Modeling leaflet correction techniques in aortic valve repair: A finite element study.

In aortic valve sparing surgery, cusp prolapse is a common cause of residual aortic insufficiency. To correct cusp pathology, native leaflets of the valve frequently require adjustment which can be performed using a variety of described correction techniques, such as central or commissural plication, or resuspension of the leaflet free margin. The practical question then arises of determining which surgical technique provides the best valve performance with the most physiologic coaptation. To answer this question, we created a new finite element model with the ability to simulate physiologic f

Detecting patient motion in projection space for cone-beam computed tomography.

Cone-beam X-Ray systems strictly depend on the imaged object being stationary over the entire acquisition process. Even slight patient motion can affect the quality of the final 3D reconstruction. It would be desirable to be able to discover and model patient motion right from the actual projection images, in order to take it into account during reconstruction. However, while the source-detector arrangement is rotating around the patient, it is difficult to separate this motion from the additional patient motion. We present a novel similarity metric for successive X-Ray projections, which is a

A genetic risk score combining ten psoriasis risk loci improves disease prediction.

Psoriasis is a chronic, immune-mediated skin disease affecting 2-3% of Caucasians. Recent genetic association studies have identified multiple psoriasis risk loci; however, most of these loci contribute only modestly to disease risk. In this study, we investigated whether a genetic risk score (GRS) combining multiple loci could improve psoriasis prediction. Two approaches were used: a simple risk alleles count (cGRS) and a weighted (wGRS) approach. Ten psoriasis risk SNPs were genotyped in 2815 case-control samples and 858 family samples. We found that the total number of risk alleles in the c

Stress sensitization following a disaster: a prospective study.

BACKGROUND: According to the stress sensitization hypothesis, prior exposure to extreme stressors may lead to increased responsiveness to subsequent stressors. It is unclear whether disaster exposure is associated with stress sensitization and, if so, whether this effect is lasting or temporary. This study aimed to investigate the occurrence and duration of stress sensitization prospectively following a major disaster. METHOD: Residents affected by a fireworks disaster (n=1083) participated in surveys 2-3 weeks (T1), 18-20 months (T2) and almost 4 years (T3) after the disaster. Participants r

A model to optimise the requirements of lactating dairy cows for physically effective neutral detergent fibre.

This study modelled multiple physiological responses of dairy cows to physical and chemical characteristics of a diet aiming to optimise their requirements for physically effective neutral detergent fibre, expressed inclusive of particles-dry matter > 8 mm (peNDF > 8). Extensive research data, comprising a wide range of feeding conditions (n = 64 studies and 257 different dietary treatments), were used to parameterise the model, while statistical modelling was used to account for the inter- and intra-experiment variation as well as to derive the model estimates. Physiological thresholds and 's

Partitioning reference intervals by use of genetic information.

BACKGROUND: Reference intervals that incorporate genetic information could reduce the misidentification of unusual test results caused by non-disease-associated genetic variation and increase the detection of results indicating underlying pathology. Subdividing reference groups by genetic effects, however, may lead to increased uncertainty around reference interval endpoints (because of the smaller subgroup sample sizes), thus offsetting any benefits. METHODS: We evaluated CLSI guidelines to develop a method appropriate for partitioning reference intervals on the basis of genetic variants wit

Decision support system for resource allocation in disaster management.

Natural and man-made disasters, such as earthquakes, floods, plane crashes, high-rise building collapses, major nuclear facility malfunctions, pose an ever-present challenge to public emergency services. Disasters may result in a large volume of responders arriving on-scene to provide assistance to victims. Coordination of responding resources is a major problem in disasters. In this paper we introduce a decision support framework built on rapid information collection and resource tracking functionalities. Based on this information collected from emergency response service agencies, operation

Building an inner sanctuary: complex PTSD in chimpanzees.

Through the analysis of case studies of chimpanzees (Pan troglodytes troglodytes) in residence at a sanctuary, who previously sustained prolonged captivity and biomedical experimentation, we illustrate how human psychological models of diagnosis and treatment might be approached in great apes. This study reflects growing attention to ethical, scientific, and practical problems associated with psychological well-being of animals. The analysis concludes that a diagnosis of Complex PTSD in chimpanzees is consistent with descriptions of trauma-induced symptoms as described by the DSM-IV and human

Promoter methylation of cancer-related genes in gastric carcinoma.

UNLABELLED: Genetic changes associated with gastric cancer are not completely known, but epigenetic mechanisms involved in this disease seem to play an important role in its pathophysiology. One of these mechanisms, an aberrant methylation in the promoter regions of genes involved in cancer induction and promotion, may be of particular importance in gastric cancer. AIM: To analyze the methylation status of eight genes: Apaf-1, Casp8, CDH1, MDR1, GSTP1, BRCA1, hMLH1, Fas in gastric cancer patients. METHODS: The methylation pattern of the genes was assessed by methylation specific restriction

Strategies to improve pediatric disaster surge response: potential mortality reduction and tradeoffs.

