A pain management program for chronic cancer-treatment-related pain: a preliminary study.

UNLABELLED: A large proportion of patients may develop chronic pain following cancer treatments such as surgery, radiotherapy, or chemotherapy. These patients can experience significant levels of physical and psychological morbidity. Our aim was to investigate a cognitive-behavioral pain management program (PMP) for cancer patients with chronic treatment-related pain. Thirteen patients (1 man, 12 women; mean age 52 yrs) completed the study, 9 of whom had a history of breast cancer and had received extensive medical treatment, including surgery. A combination of physical and psychological techn

Effective Sufentanil Intrathecal Pump Treatment in a Patient With Refractory Complex Regional Pain Syndrome: A Case Report.

BACKGROUND: Many treatments for severe complex regional pain syndrome (CRPS) are ineffective as patients may still experience pain even after various therapeutic approaches. Therefore, it is important to consider novel and creative treatment options for patients suffering from unremitting refractory pain. CASE REPORT: We report the case of a patient with severe CRPS, which proved to be refractory to numerous conventional treatments. CRPS is a disease that causes chronic pain, typically in distal extremities. CONCLUSIONS: Treatment for CRPS includes pharmacotherapy, sympathetic nerve block, a

Pain management in the surgical ICU patient.

PURPOSE OF REVIEW: Acute pain management in the surgical ICU is imperative. Effective acute pain management hastens a patient's return to normal function and avoid the negative sequelae of untreated acute pain. Traditionally, opioids have been the mainstay of acute pain management strategies in the surgical ICU, but alternative medications and management strategies are increasingly being utilized. RECENT FINDINGS: Extrapolating from lessons learned from enhanced recovery after surgery protocols, surgical intensivists are increasingly utilizing multimodal pain regimens (MMPRs) in critically il

Peripheral nerve stimulation: black, white and shades of grey.

Unbearable chronic neuropathic pain, which can often not be resolved by conservative pain management techniques, calls for peripheral nerve stimulation. The technique, based on the Gate control hypothesis, uses low intensity current to block depolarization and conduction of neuronal membrane. Conditions such as intractable migraine, occipital and trigeminal neuralgia, burn injury pain, transformed migraine, among others, which cause intense pain can be treated by PNS, especially when the pain is localised and can be traced back to a specific nerve. Complication rates in PNS vary from 5% in som

Trigeminal Neuralgia, Glossopharyngeal Neuralgia, and Myofascial Pain Dysfunction Syndrome: An Update.

Neuropathic pain is a common phenomenon that affects millions of people worldwide. Maxillofacial structures consist of various tissues that receive frequent stimulation during food digestion. The unique functions (masticatory process and facial expression) of the maxillofacial structure require the exquisite organization of both the peripheral and central nervous systems. Neuralgia is painful paroxysmal disorder of the head-neck region characterized by some commonly shared features such as the unilateral pain, transience and recurrence of attacks, and superficial and shock-like pain at a trigg

Pain relief during delivery. An evaluation of conventional methods.

The study evaluates the effect of conventional obstetric analgesia on 544 parturients and their newborn infants. The parturients' pain situation was evaluated with respect to both the effect of pain relief given, and the total pain experience in the first and second stage. This gave a more correct view of the parturients' pain situation and effectiveness of the analgesic procedures that if one or the other estimation was used separately. Nearly 20% of all parturients experienced good effect on low-back or suprapubic pain in the first stage. Severe to almost unbearable pain was experienced by a

Evaluation of pulsed radiofrequency denervation in the treatment of chronic facetjoint pain: an observational study.

BACKGROUND: Low back disorder is the most common problem in the entire spinal axis. About two-thirds of adults suffer from low back pain (LBP) at some time. Pain generators in the lumbar spine include the annulus of the disc, the posterior longitudinal ligament, a portion of the dural membrane, the facet joints, the spinal nerve roots and ganglia, and the associated paravertebral muscle fascia. There is no doubt that the facet joint is a potential source of chronic LBP. Facet joints are true synovial joints that have a joint space, hyaline cartilage surfaces, a synovial membrane, and a fibrous

Minimally Invasive Posterior Cervical Foraminotomy.

