Post-traumatic cerebral fat embolism prior to operative repair of femoral and tibial fractures.

Rare cases of primary cerebral fat embolism as a postoperative complication have been reported. In addition, cerebral fat embolism occurring before operative repair without shunt lesion are more rarely reported. We report a patient with a posttraumatic cerebral fat embolism resulting in severe neurologic dysfunction without right to left shunt. A 25-year-old man was brought to the hospital immediately after a traffic accident. He sustained a right segmental femoral shaft fracture and a left tibial shaft fracture. The patient was alert with no neurological deficits. Approximately 13 hours after

[Open elbow dislocation with distal ischemia: a therapeutic emergency].

We report the case of a 13-year-old boy with a traumatic elbow dislocation, open stage 2 (Cauchoix and Duparc), with distal ischemia. Exposure of the neurovascular humeral bundle through the wound accentutated the clinical emergency. This clinical observation consisted of elbow dislocation with all severity criteria: (i) opening of the skin, (ii) association with a fracture of the medial humeral epicondyle, and (iii) neurological deficit in the territory of the median nerve. The purpose of this report is to remind physicians that no investigation should delay surgery in elbow dislocation. Desp

Management of calcaneus fractures by a new "Below-the-ankle" ilizarov frame: A series of 10 cases.

BACKGROUND: The optimal management of intra-articular calcaneal fractures is still controversial. Open reduction and internal fixation are always associated with serious complications. AIM: Various alternative methods have been used with variable effects. This retrospective study aimed to analyze the clinical efficacy and safety of a new "below-the-ankle" Ilizarov frame in patients with calcaneal fractures caused by high-energy trauma. PATIENTS AND METHODS: We retrospectively explored ten patients with calcaneal fractures, of which four, five, and one were Sanders type II, III, and IV, respe

Screw Fixation of Lateral Condyle Fractures: Results of Treatment.

BACKGROUND: Fixation of lateral condyle distal humeral fractures has traditionally been achieved with K-wires. Screw fixation provides the advantage of compression across the fracture site, but the results of screw fixation and risk of iatrogenic physeal damage are not well defined. This study was designed to evaluate the efficacy of screw fixation for lateral condyle fractures. METHODS: A retrospective study of patients with lateral condyle elbow fractures treated using screw fixation at a single institution was undertaken. Patients 12 years and younger with isolated fractures were included.

Pediatric Monteggia fractures: a single-center study of the management of 40 patients.

INTRODUCTION: Early identification and conservative management of pediatric Monteggia fractures has been shown to correlate with good results. Nevertheless, several authors advocate more aggressive management with open reduction and internal fixation (ORIF) for unstable fractures. We herein present the experience of a tertiary pediatric hospital in the management of Monteggia fractures. METHODS: Forty patients with Monteggia fractures (26 male and 14 female) were admitted and treated over a period of 20 years (1989 to 2009). The age of the patients ranged between 3 and 14 years (mean 7.5 y).

Comparison of pullout strength between 3.5-mm fully threaded, bicortical screws and 4.0-mm partially threaded, cancellous screws in the fixation of medial malleolar fractures.

Displaced medial malleolus fractures are considered unstable and typically require open reduction and internal fixation for anatomic reduction and early joint range of motion. These fractures are usually fixated with either compression lag screws or tension band wiring depending on the fracture pattern, size of the distal fragment, and bone quality. When fracture fixation fails, it is typically in pullout strength. Failure of primary bone healing can result in nonunion, malunion, and need for revision surgery. The current study wished to explore a potentially stronger fixation technique in reg

Do Routine Postoperative Radiographs Influence the Management of Distal Radius Fractures Following Volar Locking Plate Fixation?

BACKGROUND: Distal radius fractures (DRFs) are a common orthopaedic injury, often requiring surgical intervention. Routine postoperative radiographs are frequently obtained after surgical fixation to ensure adequacy of fixation and rule out early complications, yet their necessity remains unclear. Through this study, we tried to evaluate the impact of routine postoperative radiographs on the management of DRFs. The objective was to determine whether routine postoperative radiographs are necessary for the effective management of patients following surgical fixation of DRFs using volar locking p

Increased wound complication with intramedullary screw fixation of clavicle fractures: Is it thermal necrosis?

PURPOSE: Adult mid-shaft clavicle fractures are common injuries. For displaced fractures, open reduction with plate or intramedullary (IM) fixation is the widely used techniques. All methods have their own potential drawbacks, especially related to local soft tissue complications. There is little information about outcome and management of local wound complications after clavicle fracture fixations. METHODS: Ninety-seven patients underwent open reduction and internal fixation, 17 were treated with IM screw fixation and 80 with plate fixation. Wound complication occurred in eight patients (8.2

Does cerclage wiring interfere with fracture healing of osteosynthesis in comminuted midshaft clavicle fractures? A multicenter study.

INTRODUCTION: Historically, cerclage wires were not used in the treatment of clavicle fractures because of their invasiveness. The purpose of this study was to evaluate the radiologic results and the incidence of complications following cerclage wire application and plate fixation in the treatment of comminuted mid-shaft clavicle fractures. MATERIALS AND METHODS: A total of 116 patients with comminuted mid-shaft clavicle fractures who underwent open reduction and internal fixation were reviewed. Osteosynthesis of fractures after elbow ankylosis treated with an Ilizarov external ring fixator: A case report.

Treatment with an Ilizarov ring external fixator is less invasive of soft tissues than is open reduction and internal fixation (ORIF) surgery. Operative treatment of fractures with an Ilizarov ring external fixator after elbow ankylosis has not been reported. We describe our experience with this surgical treatment (i.e., placement of an Ilizarov external ring fixator) for a fracture that occurred after elbow ankylosis. A 63-year-old Japanese woman fell from standing height onto her left elbow. A transverse fracture was observed at the ankylosed site and the patient underwent surgery 8 days aft
