Risk factors for pregnancy among rural and urban-dwelling adolescents in a high adolescent-pregnancy setting; a case control study.

BACKGROUND: In Uganda, 1 in 4 girls has been pregnant by 18 years. This was worse during the COVID 19 pandemic. We determined risk factors for pregnancy among rural and urban dwelling adolescents in Uganda. METHODS: We conducted a mixed-methods study among girls aged 13 to 19 years in a rural and urban district in Uganda. Quantitatively, we used an unmatched case control design involving 200 adolescents with pregnancy experience and 400 controls with no pregnancy experience. Quantitative data was analyzed using STATA version 14. Qualitative methods included 17 focus group discussions, 10 in-d

Burnout of the seafarers - a systematic review of burnout in the maritime sector.

Seafaring is a strenuous profession, involving prolonged periods of isolation and a high workload in an unpredictable workplace. These factors contribute to increasing development of burnout in seafarers, a significant problem in this group, with moderate to severe emotional exhaustion (EE), depersonalization (DP), and reduced personal performance. Despite the impact on mental health, burnout has not yet been systematically investigated. The aim of this systematic review is to examine, analyze, and evaluate the quality of the literature on burnout in the maritime industry. We searched various

Thermal Abandonment: Best Practices to End Correctional Heat Death for Menopausal Black Women in Prison.

Menopausal Black women in U.S. prisons face a convergence of three accelerating crises: bodies made biologically older by structural racism and incarceration exposure, a menopausal transition that arrives earlier and manifests more severely for Black women than for any other group, and thermally hostile carceral infrastructure designed without their safety in mind. Estrogen decline disrupts thermoregulation-producing hot flashes, cardiovascular strain, and syncope risk-that becomes life-threatening when ambient temperatures reach 112°F-118°F and access to cooling, hydration, and hormone therap

Bracing AdoleScent Idiopathic Scoliosis after skeletal maturity (BASIS 2): study protocol for a randomized controlled trial within a larger trial.

AIMS: Adolescent idiopathic scoliosis affects 0.2% to 0.5% of adolescents, often requiring bracing to reduce the risk of curve progression. While bracing is typically discontinued at skeletal maturity, significant curve progression can occur afterwards, potentially necessitating surgery. The Bracing AdoleScent Idiopathic Scoliosis (BASIS) 2 study, nested within the larger BASIS trial, aims to evaluate the efficacy of prolonged full-time and night-time bracing beyond skeletal maturity in reducing curve progression. The aim is to determine if six months of additional bracing at normal prescripti

Why Participants Leave: A Qualitative Study Exploring Dropout from a Medication for Opioid Use Disorder (MOUD) Clinical Trial.

BACKGROUND: High dropout rates in clinical trials can undermine data validity, particularly among individuals with substance use and mental health disorders. This qualitative study examined reasons for participant dropout from a randomized trial evaluating the Minds and Mentors Program, a Mindfulness-Based Relapse Prevention and peer support intervention, among adults receiving Medication for Opioid Use Disorder (MOUD). METHODS: Using a qualitative descriptive design, semi-structured interviews were conducted with eight participants who withdrew before completing the 12-week intervention or f

A Participatory Mixed-Methodology Study of HIV and STI Testing Barriers Among Latino College Students in the California Inland Empire.

Young adults in the United States experience a disproportionate burden of HIV and sexually transmitted infections (STIs), with Latino populations and residents of California's inland regions facing compounded disparities. This study examined multilevel barriers to HIV and STI testing among Latino college students in the California Inland Empire using Group Concept Mapping (GCM), a mixed-methods participatory approach. Twenty-six students participated in brainstorming sessions, and nineteen completed sorting and rating activities. Analyses identified five clusters of barriers: access, awareness

Modified single-stapled anastomosis in laparoscopic or robotic low anterior resection for rectal cancer: protocol for a multicentre randomised controlled trial.

INTRODUCTION: Anastomotic leakage remains one of the most serious complications after low anterior resection for rectal cancer. The modified single-stapled technique (MST) eliminates the intersection between linear and circular staple lines and may reduce leakage compared with the conventional double-stapled technique (DST). This trial aims to determine whether MST reduces anastomotic leakage in patients undergoing laparoscopic or robotic low anterior resection. METHODS AND ANALYSIS: This is a multicentre, parallel-group and superiority randomised controlled trial conducted in six tertiary ho

Plant polyphenols, terpenes, and terpenoids in oral health.