The minimally invasive posterior cervical foraminotomy procedure has become a common and successful procedure for the treatment of cervical radiculopathy. Specifically, the minimally invasive approach allows for decreased postoperative pain, blood loss, and length of hospitalization by preserving the surrounding soft tissue as compared with the traditional open approach. This article and accompanying video demonstrates the technique for a primary, single-level minimally invasive posterior cervical foraminotomy as performed through a tubular retractor.

Salvage of a failed Sauve-Kapandji procedure using a total distal radio-ulnar joint replacement.

This is the first report in the literature of a patient treated with a DRUJ replacement after Sauvé-Kapandji procedure failed due to pain and instability. The DRUJ replacement is an unconstrained, biomechanically more advantageous implant which can confer stability in cases where soft tissues are inadequate. We describe the treatment and outcome of persistent ulnar instability with a distal radio-ulnar joint replacement following failed salvage procedures for a malunion of a distal radius fracture.

Vascularized metacarpal bone graft for scaphoid non-union and Kienböck's disease.

A new vascularized bone graft from the base of the second metacarpal was used to treat scaphoid non-union and Kienböck's disease. In two patients with scaphoid non-union, the procedure promoted healing, even when the proximal segment was poorly vascularized. In one patient with Kienböck's disease, the graft held stable and wrist pain was markedly reduced. Further clinical experience may establish this procedure as an option for carpal bone fractures or disease.

The Maquet procedure in the treatment of patellofemoral osteoarthrosis. Long-term results.

Long-term results are reported for 35 knees with patellofemoral osteoarthrosis treated by Maquet procedure. Eighty percent were rated very good or good, and 20% fair or poor. The poor results are attributed to errors in the diagnosis or to inadequate surgical technique. The Maquet procedure thus provides good results for painful knees with patellofemoral osteoarthrosis. A tibial tubercle elevation of at least 2 cm is necessary to decrease joint pressure sufficiently.

Minimally Invasive and Conservative Interventions for the Treatment of Sacroiliac Joint Pain: A Review of Recent Literature.

Sacroiliac joint (SIJ) pain is responsible for approximately 15-25% of reported back pain. Patients with SIJ pain report some of the lowest quality of life scores of any chronic disease. Understanding of the physiology and pathology of the SI joint has changed dramatically over the years, and SI joint pain and injury can now be thought of in two broad categories: traumatic and atraumatic. Both categories of SI joint injury are thought to be caused by inflammation or injury of the joint capsule, ligaments, or subchondral bone in the SI joint. Treatment of SI joint pain usually involves a multi-

The use of EMLA cream and 1% lidocaine infiltration in men for relief of pain associated with the removal of genital warts by cryotherapy.

BACKGROUND: Surgical procedures used to remove genital warts (cryotherapy, electrodesiccation) are painful. Attempts to reduce the discomfort of surgery by prior lidocaine infiltration anesthesia are compromised by the pain of the infiltration. OBJECTIVE: Our purpose was to determine the efficacy of topically applied lidocaine/prilocaine cream to reduce the pain of lidocaine infiltration and the pain associated with cryotherapy to remove genital warts. METHODS: Men, scheduled for removal of genital warts by cryotherapy, were randomly selected to receive one of three treatments: (1) lidocaine

The percentage of sacroiliac fusions done in the U.S. by non-surgical specialties has increased.

BACKGROUND: Surgical fusion of the sacroiliac (SI) joint is often performed to manage chronic lower back or buttock pain. When Current Procedural Terminology (CPT) codes were introduced, SI joint fusion procedures were done primarily by orthopaedic surgeons and neurosurgeons. The purpose of this study was to examine the utilization of SI joint fusion CPT codes by physician specialty over time. METHODS: A retrospective cohort study was conducted using the PearlDiver database. The database was queried using CPT codes to identify patients who underwent SI joint fusion via percutaneous, open, or