OBJECTIVE: To estimate the potential for disaster mortality reduction with two surge response strategies: 1) control distribution of disaster victims to avoid hospital overcrowding near the scene, and 2) expand capacity by altering standards of care to only "essential" interventions. DESIGN: Quantitative model of hospital mortality. SETTING: New York City pediatric intensive care unit and non-intensive care unit pediatric hospital capacity and population. MEASUREMENTS AND MAIN RESULTS: Mortality was calculated for a hypothetical sudden disaster, of unspecified mechanism, assuming 500 childr

Universal mass vaccination against varicella in Germany using an MMRV combination vaccine with a two-dose schedule: an economic analysis.

Former economic analyses have shown that universal mass vaccination of infants against varicella using a one-dose schedule is cost-saving in Germany. In July 2006, an MMRV combination vaccine has been approved in Germany which shall be given in a two-dose schedule. We re-analysed our former analysis with the EVITA model in order to prove whether our former conclusion that universal mass vaccination against varicella is cost-saving is still valid when using a two-dose schedule vaccine. Indeed we found that universal mass vaccination of infants against varicella with a two-dose vaccine is cost-s

Tracking tremor frequency in spike trains using the extended Kalman smoother.

Tremor is one of the most disabling symptoms in patients with many movement disorders including Parkinson's disease (PD) and essential tremor (ET). Neural tremor manifests itself as a quasi-periodic fluctuation of the firing rate. We describe a frequency tracking method based on the extended Kalman smoother (EKS) to estimate the instantaneous tremor frequency (ITF) exhibited in binary spike trains detected from neural recordings. Simulation results demonstrate that the EKS frequency tracker can estimate the ITF accurately, even though the signal of interest is not sinusoidal and the noise is n

Proceedings of the SMBE Tri-National Young Investigators' Workshop 2005. Positional conservation of clusters of overlapping promoter-like sequences in enterobacterial genomes.

The selective mechanisms operating in regulatory regions of bacterial genomes are poorly understood. We have previously shown that, in most bacterial genomes, regulatory regions contain high densities of sigma70 promoter-like signals that are significantly above the densities detected in nonregulatory genomic regions. In order to investigate the molecular evolutionary forces that operate in bacterial regulatory regions and how they affect the observed redundancy of promoter-like signals, we have undertaken a comparative analysis across the completely sequenced genomes of enteric gamma-proteoba

Transcutaneous electrical nerve stimulation for the management of neuropathic pain: the effects of frequency and electrode position on prevention of allodynia in a rat model of complex regional pain syndrome type II.

BACKGROUND AND PURPOSE: Complex regional pain syndrome type II (CPSII) is a painful condition that develops following a nerve injury. Although transcutaneous electrical nerve stimulation (TENS) relieves the pain of CPSII, the stimulation parameters that would best prevent the development of the condition are not known. The purpose of this study was to compare the ability of several different stimulation strategies to reduce the development of allodynia. SUBJECTS: Sprague-Dawley rats were used in the study. METHODS: A chronic constriction injury (CCI) to the right sciatic nerve was used to in

Survival model predictive accuracy and ROC curves.

The predictive accuracy of a survival model can be summarized using extensions of the proportion of variation explained by the model, or R2, commonly used for continuous response models, or using extensions of sensitivity and specificity, which are commonly used for binary response models. In this article we propose new time-dependent accuracy summaries based on time-specific versions of sensitivity and specificity calculated over risk sets. We connect the accuracy summaries to a previously proposed global concordance measure, which is a variant of Kendall's tau. In addition, we show how stand

Simple rules underlying gene expression profiles of more than six subtypes of acute lymphoblastic leukemia (ALL) patients.

MOTIVATIONS AND RESULTS: For classifying gene expression profiles or other types of medical data, simple rules are preferable to non-linear distance or kernel functions. This is because rules may help us understand more about the application in addition to performing an accurate classification. In this paper, we discover novel rules that describe the gene expression profiles of more than six subtypes of acute lymphoblastic leukemia (ALL) patients. We also introduce a new classifier, named PCL, to make effective use of the rules. PCL is accurate and can handle multiple parallel classifications.

A longitudinal study of a natural lice infestation in growing cattle over two winter periods.

A group of 61 cattle which were naturally infested with lice was followed over two winter periods. Data were collected on the number of lice found at various body sites over this period. Summary statistics were produced and due to the repeated measured and hierarchical structure of the data, multi-level analysis was used to model the population dynamics of Bovicola bovis and assess the influence of the various hierarchical levels. A four level Poisson model was produced-level one, the individual number of lice at each parting; level two, the body site examined (shoulder, midline or rump); leve

Clinical prediction rule for 30-day mortality in Björk-Shiley convexo-concave valve replacement.

BACKGROUND AND OBJECTIVE: We aimed to develop a clinical prediction rule for 30-day mortality in patients undergoing Björk-Shiley convexo-concave (BScc) valve replacement, based on primary valve implantation procedures. METHODS: We studied 30-day mortality in 2,263 patients who received a BScc valve between 1979 and 1985 in The Netherlands. A logistic regression model was constructed and internally validated with bootstrapping techniques. RESULTS: Predictors for mortality were emergency operation, coronary artery disease, mitral or double-valve replacement, BScc implant being a reoperation,