INTRODUCTION: Herbal products are increasingly being used for their potential to promote oral health. Phytochemicals can act as scavengers of reactive or toxic chemicals, selective growth factors for beneficial oral bacteria, fermentation substrates for beneficial oral bacteria, and selective inhibitors of harmful oral microflora. Plant-derived secondary metabolites encompass a variety of natural products, including alkaloids, polyphenols (including flavonoids and tannins), terpenes, terpenoids, steroids, saponins, quinones, coumarins, glycosides, and carotenoids. OBJECTIVES: This review expl

Enhancing Care Transition Pathways in Regional Acute Mental Healthcare Settings: Insights From Lived Experience Group Model Building Workshops.

Effective care transitions are critical to patient safety and recovery in mental healthcare, yet they remain a point of vulnerability due to fragmented communication, restrictive practices and systemic pressures. This study aimed to identify priority issues and leverage points for improving care transitions in a regional acute mental health service using group model building (GMB), a structured systems thinking co-design approach. Seventeen participants-including service users, family carers and healthcare workers-engaged in a series of participatory workshops to map the factors influencing ca

A longstanding challenge in the behavioral addictions field has been determining the point at which gaming involvement becomes clinically si … oredom, or facilitating social interaction is not sufficient for a diagnosis; and that cultural and peer group norms should be considered in diagnosis. In this paper, we examine gaming normality-disorder boundary issues in the areas of conceptualization, assessment, and interventions. Electrodiagnostic criteria for neuromuscular transmission disorders suggested by a European consensus group.

OBJECTIVE: Electrodiagnostic testing plays an important role in diagnosing disorders of neuromuscular transmission (NMT), especially in seronegative myasthenia gravis. However, electrodiagnostic criteria for the diagnosis are sparse. This study aimed at inferring evidence-based recommendations for the electrodiagnostic examination of NMT disorders. METHODS: A total of 164 cases with a consensus diagnosis of NMT disorder obtained by peer review by eight experienced neurophysiologists were analysed for differences in examination strategy, the sensitivity of different tests, and inferring minima

Increasing engagement of Spanish-speaking transgender Latinas in research: strategies from ChiCAS, a community-based participatory research intervention trial.

Minoritized racial, ethnic, sexual, and gender communities and populations face profound health disparities and their engagement in research remains low. In a randomized controlled trial, our community-based participatory research partnership tested the efficacy of ChiCAS, an HIV prevention intervention designed to increase pre-exposure prophylaxis use among Spanish-speaking transgender Latinas. Of 161 eligible Spanish-speaking transgender Latinas screened, we enrolled 144, achieving an 89% participation rate, and retained 94% at 6-month follow-up. Given these participation and retention rates

Long-term impact of sevoflurane exposure on behavioural and neurocognitive outcomes in preschool children: protocol for a prospective cohort study.

BACKGROUND: The use of sevoflurane, a commonly used paediatric anaesthetic, raises concerns about potential long-term neurotoxic effects on behavioural and neurocognitive development, particularly during critical neurodevelopmental stages in preschool children. Endogenous hydrogen sulfide (H2S), a neuroprotective gasotransmitter, may be affected by anaesthetic exposure, but its role in sevoflurane-induced neurotoxicity remains unclear. METHODS/ANALYSIS: This prospective cohort study aims to evaluate behavioural and neurocognitive outcomes in 200 preschool children aged 4-6 years (1:1 allocati

Oxytocin Amplifies Negative Response to Ambiguity in Adolescent Females With and Without Eating Disorders.

OBJECTIVE: Eating disorders (ED) typically emerge in adolescence, a critical period for brain development and peer bonding. Interpersonal difficulties-particularly social anxiety-frequently co-occur with ED. Oxytocin is a neuropeptide that modulates social cognition and linked to prosocial effects. To date, no study has investigated oxytocin's effects on negative interpretation bias toward ambiguous information in adolescents with ED. METHODS: Forty-eight female adolescents aged 16 to 17 years with and without EDs took part in a placebo-controlled, double-blinded, randomised, crossover trial

Development of a Quality Measure to Improve HIV and Syphilis Screening in the Emergency Department: A Modified Delphi Approach.

Emergency department (ED) screening is an important strategy to address the persistent human immunodefiency virus (HIV) epidemic; however, ED screening rates remain low. This study describes the development of a quality measure to improve screening rates for HIV among ED patients. A panel of 10 emergency physicians with expertise in ED-based preventive health and quality measurement employed a 3-round modified Delphi process. The panel reviewed peer-reviewed literature and national guidelines to identify high-risk patient populations; participated in anonymous surveys to rank ED screening appr
