[Magnetic resonance imaging in multiple sclerosis: investigation of brain and spinal cord lesions].

Magnetic resonance imaging (MRI) of the brain was performed on forty-five patients with multiple sclerosis (MS), using T1-weighted inversion recovery and T2-weighted spin echo images, and the results were compared with X-ray computed tomography (CT). Some of the 45 MS patients were also examined by neurophysiological studies (visual evoked potentials and auditory brainstem responses) to compare with the brain MRI findings. MRI showed demyelinating plaques of the brain in 20 (74%) of 27 patients with brain symptoms, 11 (61%) of 18 patients without symptoms and 31 (69%) of all 45 patients. In 27…

Bullous pemphigoid and multiple sclerosis: more than a coincidence? Report of three cases.

In three patients with long-standing multiple sclerosis, bullous pemphigoid developed. The diagnosis of bullous pemphigoid was based on histologic findings, direct and indirect immunofluorescence, and Western blots showing IgG reacting with the 220 to 240 kD bullous pemphigoid antigen in the serum of three patients. Contrary to previous observations, bullous pemphigoid associated with multiple sclerosis was not different from bullous pemphigoid alone. Three similar cases have been reported previously, so the occurrence of bullous pemphigoid in patients with multiple sclerosis may be more than …

[Multiple sclerosis in health care in our country: electrophysiological, immunological and clinical analysis].

Among 70 patients referred for suspected multiple sclerosis (MS) a total of 40 met criteria for definite (17), likely (12) or possible (11) diagnosis of the disease. Visual evoked potentials provided evidence of CNS lesion in 13 cases. Abnormalities of somatosensory evoked potentials, particularly on stimulation of the posterior tibial nerve, were present in 90% of patients. Brainstem auditory evoked potentials were of little use, contributing to diagnosis of one subclinical lesion; they were normal in 14 cases with suspected brainstem involvement. A definite diagnosis of MS was made in 4 case…

[Comparative study of visual evoked potentials and magnetic resonance imaging in multiple sclerosis].

19 patients with clinically definite, probable or possible multiple sclerosis were examined by means of visual evoked potentials and magnetic resonance imaging. The patients were classified in three groups according to the criteria of Mac Alpine. 11 patients had definite multiple sclerosis: 8 of these patients, who had visual disorders evoking an optic neuropathy, had abnormal visual evoked potentials and 7 of these 8 patients had a pathological magnetic resonance imaging. In the other 3 patients of this group without optic neuropathy. 2 of them had abnormal visual evoked potentials and the 3 …

Evoked potentials in suspected multiple sclerosis: diagnostic value and prediction of clinical course.

Pattern visual, somatosensory and brainstem auditory evoked potentials (EPs) of 14 patients with definite multiple sclerosis, 222 patients suspected of having multiple sclerosis, 26 patients with isolated optic neuritis and 40 patients with a chronic not diagnosed neurologic disorder, were compared with their clinical diagnoses on 2 1/2-year follow-up. In the MS suspects, an EP abnormality demonstrating a clinically silent lesion in any modality (65 patients) was associated with a 71% chance of clinical deterioration (48% chance of definite MS within the follow-up period). Normal EPs (121 pati…

Induction of oligodendrocyte proliferation and remyelination after chronic demyelination. Relevance to multiple sclerosis.

Optic nerve and spinal cord tissue from untreated guinea pigs with chronic relapsing experimental autoimmune encephalomyelitis, guinea pigs … emyelinating oligodendrocytes might possess progenitors located in these regions. Observations from multiple sclerosis lesions showed that oligodendrocyte proliferation and CNS remyelination occur in human subcortical white matter, but to a lesser degree than that seen in the CNS of MBP/GC/treated guinea pigs. Immunocytochemical examination of CNS tissue from experi …

Immunocytochemical search for JC papovavirus large T-antigen in multiple sclerosis brain tissue.

The large T-antigens of papovaviruses JC (JCV) and BK share a C-terminal subsequence with myelin basic protein (MBP). Since this sequence functions as a phosphate acceptor site in MBP, expression of a competing T-antigen sequence in oligodendroglia might adversely affect their ability to post-translationally process MBP and thus to maintain myelin. We have used techniques which demonstrate JCV T-antigen in small oligodendroglial cells from progressive multifocal leukoencephalopathy tissue to search for a possible latent JCV infection expressing T-antigen in nine cases of multiple sclerosis (MS…

Pattern visual evoked potentials and spatial vision in retrobulbar neuritis and multiple sclerosis.

In an attempt to resolve some of the reported disagreements between visual evoked potential (VEP) recordings and Snellen visual acuity in patients with multiple sclerosis, we compared these test results with sine-wave grating-contrast sensitivity curves. Disease that depressed visual sensitivity for high spatial frequencies, sparing low spatial frequencies, was associated with depressed visual acuity and attenuated small-check VEPs in the affected eye, while large-check VEPs were not attenuated. When visual sensitivity to all spatial frequencies was depressed, both large-check and small-check …

Thymectomy-induced decrease in T gamma cells and OKT8+ cells in multiple sclerosis.

