quality of available mHealth apps relevant to COPMI; (2) investigate experts' p

Understanding hairy cell leukemia in the context of mature B-cell neoplasms: tumor microenvironment and extracellular vesicle contribution to disease pathogenesis.

Mature B-cell neoplasms constitute a biologically and clinically heterogeneous group of hematologic malignancies, defined by the clonal proliferation and accumulation of monotypic mature B lymphocytes, which may involve the peripheral blood, bone marrow (BM), lymphoid tissues, or present primarily in extranodal sites. While the pathogenesis of common subtypes, such as chronic lymphocytic leukemia, multiple myeloma, Hodgkin Lymphoma and Diffuse Large B-cell Lymphoma has been extensively studied, those of rare entities like Hairy Cell Leukemia (HCL) remains poorly understood. HCL is a distinct B