Studies were performed on lymphocytes from patients with multiple sclerosis before and after thymectomy (group I) and before and after thymectomy and azathioprine therapy (group II), and from matched control patients with multiple sclerosis, control patients with other neurological diseases, and healthy control subjects. Following treatment the percentage of T gamma cells in both group I and group II patients decreased from above the mean normal control levels to below this level; OKT8+ cell numbers in group I became lower than in any of the control groups; the percentage of concanavalin A-ind…

Isoelectric focusing of IgG eluted from multiple sclerosis and subacute sclerosing panencephalitis brains.

Elevated IgG distributed in oligoclonal bands is characteristically observed in the cerebrospinal fluid (CSF) of patients with multiple sclerosis (MS) or subacute sclerosing panencephalitis (SSPE). Similarly, IgG in bands has been detected in neutral saline (NS) and acid eluates of brain material from these two diseases. We have now used isoelectric focusing (IEF) to compare IgG eluted from control brain, three plaques and a white matter pool of an MS brain, and three regions of an SSPE brain. A direct peroxidase-conjugated anti-human IgG staining technique was used to stain IgG exclusively an…

[Vestibular meanifestations of multiple sclerosis. A new diagnostic element revealed by bithermic tests].

We studied 56 cases of disseminated sclerosis, all during an attack of the disease and all presenting with symptoms of incoordination. The diagnosis was certain both clinically and biochemically. Out of this population, 64% of the patients had unprovoked nystagmus; of these patients 48% had central type nystagmus (multiple, vertical superior or vertical inferior, or bilateral "gaze nystagmus"). We investigated particularly abnormalities in the bithermic tests (vestibular hypovalency, directional preponderance of nystagmus, thermic preponderance) and in particular we studied an index which to t

Visual evoked responses in multiple sclerosis: comparison of two methods for pattern reversal.

Visual evoked responses have been elicited by reversal of a black and white checkerboard pattern and also by a display of light-emitting diodes (LED) generating a reversal of a pattern of illuminated red circular areas. In a control group the LED display produced responses of lower amplitude and shorter latency compared to responses to checkerboard stimulation. In patients with a definite or probable diagnosis of multiple sclerosis the stimulators were equally effective in evoking pathological responses; in patients with a possible but less certain diagnosis abnormal responses were found in a …

Herpes simplex virus latency in patients with multiple sclerosis, lymphoma and normal humans.

Herpes simplex virus (HSV) was isolated from the trigeminal ganglia (TG) of 12 cadavers (10 traumatic deaths, one lymphoma and one multiple sclerosis). The cadaver with multiple sclerosis showed large bilateral trigeminal nerve root entry zone areas of demyelination. It is hypothesized that HSV is capable of migrating to the trigeminal nerve root entry zone and initiating demyelinating disease.

In vitro-labeled DNA for detecting viral genomes in multiple sclerosis: I. Papovaviruses.

Papovaviruses appear to be neurotropic and one, JC virus, is implicated as the cause of one type of demyelinating disease, progressive multifocal leukoencephalopathy. To investigate whether human papovaviruses play a role in multiple sclerosis, radioactively labeled DNA from BK virus, human papilloma virus, and simian virus 40 was used as a probe in order to detect related unlabeled DNA sequences in DNA isolated from multiple sclerosis brain and/or spinal cord. Labeled viral probes were denatured and DNA allowed to reassociate in the presence of excess unlabeled DNA from multiple sclerosis tis…

Anesthetic management for cesarean section and tubal ligation in a patient with Marfan syndrome, multiple sclerosis, and multiple postdural puncture headaches.

We report a 29-year-old woman with Marfan syndrome, multiple sclerosis, and multiple postdural puncture headaches who presented for a scheduled repeat cesarean delivery with bilateral tubal ligation at 37 weeks gestation. During an outpatient preoperative visit, a general anesthetic plan was ultimately selected through a shared decision-making process. The patient had an uneventful general anesthetic that included a rapid sequence induction with direct laryngoscopy. Neonatal Apgar scores were 8 at 1 minute and 9 at 5 minutes. Prior to emergence, fentanyl, acetaminophen, and ketorolac were admi…

Who declines neurocognitive testing? A cross-sectional study in people with multiple sclerosis.

In 334 people with multiple sclerosis, a backward stepwise regression analysis was conducted to identify clinical characteristics that predict who declines neurocognitive testing (p < 0.05), from demographics, disease-related variables, psychiatric diagnoses, and self-reported cognitive concerns. The mean age was 44.1 years (standard deviation = 11.4), the median Expanded Disability Status Scale was 2.5 (1.5-4.0), 79.8% had relapsing-remitting MS, and 14.7% declined testing. Independent correlates of declining neurocognitive testing included progressive MS (odds ratio = 2.1, 95% confidence int…

Multi-scale Gaussian product-of-experts for missing sequence multiple sclerosis lesion segmentation.

OBJECTIVE: To develop and evaluate a unified framework for multiple sclerosis (MS) white matter lesion segmentation that operates robustly under clinically prevalent missing sequence magnetic resonance imaging conditions. METHODS: We propose a multi-scale Gaussian product-of-experts framework (MG-PoE) integrated into nnU-Net, where sequence-specific inputs are independently processed through a shared encoder and parameterized as Gaussian experts before combination through precision-weighted product-of-experts (PoE) fusion at each encoder scale. The unified fusion rule handles different sequen…
